概要. 一本のねじれのない紙の閉じた輪を折って,正四面体の形にすることができる.そのような折り紙展開図は,既約分数p/qで番号づけされる.また,そのような紙の輪を変形させて切ることにより,正四面体を凸多角形に切り開く展開図が系統的に得られる.

The double exponential formula, or the DE formula, is a high-precision integration formula using a change of variables called a DE transformation. However, it has a disadvantage that it is sensitive to singularities of an integrand near the real axis, and Slevinsky and Olver (SIAM J. Sci. Comput., 2015) attempted to improve it by constructing conformal maps based on the locations of singularities. In this letter, we construct a new transformation formula based on their ideas. This can be regarded as a generalization of the DE transformations. We confirm the effectiveness of this by a numerical experiment.

Background:We aimed to investigate the efficacy and safety of landiolol in Japanese patients with recurrent hemodynamically unstable ventricular tachycardia or recurrent ventricular fibrillation (recurrent VT/VF).Methods and Results:This was an open-label, uncontrolled, multicenter study. Patients with hemodynamically unstable VT or VF 24 h prior to providing informed consent, and who were refractory to class III antiarrhythmic drugs, were enrolled. Landiolol was started at a dose of 1 μg/kg/min, after VT/VF was suppressed with electrical defibrillation. Landiolol was titrated up to 10 μg/kg/min in 1 h and adjusted between 1 and 40 μg/kg/min for the efficacy assessment (1–49 h). The primary efficacy endpoint was the proportion of patients free from recurrent VT/VF. Secondary efficacy endpoints included the number of recurrent VT/VF events and the survival rate 30 days after the start of landiolol treatment. Adverse events (AEs) were assessed for safety; 27 and 29 patients were analyzed for efficacy and safety, respectively. The proportion of patients free from recurrent VT/VF was 77.8% (95% CI 57.1–89.3). The mean (±standard deviation) number of recurrent VT/VF events was 9.3±7.9. The survival rate was 96.3%. The overall incidence of AEs and of serious AEs was 72.4% and 6.9%, respectively.Conclusions:Landiolol may be useful for Japanese patients with recurrent VT/VF who do not respond to class III antiarrhythmic drugs.

This study aimed to clarify the influence of dietary habits and the daily intake of fermented milk product containing Bifidobacterium breve strain Yakult, 1.0 × 1010 cfu/100 mL/day on bowel habits in healthy young Japanese women. To clarify the interaction of a fermented milk product containing Bifidobacterium breve strain Yakult, 1.0 × 1010 cfu/100 mL/day, and food intake on bowel habits in healthy young Japanese women. A total of 150 female university students participated in this study. They recorded the amount of food intake using a food-frequency questionnaire (FFQ) during the pre-experiment period and bowel habits in a diary during the study. After the exclusion of 30 subjects who defecated every day, 120 subjects were randomly divided two groups.Pre experiment they recorded the amount of food intake using food-frequency questionnaire (FFQ) and bowel habits. After excluded 30 subjects who defecated every day, we divided two groups. We conducted an open cross-over ,study, which was composed of a non-intake period (4 weeks), a washout period (2 weeks), and an intake period (4 weeks). Sixty-two subjects, who completed the study and fully recorded in a diary, were included in the analysis. We conducted an open cross-over study. The study period was 10 weeks including a non-intake period, washout period, and an intake period. During the intake period the defecation frequency was increased from 72.2 ± 18.6 to 77.7 ± 18.8% (p < 0.01). The Bristol Stool Scale was increased from 3.2 ± 0.8 to 3.4 ± 0.9(p < 0.01). As assessed by a 5-point Likert scale, the average scores of straining and feeling of incomplete evacuation was decreased from 2.0 ± 0.8 to 1.7 ± 0.7 (p < 0.01). As assessed by a visual analog 10-point scale, the average scores of constipation awareness was decreased from 4.1 ± 2.0 to 3.3 ± 1.9 (p < 0.01). In contrast, tThe amount of calorie intake was remarkably smaller than that in recommended value (1532 ± 314 v.s.1950 kcal). Especially the consumption of potato, beans, vegetable, and fruits were smaller than recommended value. The defecation frequency during the intake period was positively correlated with each amount of calorie intake and carbohydrate intake(each p < 0.05)but not with the amount of dietary fiber intake. These findings suggest that a daily intake of fermented milk containing Bifidobacterium and an adequate food intake might provide benefits improving the bowel habit in healthy young Japanese women.

