チエノピリジン系抗血小板薬であるクロピドグレルは,経皮的冠動脈インターベンションのステント留置例において,亜急性血栓性閉塞の予防に寄与する重要な薬剤である。しかし,クロピドグレルは肝臓において薬物代謝酵素の1 つであるCYP2C19 により代謝され薬効を発揮するため,その代謝活性が低い場合効果を示さないことがある。今回われわれは,クロピドグレル服用中でステント留置後に亜急性血栓性閉塞を繰り返した症例を経験した。この症例に対し,VerifyNow®P2Y12 を用いてチエノピリジン系薬剤に特異的な血小板凝集活性を測定したところ,クロピドグレルの薬効がまったく認められなかった。そこで,本患者のCYP2C19遺伝子多型解析を行ったところ,クロピドグレルの代謝能力が低いタイプであることが判明した。この結果をふまえ,クロピドグレルをチクロピジンに変更し,VerifyNow®P2Y12 により抗血小板活性を測定したところ,薬効が認められるようになった結果,合併症を出現せずに経過している。今回の知見で示したように,VerifyNow®P2Y12 とCYP2C19 の遺伝子多型解析の組み合わせは,今後のクロピドグレル適正使用に貢献できるものと考える。

[Purpose] This study aimed to assess the effect of a virtual reality exercise program accompanied by cognitive tasks on the balance and gait of stroke patients. [Subjects] Twenty stroke patients were randomly assigned to two groups 10 to an experimental group that performed a virtual reality exercise program accompanied by cognitive tasks and 10 to a control group. The control group performed a proprioceptive neuromuscular facilitation exercise program. Balance was measured with the Berg Balance Scale. Gait was assessed using the Timed Up and Go Test. The paired t-test was used to compare groups before and after the experiment. The independent t-test was conducted to assess differences in the degree of change between the two groups before and after the experiment. [Results] Within-group comparison in the experimental group showed significant differences in the Berg Balance Scale and Timed Up and Go Test. In a comparison between groups, the differences in the Berg Balance Scale and Timed Up and Go Test in the experimental group appeared significant compared with the control group. [Conclusion] The results of the experiment indicate that a virtual reality exercise program accompanied by cognitive tasks has a positive effect on the balance and gait of stroke patients.

In this paper, we focused on the fact that the ease of driving changed according to torsional rigidity of the electric power steering system for the purpose of the improvement in control performance of vehicle. First, the influence on transfer characteristics from a steering angle(steering wheel) to front wheel angle(front tires) by torsional rigidity was considered theoretically. As a result, it turned out that a front wheel angle was determined by not only a steering angle but also the influence of the dynamic state of vehicle, and it was shown that the influence of the dynamic state of vehicle can be shut by making steering torsional rigidity into high torsional rigidity. Next, the validity of a theoretical verification was confirmed through numerical simulation of steering correction at the time of a disturbance input. Finally, the lower limit value of torsional rigidity for practical use of a high torsional rigidity steering was determined.

Sarcopenia is an age-related systemic syndrome with progressive deterioration in skeletal muscle functions and loss in mass. Although the senescence-accelerated mouse P8 (SAMP8) was reported valid for muscular ageing research, there was no report on the details such as sarcopenia onset time. Therefore, this study was to investigate the change of muscle mass, structure and functions during the development of sarcopenia. Besides the average life span, muscle mass, structural and functional measurements were also studied. Male SAMP8 animals were examined at month 6, 7, 8, 9, and 10, in which the right gastrocnemius was isolated and tested for ex vivo contractile properties and fatigability while the contralateral one was harvested for muscle fiber cross-sectional area (FCSA) and typing assessments. Results showed that the peak of muscle mass appeared at month 7 and the onset of contractility decline was observed from month 8. Compared with month 8, most of the functional parameters at month 10 decreased significantly. Structurally, muscle fiber type IIA made up the largest proportion of the gastrocnemius, and the fiber size was found to peak at month 8. Based on the altered muscle mass, structural and functional outcomes, it was concluded that the onset of sarcopenia in SAMP8 animals was at month 8. SAMP8 animals at month 8 should be at pre-sarcopenia stage while month 10 at sarcopenia stage. It is confirmed that SAMP8 mouse can be used in sarcopenia research with established time line in this study.

