Purpose : High-flow nasal cannula oxygen therapy (HFNC) is a new type of non-invasive respiratory support for acute respiratory failure patients. However, patients receiving HFNC often develop sleep disturbances. We therefore examined whether dexmedetomidine could preserve the sleep characteristics in patients who underwent HFNC. Patients and Methods : This was a pilot, randomized controlled study. We assigned critically ill patients treated with HFNC to receive dexmedetomidine (0.2 to 0.7 µg / kg / h, DEX group) or not (non-DEX group) at night (9:00 p.m. to 6:00 a.m.). Polysomnograms were monitored during the study period. The primary outcomes were total sleep time (TST), sleep efficiency and duration of stage 2 non-rapid eye movement (stage N2) sleep. Results : Of the 28 patients who underwent randomization, 24 were included in the final analysis (12 patients per group). Dexmedetomidine increased the TST (369 min vs. 119 min, p = 0.024) and sleep efficiency (68% vs. 22%, P = 0.024). The duration of stage N2 was increased in the DEX group compared with the non-DEX group, but this finding did not reach statistical significance. The incidences of respiratory depression and hemodynamic instability were similar between the two groups. Conclusions : In critically ill patients who underwent HFNC, dexmedetomidine may optimize the sleep quantity without any adverse events. J. Med. Invest. 69 : 266-272, August, 2022

Swarm intelligence (SI) algorithms have been studied in solving real-world optimization problems called black-box optimization problems. Typical features of SI algorithms are: (1) being a population-based metaheuristics; (2) using fitness values of a given objective function; and (3) having very simple search rules which search agents follow. As such, SI algorithms have been applied to various black-box optimization problems. Particle swarm optimization is one of powerful SI algorithms, in which a swarm consists of plural particles as solution candidates. Particles directly fly a search space and share their own information each other, and thus PSO can find good quality of solutions. However, a PSO swarm is easily stuck in solving optimization problems whose search space is high-dimensional and complicated. In order to solve such problems, large numbers of particles and reference frame invariance are needed for PSO algorithms. Herein, we suggest a piecewise-linear particle swarm optimizer (PPSO) which is a deterministic PSO. PPSO has two simple search modes switched to another mode dynamically, whose search dynamics are complex. As such, PPSO algorithm can be implemented on hardware with low hardware costs because PPSO algorithm must not require many random number generators. In addition, PPSO algorithm can find a good quality of solution in solving complex optimization problems. We studied search performances of PPSO compared to PSO algorithms and provide theoretical analysis of reference frame invariance for PPSO. In order to verify search performances and theoretical analysis, we performed numerical simulations.

In this study, we explored the impacts of midlatitude western North Pacific (WNP) sea surface temperature (SST) on tropical cyclone (TC) activity at intraseasonal to seasonal time scales during the 2018 boreal summer. During this period, a positive SST anomaly occurred in the midlatitude WNP and subtropical central Pacific; TC activity was abnormally high under the influence of the strong Asian summer monsoon. We performed sensitivity experiments using a global cloud system-resolving model for global SST (control, CTL) and SST that were regionally restored according to midlatitude WNP climatology (MWNPCLM). TC track density in the eastern WNP was higher in CTL than in MWNPCLM, in association with large-scale atmospheric responses; enhanced monsoon westerlies in the subtropical WNP, moist rising (dry subsiding) tendencies, and reduced vertical wind shear in the eastern (western) WNP. Enhanced TC activity in the eastern WNP was more distinct for intense TCs and during the active phase of intraseasonal oscillation (ISO). These results suggest that the impacts of midlatitude SST anomalies can reach lower latitudes to affect TC activity via large-scale atmospheric responses and ISO, which are usually overwhelmed by the impacts of SST anomalies in the tropics and subtropics.

Objective Peripherally inserted central catheters (PICCs) are widely used in patients with hematologic malignancies. However, the risks of PICC-related complications during chemotherapy for acute myeloid leukemia (AML) are not fully understood. Methods We conducted a retrospective review of 128 adult patients with AML who received induction therapy by way of PICC insertion between 2012 and 2019. Results The median duration of PICC insertion was 30 days. The incidence rate of catheter-related bloodstream infection (CRBSI) was 2.4% at 30 days, and women were more likely to suffer from CRBSI than men. Local reactions at the insertion site were observed in 56 patients; however, these events did not predict CRBSI. The incidence rates of catheter-related thrombosis (CRT) were 1.6% at 30 days. Obesity put patients at an increased risk for CRT. Unexpected PICC removal occurred in 59 patients, and women were at a higher risk of catheter removal than men. Conclusion Low PICC-related complication rates, possibly associated with high rates of catheter removal, were observed during intensive chemotherapy for AML. Women and obese patients require careful monitoring of their PICC. Procedures to achieve appropriate PICC removal without increasing the complication rate need to be considered.

[Purpose] No previous studies have confirmed whether the effects of visual feedback on motor learning vary according to learner skill level for a learning task. The purpose of this study was to clarify whether differences in skill influence the effects of visual feedback on motor learning. [Participants and Methods] Sixty-four participants were assigned to one of four different feedback groups (concurrent-100%, concurrent-50%, terminal-100%, or terminal-50%). The learning task was to adjust the load amount continuously to the left lower limb in accordance with sound stimulation at intervals of 1 Hz. The four groups performed a pretest, practice sessions, and a retention test 24 hours after practice. After completing these measurements, the participants were classified as either high- or low-skilled based on the results of the pretest. [Results] Only the groups of low-skilled participants who used concurrent feedback showed lower root mean square errors in the retention test compared to in the pretest. [Conclusion] Differences in skill level for the same task influenced the effects of visual feedback on motor learning. Furthermore, concurrent visual feedback can help improve motor learning in low-skilled learners for the same task.

