近年,BERT のような事前学習モデルを利用することで,自然言語処理システムの性能が大きく向上している.BERT は,Transformer の Multi-head Attention を用いることで文脈に応じた単語の埋め込み表現列を得ることのできるモデルである.文書分類のタスクの場合,文書を BERT に入力し,その出力から文書の特徴ベクトルを構築する方法によって処理できる.しかし,BERTに入力できるシーケンスの長さには上限がある.この制限によって,長い文書を扱う場合,標準的な手法では文書分類に必要な情報を十分に得られないと考えられる.そこで,BERT から長い文書内の全ての単語に対応する埋め込み表現を得て,そこから文書の特徴ベクトルを作成する手法を提案する.

COPD患者の多次元的重症度評価は生命予後の観点から重要であるといわれている.しかし,呼吸リハビリテーションのBODE indexの重症度別の効果は明らかでない.対象はADLの自立している外来呼吸リハが可能なCOPD患者37名.週に1回,3ヵ月間の介入を行った.介入前後で呼吸機能検査,下肢筋力検査,6分間歩行試験,QOL評価を行った.BODE indexの重症度別にⅠ群(軽症),Ⅱ群(中等症),Ⅲ群(重症),Ⅳ群(最重症)と群分けし,効果を検討した.Ⅱ,Ⅲ群は介入前後で呼吸機能,運動機能の有意な改善が認められた.群間での改善率の比較では,呼吸機能でⅡ,Ⅲ群はⅠ群と比較し有意に高く,運動耐容能ではⅢ群はⅠ,Ⅳ群と比較し有意に高い結果であり,Ⅲ群で最も改善率が良好であった.呼吸リハビリテーション効果はBODE indexの重症度により改善に差があることが確認された.

目的 ビタミン、ミネラルなどをはじめとしてフィトケミカルを豊富に含む野菜や果物は、健康維持や生活習慣病の予防に重要であるが、十分に摂取できていないのが現状である。本研究では、作り置きにより野菜を手軽に摂取するための一つの方法としてピクルスに注目し、野菜に含まれる栄養素の中でも調理等により壊れやすいビタミンCに着目した。方法 ピクルスの野菜やピクルス液の配合のレシピ調査をもとに、パプリカ、キャベツのピクルスを調製した。野菜を下漬け後、ピクルス液に浸漬して、冷蔵庫で保存し、経時的に野菜とピクルス液に分けてビタミンCを抽出し、HPLC-UV法により還元型ビタミンC(AsA)量および酸化型ビタミンC(DAsA)量の両者を定量した。結果 パプリカのAsA量は、1日で浸漬前の約2/3に、7日で約1/3に減少したが、その後大きな変化はなかった。一方、ピクルス液のAsA量は経時的に増加したことから、パプリカのビタミンCのピクルス液への溶出が示された。キャベツの総ビタミンCはパプリカと比較して減少速度が大きく、パプリカよりもDAsAの割合が高かった。以上の結果から、ピクルスのビタミンCは浸漬後短時間で溶出しやすいが、1週間あるいは3週間後でもピクルスのビタミンCは残存していた。

The quality-adjusted life year (QALY) and incremental cost-effectiveness ratio (ICER) are important concepts in cost-effectiveness analysis, which is becoming increasingly important in Japan. QALY is used to estimate quality of life (QOL) and life years, and can be used to compare the efficacies of cancer and cardiovascular treatments. ICER is defined as the difference in cost between treatments divided by the difference in their effects, with a smaller ICER indicating better cost-effectiveness. Here, we present a review of cost-effectiveness analyses in Japan as well other countries. A number of treatments were shown to be cost-effective, e.g., statin for secondary prevention of cardiovascular disease, aspirin for primary prevention of cardiovascular disease, DOAC for high-risk atrial fibrillation, beta blockers, ACE inhibitors, and ARB for heart failure, sildenafil and bosentan for pulmonary hypertension, CABG for multi-vessel coronary disease, ICD for ventricular tachycardia, and CRT for heart failure with low ejection fraction, while others were not cost-effective, e.g., epoprostenol for pulmonary hypertension and LVAD for end-stage heart failure. Further investigations are required regarding some treatments, e.g., PCSK-9 inhibitors for familial hypercholesterolemia, PCI for multi-vessel coronary disease, catheter ablation for atrial fibrillation, and TAVI for severe aortic stenosis. Ethical aspects should be taken into consideration when utilizing the results of cost-effectiveness analysis in medical policy.

気功は、中国の心身鍛練方法の一種と言われている。その気功練習者の身体からある周期に変調された放射赤外線が検出されたという報告がある。さらに、その周期は呼吸周期と一致するという報告もある。著者らは、放射赤外線を検出する装置を自作し実験を行った。この放射赤外線検出装置においても放射赤外線に呼吸周期と一致する周期が測定された。また、放射赤外線は皮膚表面温度と深く関係しているので、皮膚に直接温度センサを付けた実験も行われた。このセンサにおいても呼吸周期と一致する周期が測定された。また、他の分野で、皮膚表面温度から呼吸数を推定する研究報告がある。本研究によって、気功時における放射赤外線の変調は、呼吸の変化が皮膚表面温度の変化として表れたことに起因すると示唆された。

