2015年8月から,他院で化学療法中の患者を緩和ケアとして併診する目的で,早期からの緩和ケア外来(EPC外来)を開設.2015年8月~2016年1月に,EPC外来を受診した患者について,初診診察時間,診察の内容,入院・死亡までの期間などについて,診療録から後ろ向きに調査を行い,同時期に腫瘍内科を受診した患者と比較検討した.結果,EPC外来群19名,腫瘍内科外来群11名が,それぞれ延べ80回および117回外来受診.初診外来での診察時間中央値は各45分(10〜106),38分(23〜60)であった(p=0.17).診察の内容は,症状緩和,コーピングなどについてはEPC外来群が有意に多かった.EPC外来群では初診から60日以内死亡が5名(26%)であった.EPC外来で初診に要する時間は腫瘍内科外来と同程度であった.紹介されてくる時期が遅い患者も多く,今後の啓発と継続した実践が重要である.

「畏まりました」「恐れ入ります」の類の言葉を日本列島ではよく耳にする。拝して曲腰をして恐縮する姿を型としている作法に合わせて「恐れ」の感を言い表わす礼儀は依然として根強く育まれている。「型」である。拙論では、仮にそれを「恐怖の修辞」と呼び、観察の焦点を挨拶の言葉に限定し、その普及と徹底的な定着の所以を問う。要するに「恐怖の修辞」形成過程の復元と、その由来の歴史的考察とを五段階を経て行いたい。

We performed a 20-year numerical experiment over the period 1985 to 2004 using a high-resolution North Pacific Ocean General Circulation Model (NPOGCM) and a 20 km-resolution regional climate model (RCM20) to clarify the impact of the Kuroshio large meander (LM) on the climate around Japan. The NPOGCM reproduced the two primary quasi-stationary states, straight path (SP), and large meander (LM), although the periods during which each state prevailed differed from those indicated in the observational data. The NPOGCM result also showed that the Kuroshio LM causes a cold sea surface temperature anomaly to the south of the Pacific coast of the central Japan. Using the result as a lower boundary condition, a continuous numerical integration was performed by the RCM20. An 8-year composite analysis of the atmospheric circulations of the RCM20 simulation for the Kuroshio LM and SP showed that, in both winter and summer, substantial decreases in the upward surface turbulent heat flux, the frequency of precipitation, and the frequency of steep horizontal gradients in equivalent potential temperature over the ocean are caused by the cold sea surface temperature anomaly. Similar effects are evident over the land area of Japan, although they are less intense, at most 20-50 % of magnitude over the cold sea surface temperature anomaly area, and limited to the coastal region on the Pacific Ocean side in the central part of the country.

【目的】消化管閉塞をきたした膵がん患者に対し倫理的考察を行い, チームが共通の理解基盤のもとで治療方針を決定し, 技術的工夫を行った減圧目的の経皮内視鏡的胃瘻造設術と摂取可能な食品の検討で患者のQOLが改善したため報告する. 【症例】80歳代, 女性. 膵がん末期でbest supportive careにて自宅療養をしていたが, 十二指腸浸潤から通過障害をきたした. われわれは患者・家族の思いを確認し, 臨床倫理の4分割表を用いて倫理的考察を行ったうえで減圧PEGを施行した. 減圧PEGでは胃内容物のリークによる腹膜炎や胃瘻チューブ閉塞が問題となるが, 胃壁固定の継続や人工胃液を用いて摂取可能な食品を検討した. 形態は限られるが, 経口摂取再開と外出が可能となったことから, QOLが改善した. 【結論】倫理的考察には臨床倫理の4分割表を用いたカンファレンスが有効であり, 減圧PEGや摂取可能な食品の検討でQOL改善の可能性がある.

Private sector funding was used in the government’s economic stimulus package after thecollapse of the Bubble economy (1991-1993). As a result, outstanding local bonds increased,and differentials expanded due to erosion of borrower credibility in the market. In March 1999, Japan Rating and Investment Information Inc. reported for the first time theJapanese municipal bond rating. In November 2006, Yokohama City obtained R & I. Withthis Japan has rapidly facilitated the use of municipal bond rating. Accordingly, much previousresearch has predicted that the acquisition of municipal bond rating will increase. However,this is not the case at present. Here the municipal bond rating is examined, and the following three issues are discussed.The first is the relationship between implicit security and municipal bond rating. I show thatfor local governments ratings are not essential, because of the macro and micro revenueprotection under the Ministry of Internal Affairs. The second is that municipal bond ratingsdone for local governments do not properly reflect financial benchmarks, resulting in a lackof confidence for these ratings. The third is that investor’s trust in municipal bond rating isquestionable. The discussion clarifies the need for rating companies to disclose accurate information,and the necessity for dialogue with the parties concerned for higher evaluation from localgovernments and investors.

In recent years, India has a big inflection point for the next growth due to reasons of expansion of personal consumption by improving income level, increase in young people in India and consumption behavior and reasons such as business opportunities in rural market. In this paper, I have taken up the India consumer electronics market, which has achieved rapid growth, and theoretically and empirically examined the competitive strategies of multinational corporations, which have advanced into said market from the point of view of multinational corporation theory. I have expounded upon various strategic activities centrally including the market entry strategy conducted by LG Electronics India Pvt. Ltd., Samsung India Electronics Pvt. Ltd. and other consumer electronics makers. The localization strategies of multinational corporations include many multidimensional factors. Companies that are considering business in India or companies that want to expand their positioning maps from now on are most important to determine the positioning of the company in the long term after understanding the multidimensional concept of the Indian market
