Historical records of large slope failures in Japan and foreign countries were collected andanalyzed to investigate occurrence frequency of the large slope failures and some dynamical andgeometrical features of debris deposits of the slope failures.It is shown that large slope failures have occurred in Japan much more frequently than inWestern countries. The relationship between the volume of slided debris and the equivalentcoefficient of friction for the debris motion is common to Japan and Western countries. Thefeatures of deposits spreading of slided debris showed a similar relationship between deposit areaand volume, but different shape over a wide range of debris volume with various types of slopefailures.

23歳のステロイド緑内障の症例に,急激な眼圧上昇と弱主経線屈折力にして-5.5Dの片眼性の急性近視が生じた。水晶体厚の有意な増大と浅前房及び硝子体長の延長には,ピロカルピン点眼,D—マンニトールおよびメタゾラミド投与の影響が考えられた。眼圧上昇の著明な右眼に,角膜曲率半径の減少と眼軸長・硝子体長の延長があった。右眼に対する線維柱帯切除術とD—マンニトールおよびメタゾラミドの中止後,眼圧低下とともに近視は約4か月で-0.75Dに回復し,角膜曲率半径は有意に減少した。本症例は抗緑内障薬による近視化に加え,急激に眼圧が上昇した片眼に角膜突出と眼軸長の延長が生じた一過性近視が特徴であった。

So far, the use of superconducting magnets has been limited to chemical analysis and medical inspection. To develop a new use, we have tried to apply common MRI devices as nondestructive sugar content detector of watermelon. To estimate the sugar content, a new technique using multiple regression analysis of two NMR relaxation times was developed. It was found that the coefficient of multiple regression at the center of the watermelon exceeded 0.9 and the standard error of prediction was around 0.5, and that the over-ripened part of flesh called ‘nieka’ in watermelon could be distinguished because its T2 was much longer than that of the oridinary part. An evaluation rate of sugar content could be below 6s per watermelon. It is concluded that multiple analysis of T1 and T2 on intact watermelon can be applied as a non-invasive, nondestructive indicator of sugar content.

本研究は、東京都心の医療機関における、1990年から2001年まで12年間の外国人分娩事例のうち、日本語によるコミュニケーションが困難な事例について検討することにより、言語の問題に伴う在日外国人の周産期医療上の課題を明らかにし、その対策について考察した。日本語によるコミュニケーションが困難なことにより、医療従事者と妊産婦との適切な意思伝達の阻害、保健・医療・福祉に関する情報不足、の2点が特有の問題として挙げられた。特に意思伝達の阻害は、病歴や自覚症状の確認困難、相互信頼関係形成と精神的支援の阻害、医療従事者の負担の増大を引き起こし、さらにインフォームドコンセントに基づく医療サービス提供の妨げになっていた。外国人妊産婦に対して、日本人と同じようにインフォームドコンセントに基づいた医療を提供するには、医療通訳制度を整えることが急務の課題であると考えられた。

The effect of chymotrypsin inhibitors on the intestinal absorption of insulin was investigated in conscious and unrestrained rabbits. Insulin at a dose of 25U/kg with 10mg/kg of chymotrypsin inhibitors was administered intraduodenally via an indwelled catheter. The blood glucose level was decreased following an administration of insulin in combination with each chymotrypsin inhibitor. The order of the effect on the intestinal absorption was related with the order of chymotrypsin inhibitory activity. Maximal decrease was observed when insulin was administered with such a strong inhibitor as FK 448, [4-(4-isopropylpipe-radinocarbonyl) phenyl 1, 2, 3, 4-tetrahydro-1-naphthoate methanesulfonate], whose IC50 value was 7×10-7M, and the decrease of the glucose level was 25%, compared with the level before administration. The effect of inhibitors which inhibit both chymotrypsin and trypsin was almost the same with that of chymotrypsin specific inhibitors.

