1)Sheldonらの分類でstage IIIA迄の尿膜管癌の初回手術として,臍・尿膜管全摘除術を加えた膀胱部分切除術にて長期の生存,膀胱温存が得られる可能性が示唆された.2)CEAは患者血清4例中2例,尿膜管癌組織5例中5例で陽性であり,尿膜管癌の腫瘍マーカーとなる可能性が示唆された.3)p53は尿膜管癌組織5例中4例で陽性であったが,stageや予後との相関は認められなかった

A 49-year-old female presented complaining of gross hematuria. Cystoscopy and magnetic resonance imaging revealed a papillary tumor on the bladder dome. At biopsy pathology the tumor was diagnosed as adenocarcinoma. We diagnosed the tumor as urachal adenocarcinoma and performed partial cystectomy of bladder dome with en-bloc resection of the urachal ligament up to the umbilicus. In surgical pathology, the tumor had invaded to the fat tissue around the urachal ligament with metastasis to the lymph node. Therefore the tumor was diagnosed as a stage IVA (Sheldon’scategory) urachal adenocarcinoma. After surgery, 6cycles of chemotherapy with TS-1 and cisplatin (CDDP) were performed. There has been no relapse 5years after surgery. This is the first report of successful adjuvant chemotherapy with TS-1/CDDP for advanced urachal adenocarcinoma.

A 39-year-old man visited our clinic with gross hematuria. Cystoscopy revealed a papillary tumor at the urinarybladder dome. Abdominal magnetic resonance imaging (MRI) and computed tomography(CT) demonstrated a tumor extending from the umbilicus to the bladder dome. Transurethral resection of bladder tumor (TUR-Bt) was performed and histopathological findings revealed adenocarcinoma. Chest CT and examination of the upper gastrointestinal did not reveal anyabnormal findings. The tumor was diagnosed as stage IIIA urachal carcinoma, and en bloc segmental resection was performed. About 10 months later, chest CT demonstrated multiple lung metastases. After two courses of combination chemotherapywith methotrexate (MTX), 5-fluorouracil (5-FU), epirubicin (epiADM), and cisplatin (CDDP), the multiple lung metastases completelydisappeared. The patient has survived 23 months to date with no evidence of disease and is receiving adjuvant chemotherapywith tegafur uracil.

In overseas universities many instructors have begun to use a classroom device commonly called a clicker, which allows students to respond to questions with small keypads. Students answer multiple choice questions projected on a classroom screen by pressing the keys of the clicker. It is reported that clickers increase the interaction with students and improve their learning, but few Japanese instructors use them now. I think we would benefit from the use of clickers but they are expensive. Therefore I made an inexpensive clicker system for testing in the classroom.

This article explores the history and politics of American-funded food demonstration buses (“kitchen cars”) in postwar Japan. Their express mission was to transform the Japanese national diet. I make two primary arguments. First, at least in the short to medium term, the kitchen cars were a win-win for both the United States and Japan. On the one hand, Japan benefited because the kitchen cars taught Japanese women how to cook cheap, nutritious, mostly easy dishes to improve the health of their families and the nation. On the other hand, these menus were planned specifically to increase consumption of American agricultural products, especially wheat, soy, and corn. For US agricultural and political interests, in addition to supporting the economic recovery and political stability of a Cold War ally, the kitchen cars—along with the school lunch program—were instrumental in teaching Japan to accept and consume American produce. My second argument concerns the reasons for the kitchen cars’ success. I identify the following two factors: staffing by mostly female professional nutritionists, who combined authority with approachability for the kitchen cars’ main audiences of middle-aged, married women; and the kitchen cars’ mobility, which allowed them to reach even remote villages and hamlets.

A new Early Triassic ichthyosaurus, Utatsusaurus hataii gen. et sp. nov., is described. The materials were collected from the upper part of the Osawa Formation on the coast at Tatezaki, the Kitakami Massif, Northeast Japan which is correlative of the Prohungaritan-Columbitan ammonoid stages of Spath (1930, 1934). The present form is of the most primitive type of Ichthyosauria from the view point of specialization of the pectoral limb.