外科的顎矯正治療後に, 咬合の異常感の他, 姿勢のねじれ, 全身倦怠など多彩な全身的不定愁訴を呈していた39歳, 女性の顎関節症患者に, SNRIであるmilnacipranによる治療を行った. SNRIを6ヶ月かけて30mg/dayから120mg/dayまで増量したところ, 咬合の異常感, 広範囲の疼痛や随伴症状の睡眠障害, 慢性疲労, 全身のこわばり, 痺れ感, 抑うつ気分などが, 経時的および用量依存的に改善していった. 本症例は線維筋痛症の診断基準も満たしており, 両者の随伴症状の重複や抗うつ薬への反応の共通性から, このタイプの顎関節症の背景には線維筋痛症と類似した中枢性の病態生理学的機序が存在するのではないかと考えられた.

片側顔面痙攣に対して大承気湯加茵蔯蒿が奏効した症例を経験したので報告する。症例は57歳女性。2年前から,左眼瞼攣縮や左下顎など顔面の痙攣が,緊張時に発作的に起こるようになった。脂肪肝,喘息の治療を目的として,X 年2月に当研究所を受診した。臍へ向かって堅く膨隆している腹を目標に,大承気湯を処方した。服用開始後,喘息発作は消失し,3ヵ月後には左顔面痙攣が減少した。7ヵ月後には肝障害に対して茵蔯蒿を加味した。人間ドックの検査結果を2年間追跡すると,本方投与後に体重が減少し,肝機能が改善していた。本症例の顔面痙攣は,左顔面の発作性不随意収縮であり,症状が片側性であったことから片側顔面痙攣と診断した。痙攣症状が改善したのは,大承気湯の筋弛緩・抗痙攣作用や抗不安作用によるものと考えた。

Contact corrosion of couples of aluminum-mild steel, aluminum-stainless steel, anodized aluminummild steel, and anodized aluminum-stainless steel was studied. Corrosion current, weight loss, polarization measurement and salt spray test were carried out on these couples in various inorganic corrosive media: 1N NaCl, 0.1N NaCl, 1N CaCl2, 0.1N CaCl2, 1N Na2SO4, satd. Ca(OH)2 (pH=12.5), 0.38N Na2CO3, 0.5N Na2CO3, 0.1N NaCl, 0.069N NH4OH, 0.09N H2SO4, 0.1N HCl.Under various experimental conditions, both aluminum and anodized aluminum were in general less noble than mild steel and stainless steel except in sodium sulfate and sulfuric acid solutions, in which mild steel was anodic against aluminum and anodized aluminum.In most of the experimental conditions, aluminum and anodized aluminum sacrificially protected mild steel and stainless steel from corrosion completely. Weight loss of aluminum was, however, much greater than that of anodized aluminum. Corrosion current by the aluminum-mild steel and aluminum-stainless steel couples in alkaline solutions was about ten to one hundred times larger than in neutral solutions, while, in the case of anodized aluminum-mild steel and anodized aluminum-stainless steel couples, except in neutral and acidic solutions containing Cl ion, only a small corrosion current was measured.

In this paper, we proposed a new particle method for numerical simulation of droplet dynamics. In the proposed method, the moving surface mesh is used to define interface between gas and liquid. The volume enclosed by the mesh represents the liquid droplet, and the outer domain is inactive gas with constant pressure. The incompressible liquid flow is calculated using a particle method, in which spatial derivatives are evaluated using an arbitrary high order accurate scheme. On the free surface, the surface stress balance equations, including surface tension and viscous stress, are adopted for the boundary conditions. Deformation of the gas-liquid interface is explicitly calculated by surface nodes that move in a Lagrangian fashion. Surface tension force is directly evaluated with high accuracy on each node utilizing the mesh shape. As numerical verification, simulations of three benchmark problems, namely circular patch test, Laplace pressure test and 2D droplet (liquid column) oscillation problems with different oscillation modes, have been carried out. The computation results were compared with the theoretical solutions, and excellent agreements were obtained. As a result, high accuracy and validity of the proposed computational method were confirmed.

