In 1945, the United Nations Charter was signed in San Francisco after the United Nations Conference on International Organization. In the first chapter, “Purposes and Principles,” four purposes are stated, and one of them refers to racial equality: to “achieve international co-operation . . . in promoting and encouraging respect for human rights and for fundamental freedoms for all without distinction as to race, sex, language or religion.” Considering that President Wilson, 25 years ago, refused to accept the Japanese proposal for inserting a short clause about the abolition of racial discrimination into the Covenant of the League of Nations, the United Nation Charter accepted to bear a responsibility this time. Based on official documents of the time and secondary works, this article examines the origins and the driving forces both within and outside of the Roosevelt Administration that led to the inclusion of the clause. It draws attention to the substantial contribution of a private organization such as the Commission to Study the Organization of Peace and its close relationship with government officials. It concludes that the reference to racial equality was not the result of single organization or country, but of various countries and people who were concerned about the nature of the new international organization.

Evidence Based Practice (EBP) appeared at the beginning of 1990s.Social workers have been required “Explanatory Power of Language for Practice” lately. Social work is a professional field. It means that its practice has to be based on theories and research; moreover, social workers have to explain the reasons why they provide the services for the clients. In order to make people aware of social work as a professional, social work practices need to be evidence based. This article has four purposes. First of all, the principles of EBP and introducing EBP to social work field are summarized. Second, significances of EBP in clinical social work are examined through a case of elder abuse. Also, this article clarifies what evidences are and considers how EBP should be applied in clinical social work field. Finally, how EBP works in social work field is examined; moreover, further issues of EBP in social work field are considered.

近年,高等教育機関では,PBL(Project-Based Learning:課題解決型学習)と呼ばれる教育手法が広まってきている.PBLは,「課題を発見し,解決する」というプロセスにより学ばせる教育手法であり,問題発見力,問題解決力などの向上に高い効果があると言われている.愛媛大学工学部情報工学科の分散処理システム研究室および愛媛大学大学院理工学研究科電子情報工学専攻ICTスペシャリスト育成コースでは,社会に実際に存在する課題を題材としたPBL(実課題PBL)を実施している.本稿では,分散処理システム研究室およびICTスペシャリスト育成コースで実施している実課題PBLの実例と,実施に際しての工夫点などを紹介する.

教育の情報化が加速する中で,学習管理システム(Learning Management System : LMS)等の教育プラットフォームに蓄積される膨大な学習ログを活用し,学習者の行動履歴を解析するラーニングアナリティクス(Learning Analytics:LA)と呼ばれる研究が世界中で活発になってきている.今後さらに学習ログの価値が見直され,LA が当たり前になったとき,教育改善のための試行錯誤は,個人的な経験則だけでなく客観的なデータと共通の理解に基づいたものになることが期待されるだろう.

(1)縦断解析:インスリン抵抗性と冠動脈硬化進展、歩数とメタボリックシンドローム発症との関連、(2)国際共同研究:米国住民コホートMESAとの冠動脈硬化日米比較、ピッツバーグ大学等との共同研究にてオメガ‐3脂肪酸と冠動脈石灰化発症との関連等、(3)遺伝子、メタボローム、新興バイオマーカー:アルデヒドデヒドロゲナーゼ遺伝子多型とLDL-コレステロール、リポプロティン関連ホスフォリパーゼA2とその遺伝子多型と潜在性動脈硬化との関連等を明らかにした。尿中メタボローム・酸化変性LDLも測定済みであり引き続き有用なバイオマーカーの探求を続ける。

We have previously shown that chronic donepezil treatment induces nicotinic acetylcholine receptor up-regulation and enhances the sensitivity of the neurons to the neuroprotective effect of donepezil. Further analyses revealed that the nicotinic receptor is involved in this enhancement. In this study, we examined whether nicotinic receptor stimulation is sufficient to make neurons more sensitive to donepezil. We treated primary cultures of rat cortical neurons with nicotine and confirmed that chronic nicotine treatment induced nicotinic receptor up-regulation and made the neurons more sensitive to the neuroprotective effects of donepezil. Analyses with receptor antagonists and kinase inhibitors revealed that the effects of chronic nicotine treatment are mediated by nicotinic receptors and their downstream effectors including phosphatidylinositol 3-kinase. In contrast to chronic donepezil treatment that enhanced the level of nicotine-induced Ca2+ influx, chronic nicotine treatment did not significantly alter the level of Ca2+ influx.

本学の医学部5年生を対象に「整形外科はメジャー(大診療科)だと思うか,マイナー(小診療科)だと思うか」について無記名でアンケートをとった.回答のあった89名中,メジャーと答えた者が48名(54%),マイナーと答えた者が41名(46%)であった.メジャー,マイナーがほぼ相半ばしたが,整形外科が医師国家試験や卒後臨床研修の必修科目に入っていない割には,メジャーととらえている学生が意外に多いと言えるのかもしれない. 医学教育の現場では,整形外科はマイナー科として扱われることが多い.本学を例にすると,整形外科の必修臨床実習期間はマイナー科とみなされ1週間のみである.内科などのメジャー科は2週間の配分だが,内科は第1から第4まであるので計8週間となり整形外科との差は歴然である(現在,不均衡の是正を交渉中である).厚生労働省により平成16年度(2004年)から始められる,医師免許取得後の臨床研修制度においても,必修科目は内科,外科,救急部門(麻酔科を含む),小児科,産婦人科,精神科,地域・保健医療であり,その中に整形外科は入っていない.