NaClOと過酸化水素水との反応で発生する酸素量を自作のガスビュレットで測定し, 簡単に塩素系漂白剤の濃度決定を行う方法を開発した.得られた結果は次のように要約される.(1) ガスビュレットで求めた発生酸素量V∞ (mmol) と, 常法の酸化還元滴定で求めた有効塩素濃度H (%) との間によい相関が認められ, 本方法が有効塩素濃度の決定に用い得ることがわかった.(2) 本法は, 漂白に関係する酸素を測定するので, 原理がわかりやすい.経時的な測定では, 反応速度定数が求められ, 基礎から応用まで幅広く活用可能である.過酸化水素水を用いる本法は, 安全で廃液の処理も簡単であり, 学生実験にも適用できる.(3) みかけの反応速度は10~40℃の範囲では, 温度に比例して大きくなることが認められた.(4) 開封して長期間使用している市販塩素系漂白剤の発生酸素量を測定した結果, 開封後1年で当初の約70%, 2年後で約50%の濃度に低下していることが示された.(5) 保管条件による濃度低下を検討した結果, 温度, 光の影響を強く受けることがわかった.なお, 本研究の概要は, 日本家政学会第49回大会 (1997) において発表した.

ニューキノロン系合成抗菌薬のpazufloxacin mesilate (PZFX) の腎不全患者における体内動態に関する検討は十分でない。今回, のう胞腎感染症と診断された4名 (年齢65.3±9.7歳, 体重49.0±4.1kg, 男1名) の血液透析 (HD) 患者を対象として, PZFX 300mgを週3回HD後に投与し, 体内動態を評価した。初回投与時のPZFXのCmaxは10.47±2.19μg/mL, AUC(0-∞)は421±175μg・h/mLであり, 腎機能正常者に比べ消失の著しい遅延が認められた。一方で, 3回目投与時の投与終了2時間後の血漿濃度は11.12±2.30μg/mLであり, この値は健常成人に常用量を反復投与した成績と同レベルであり, 消失の遅延から予測される程度には上昇していなかった。HD除去率は4時間のHDによりみかけ上58.7±7.7%であったが, HD終了1時間後までに認められた平均13.5%のリバウンド現象を考慮すると45.2±5.9%と算出された。全例において治療期間内に血漿濃度依存的な毒性は認めなかった。以上より, HD患者におけるPZFXの消失は腎機能正常患者に比べ大幅に遅延しているものの, HD除去性が高いために血漿濃度の蓄積性は小さいと考えられ, 毎HD後の投与が合理的であると考えられた。

[Purpose] The purpose of this study was to assess the effects of flexi-bar exercises and non-flexi-bar exercises on trunk muscle activity in different postures in healthy adults. [Subjects] Twenty healthy right-hand dominant adults (10 males and 10 females) were selected for this study. None of the participants had experienced any orthopedic problems in the spine or in the upper and lower extremities in the previous six months. [Methods] The subjects were instructed to adopt three exercise postures: posture 1, quadruped; posture 2, side-bridge; and posture 3, standing. Surface electromyography of selected trunk muscles was normalized to maximum voluntary isometric contraction. [Results] The external oblique, internal oblique, and erector spinae muscle activity showed significant differences between flexi-bar exercises and non-flexi-bar exercises. [Conclusion] The results of this study suggest that flexi-bar exercises are useful in the activation of trunk muscles.

Minimally invasive transforaminal lumbar interbody fusion (TLIF) as a short fusion is widely accepted among the spine surgeons. However in the long fusion for degenerative kyphoscoliosis, corrective spinal fixation by an open method is thought to be frequently selected. Our objective is to study whether the mini-open TLIF and corrective TLIF contribute to the improvement of the spinal segmental and global alignment. We divided the patients who performed lumbar fixation surgery into three groups. Group 1 (G1) consisted of mini-open TLIF procedures without complication. Group 2 (G2) consisted of corrective TLIF without complication. Group 3 (G3) consisted of corrective TLIF with instrumentation-related complication postoperatively. In all groups, the lumbar lordosis (LL) highly correlated with developing surgical complications. LL significantly changed postoperatively in all groups, but was not corrected in the normal range in G3. There were statistically significant differences in preoperative and postoperative LL and mean difference between the pelvic incidence (PI) and LL between G3 and other groups. The most important thing not to cause the instrumentation-related failure is proper correction of the sagittal balance. In the cases with minimal sagittal imbalance with or without coronal imbalance, short fusion by mini-open TLIF or long fusion by corrective TLIF contributes to good clinical results if the lesion is short or easily correctable. However, if the patients have apparent sagittal imbalance with or without coronal imbalance, we should perform proper correction of the sagittal spinal alignment introducing various technologies.