In pregnant women, non-obstetric surgery should be performed during an appropriate period to prevent congenital anomaly and to avoid miscarriage or premature labor. The second trimester of pregnancy is better for surgery because the risk of maternal, anatomical and physiological changes is lower. Neurotoxicity and the teratogenicity of anesthetic agents should be considered. In addition, blood flow to the placenta must be maintained during the surgery.  We performed anesthesia for an 18-week pregnant woman who required a radical neck dissection because of the metastasis of tongue cancer. She was 29 years old, 160 cm tall, and weighed 46.1 kg. Before the induction of anesthesia, the patient was placed in a left half-lateral position by placing towels under the right lower back to avoid supine hypotension syndrome, as the patient complained of a sense of abdomen distension while in a supine position. Her face was twisted to the right to secure a surgical field. General anesthesia was induced with intravenous propofol, atropine, fentanyl and remifentanil. Two milliliters of 4% lidocaine was sprayed on the subglottic area and the glottis for topical anesthesia. A tracheal tube (ID, 6.0 mm) was intubated orally using McGRATH. The anesthesia was maintained with the inhalation of 1.5%-1.7% sevoflurane in oxygen (Fio2, 0.4), as sevoflurane may be less neurotoxic to a fetus than isoflurane or desflurane. Ephedrine was administered intravenously to maintain blood flow to the placenta when the blood pressure decreased. The operation and anesthesia were completed uneventfully.  After the operation, the obstetrician performed an ultrasonography examination to confirm that the fetus had good fetal movement and a normal pulse.

In this paper, we develop a three-legged robot without a brain. This tripedal walking robot generates gait patterns adaptively and autonomously with no sensor, or controller. The key of this adaptability is to utilize a “weakness” of low-torque motors in each leg. Each motor drives leg joint and adjusts its own phase in response to a reaction force from the ground. As a result of the dynamics of body–motor–environment interaction, the weak motors sync each other.

A passive dynamic walking robot can walk down a gentle slope naturally without any actuator or controller, only by potential energy. Therefore, it is thought that the principle of walking exists in this phenomenon. By current researches, the passive dynamic walking has been confirmed for two and four. However, it seems that it is also important to study passive dynamic walking for more than 6 legs because there are many creatures that have more than 6 legs. In this research, a passive dynamic walking robot with more than six legs was analyzed by a simulator. As a result, it was confirmed that passive dynamic walking even for 20 legs could be achieved. Furthermore, its gait could be changed by changing the body structure. In addition, the differences of stability depend on the number of legs resulted, and this differences disappeared by changing the position of the pitch spin parts. Moreover, its stability was very high, so it could walk on the contoured ground.

The purpose of this study was to develop a self-report instrument of mood states consists of eight factors based on a three-dimensional model. The three-dimensional model consisted of energy, tension, and hedonic tone. Eight factors were defined to be influenced by three dimensions; energy, tension, and hedonic tone. The Three-Dimensional Checklist of Affect (3DCLA) was administered under four conditions: after doing relaxation training, after watching an amusing video, after taking a test, and after attending a lecture. An instrument called the 3DCLA with 7 subscales was developed. The reliability and the validity of each scale were investigated. Results have shown that the subscales are highly internally consistent and factors valid.

The top clinometer is a newly developed payload tool which enables a submersible vehicle to directly measure orientation of planar geological structures (e.g. bedding planes, faults) on seafloor outcrops. It consists of a disc and central vertical bar, both graduated at 1 cm scales, and a handle. On seafloor outcrops, the disc is placed on the geological surface of interest by a manipulator, and is captured by a still camera. The orientations are determined via simple onboard graphic analyses of the images obtained and the submersible log data. Strike and dip of the surface structures are routinely calculated by a macro program within a Microsoft Excel worksheet. Theoretical and laboratory tests suggest errors of the measurements in the same order as magnetic clinometer compasses commonly used for on-land geological surveys. Camera installation angles to the submersible Shinkai 6500 were also calibrated based on on-deck tests during R/V Yokosuka YK08-05 and YK10-13 Leg2 cruises. Results of three practical measurements suggested that speed of the operation depends heavily on the time spent looking for the target surfaces and the time for communication between operators and scientists. Besides these factors, a measurement can be taken in as little as five minutes. This simple and quick method improves the quality of structural measurements for submarine geology.

Objective:The purpose of this survey was to identify the roles,responsibilities and skills of medical science liaisons(MSLs) in Japan. In addition,we compared to the prior survey results in 2011,2013 and 2015.Method:We contacted 47 pharmaceutical companies with a questionnaire survey on MSLs which included 22 items and analyzed the anonymized results using a web response system.Results:The total number of MSLs increased compared to prior surveys(ranged from 0 to 110). Many companies need MSLs with medical professional qualifications and sophisticated medical expertise. The roles and responsibilities MSLs were expected to perform included managing thought leaders(TL)and/or key opinion leaders (KOL)and implementing medical strategies. On the other hand, issues reported included management of MSLs and cooperation with other stakeholders in the company,and a still low level of recognition of MSLs.Conclusion:The roles of MSL are diverse,and while their activities and status are becoming established they are not yet unified across companies. It is recommended that at the earliest opportunity the roles,responsibilities and key performance indicators(KPI)of MSLs are defined,and educational programs established so that they can act as effective liaisons with medical professionals.

Neonicotinoids are the only major new class of insecticides introduced in the past three decades. They act as selective agonists at the insect nicotinic acetylcholine receptor and are therefore highly toxic towards important insect pests but relatively safe to mammals. However, the excellent selective toxicity may not be evident with their metabolites or analogous compounds. The aim of this paper is to consider the effects of neonicotinoids and derivatives in mammalian cells and mice involving up-regulation of nicotinic receptor levels and activation of the intracellular signal integration cascade elicited by chronic or sustained exposure and analgesic and toxic effects in mice.
