褐色細胞腫からの急激なカテコールアミン分泌により,著明な高血圧,全身症状,標的臓器障害を呈する病態を褐色細胞腫クリーゼと称する.様々な刺激が誘因となって発症するもので,予後は不良であることから,早期の診断と適切な治療が必須である.カテコールアミン,代謝産物の過剰および画像診断による腫瘍の確認で診断する.α遮断薬フェントールアミンの静脈内投与後,点滴静注を継続し,血圧が安定したら選択的α1遮断薬の経口投与を行う.安定期には所定の準備期間を経て腫瘍の手術的摘出を行う.

Cet article vise à mesurer l’influence de Jean Wahl sur la formation de la pensée d’Emmanuel Lévinas. L’influence qu’aurait pu exercer Wahl sur Lévinas n’a pas été entièrement étudié jusqu’ici alors que c’est ce premier qui a soutenu la publication de Totalité et Infini et conduit ce dernier au milieu universitaire. Au fait, c’est Wahl qui a introduit Hegel, Kierkegaard et la philosophie angloaméricaine en France au début du 20ème siècle et en particulier sa nouvelle lecture de Hegel à partir du motif de la « conscience malheureuse » qui a eu une grande influence sur le monde académique français de l’époque. Face à la séparation entre « le fini » et « l’infini », selon Wahl, Hegel affirme que la « souffrance » comme « conscience malheureuse » forme la force motrice du développement dialectique de la conscience de soi. Par rapport à cette interprétation wahlienne, Lévinas lui-même considère la « séparation » comme relation entre « le fini » et « Infini » et la définit comme notion central de Totalité et Infini, bien qu’il critique à plusieurs reprises la dialectique hégélienne dans son oeuvre. Toutefois, curieux que cela semble-t-il, dans son oeuvre Lévinas garde le silence total sur cette interprétation wahlienne malgré l’affinité de leur pensée. Nous devrions donc creuser la cause de ce silence: Levinas n’aurait-il pas pris en compte de l’interprétation wahlienne ? Pour aborder cette question, nous nous focalisons sur l’analyse de la notion de la « séparation » dans Totalité et Infini par comparaison avec la « séparation » hégélienne révélée par Wahl dans Le Malheur de la conscience dans la philosophie de Hegel. Après la comparaison de ces notions, nous tenterons de réévaluer l’impact de la lecture wahlienne de Hegel sur la genèse de Totalité et Infini, qui n’a jamais été mis en lumière jusqu’à présent.

日本の伝統芸能猿回し(猿舞師)では、サルは調教師であるヒトの指示を聞き、観衆の面前で様々な芸をすることができる。しかしサルは芸が初めからできるわけではない。では、「芸ができるようになる」とはどのようなプロセスなのだろうか。エリコ(第一著者)は調教師として茨城県の動物レジャー施設、東筑波ユートピアにおいて、餌を報酬としたオペラント条件付けによる芸の調教を行っている。アカネと名付けられたニホンザル(4歳♀)に、今までやったことのない芸「ケーレイ」を覚えさせるべく調教を行ったアカネはすでに「二足立ち」、「手を出すと前肢をのせる」などができていた。7日間(1日20分間)の調教を行い、その全てをビデオカメラに記録した。動画解析ソフトELANを用いアカネとエリコのパフォーマンスを、ジェスチャー論の枠組みを援用し、コマ単位で分析した(坊農・高橋 2009; Kendon 2004; McNeill 2005)。ストローク長(右手がアカネの場合右背側部、エリコの場合右大腿部で静止してから額に付き静止するまでの動作時間)と、開始・終了同調(エリコのパフォーマンス開始・終了コマに対する、アカネのパフォーマンスの遅れ)の調教日ごとの平均値・標準偏差を算出した。アカネ・エリコ双方において、ストローク長の標準偏差は後半になるにつれ小さくなっていき、平均値は最初と最後でほとんど変化が見られなかった。開始・修了同調は、後半で平均値は0に近づき、標準偏差は減少した。古典的モデルによればエリコは「情報」を教える側であり、芸ができるのでばらつきは最初から小さいと予想されたが、予想とは逆に、エリコもアカネの変化に合わせて自分のパフォーマンスを変化させていたことが示唆される。調教とはサルとヒトの双方が状況と互いに他のパフォーマンスに応じて自分のパフォーマンスを調整し同調させてゆくプロセスのことであり、芸を完成させていく異種間相互行為であると考えられる。

本稿では,近代民主主義における直接行動の意義を確認する.近代の代議制民主主義では「民主主義イコール選挙」のイメージが強いが,デモや集会などの直接行動は議会に対する圧力として重要である.2010 年代日本の直接行動では「普通の人たち」の参加が強調されたが,1960 年代の直接行動を見ると大衆のラディカリズムが強烈だった.既存の秩序を打破するラディカリズムが宿る直接行動には,代議制民主主義に緊張感を与える役割が期待される.

摂食嚥下障害診療は医師(主治医・リハビリテーション科医・耳鼻咽喉科医),歯科医師,言語聴覚士,理学療法士,作業療法士,看護師,薬剤師,その他多くの医療職が関与し円滑なチーム医療を行うことで初めて成り立つ.チームの構築には「テクニカルスキル」「ノンテクニカルスキル」の両面からのアプローチが求められる.2020年度の診療報酬改定では,摂食機能療法において,摂食嚥下機能に障害のある者に対する多職種チームによる介入を評価する「摂食嚥下支援加算」が新設されたことから,各施設において摂食嚥下障害診療のチーム医療への積極的な参加が望まれる.

植物食の鳥脚類恐竜の頭骨には,Pleurokinesisという上顎の側方運動があることが形態学的観察から提唱されている.このPleurokinesisを生み出す頭骨内の関節構造について有限要素法で応力解析を行い,Pleurokinesisが頭骨と歯列に及ぼす影響を力学的な観点から考察した.解析の結果,可動性のある関節によって,咀嚼による負荷が頭頂部に分散することが明らかとなった.このように恐竜などの古脊椎動物学においても力学的観点からの解析とその解釈により,これまで以上に彼らの生態が解明されることが期待できる.[本要旨はPDFには含まれない]

The first case ; Male, 26 years old, employee. Shimizu city, Shizuoka prefecture. Chief complaints : stomachache, nausea, anorexia and slight fever since a few years ago. The general condition was rather good. The results of examinations of both urine and blood were almost normal. No helminth ova were found, but occult blood was positive in the fecal examination. Gastrectomy was carried out under the diagnosis of gastric ulcer. A cherry-sized and localized tumor was found at Pylorus of the stomach. Eosinophilic abscess surrounded with eosinophilic granulation in submucosa of the stomach, and two or three transverse sections of the parasite in the center of this abscess were observed histophathologically. The worm sections were examined morphologically. From the morphological characters of the cuticular spines, intestine and the other organs of this parasite, it was identified as the larva of 4-5 mm in length of Gnathostoma spinigerum Owen, 1836. The second case ; Male, 31 years old, employee. Kanazawa city, Ishikawa prefecture. The occult blood of gastric juice and feees were strong positive, but the results of examinetions of urine and pepipheral blood were normal and no helminth ova were found in feces. Gastrectomy was carried out under the diagnosis of gastric ulcer, like as the first case. A grapesized tumor was found in the ventral site of the greater curvature of the stomach. Histopathological finding was eosinophilic granulation with two transverse sections of the worm. Morphological observations of the cuticule, oesophagus, lateral lines, intestine on the transverse sections of the worm were carried out, and the worm was identified as the immature worm of 30〜35 mm in length of Ascaris lumbricoides.
