Spontaneous regression of any malignant tumor is a rare event, occurring in about 1 per 60, 000-100, 000 among the total number of cases of malignant tumor. We report a case of the spontaneous regression of the hypopharyngeal carcinoma with multiple pulmonary metastases.The patient was a 75-year-old man who complained of dysphagia. He was found to have squamous cell carcinoma of the hypopharynx with multiple pulmonary metastases. Since his airway was almost occluded by the tumor, tracheostomy was performed urgently. About two weeks later, while preparing for chemotherapy for the tumor, the primary tumor began to reduce without any treatment. Subsequently not only the primary tumor but also metastatic lesion continued to reduce spontaneously, and about four months later, they disappeared completely.It is thought that elucidation of the mechanisms of spontaneous regression of malignant tumors could possibly lead to a more effective therapy for cancer.

背景.原発性肺癌の自然退縮は非常に稀な現象である.症例.55歳,男性.右頚部腫瘤を自覚し,前医にて頚部リンパ節生検を施行されたが壊死が強く確定診断はできなかった.右肺尖腫瘤もあり,当科にて経気管支肺生検を行ったところ,組織像は瘢痕像のみでやはり確定診断は得られなかった.その後,肺腫瘤および頚部リンパ節はともに無治療で縮小を続けたが,8か月後に別の右頚部リンパ節腫大が新たに出現し生検にて分化度の低い癌細胞を認めた.さらに右肺上葉切除術を施行した結果,肺大細胞癌と確定診断された.肺原発巣および頚部リンパ節ともにHLA class Iの強い発現とCD8リンパ球浸潤を認めた.また経過中,病状悪化時には白血球数および血清G-CSFも高値であり,腫瘍細胞も免疫組織学的にG-CSF陽性であったため,G-CSF産生腫瘍と考えた.結論.悪性腫瘍の自然退縮はいまだその機序は解明されていないが,HLA class I発現やCD8リンパ球浸潤など腫瘍免疫が深く関与していると考えられる.本症例はG-CSF産生肺大細胞癌が自然退縮を示したと推察される興味深い症例と考え報告した.

症例は82歳女性。気管支喘息,高血圧の加療中,消化器症状にて上部消化管造影を施行し,食道癌を疑われた。上部消化管内視鏡にて歯列より約30cm,2時方向に小結節扁平隆起性病変を認め,X線,内視鏡所見,生検より表在隆起型食道癌(中分化型扁平上皮癌)と診断した。内視鏡的切除など各種治療を検討したが,年齢,全身状態などにより未治療のまま経過観察となった。定期的に内視鏡を実施していたところ,約2年後の内視鏡検査において著明な退縮を認めた。悪性腫瘍の自然退縮は極めてまれな現象であり,頻度は悪性腫瘍6万例から10万例に1例とされており,とりわけ消化器癌ではまれとされている。今回,自然退縮した食道癌を内視鏡的に経過観察しえた1例を経験したので報告する。

Obesity-related renal diseases have been a worldwide issue. Effective strategy that prevents high fat-diet induced renal damage is of great significance. Resveratrol, a natural plant polyphenol, is famous for its antioxidant activity, cardioprotective effects and anticancer properties. However whether resveratrol can play a role in the treatment of renal diseases is unknown. In this study, we added resveratrol in normal glucose or high glucose medium and provide evidences that resveratrol protects against high-glucose triggered oxidative stress and cell senescence. Moreover, mice were fed with standard diet, standard diet plus resveratrol, high-fat diet or high-fat diet plus resveratrol for 3 months, and results show that resveratrol treatment prevents high-fat diet induced renal pathological damage by activating SIRT1, a key member in the mammalian sirtuin family that response to calorie restriction life-extension method. This research confirms the potential role of resveratrol in the treatment of renal diseases and may provide an effective and convenient method to mimic the beneficial effects of calorie restriction.

[目的]本研究の目的はドロップジャンプ(以下DJ)時の下肢の運動学的特徴とStretch-shortening cycle(以下SSC)を跳躍に利用する能力との関係を調べることである。[対象]対象は健常男性28名とした。[方法]20 cm DJ時における下肢関節角度の変化に加え,垂直跳び(以下VJ)とDJの跳躍高を測定した。DJ時の関節角度の変化より,着地直前と踏切動作時の関節角度,踏切動作時の足関節運動の切り換わりから膝関節運動の切り換わりまでの時間を算出した。VJよりDJ跳躍高が増加した群と減少した群に分類し,両群の違いを比較した。[結果]増加した群は,着地直前の足関節底屈角度が有意に少なかった。また,足関節の切り換わりから膝関節の切り換わりまでの時間が有意に長く,足関節が膝関節より早く切り換わるものが多かった。[結語]少ない足関節底屈角度での着地に加え,足関節を膝関節より早く伸展運動に切り換えるような踏切動作をとることでSSCを効率的に利用したDJを行うことができることが示唆された。

