The authors investigated 8 cases of infarction in the territory of the anterior cerebral artery (5 cases in the left hemisphere, 2 in the right and 1 bilateral) as to their language functions. Serial examinations on the language function were performed clinically and by using SLTA and Syntax Test of Aphasia from the acute stage. The lesions were confirmed by CT scan and cerebral angiography.    The characteristics of the symptoms were :    1) a striking suppression of spontaneous speech (mutism) in the acute stage,    2) intact or minimal impairment of verbal and literal comprehension and repetition,    3) good recovery of language function,    4) persistance of some difficulty in speech initiation and in syntactic ability,    5) association with a generalized decrease in voluntary movements (akinesia),    6) these dysfunctions were equally demonstrated in each hemisphere lesion,    7) when the lesion in the left hemisphere extended beyond the medial frontal region (two cases), it showed persistent classical transcortical motor aphasia.    We concluded that the language disorders due to the lesion limited to the medial frontal region might be different from the classical transcortical motor aphasia because of their occurrence also in the non-dominant hemisphere lesion and good recovery.    It is suggested that these disorders are a part of generalized akinesia caused by dysfunction of each medial frontal cortex especially that associated with the supplementary motor area.

標準失語症検査 (SLTA) における単語の呼称は4割可能である一方,家族名の呼称の正答率が約1割というウェルニッケ失語の症例について報告した。本症例は,はじめ,家族名のみの呼称障害が選択的と思われたが,その他の意味カテゴリーを含む呼称検査において,カテゴリーによって段階的に異なる正答率を示した。すなわち,身体部位が 75%以上と最も高く,次いで,乗り物,果物,野菜,動物,楽器が 25~50%の間,貨幣,日本国内の名所,家族,手指,色は,25%以下の低い正答率を示した。本研究では,本症例に固有の「意味カテゴリーの階層構造」を仮定し,以上の検査結果を対応させることにより,本症例の呼称障害のメカニズムを説明することを試みた。そして,本症例の呼称障害は,特定のカテゴリーに限定して生ずるのではなく,階層構造にしたがって段階的な重症度をもって出現するのではないかと考えた。

意識障害を契機に発見されたPTHrP産生肺癌の1例を経験したので報告する.症例は76歳,男性.2007年4月下旬頃より,呼びかけに対する応答が鈍くなったと意識障害を主訴に,同年6月に通院中の近医を受診した.心臓,脳の精査を施行したが,これらに異常は認められなかった.高カルシウム血症と胸部X線検査にて異常を指摘され,当院に紹介となった.諸検査にてPTHrP産生肺癌による高カルシウム血症と診断し,同年8月左下葉切除術および縦隔リンパ節郭清(Node Dissectoin 2a-1:以下ND2a-1)を施行した.術直後に一過性の低カルシウム血症を認めたが,意識レベルや腎機能障害も改善し,術後10日目に退院となった.術後1カ月後の血清カルシウム値,PTHrPは正常化し,intactPTHは改善傾向であった.現在術後3年5カ月経過しているが,再発なく経過している.

The collision frequency of inertia particles in turbulent flows is governed by a wide range of scales of flow motion. Recent studies have shown that large-scale energetic eddies dominate the relative velocity between two colliding particles (the turbulent transport effect), whereas small-scale dissipative eddies can enhance the collision frequency significantly by inducing local non-uniform particle distribution (the accumulation effect). In this study, we have developed an integrated collision kernel model, which takes into account both the turbulence effects and can predict collision frequencies at arbitrary Reynolds numbers and particle inertia. In addition, we have implemented the developed collision kernel model into a large-eddy simulation (LES). We have performed our developed LES for particle collision growth in an isotropic evolving turbulence. A direct numerical simulation (DNS) for the same system has also been done. Comparison between our LES and DNS predictions has confirmed that our LES can predict the particle collision growth in the turbulent flow.

シンポジウム、オープンフォーラム、ワークショップを通じて、水と教育に関する専門家のコミュニティが形成された。有効的に教育するための知識と方法に関して、建設的な討論はいまも継続中である。本研究では、水に関する基礎的知識(ウォーターリテラシー)評価ツールを開発し、それは様々な地域で妥当性の検討のために試験的に導入された。その結果、水に関する知識、スキル、そして開発しやすい性質の文化的で環境的に適切な側面は社会全体に認識されたといえる。

Evidence indicates low 25-hydroxyvitamin D [(25(OH)D] concentrations in European adolescents. Identification of potential determinants is therefore essential to guide public health initiatives aiming at optimizing vitamin D status across Europe. The aim of the study was to identify potential influencing factors of 25(OH)D concentrations in European adolescents aged 12.5 to 17.5 y, participating in the multi-centre cross-sectional Healthy Lifestyle in Europe by Nutrition in Adolescence (HELENA) study. A subset of 1,006 participants (46.8% males) was drawn from the main study. Measures of body composition, biochemical markers, socioeconomic status, dietary intake, physical activity, fitness, sleep time and vitamin D genetic polymorphism (rs1544410) were assessed. Stepwise multivariate linear regression analysis was conducted stratified by gender. In males, linear regression of 25(OH)D, suggested that (1) winter season (β=−0.364; p<0.01), (2) higher latitudes (β=−0.246; p<0.01), (3) BMI z-score (β=−0.198; p<0.05) and (4) retinol concentration (β=0.171; p<0.05) independently influenced 25(OH)D concentrations. In females, (1) winter season (β=−0.370; p<0.01), (2) sleep time (β=−0.231; p<0.01), (3) supplement intake (β=0.221; p<0.05), (4) flexibility (β=0.184; p<0.05), (5) body fat % (β=0.201; p<0.05) (6), BMI z-score (β=−0.272; p<0.05), (7) higher latitudes (β=−0.219; p<0.01) and (8) handgrip strength (β=0.206; p<0.05) independently influenced 25(OH)D concentrations. Season, latitude, fitness, adiposity, sleep time and micronutrient supplementation were highly related to 25(OH)D concentrations found in European adolescents.

埼玉県立大学では、平成17年度から看護職の生涯学習の一環として、また本学の公開講座の専門職講座として8ヶ月間の認定看護師教育課程を開講している。受講生は、臨床5年以上、専門分野3年以上のキャリアをもつ看護師であるが、経験や基礎教育課程などはさまざまである。そこで、各自が主体的に自己の課題に向かって学びを深められるように、開講時よりポートフォリオを活用している。今回は、緩和ケアコースの研修期間を修了した受講生を対象に、研修修了時に実施したインタビューをもとにポートフォリオの活用についてその効果を分析した。その結果、ポートフォリオの活用は、学習意欲の他、緩和ケア認定看護師の実践能力に不可欠な対人関係能力や対象者の心情にかかわるケアに必要な能力の向上が期待できると考えられた。

1998年8月27日から31日にかけての集中豪雨により、福島県内の阿武隈川流域のモモ地帯が浸水し、甚大な被害が発生した(第1報)。このため、被災者農家を対照としたアンケート調査により、浸水害の発生した地区の被害状況を把握するとともに事後に構じた技術対策の有効性について調査を実施した、本報ではこの結果を報告する。