In Japan, the proportion of elderly people had reached up to 23% in 2009. The number of elderly people in long-term nursing homes or nursing facilities will increase in the next decade. By 2025, the majority of the elderly people would have developed cancer, stroke, cardio-vascular diseases, and dementia. Almost all of them would be treated with prescribed drugs. They would also have dysphagia and have difficulties in remembering their medications in the long term. Therefore, for the benefit of such a community, the work force, especially in the field of drug distribution, will need to be increased to prevent the incidence of patients who forget to take their medications. Further, the educational curriculum for pharmaceutical students has been changed to a new version, and some Japanese pharmacy shops have been switching over to “Pharmacy 3.0,” which is the next generation model. In this pharmacy, the pharmacists will play an additional new role; they will not only dispense drugs but also support home recuperation leveraging some vital signs and physical assessments. In my opinion, this novel scheme of medical service developed with pharmacists playing this new role may be a boon to the patient/elderly community in Japan who are facing the collapse of healthcare systems. In conclusion, Collaborative Drug Therapy Management (CDTM) in the practice of the pharmacists is essential for increasing the efficiency of the Japanese healthcare systems.

【目的】放射線と化学発がん物質が複合曝露された時に発生するがんにおいて、がん関連遺伝子の変異の蓄積がどのように変化するかについての情報は未だ少ない。そこで、放射線とエチルニトロソウレア(ENU)の複合曝露により胸腺リンパ腫(TL)を誘発し、その<i>Kras</i>の点突然変異の頻度とスペクトラムが単独曝露とどのように異なるか、また、その変化が<i>Ikaros</i>の点突然変異とどのように異なるか比較した。<br>【材料と方法】B6C3F1マウスにX線0.8~1.0Gyを1週間間隔で4週間全身照射、もしくは、ENUを飲料水として100~200ppmを4週間投与した。処理は1)4週齢または8週齢からX線照射、2)4週齢または8週齢からENU投与、3)4週齢からX線照射した後8週齢からENU投与(X to ENU)、4)4週齢からENU投与した後8週齢からX線照射(ENU to X)、5)4週齢からX線照射とENU投与を同時曝露(X+ENU)の条件で行った。<i>Kras</i>ならびに<i>Ikaros</i>の変異は、cDNAのダイレクトシークエンスにより調べた。<br>【結果】TLの発生頻度はX線単独またはENU単独での発生頻度と比較して、(X to ENU)群でも(X+ENU)群でも相乗的に、(ENU to X)群では亜相加的に増加した。<i>Kras</i>の点突然変異はX線単独でもENU単独でも、4週齢から処理したものに比べ、8週齢から処理したもので減少していた。<i>Ikaros</i>では週齢による点突然変異の現れる割合に変化はほとんどなかった。次に(X to ENU)群では、<i>Kras</i>と<i>Ikaros</i>のそれぞれで、点突然変異が(超)相加的に増加した。しかし、(ENU to X)群では、<i>Kras</i>の点突然変異は相加的な増加が見られたが、<i>Ikaros</i>では見られなかった。また、(X+ENU)群では、(X to ENU)群と比較して、<i>Kras</i>の点突然変異は著しく減少したのに対し、<i>Ikaros</i>は増加することがわかった。<br>これらの結果から、発がんの複合曝露効果は、曝露の順番などの曝露様式に依存し、それは、がん関連遺伝子の点突然変異の誘発頻度によって一部説明できると考えられた。

We investigated the effect of serofendic acid, a neuroprotective substance derived from fetal calf serum, on the morphological changes in cultured cortical astrocytes. Cultured astrocytes developed a stellate morphology with several processes following exposure to dibutylyl cAMP (dbcAMP), a membrane-permeable cAMP analog; 8-Br-cGMP, a membrane-permeable cGMP analog; or phorbol-12-myristate-13-acetate (PMA), a protein kinase C activator. Serofendic acid significantly accelerated the stellation induced by dbcAMP- and 8-Br-cGMP. In contrast, the PMA-induced stellation was not affected by serofendic acid. Next, we attempted to elucidate the mechanism underlying the dbcAMP-induced stellation and explore the site of action of serofendic acid. Both the stellation induced by dbcAMP and the promotional effect of serofendic acid were partially inhibited by KT5720, a specific protein kinase A (PKA) inhibitor. Furthermore, serofendic acid failed to facilitate the stellation induced by Y-27632, an inhibitor of Rho-associated kinase (ROCK). These results indicate that serofendic acid promotes dbcAMP- and 8-Br-cGMP-induced stellation and the promotional effect on dbcAMP-induced stellation is mediated at least partly by the regulation of PKA activity and not by controlling ROCK activity.

A high-luminosity striation, which rotates within a flux surface of the plasma boundary having helical structure, has been observed in the LHD plasmas by means of a fast camera. The striation appears when the plasma is shrinking due to excessive gas fueling despite the existence of neutral beam heating.
