In papillary thyroid carcinoma (PTC), macroscopic extrathyroid extension (Ex) and clinical node metastasis (N) are prominent prognostic factors. Ex is divided into two grades in the UICC TNM classification: minimal and massive Ex. Massive Ex significantly affects patients’ prognoses, whereas minimal Ex has little prognostic value. N is also divided into two grades in the TNM classification: N1a and N1b, depending on the location of metastasis, with N1b graded higher than N1a. However, massive Ex and/or N-positive PTC includes patients with a wide range of biological characteristics and prognoses, depending on their degrees of Ex and N. Other clinicopathological features such as age, gender, and tumor size also influence the prognosis. In evaluations of the biological characteristics of PTC patients with Ex and/or N, we should consider the degrees and relationships of _Ex and N _ with other clinicopathological features.

In the circumference of the Ôsaka region, there are widely distributed Plio-Pleistocene deposits, the Ôsaka group. In September of 1964, nearly complete, but without some limbs and tail bones, skeletons of a fossil crocodile were excavated from the upper part of the Ôsaka group, at the ground of the Ôsaka University, Machikaneyama, Shibahara, Toyonaka City, Ôsaka Pref.. The horizon was the fresh water sandy clay bed which lay between the 8th marine clay above and the 7th marine clay below, of that group. Thus, the geological age of this horizon is estimated to be the Early Pleistocene. It was the first discovery of the crocodile fossil in Japan, and then, this long snout crocodile was named as Tomistoma machikanense Kamei et Matsumoto (n. sp.).

Subclinical hypothyroidism (SCH) has been associated with type 2 diabetes mellitus. However, it is unknown whether common complications of type 2 diabetes, such as diabetic nephropathy, are also present with SCH. Here, we investigated the association between SCH and diabetic nephropathy among Japanese patients with type 2 diabetes mellitus. In this multicenter cross-sectional study, we recruited 414 such patients who had no previous history of thyroid disease. Serum thyroid hormone levels and the urinary albumin:creatinine ratio were measured. SCH was defined as an elevated thyroid-stimulating hormone (TSH) level (>4.0 mIU/L), and diabetic nephropathy was defined as creatinine levels ≥300 mg/g. The prevalence of SCH was 8.7% (n = 36) among patients with type 2 diabetes mellitus. The SCH group had a higher prevalence of dyslipidemia (p = 0.008) and diabetic nephropathy (p = 0.014) than the euthyroid group. Multivariate analysis identified significant positive associations between diabetic nephropathy and SCH (odds ratio [OR], 3.51; 95% confidence interval [CI], 1.10-10.0; p = 0.034), hypertension (OR, 4.56; 95% CI, 1.69-14.7; p = 0.001), and smoking (OR, 3.02; 95% CI, 1.14-7.91; p = 0.026). SCH may be independently associated with diabetic nephropathy in Japanese patients with type 2 diabetes mellitus.

National Committee for Nuclear Energy Research, Japan Science Council has completed a future plan for basic nuclear energy research by university researchers. The JSC has recommended the promotion of basic research for nuclear energy based on the plan in 1983. The future plan consists of four main research fields, namely, (1) improvements of reactor safety, (2) down stream, (3) thorium fuel reactors, and (4) applications of research reactor and radioisotopes.

The purpose of this study was to clarify the effect of standing balance training on walking speed (short-term outcome) and cardiac events (long-term outcome) in elderly ischemic heart disease (IHD) patients. This was a retrospective cohort study. Ninety-two elderly (≥ 65 years) IHD patients who underwent an inpatient cardiac rehabilitation program were assigned to two groups: a balance group that received standing balance training in addition to conventional (aerobic and resistance) training and a conventional group. Standing balance was assessed by one-leg standing time and a postural stability index reflecting dynamic balance, and normal walking speed was measured at baseline and hospital discharge. Patients were followed for up to 3 years or until a cardiac event occurred. There were no significant differences in clinical characteristics between the groups. Both groups showed a significant change in normal walking speed from baseline to hospital discharge (P < 0.001, respectively), and normal walking speed was significantly higher in the balance group compared to the conventional group (P = 0.001). The postural stability index improved significantly only in the balance group (P = 0.005). Multivariable analyses using Cox proportional hazards model confirmed that standing balance training (hazard ratio [HR]: 0.408; 95% confidence interval [CI]: 0.162-1.029; P = 0.058) and fast walking speed (HR: 0.362; 95% CI: 0.137-0.957; P = 0.041) were associated with cardiac events. These findings show that standing balance training improves walking speed and reduces cardiac events, and suggests that such training can be an effective intervention for elderly IHD patients.

