南海トラフ巨大地震が発生すると,津波の影響を受ける沿岸部のすべての人は,すみやかに避難しなければならない。強い余震が頻発する可能性を考えると,避難を強いられる期間は数日に及ぶことも視野に入れておく必要がある。津波浸水域外に避難することが最善の避難方法であるが,高台がない,避難経路が閉塞されている,体が不自由である場合などは,津波浸水域外への避難が困難な場合も考えられる。沿岸部において津波からの避難の可能性を広げるためには,避難場所の選択肢をいくつか設けておく必要がある。本研究では,まず,津波避難に向けた準備の現状を調査する。次に津波からの避難の可能性を広げるため,津波避難ビルを増やすための住民主体のワークショップを提案する。

高齢者における口腔乾燥は、自浄作用の低下や粘膜の潤滑作用消失のために様々な悪影響を及ぼす。そのため口腔ケアでは、口腔内の保湿がきわめて重要となる。そこで今回、口腔ケアチームによる、口腔ケアラウンドと口腔乾燥に関する勉強会を実施し、その前後における入院患者の口腔乾燥状態を比較した。ラウンドでは乾燥患者の把握と対処方法の伝達により技術面へのフォローを行い、勉強会では口腔ケア時の対策方法、口腔乾燥用保湿・湿潤ジェルの正しい使用方法などの知識向上をはかった。その結果、舌の乾燥患者は約半数にまで減少し、それが舌苔の改善にもつながった。しかし、現在も口腔乾燥を認める患者は多く、ラウンドにおける問題点も挙がった。今後は、病棟の特色や個々の患者の態度にあわせた対処方法の検討とラウンドの実施方法の見直しが課題である。

人間は知覚経験を通して生じた現象の解釈を行い,受動的適応・能動的適応という二つの対立した戦略を動的に選択していると考えられる.我々は,この選択の実現に「予測精度の予測」が重要であると考え,それが実現可能な再帰結合神経回路モデルを提案する.提案モデルをロボットに実装し,適応行動の生成学習実験を行った結果,受動的・能動的な行動を実現する二つの神経メカニズムが同一の神経回路に自己組織化された.

The low cost improved recognition of an uncooled infrared image processing has been required urgently for the development of high sensitive uncooled sensor like a cooled sensor. In this study, the uncooled infrared image taken by YBaCuO bolometer camera was improved with applying a special low cost image processing systems. The improved image with this processing exhibited not only the edge enhancement in a large contrast area but also the suppression of heat noise in a small contrast area. It is clarified that this improved recognition image processing is easy to realize at low cost, and the demand for this image processing will be expected to grow up as the use of uncooled infrared sensor system increases.

Introductory quality engineering education was studied. There has been much dispute regarding this type of education, in which the participants must struggle to make the initial paradigm shift. Quality engineering instructors have had much difficulty in motivating their students to take up quality engineering. This study identified current problems in introductory quality engineering education. In particular, instructional materials that will efficiently show the effectiveness of quality engineering and enable students to be educated effectively were studied.

The clinical efficacy of long-term roxithromycin (RXM) treatment was examined objectively in 8 patients with chronic diffuse sclerosing mandibular osteomyelitis. RXM was administered orally at a dose of 300mg per day for 68 days to 66 months.As a result, the results of 7 out of 8 cases (87.5%) were assessed effective, and the symptoms had disappeared in 1 to 12 months. One case assessed poor had several operations and various kinds of antibiotic therapy for more than 11 years before this therapy. In 7 cases assessed effective, the improvement of osteolytic changes on X-ray was observed. However, most of the X-ray findings remained osteosclerosis or the osteosclerosis became more predominant, when this therapy was terminated. It was considered that X-ray findings would take a long time to be normalized. Therefore, the optimum duration of administration which is the time to terminate this therapy should be decided according to the improvement of symptoms along with the disappearance of osteolytic findings on X-ray. Diarrhea and stomach discomfort in 1 case, and liver dysfunction in 1 case were found, however, these side effects were slight.The mechanisms of RXM are unclear however, these results indicate that long-term RXM treatment has therapeutic possibilities for diffuse sclerosing mandibular osteomyelitis and should be used as the first choice followed by surgical treatment if necessary.

