This paper aims to contribute to the discussion of Japan's end-to-life policy by comparing the institutional framework concerning Advance Care Planning (ACP) and Advance Directive (AD) of elderly people. Upon making the comparative analysis, the paper concludes by making the following policy recommendation for Japan: (1) establishment of the death with dignity act; (2) improvement of decision-making system at the end of life; (3) development of information sharing system for advance directives.

The cascade damage and its kinetic behavior in various kinds of ceramics irradiated with fission- and fusion-neutrons have been examined through transmission electron microscopy. Fission- and fusion-neutrons introduce relatively dilute concentration of point defects around primary knock-on atoms through the displacement cascade process in low-Z ceramics. Higher concentration of point defects, which induces the contrast corresponding to the cascade damage though TEM, is induced only in covalent crystals consisting of relatively high-Z elements. The nucleation and growth process of interstitialloops is strongly affected by the crystal structure and structural vacancy.

対人評価において,被評価者の衛生マスク(以下,マスク)の着用は,印象や魅力の知覚に影響することが明らかにされている(Miyazaki & Kawahara, 2016)。本研究は,マスクを着用している人物に対する印象や顔の魅力の知覚に及ぼす影響について,マスクの色による違いに着目する。具体的には,黒もしくは白色のマスクを着用している人物に対する潜在的・顕在的な態度(例えば,良い‐悪い)及び顔の魅力を調査・測定した。その結果,潜在的・顕在的態度の両方で黒いマスクを着用している人物に対して非好意的な態度が確認された。また黒いマスクの着用者の顔の魅力は,白いマスクの着用者に比べて低い場合があった。これらの結果は,(1)印象や顔の魅力の知覚にマスクの着用だけではなく,マスクの色特有の影響が生じること,(2)慣れ親しみの高い従来のマスク(白色)に比べて,慣れ親しみの低いマスク(黒色)を着用している人物に対して非好意的に知覚されることを反映している。

本稿は、1950年代に乳児院収容児に対して行われた2 つの人体実験、名古屋市立大学医学部の特殊大腸菌感染実験および神戸医科大学医学部の乳児栄養実験を取り上げる。これらの事例について、乳児院、医学者や関連産業をめぐる背景を検討し、人体実験が問題化された経緯と文脈をたどる。産婆による乳児保護システムの解体、第二次大戦中からつづく細菌学実験の系譜や、隆興する乳児栄養産業の影響等の条件が重なった結果、一部の大学病院内の乳児院で引き取り予定のない収容児等に対する人体実験が行われた。実験の告発は、小児科医や看護師によって行われ、日本弁護士連合会や神戸地方法務局が調査を実施し、「(基本的)人権の侵害」であると非難された。また法学者によって刑法学上の責任追及の可能性も示唆された。

SARS-CoV-2 infects a variety of tissues, including the oral cavity. However, there are few reports examining the association of SARS-CoV-2 with tongue mucosal tissues with sticky tongue debris. This study investigated the presence of SARS-CoV-2 and its associated molecules by dissecting tongue tissue from autopsy specimens of 23 patients who died of COVID-19-related illness (pneumonia). Immunohistochemical staining, electron microscopy, and PCR analysis were performed on the tongue tissue specimens. The mucosal epithelium of the tongue formed a very thick keratinized with well-developed filiform papillae in all cases. ACE2 and TMPRSS2 were consistently co-expressed in all samples in the epithelium. The S-protein was strongly expressed in basal cells and the epithelial surface. S-protein-positive viral particles were detected in the tongue’s stratified squamous epithelium via an immunoelectron microscope. Based on PCR amplification of the N1 and N2 regions, the SARS-CoV-2 gene was detected on the tongue epithelium, tongue submucosa, and in tongue debris. This suggests that tongue debris, including the squamous epithelial tissue, could be a source of SARS-CoV-2 in saliva. Furthermore, removing tongue debris may decrease the amount of SARS-CoV-2 in the oral cavity.

Preservation of molluscan specimens for DNA analysis is one of the most important collection management issue in modern malacology. The merits and demerits of two simple methods of DNA preservation, storage in ultracold freezer and alcohol preservation, are compared. Preferred protocols for alcohol preservation are described.

眼窩病変に対する手術は境界領域であり, 眼科 (眼形成外科), 耳鼻咽喉科, 脳神経外科, 形成外科などがかかわる. アプローチもさまざまなものがあるが, 内視鏡は外切開が不要, 内側・後方の操作に強いなどの強みがある. 適応疾患も数多い. それぞれについて概説する. 眼窩病変は非典型的なケースも多く, それぞれの疾患においても個々の症例に対応することが求められる. 手術テクニックもさることながら, まずは手術適応, アプローチについて広く原則を知っておくことで個々の症例にも対応できるようになる.

スギ花粉症の有病率は,日本の「国民病」といえる程に高い.一般的な治療には,マスクやメガネなどを使用したスギ花粉の回避によるセルフケアと第2世代抗ヒスタミン薬や鼻噴霧用ステロイド薬を用いたメディカルケアがある.しかし,医療機関を受診せず,いわゆる健康食品を利用した「セルフメディケーション」を行っている患者も多い.その1つに,スギ花粉をカプセルに詰めたものがある.2007年2月に,そのカプセル服用後に,アナフィラキシーショックを呈した49歳,女性を経験した.この症例の特異的IgE抗体やヒスタミン遊離試験はスギ花粉に対して陽性であった.内容物に含まれる主要抗原のCryj1量は少なかったが,この製品に含まれるスギ花粉以外にアナフィラキシーショックの原因は考えられず,服用後行ったテニスによる運動誘発アナフィラキシーショックと考えるのが妥当と結論した.