症例の概要:初診時54歳の男性.下顎右側臼歯部ブリッジの違和感と左側臼歯部欠損による咀嚼障害を主訴に来院した.下顎右側臼歯部ブリッジ予後不良と診断され,ブリッジを除去し,ブリッジの支台歯を抜歯したため両側遊離端欠損となり咬合支持を考慮してインプラント補綴治療を行った. 考察:インプラント支台による固定性補綴の咬合支持は,良好な咀嚼機能回復が行うことができた.最終補綴物装着から現在3年以上が経過しているが,約4カ月毎のメンテナンスと口腔衛生指導を継続していることでインプラント部と残存歯を経年的に維持できることが示された. 結論:適切なインプラントの位置と上部構造の設計,補綴物装着後のメンテナンスは,可撤性の両側遊離端義歯に比べ咀嚼機能の良好な回復と有効な咬合支持を獲得できることが示された.

We conducted an 8-year longitudinal study to investigate aging-related changes in food and nutrient intake in a cohort of elderly subjects living in an urban community, and attempted to relate food intake with vital prognosis. The first (baseline) nutrition survey was conducted in 1991 on 161 subjects (72 males and 89 females; aged 65 to 79) living in Koganei City. The second nutrition survey was conducted 8 years later in 1999. Excluding death or illness, 98 subjects (86%) were available for follow-up. Nutrition survey was conducted by a three-day dietary record method with daily home visits by dieticians. Aging-related changes in physical attributes, food intake, nutrient intake, and intake adequacy were analyzed. The relationship between nutritional intake and mortality was analyzed by Cox proportional hazard model. In the present cohort, although nutrient and food intake changes with aging, nutrient intake was higher than the recommended dietary allowances. These results show that a “diet for healthy longevity” is achieved by continuing to maintain the recommended dietary allowances despite age advancement.

大柴胡湯によるコレステロール胆石形成に対する影響を, コレステロール胆石形成モデルを用いて検討した. 雄性ゴールデンハムスターを, 普通食投与群 (N群: n=12), グルコース食投与群 (L群:n=14), 1%大柴胡湯添加グルコース食投与群 (L+D群:n=12) の3群にわけ4週間飼育した後, 胆石出現率, 胆汁脂質, 血清脂質および肝組織中の脂質の差を検討した. コレステロール胆石の出現率は, L群において71%と高率であったが, NおよびL+D群においては胆石形成を認めなかった. 胆汁中の胆石形成指数は, N群0.44±0.28, L群0.85±0.19, L+D群0.43±0.24 と大柴胡湯投与により有意に低下していた. 一方, 血清脂質は, コレステロール, 中性脂肪ともにL群においてN群, L+D群に比し有意に上昇したが, N群, L+D群間に有意の差は認めなかった. 肝組織中の脂質はN群に比べL+D群において遊離コンステロールの低下, コレステロールエステルの上昇, L群においては逆の変化を認めた. 以上より, 大柴胡湯は, 腸管でのコレスデロール吸収抑制, 肝でのコレステロール合成抑制および胆汁酸への異化亢進により, 胆汁中コレステロール飽和度を低下させ, 胆石形成に抑制的に作用することが示唆された.

我が国の水道施設は, 人々の活動を支える社会基盤として欠かすことのできない存在である. しかし, 資源やエネルギーの大量投資に伴って環境負荷を与えている一面もある. さらに, 高度経済成長期に集中的に建設された施設の老朽化が進行し, 現在では大更新期といった難局面も迎えている. ライフラインとしての水道管路を今後も維持して行くためには, 可能な限り環境負荷の小さいシステムへ移行することが重要な検討課題の1つに挙げられる. 本論文では, 管路施設とポンプ施設から構成される水道管路システムに着目し, LCA(Life Cycle Assessment)の観点から, 建設, 運転, 維持管理, 更新の各々のプロセスにおけるCO2排出量の算定を行った. 車両輸送と管路輸送の比較により, 後者(管路)による水輸送の優位性を示した. さらに, 人口密度の大小, 管路配置形状の違い, 管路口径の大小がCO2排出量にどのような影響を及はすか多面的に明らかにした.

Metagenomic screening and conventional cultivation have been used to exploit microbial lipolytic enzymes in nature. We used an indigenous forest soil (NS) and oil-fed enriched soil (OS) as microbial and genetic resources. Thirty-four strains (17 each) of lipolytic bacteria were isolated from the NS and OS microcosms. These isolates were classified into the (sub)phyla Betaproteobacteria, Gammaproteobacteria, Firmicutes, and Actinobacteria, all of which are known to be the main microbial resources of commercially available lipolytic enzymes. Seven and 39 lipolytic enzymes were successfully retrieved from the metagenomic libraries of the NS and OS microcosms, respectively. The screening efficiency (a ratio of positive lipolytic clones to the total number of environmental clones) was markedly higher in the OS microcosm than in the NS microcosm. Moreover, metagenomic clones encoding the lipolytic enzymes associated with Alphaproteobacteria, Deltaproteobacteria, Acidobacteria, Armatimonadetes, and Planctomycetes and hitherto-uncultivated microbes were recovered from these libraries. The results of the present study indicate that functional metagenomics can be effectively used to capture as yet undiscovered lipolytic enzymes that have eluded the cultivation-based method, and these combined approaches may be able to provide an overview of lipolytic organisms potentially present in nature.