This paper proposes a new switching method of control mode for a robot manipulator coming in contact with environment that has elasticity. First, the basic concept of the switching of the control mode in a robot manipulator is explained. Second, we propose a new method of switching the control mode that selects the control mode at the intersection of “Characteristic curve of position and force” and “Target value of the work composed by the desired value of position and force”. Next, the switching algorithm based on this method is obtained about a certain work coordinate with a Multi-DOF robot manipulator. In this paper, the switching method is named “A Method for Switching Control Mode Based on Position and Force Errors”. Furthermore, when a One-DOF robot manipulator driven by DC servo motor with reduction gears using an elastic joint came in contact with the object that has elasticity, the simulation and the experiment on various angle and torque controls were done. As a result, it is shown that the proposed switching method is useful and has stable and flexible characteristics.

Neuropathy is a common disease. Recent progress of neurotherapeutics enabled us to treat and cure some intractable neuropathies ; this means that neurologists are required to diagnose and pick–up “treatable neuropathy” patients properly, and to catch–up the recent progress of neuropathy treatment. In this article, I briefly present practical points of diagnosis and treatment in representative treatable neuropathies. In Guillain–Barré syndrome (GBS), first line treatments (plasmapheresis and IVIg) were established in 1980s and 1990s, respectively, and no novel therapeutic regimen has been proposed in these 25 years. However, because a substantial number of patients, especially with severe disease, showed poor recovery and residual deficits, a breakthrough therapeutic strategy has eagerly been awaited. In 2016, Japanese neurologists completed a prospective, multi–center, placebo–controlled double–blinded, randomized phase–II study to evaluate the efficacy of eculizumab in GBS and got favorable results. Future phase–III trials are expected. In chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), three first line treatments (corticosteroids, plasmapheresis, and IVIg) were already established. However, nobody knows which therapeutic regimen is most appropriate in individual patient. Recent discovery of autoantibodies in CIDP (e.g. anti–neurofascin 155) may subdivide CIDP patients, and enable us to choose best therapy in each patient. Vasculitic neuropathy needs prompt diagnosis and rapid commencement of immunotherapy. Pathological confirmation of vasculitis is most important in the diagnosis of this disorder and most of the patients show multiple mononeuropathy, but patients presenting polyneuropathy are not rare. Neuropathy due to vitamin B1 deficiency (beriberi) shows motor–dominant acute polyneuropathy and sometimes misdiagnosed as GBS.

運動時の暑熱障害事故予防のための基礎資料を得るために, 男子大学生を対象に運動場の環境条件 (WBGT) の実態と, それに伴う発汗量や飲水量及び体温との関係ならびに運動時の水分補給の有無が体温調節反応に及ぼす効果について検討した.得られた主な結果は, 次の通りである.1.4月から9月までの期間, 同一時刻に練習を行った際のWBGTは4月以降上昇し, 8月に最高値に達した後下降した.2.自由飲水時の体重減少量は4.8~5.7g/kg・hr-1の範囲にあり, 各月間に有意な差異は認められなかった.3.発汗量および水分補給量はWBGTの上昇に伴って増大し, 両者の問に高い相関関係が認められた.しかし, 自由飲水時の口内温の変化量はWBGTの上昇にもかかわらず0.52±0.08℃であり, WBGTの増加に伴う変動は認められなかった.4.運動時の口内温の上昇は水分補給の有無によって異なり, 水分補給の場合で0.62±0.30℃, 非補給では1.09±0.54℃上昇し, 両者の間に有意な差異が認められた (p<0.001) .5.水分補給の場合の発汗量は10.455±2.272g/kg・hr-1, 一方非補給のそれは8.279±1.271g/kg・hr-1であり, 両者の間に有意な差異が認められた (p<0.01) .