Feline morbillivirus (FmoPV) is a new virus species and its detection is important, since correlation has been reported between FmoPV virus infection and tubulointerstitial nephritis in cats. Here, we report a real-time reverse transcription (RT)-PCR system that can detect the FmoPV L-gene sequence with more than 10-time higher sensitivity than a conventional PCR system, resulting in detection of less than 10 copies of the template DNA. The total FmoPV positive rate of urine samples from veterinary clinics and hospitals in Japan was 15.1% (25/166) using this system. This study demonstrates usefulness of the real-time RT-PCR system for detection of FmoPV for cat urine samples.

[Purpose] We performed early physiotherapy for elderly patients with pneumonia admitted to an intensive care unit (ICU), and examined the effects of this early physiotherapy on the severity of pneumonia. [Subjects and Methods] Patients for whom physiotherapy was started the day after admission to the ICU (acute phase) were assigned to the early intervention group and compared with patients in the standard intervention group. All patients were divided into three groups (Groups I, II, and III) based on the severity of pneumonia. We evaluated the ICU admission period, hospitalization period, and activities of daily living (ADL) before and after admission. [Results] With respect to the severity of pneumonia, Group II showed significant differences in the ICU admission period and rates of change in the operating range, cognitive domain, and Functional Independence Measure (FIM). Group III showed significant differences in the ICU admission period and rate of change in the cognitive domain (FIM item). The results were more favorable in the early intervention group than in the standard intervention group. [Conclusion] The ICU admission period was shorter and a reduction in the ADL level was prevented in Groups II, and III compared to Group I. This may have occurred because of the early rehabilitation.

[Purpose] To describe the functional consequences of patients with cardiac diseases and analyze associations between activity limitations and quality of life. [Subjects and Methods] Seventy subjects (mean age: 60.1±12.0 years) were being treated by Physical Medicine and Rehabilitation and Cardiology Departments were included in the study. Activity limitations and participation restrictions as perceived by the individual were measured by the Canadian Occupational Performance Measure (COPM). The Nottingham Extended Activities of Daily Living (NEADL) Scale was used to describe limitations in daily living activities. To detect the impact of activity limitations on quality of life the Nottingham Health Profile (NHP) was used. [Results] The subjects described 46 different types of problematic activities. The five most identified problems were walking (45.7%), climbing up the stairs (41.4%), bathing (30%), dressing (28.6%) and outings (27.1%). The associations between COPM performance score with all subgroups of NEADL and NHP; total, energy, physical abilities subgroups, were statistically significant. [Conclusion] Our results showed that patients with cardiac diseases reported problems with a wide range of activities, and that also quality of life may be affected by activities of daily living. COPM can be provided as a patient-focused outcome measure, and it may be a useful tool for identifying those problems.

In the field of physiology, it has generally been Considered that changes in the phase boundary potential do not affect membrane potentials. The present review shows that the phase boundary potential directly contributes to the membrane potential of a number of cells. (1) The K+-channels of mouse neuroblastoma (N-18 cells) are open at high external K+-concentration but closed at low K+-concentration including the physiological one. Under physiological conditions, the resting membrane potentials of N-18 cells stem significantly from the phase boundary potentials. Theoretical consideration shows that changes in the phase boundary potentials directly affect the membrane potential when no selective channels are open. (2) Changes in the phase boundary potential also contribute to the receptor potentials in taste cells, olfactory cells and Tetrahymena as well as to the resting potentials of these cells: adsorption of chemical stimuli on the receptor membranes of these cells induces changes in the phase boundary potential, which leads to depolarization of the cells.

透析中に体内電解質の移動を制御する方法の一つとして透析膜に荷電を付加することが有効と考えられる。本報では、血液透析膜を用いて、無機リンおよびナトリウムイオンの膜透過性、無機リンの溶質透過性. 膜表面電位(ζ電位)を測定し膜透過性と膜表面電位の関係を検討した。ζ電位は湿潤状態の中空糸透析膜1本について、流動電位測定装置を用いて中空糸内にKCi水溶液を圧入して測定した。無機リンの溶質透過係数は放射性同位元素で標識した32P-Na2HPO4 (5μCi/ml)の水溶液を中空糸透析膜1本に充填して一席時間透析を行い、中空糸内部の残存溶質濃度を測定して算出した。再生セルロース膜では、無機リンの総括物質移動係数および無機リンの溶質透過係数Pmに及ぼすζ電位の影響はない。PMMA膜では、ζ電位が無機リンおよびナトリウムイオンの総括物質移動係数および無機リンの溶質透過係数Pmに影響を及ぼしている。