Synthesis of water-soluble chymotrypsin specific inhibitors was attempted to study the roles of chymotrypsin-like enzymes in vivo. Previously we reported that the esters of carboxylic acid containing a condensed ring showed stronger activity than those containing a single ring system. Then we synthesized 4-substituted phenyl esters of carboxylic acid containing a condensed ring, such as tetralin, naphthalene, indole etc., and their inhibitory activities were compared. Among these compounds, esters of tetralin-1-carboxylic acid (FK-448) and 1-naphthylacetic acid showed the strongest activity, and their IC50 values were 8×10-7, 5×10-7 M, respectively. Tetralin-2-carboxylate and 2-naphthylacetate inhibited weaker than 1-analogues. Esters of basic quinoline carboxylic acid and bulky carboxylic acids containing a three-ring system inhibited poorly. Chymotrypsin produced equimolecular 4-substituted phenol rapidly and thereafter the amount of 4-substituded phenol increased slowly, when incubated with FK-448 at 37°C.

〔目的〕都市における結核発病の実態を報告し,今日の都市結核対策を検討する。〔方法〕症例研究を中心に行った。各症例の社会背景と結核菌DNA指紋分析を加えた菌情報により感染経路を調査した。〔結果〕2005年2月よりの1年5カ月の間に,川崎市川崎駅周辺の約500m四方の地域で9例の結核発病を確認した。9症例は16~55歳の比較的若い年齢層で,3例はホームレスであった。接触者健診の過程で,すべての症例が川崎駅周辺を生活活動圏としており,ネットカフェ等での関連が推測された。9例中7例はSM耐性菌であり,そのうち5例はDNA指紋分析により同一パターンを呈した。〔考察〕本事例はネットカフェ等の不特定多数利用施設を中心とした感染と考えられた。都市にはこのような施設が多く,若年者層とともにホームレス等の社会的弱者も利用する。結核未感染の若年者層と結核ハイリスク層とが閉鎖的空間を長時間共有する環境は,いったん結核菌の喀出があれば,容易に感染が起こりうることを示唆した。結核の都市偏在にはこのような社会環境も影響しており,それらを加味した総合的対策が求められる。

噴気ガスの化学組成が火山活動の激しさの指標になる可能性が指摘されてきた。本研究では, 噴気ガスとともに放出される揮発性金属元素の量の変化と, 同位体組成の変動に相関が見られるか, 相関があれば脱ガスによるレイリーモデルにより同位体組成の変動が説明できるか検討する。三宅島の2000年噴火の火山灰試料とラバウルTavurvur火山の火山灰試料について検討する。

株式会社ERiは会社設立当初からBluetoothを活用した無線通信技術を一つの核として事業展開してきた。自社オリジナルのアイデアに基づき商品化に注力した結果、2012年に世界初のスマートフォン向けBluetoothビーコン発信機『BLUETUS(R)(ブルータス)』の商品化に成功した。また、現在は、BLUETUSの製品展開として屋内位置推定システム『BLEGONIO(ブレゴニオ)』」を研究開発中である。なお、研究開発では近隣大学と積極的に共同研究に取り組んでいる。本事例紹介ではこれら2商品の開発について紹介する。<br>*(R)は登録商標マークRの囲み文字を示す。

血清コリンエステラーゼ (ChE) が異常に低値を示した症例の麻酔を経験した. 症例は, 74歳, 女性. 今回, 胆石症に対し開腹胆嚢摘出術が予定された. 手術・麻酔歴はなく, 既往として悪性貧血, 高血圧を指摘されていた. 近医での血液検査でChE値の異常低値が20年にわたり認められていたが, 自覚症状がないため経過観察とされていた. 麻酔は, 胸部硬膜外麻酔併用全身麻酔を選択し, 術中は脱分極性筋弛緩薬, エステル型局所麻酔薬を避け, ベクロニウム, メピバカインを使用し特に問題なく手術, 麻酔を終了した. 血清コリンエステラーゼは薬物代謝に関与しているので, 術中に使用する薬剤の選択には十分な注意が必要と考える.

