Background:We recently demonstrated that the acute reconnection rate detected with adenosine provocation test (APT) was significantly lower after pulmonary vein isolation (PVI) with visually guided laser balloon ablation (VGLB) than with RF ablation (RF). We evaluated the recurrence rate of atrial arrhythmias at 12 months after VGLB vs. RF and the significance of APT results for the outcome.Methods and Results:Fifty patients with paroxysmal AF were randomized to either RF or VGLB ablation in a 1 : 1 fashion. After PVI each PV underwent an APT. All patients underwent a 3-day Holter and clinical follow-up every 3 months. Significantly less PVs reconnected during APT in the VGLB-arm (10 PV (10.8%) vs. 29 PV (30.9%); P=0.001). Significantly less patients had a recurrence of atrial arrhythmia in the VGLB-arm (3 vs. 9; P=0.047). In the VGLB-arm no recurrence was seen in those patients with a negative APT (negative predictive value (NPV)=100%). Only 3 of the 8 patients with a positive APT in the VGLB-arm had a recurrence (positive PV (PPV)=37%). Recurrences in the RF-arm were seen in 3 patients with positive APT as well as in 6 patients with negative APT (PPV=18% and NPV=33%).Conclusions:There was significantly less recurrence of atrial arrhythmias at 12 months after PVI with VGLB. A negative APT after PVI with VGBL predicted freedom from AF with a very high NPV meaning that the high acute efficiency of the VGLB persisted long term.

「某氏からデカルトへ パリ1641年7月」という差出人不明のラテン語書簡が存在する.長文であり,その内容はデカルトの「第五答弁」に対してガッサンディ主義の立場から細かな点を再反論する,という重いものとなっている.当初,デカルトはこれに答弁を付して『省察』の「反論と答弁」の最終章にしようと考えていたほどである.本稿では,その主要な論点を明らかにしつつ,全訳を試みる.

The cooling history of the Crateceous Toki granite, exposed in the eastern Sanyo Belt, Central Japan, was constructed from various geochronological data. The granite yields K-Ar biotite ages of 74±2 to 72±2 Ma (±1σ) and K-Ar hornblende ages of 75±4 to 74±4 Ma, which are concordant with a previously reported Rb-Sr whole-rock isochron age. Fission-track ages of zircon and apatite in the granite give ages 73±3 to 64±3 Ma and 40±4 to 37±4 Ma, respectively. These above thermochronological data suggests two distinctive cooling stages for the Toki granite; 1) a first rapid cooling stage in which the granitic magma was cooled to the temperature of host rock soon after intrusion at depths of 5-7 km in the upper crust, and 2) second slow cooling stage (7-9 °C/m.y.) associated with the uplift and subsequent erosion of the granite intrusion after ∼ 70 Ma.

脳脊髄液や脳間質液には,前頭蓋底部の嗅神経に沿って鼻粘膜リンパ管に入り頸部リンパ節にいたる解剖学的吸収路が存在し,脳リンパ排液(brain lymphatic drainage)という概念が成立する.脳灰白質では,脳動脈周囲の血管周囲腔(perivascular space)が機能的にリンパ管に相当し,リンパ流は血流と逆方向に流れていると考えられている.この排液機構は,Aβの蓄積などによる通過障害や動脈硬化による排液駆動力の低下の結果,機能障害を生じ,iNPH,脳小血管病,アルツハイマー病の3疾患の重複病理(comorbidity)を説明しうる機序のひとつであることが推察される.

This paper addresses the issue of design drawing in order to discuss about creativity. Purpose of designers' drawing is to generate enormous Fields of Creative Perception for themselves. Why does a design teacher requires his/her students to draw hundred sketches in one night at home work? What is achievement of the students through the hard work? The answer is that the hundred drawings become fields of their diverse inventions on the process. Design creativity is emerged out of number of expressions derived from coupling of perception/seeing and action/drawing. Beautiful original forms are only extracted from a large number of ideas and scrubbed of the extractions. Therefore ability of generating Fields of Creative Perception should be fostered on design students in their learning. Creativity is abilities of drawing enormous sketches and organizing them into creative fields as professional designers do. Discussion of this paper consists of reading perceptual experience of Rudolf Arnheim, coupling of perception and action, Fields of Creative Perception and expression then creativity and self.

Fibrodysplasia ossificans progressiva (FOP) and diffuse intrinsic pontine glioma (DIPG) are diseases that typically manifest in childhood and are associated with severely reduced life expectancy. However, there are currently no effective therapies for these diseases, which remain incurable. Activin receptor-like kinase-2 (ALK2), encoded by the ACVR1 gene, is a bone morphogenetic protein (BMP) type-I receptor subtype that plays an important physiological role in the development of bones, muscles, brain, and other organs. Constitutively active mutants of ALK2 have been identified as causative of FOP and involved in the tumorigenesis of DIPG owing to abnormal activation of BMP signaling, and therefore have emerged as promising treatment targets. Here, we describe these two diseases, along with the link to ALK2 signal transduction, and highlight potential ALK2 inhibitors that are under development to offer new hope for patients with FOP and DIPG.

1938年の内務省による小活字・ルビ禁止政策は,日本近代史の中へ位置づけてはじめて正確な理解が可能になることを示した。具体的には,(1)内務省政策はもっぱら活字の大きさを問題にしてのルビ禁止であったが,この当時,近視が国防上の不利益であるという認識,予防策の策定は他の官庁にも見られる。(2)そのうち,厚生省は同年設立の新しい官庁であるが,出自をたどると,内務省衛生局を母体とし,内務省とは密接な関連のあった官庁であり,陸軍省の構想と圧力によって設立を見た官庁であって,設立構想は,近視眼の増加を含めた「壮丁体質の悪化」による徴兵検査合格率低下への懸念に発していた。(3)厚生省の指導下に国家的組織として「視力保健連盟」が1938年9月26日に成立し,月刊誌の発行を含む全国規模の活動を展開していた。(4)同連盟と近視予防運動にとって近視は第一義的に,国防上の問題として認識されており,ルビを含む小活字の問題も,近視予防運動の立場から重要であり,近視の原因の一つと認識されていた。これは当時の行政と軍に共通した認識である。(5)内務省要綱はこのような文脈において,厚生省との連絡のもとに成立した。(6)したがって,近視予防運動の一環としての内務省要綱のルビ・小活字項目は,第一次大戦以降意識され,準備されてきた総力戦体制の整備の一環として位置づけられ,この時代の急速な戦時体制化を象徴するできごとの一つであり,軍事的意思が言語政策を併呑したところに成立した施策であった。

An earthquake swarm begun at the end of January, 2012 and the activity reached a peak in middle February, then gradually decreased by the end of April, in the Nigorikawa caldera, Hokkaido. The largest earthquake was the MJMA3.6 event which occurred on 15 February 2012 with maximum intensity 3.Temporal seismic observation with two stations carried out from 20 February to 9 April in the caldera. Detailed hypocenter distribution estimated by using both data temporal and secular observations shows two clusters, one of them is aligned along the northern caldera wall and another is located at eastern outside of caldera. Strike of alignment epicenter of the north cluster shows NE-SW direction, which is in good agreement with one of the nodal plane of focal mechanism of the largest event.