Recently the design of experiments is used to decide optimum processing conditions. However when the control factor interaction between the several control factors becomes large, the calculated accuracy using the design of experiments becomes very bad. Then everybody should check the results regarding the best and the worst conditions in the experiments. If differences between the calculated vale and the experimental value for the best and the worst conditions become large, the results using the design of experiments are never used. Therefore, in the previous research, we have developed the tool for easily finding the control factor interaction in the design of experiments. This control factor interaction between the several control factors action is large fault, an obstacle for innovation and disliked by everybody. However when the reaction between the several control factors becomes large effect which is surpassed the estimate, the control factor interaction becomes the synergistic effect which is liked by everybody, the synergistic effect finally brings large profit, excellent license and innovation. Therefore in this research, the tool for easily finding the synergistic effects between the control factors was developed and evaluated using the program in the previous research. The program was the tool for finding the control factor interaction in the design of experiments, it was improved for the easily finding the synergistic effects by using the new algorithm, and was evaluated by the several mathematical models and the experiment. It is concluded from the result that (1) the new program can clear the synergistic effects between the control factors, (2) the program also can clear the complex multiplier effects and (3) the program can clear the synergistic effects with innovative profit in the actual example.

Transluminal attenuation gradient (TAG) is expected as a noninvasive assessment of the functional significance of a stenosis, and has reported relatively high diagnostic performance. TAG measures the gradient of intraluminal radiological attenuation from the ostium at the first pass of the injected contrast agent; therefore, replacement of fluid by jet flow from a stenosis with gradually increasing contrast agent concentration should be investigated. We performed a phantom experiment and ALE fluid-structure interaction finite element simulation on pulsatile flow through a bifurcated flexible tube system with a stenosis. Experiment and simulation showed good agreement with temporal change of flow rate, pressure, and radius under 1 Hz square pulsatile flow. We varied Young modulus and rate of stenosis with 1 Hz sinusoidal input. Young modulus had little effect on the distribution of total flow, but a changed flow rate waveform and faster maximal velocity in the stenosis was observed with a smaller Young modulus. Then we simulated convection of particle tracers generated at the inlet, imitating a gradual increase in contrast agent with 80% and 95% stenosis. With 80% stenosis, axially symmetric flow resulted in reproductive tracer distributions; however, with 95% stenosis, the direction of jet flow from the stenosis and of subsequent helical flow varied every beat, suggesting this variation might lower sensitivity of TAG.

リヤプノフ安定性を基盤とする多くのニューラルネットモデルは,極小への落込みなどの問題点をもち,その動特性の限界が指摘されている.本論文ではこのようなニューラルネットモデルに対してカオス力学系を導入することにより,それらに新たな動特性を付加することを試みる.本論文で提案するカオス力学モデルは,エネルギー曲面での運動を記述する力学方程式の散逸項に周期的に変動する非線形抵抗をもつことを特徴とし,安定および不安定の位相を繰り返すことにより,状態のカオス的遍歴を実現するものである.誤差逆伝搬学習およびホップフィールド型ネットワークでの記憶想起に本力学モデルを適用した結果,これらの動的過程においてカオス的な極小遷移を確認した.更に,適当なパラメータ操作により発生するインターミテントカオスは,安定性と可塑性が共存したより柔軟な学習,構造性をもった記憶想起などの有効な動特性をネットワークに導くことが確認された.

Idiopathic scoliosis is a well-known orthopedic disease in childhood and the onset and progress are observed during adolescence. It is speculated that the progress of idiopathic scoliosis affects the maxillofacial growth and the function of the temporomandibular joint because the craniofacial skeletal is located on the top of the spinal column. Another reason may be that asymmetric craniofacial growth changes the head posture and/or spinal column. In this study, we examined the association of scoliosis with jaw deformity and temporomandibular joint disorders.<br>The subjects were 44 patients who underwent orthognathic surgery for treatment of a jaw deformity at Hiroshima University Hospital. We measured the Cobb angle, an indicator of scoliosis, and the curve of the spinal column in vertebrae thoracicae on the chest radiograph. Moreover, the lateral shift of points A and B, cant of the occlusal plane and ANB angle were measured on the postero-anterior (PA) plane and lateral cephalograms. Association between the curve of the spinal column and maxillofacial morphology was examined by correlation analysis. The degree of scoliosis was examined in association with the pathological status of temporomandibular joint.<br>The prevalence of subjects diagnosed with scoliosis was 22.7%. Among the scoliosis patients, 70% were female. In all subjects with a jaw deformity, a significantly positive correlation was not found between the following variables and the Cobb angle: ANB angle, lateral shift of points A and B, cant of the occlusal plane. Moreover, no significant association between the curve of spinal column in the vertebrae thoracicae and previous variables was revealed. In addition, there was no relationship between the degree of the scoliosis and progress of the temporomandibular joint disorder.<br>These findings suggested that the progress of scoliosis exerts no substantial influence on the growth of the maxillofacial skeleton and the progress of temporomandibular joint disorders.