Many 2-oxoalkyl arenesulfonate derivatives having straight or branched alkyl chains of different lengths, 2-oxoalkyl bis-arenesulfonate derivatives, and alkyl arenesulfonate derivatives having a ketal moiety at the 2-position on the alkyl chain were synthesized, and their esterase-inhibitory activities, as well as hypolipidemic activities, were evaluated.Among these compounds, 1-(2, 4, 6-trimethylbenzenesulfonyloxy)-2-dodecanone (III-1u), and 1-(2, 3, 4, 6-tetramethylbenzenesulfonyloxy)-2-hexanone (III-1w), -2-octanone (III-1x) and -2-decanone (III-1y) exhibited potent esterase-inhibitory activities (IC50=3×10-10, 2×10-10, 2×10<-10> and 3×<-11>M, respectively). However, the sulfonate (XV) having a ketal moiety on the alkyl chain and the bis-sulfonate (XVI) exhibited low inhibitory activities toward esterase in comparison with III and XII. Most of the compounds III and some of the compounds XII exhibited potent hypolipidemic activities corresponding to more than 50% lipid-lowering effect (plasma triglyceride and cholesterol ester) in vivo. The structure-activity relatioinships of these compounds are discussed.

Many 1-substituted 2-alkanone derivatives were synthesized and their inhibitory activities toward pancreatic lipase and esterase were examined in order to obtain hypolipidemic agents. 1-Benzenesulfonyloxy-2-pentanone (VI-2a) and 1-(2, 4, 6-trimethylbenzenesulfonyloxy)-2-pentanone (VI-2q) exhibited not only potent and selective esterase inhibitions (IC50 : 9.0×10-7M and 1.0×10-6M, respectively), but also potent hypolipidemic action (90 and 92% reductions of plasma triglyceride, and 53 and 90% reductions of plasma total cholesterol, respectively). A novel working hypothesis is presented to account for the lowering of the plasma lipids level, i.e., that inhibition of esterase and lipase activities in the small intestinal lumen may be responsible for the decrease in the plasma lipids level.

Various podophyllotoxin derivatives from desoxypodophyllotoxin (DPT) were synthesized to examine the structural relationships between the biological significance (cytotoxic effect, effects on DNA topoisomerase II and tubulin polymerization) in vitro and antitumor activity in vivo (L 1210).An intact 6, 7-methylenedioxy group of DPT is necessary to inhibit tubulin polymerization and topoisomerase II. 4'-Phenolic hydroxyl group of DPT is essential to inhibit DNA topoisomerase II and the inhibitory effect on DNA topoisomerase II contributes to a high cytotoxicity.The introduction of an aminoalkoxy group at 1-position of DPT enhances the inhibitory activity against DNA topoisomerase II and cytotoxic effect, causing the inhibitory activity against tubulin polymerization to disappear. The results of antitumor test in mice bearing L 1210 on podophyllotoxin derivatives suggest the following : 1) the strong cytotoxic effect itself is not a good indication of antitumor activity in vivo as long as it is associated with inhibition of tubulin polymerization. DNA topoisomerase II inhibitory effect contributes to an antitumor activity in vivo; 2) detailed measurements of cytotoxicity and inhibition on DNA topoisomerase II and tubulin polymerization in vitro are necessary to evaluate podophyllotoxin derivatives.

二次孔心房中隔欠損 (atrial septal defect ; ASD) に合併した僧帽弁逆流 (MR) が急激に進行し, 心不全をきたした症例を経験した. 症例は10歳女児. 5歳時に心雑音を指摘され, 近医を受診してASD, 軽度のMRと診断された. ASDに対してカテーテル治療を希望し, 至適体重まで待機中に, 経済的理由により病院を受診しなかった. 10歳で, 全身倦怠感, 起坐呼吸が出現し, 胸部X線写真では, 心胸郭比が80%と心拡大を認め, 心臓カテーテル検査では, 肺体血流比は5.1で, 重度のMRと肺高血圧を認めた. 外科的にASD閉鎖術および僧帽弁人工弁置換術を施行した. ASD症例で稀ながらMRが進行する場合があり注意が必要である.
