インターネット上でのユーザ間の情報共有はCGMと呼ばれるユーザ主体の情報発信基盤を通じて文字,写真,動画など様々な形で行われている.その中でもインターネット生放送は情報を映像とともにリアルタイムに伝える手段として注目を集めている.本論文ではユーザが創造したコンテンツを保護しつつ情報を発信できるインターネット生放送システムを提案しそのプロトタイプを構築した.そして,そのプロトタイプを用いて操作性,映像の品質,コンテンツ保護の効果の3つの項目を評価するために被験者実験を実施した.操作性では83.3%の被験者から高い評価を得られた.映像の品質は画質,音質,映像と音声の同期性の3つの指標でそれぞれ70%以上の被験者から高い評価を得られた.コンテンツ保護の効果では100%の被験者に映像の視認性について効果が見られた.また,94.4%の被験者から映像の横流しに対する抑止力があるという評価を得た.

症例は52歳の女性で,4型胃癌で2005年10月に胃全摘術を施行後に術後補助療法としてS-1を投与した.2007年6月のCTで右水腎症を認め,後腹膜再発を疑いweekly paclitaxelを開始した.2クール後に水腎症は改善するも,2008年10月に右大腿の疼痛と筋の硬化が出現した.2009年3月のCTで両大腿動脈周囲に低濃度領域を認め,疼痛も両下腿へ拡大した.後腹膜再発の大腿動脈周囲への伸展とその随伴症状と診断した.両鼠径部への放射線照射後に同部位の低濃度領域は消失したが,下肢の疼痛と硬化は進行し,硬化範囲が背部へも拡がったため,同年11月にMRIを施行した.脊柱起立筋から下腿に筋変性を認めたため,変性疾患との鑑別のため筋生検を施行した.横紋筋はほとんど間質線維に置換され,その中に異型腺管をまばらに認め,胃癌筋転移と診断された.胃癌骨格筋転移の報告は少ないが,筋変性を伴った報告はなく,本症例は極めてまれな症例と思われる.

Although a great deal of effort has been made on the analysis of the cognitive side of organizational life, little is known about how the members interpret organizational change. This paper applies the attribution theory to the organizational change study. The purpose of this research is to investigate how the causal attribution of an organizational change affects an attitude toward change, and to examine the moderating effect of the organizational schema on the relationship. Using a questionnaire survey, 180 employees (113 firms) who experienced a planned organizational change in their firm were asked about their interpretation of the change. The findings were as follows: First, two types of causal attribution influenced the attitude toward change. That is, the more a person interpreted that an organizational change took place because of a member's power and not of conflicts in the firm, the more his/her attitude toward change became positive. Second, an organizational schema moderated the relationship between the causal attribution and the attitude toward change. That is, the positive relationship between the attribution to member's power and the attitude toward change was identified only among persons who had the innovation-schema for their firm. Third, the contents of an organizational change did not show strong effect on the causal attribution. Theoretical and practical implications and suggestions for future research are discussed.

Background: Transcatheter left atrial appendage (LAA) closure is an alternative therapy for stroke prevention in atrial fibrillation (AF) patients. However; real-world efficacy, safety and complications have yet to be investigated. We sought to determine the procedural outcomes and potential complications of LAA closure in routine clinical practice at a high-volume center. Methods and Results: The study group comprised 179 patients (105 males; 72.7±9.0 years) with AF undergoing LAA closure at a single center in Germany. The rate of successful implantation was 98.9% (2 patients did not undergo implantation) and the overall procedure-related complication rate was 11.2% (major: 3.3%: tamponade 2; possibility of transient ischemic attack (TIA) 1; device dislocation 3; minor: 7.8%: pericardial effusion 2; air embolization with transient ST segment elevation 3; thrombus on device/sheath 3; puncture complications 5). At 45 days; 99.4% showed successful sealing of the LAA and 94.5% discontinued oral anticoagulation (OAC). TIA occurred in 2 patients during 6-month follow-up; but no cases of stroke were reported. There were no hemorrhagic stroke or device-related deaths. Only 1 patient was hospitalized with traumatic subdural hematoma. Minor bleeding was reported in 5 patients. Conclusions: Transcatheter LAA closure in a high-volume center is safe and feasible. Life-threatening complications are rare. Discontinuation of OAC 45 days after implantation was also safe.

症例は妊娠第15週の36歳の女子で,肉眼的血尿を主訴として当科を受診した。膀胱鏡を施行したところ,膀胱左側壁に巨大な腫瘤を認めたので生検を行ない,移行上皮癌と判明した。患者は既に3子を有しているので,人工流産を施行し,泌尿器科的精査および治療を進めていつた。膀胱造影では左側半分を占める巨大た陰影欠損を認め,骨盤部動脈造影で腫瘍血管に富む腫瘍を認めた。術前に,マイクロカプセル化されたマイトマイシンCを合計40mg使用して2度にわたつて腫瘍支配動脈の化学塞栓を行ない,化学塞栓後の膀胱造影で腫瘍が中等度縮小した時点で膀胱全摘除術および回腸導管造設術を施行した。肉眼的には大小2個の腫瘍が認められ,組織学的にはgrade I, stage Aの移行上皮癌であつた。術後5ヵ月という短い経過ではあるが,転移や再発の徴候はない。妊娠に悪性腫瘍が合併することは稀れであり,殊に尿路悪性腫瘍が合併することは極めて少ない。妊娠に合併した膀胱癌の症例は欧米の文献に7例を認めるが,本邦では最初の報告ではないかと思われた。血尿が認められれば妊娠の有無にかかわらず即時にしかも適切な検索が行なわれなけれぼならないことを強調し,かつ,妊娠中の膀胱癌の治療法および妊娠と悪性腫瘍との関係について若干の考察を加えた。