Chronic paranasal sinusitis encompasses a variety of pathologies, and eosinophil infiltration of nasal polyps is characteristic of chronic sinusitis in which allergy is a strong etiologic factor (allergic paranasal sinusitis). Suplatast tosilate is an anti-allergy agent that suppresses production of IgE antibody and inhibits tissue infiltration by eosinophils. In the present study we assessed the efficacy of suplatast tosilate immunohistochemically in surgical specimens of nasal polyps before and after drug therapy to evaluate its efficacy in allergic sinusitis.The subjects were 8 patients with allergic sinusitis associated with nasal polyps. These patients were treated with suplatast tosilate for about three months. Nasal polyps were collected before and after treatment, and 5-μm frozen serial sections were prepared and stained with HE and by immunochemical methods. The number of eosinophils and eosinophil activation were assessed by using EG2 antibody, which recognizes secretory type eosinophilic cationic protein (ECP) alone, as the primary antibody, to investigate eosinophil activation.Infiltration by numerous eosinophils, predominantly in the superficial layer of the lamina propria, was observed in the nasal polyps of allergic rhinitis patients. Significant decreases in the number of eosinophils and the number of EG2-positive cells were observed in the nasal polyps after treatment with suplatast tosilate, and a significant decrease was also observed in the ratio of the number of EG2-positive cells to the number of eosinophils. Thus, suplatast tosilate was shown to have eosinophil tissue infiltration suppressing activity in human nasal polyps, and to also possess inhibitory activity against eosinophil degranulation. Based on these findings, suplatast tosilate suppresses allergic inflammation, which is predominantly manifested by eosinophil infiltration, and was found to be effective in the treatment of allergic paranasal sinusitis.

Earthquake prediction by the MT-system was proposed by Genichi Taguchi in 1995. Studies started in the Yano laboratory at Tokyo Denki University in 2004 demonstrated that earthquake prediction was possible to some extent, but there was considerable prediction error. A new study was started in line with new developments in the MT-system. Data collection was started in Tsukuba district. This paper gives a periodic analysis of seismic waveforms in the Tsukuba district and obtains the distances of ground vibrations by using the RT method, with the variance of the period as an attribute. The possibility of prediction by a further two-tail T method is shown, based on the variation of the distance over time. The estimated S/N ratio was -0.176 db and the sensitivity was 0.016 db. The prediction error was ±10% at a vibration velocity of 20μm/s.

中国人の顔面行動を研究する手始めとしてわれわれは彼らの典型的な表情を集めて分析する作業を進めている。北京在住の演劇学生が様々な感情表出場面で演じたアドリブ演技を撮影し、どのような場面でどのような表情が表出されるかを調べてみた。一人の男子学生のアドリブ演技をFACSで表情分析したところ、7種類の明瞭な基本表情が認められた一方、付随するジェスチャーや感情生起因に中国的と思われる特徴が見られた。討議では中国人の表情研究において様々な条件で撮影した表情の動画像とその時系列分析結果、そしてそれを刺激に用いた評価実験の結果を含む統合データベースを構築することの重要性を論じた。

症例は52歳, 女性. 急に咳嗽, 高熱, 喘鳴, 呼吸困難が出現. ペニシリンやセフェム系抗生剤に反応なく, 緊急入院となった. 室内空気下でPaO249.8Torr. 胸部X線では小粒状影, 胸部CTではびまん性に小葉中心性の微少結節と気管支壁の肥厚を認めた. マイコプラズマによる急性細気管支炎を疑い, ミノサイクリンを開始後, 胸部CT上の小結節は消失した. その後血清学的にマイコプラズマ感染症と診断できたが, 低酸素血症が遷延し, 閉塞性換気障害の存在 (1秒率62.8%), 換気血流シンチの異常などより, 閉塞性細気管支炎への進展を疑った. TBLBや気管支造影では確診は得られなかったが, メチルプレドニゾロン1g 3日間, その後プレドニゾロン40mg/日投与により低酸素血症, 肺機能, 換気血流シンチは改善した. 閉塞性細気管支炎への移行が疑われ, 早期のステロイド投与が有効であったマイコプラズマ急性細気管支炎の1例を報告した.