目的:農村部における認知症予防に向けた介護予防事業への参加者と不参加者の特徴の相違を明らかにし,不参加者における適切な支援を含めた今後の介護予防のあり方を検討することである.方法:対象はA県在住の地域高齢者における認知症予防に向けた介護予防事業対象者(特定高齢者)94名(参加者47名,不参加者47名)である.方法は質問紙調査であり,基本属性,身体的特性(認知機能,生活機能),心理的特性(健康度自己評価),社会的特性(ソーシャルサポート)等を検討した.結果:参加群は平均年齢79.5(SD=5.8)歳,不参加群は82.0(SD=6.2)歳であり,基本属性に有意な差はみられなかった.不参加群は参加群に比して,認知機能の低い者の割合が有意に高く(p=0.03),手段的自立(p=0.02),知的能動性(p<0.001),社会的役割(p=0.07)が有意に低いもしくは低い傾向があった.また不参加群は参加群に比して,手段的サポートが低く(p=0.09),サポートが乏しい傾向があった.なお参加群の事業参加の動機は,孤独感の緩和や人とのつながりが最も多く,次いで,物忘れの重大さの自覚や将来の認知症への懸念などとなっていた.結論:農村部の認知症予防に向けた介護予防事業不参加者は参加者に比較して,生活習慣や対人交流の脆弱性があり,今後は,高齢者一人ひとりの特性やニーズに応じた参加への意欲や動機づけの支援が必要である.

Ketene silyl acetals have been developed as new versatile reagents for organic synthesis. The reactions of ketene silyl acetal with acid halides, carbonyl compounds, Schiff bases, and heterocummulenes and summarized. Oxidative coupling, oxidation and Mannich reaction of these reagents, and, an application of Claisen-type rearrangement of these reagents to the total synthesis of natural products are also described.

症例は66歳女性.急性心筋梗塞に対して当院心臓外科にて冠動脈バイパス術を施行した.術後右血胸を認め,術後6日目に後側方第7肋間開胸にて血腫除去術を施行した.さらに右膿胸を認めたため31日目に開窓術(第6-9肋骨後方部分切除)を施行した.膿胸腔のガーゼ交換を連日継続中の94日目に大量の壊死肺(10×5cm)の自然脱落を認めた.その後229日目に有茎腹直筋弁充填術を形成外科医と共同で施行した.しかし,充填術後に肺瘻が遷延したため,計3回気管支塞栓術を行ったが一時的な改善しか認めなかった.275日目に開胸にて肺瘻閉鎖術を行った.腹直筋弁は形成外科的な手技が必要であるが,大網や広背筋が利用できない場合は膿胸腔充填術に対して有用である.また,膿胸開窓術後の大量の壊死肺脱落は今回検索し得た文献においては稀であると考えられた.

In recent years, an increasing emphasis on conserving biodiversity in cultivated land has led to a need for environmentally friendly and sustainable agriculture that does not depend on the excessive use of agrochemicals. However, it can be difficult to generate high crop yields without using herbicides because alternative methods of restricting weed growth are often costly and labor-intensive. Here, we designed a mesocosm to examine the weeding efficacy of a cheap, commercially available, automatic robot ball, which was originally designed for house cleaning. The weeding robot performed very well, particularly for sprouts of Monochoria vaginalis var. plantaginea, a major paddy weed found in central Japan.

症例は, 29歳女性.下腹部痛を主訴として,近医受診し,鎮痛剤および点滴等の治療を受けたが症状の改善がみられず,当院産婦人科受診となった.婦人科にて骨盤内腹膜炎疑いで保存的加療を行ったが,症状は悪化し当科紹介となった.腹部CT検査により,子宮筋腫および膀胱子宮間に存在する索状物による絞扼性イレウスを疑い緊急手術を施行した.術中所見では子宮筋腫と膀胱との間に索状物が存在し,回盲部の高度炎症および回腸末端から口側約110cmにわたる小腸壊死を認めた.子宮筋腫と膀胱との間に索状物があり,子宮膀胱間を連結し,膀胱子宮窩がヘルニア門となり絞扼性イレウスが生じたものと考えられた.この索状物は,中腎傍管が癒合し子宮広間膜を形成する際に形成されたものであろうかと推測された.

Temporary abandonment operations that led to the oil spill incident in Gulf of Mexico were analyzed focusing on temporary abandonment procedures and actual operations from the negative pressure test to blowout occurrence. Temporary abandonment plan went through remarkable transitions from original plan to actual plan. Especially cement in shoe track was the only active barrier when negative pressure test was conducted under considerable different risk factors associated with cement job. The plan was changed to efficiently perform temporary abandonment operations since there were delay of the process and cost increase. In this circumstance, the negative pressure test was done and all members on site believed that cement barrier has been successfully tested regardless of presence of strange pressure behavior due to overreliance on leader's judgment. During displacement SOBM (Synthetic Oil Base Mud) with sea water in marine riser, blowout occurred. Although there was a lot of opportunity to detect the kick during this operation, nobody could stop operation until gas came in marine riser.