Tobacco smoking continues to be a major risk factor for cardiovascular disease (CVD) and the leading avoidable cause of death worldwide. Tobacco smoking has declined in high-income countries, but the average smoking rate in Japan remains high: 29.4% for men and 7.2% for women in 2017. Of note, the average smoking rate among middle-aged men remains approximately 40%, indicating that a high incidence of smoking-related CVD will continue for a couple of decades in Japan. The adverse effects of tobacco smoking on CVD are more extensive than previously thought. Physicians should be particularly alert to the development and progression of heart failure, atrial fibrillation, and venous thromboembolism, as well as ischemic CVD among tobacco smokers. Increasing use of heat-not-burn tobacco as cigarette alternatives is an emerging issue. Harmful effects do not disappear just by changing the delivery system of tobacco.

Background:This retrospective study was conducted to evaluate the safety and efficacy of ambrisentan combination therapy with phosphodiesterase type 5 (PDE5) inhibitors in Japanese patients with pulmonary arterial hypertension (PAH).Methods and Results:PAH patients who received ambrisentan for the first time in combination with a PDE5 inhibitor between January 2013 and the end of August 2015 were included in this study. Adverse drug reaction (ADR) safety analysis, as well as the efficacy analysis focusing on changes in clinical parameters, were investigated for overall cases and cases stratified by patient background. Forty-eight consecutive patients (n=21, 43.8% with idiopathic PAH; male/female, 18/30; average age, 43.3±17.4 years; World Health Organization functional class III/IV, n=22, 45.8%) who were treated with ambrisentan and a PDE5 inhibitor in Japan underwent the safety analysis. A total of 14 ADR occurred in 10 patients (20.8%). ADR included headache (8.3%), face edema (4.2%), angina pectoris (2.1%), hyperemia (2.1%), dyspnea (2.1%), pulmonary hypertension (i.e., worsening of PAH, 2.1%), nausea (2.1%), hepatic function abnormal (2.1%), edema (2.1%), and sudden death (2.1%). On analysis of hemodynamics parameters, there was a significant improvement in the mean pulmonary arterial pressure (−13.5 mmHg, P=0.0001) and pulmonary vascular resistance (−563.53 dyn·s·cm−5, P=0.0033).Conclusions:Ambrisentan combination therapy is safe and effective in hemodynamics improvement.

This paper provides a thorough description of humeral, ulnar, and radial specimens of the Nacholapithecus holotype (KNM-BG 35250). A spool-shaped humeral trochlea (and keeled sigmoid notch of the ulna) is a hallmark of elbow joint evolution in hominoids. In lacking this feature, the elbow of Nacholapithecus is comparatively primitive, resembling that of proconsulids. However, the humeroulnar joint in Nacholapithecus is specialized for higher stability than that in proconsulids. The humeroradial joint (humeral capitulum) resembles that of extant apes and Sivapithecus. This condition may represent an intermediate stage leading to the fully modern elbow in extant apes. If this is the case, specialization of the humeroradial joint preceded that of the humeroulnar joint. Nacholapithecus elbow joint morphology suggests more enhanced forearm rotation compared to proconsulids. This observation accords with the forelimb-dominated positional behavior of Nacholapithecus relative to proconsulids, which has been proposed on the grounds of limb proportions and the morphology of the phalanges, shoulder girdle, and vertebrae.

The edible purple laver, Porphyra yezoensis, contained 51.49±1.51 μg of vitamin B12 compounds per 100 g dry weight of the laver (mean±SEM, n=4). A vitamin B12 compound was purified from the lyophilized purple laver and partially characterized. The silica gel 60 TLC and reversed-phase HPLC patterns of the purified pink-colored compound were identical to those of authentic vitamin B12, but not to those of vitamin B12 analogues inactive for humans.

Aim: We conducted a pilot study to clarify the effects of the Japan Diet nutritional education program on metabolic risk factors for atherosclerotic cardiovascular disease in middle-aged men who were brought up in the westernized dietary environment of modern Japan.Methods: Thirty-three men, 30–49 years of age, attended a nutrition education class to learn food items and recommended volumes comprising the Japan Diet (more fish, soybeans and soy products, vegetables, seaweed, mushrooms and unrefined cereals, and less animal fat, meat and poultry with fat, sweets, desserts and snacks, and alcoholic drinks), and were encouraged to consume the Japan Diet for 6 weeks. Anthropometric and biochemical parameters were measured and 3-day weighted dietary records were kept before and at completion of the intervention.Results: Ninety-one percent of participants showed improvements in more than one cardiovascular risk factor after 6 weeks. Body weight, serum low density lipoprotein (LDL) cholesterol, malondialdehyde modified (MDA)-LDL and triglyceride concentrations decreased significantly, while high density lipoprotein cholesterol was unchanged. Fish, soy, and sum of seaweed, mushrooms and konjak intakes doubled, and green and yellow vegetable intakes also increased as compared to baseline. Meanwhile, intakes of refined cereals, meat and poultry, sweets, desserts and snacks, and margarine and shortening decreased. Total energy, lipid, and saturated and monounsaturated fatty acid intakes decreased, while n-3 polyunsaturated fatty acid, dietary fiber, beta-carotene, vitamins D and K, potassium, and magnesium increased, with no change in sodium intake.Conclusions: The Japan Diet is suggested to improve atherosclerotic cardiovascular disease risk factors in middle-aged Japanese men.The clinical trial registration number: UMIN000020639.
