Exercise necessitates a large supply of O2 and nutrients and rapid removal of CO2 and waste products. Histamine is a regulator of the microcirculation (which performs these exchanges), suggesting a possible involvement of histamine in exercise. Histamine is released from either mast cells or non-mast cells. In the latter, histamine is newly formed via the induction of histidine decarboxylase (HDC) in response to an appropriate stimulus, and it is released without being stored. Here, in mice, we examined the role of histamine or HDC induction in exercise. Prolonged walking (PW) (in a cylindrical cage turned electrically) increased HDC mRNA and HDC activity in quadriceps femoris muscles. Mice given a histamine H1-receptor antagonist [fexofenadine (peripherally acting) or pyrilamine (peripherally and centrally acting)] or an irreversible HDC inhibitor (α-fluoromethylhistidine) displayed less PW endurance than control mice. Ranitidine (H2-receptor antagonist) tended to reduce endurance. Other histamine-receptor (H3 and H4) antagonists had no significant effects on endurance. Mice deficient in HDC or histamine H1-receptors displayed markedly less endurance than control mice, and HDC activity in the quadriceps femoris of H1-deficient mice was rapidly elevated by PW. Fexofenadine significantly reduced the muscle levels of nitric oxide (NO) metabolites and glycogen after PW. The results support the ideas that (i) histamine is involved in protecting against exercise-induced fatigue or exhaustion, (ii) histamine exerts its protective effect via H1 receptors and the ensuing production of NO in skeletal muscle, and (iii) histamine is provided, at least in part, by HDC induction in skeletal muscles during prolonged exercise.

Endovascular therapy (EVT) has revolutionized the treatment of acute ischemic stroke. In the past few years, endovascular treatment indications have expanded to include patients being treated in the extended window, with large ischemic core infarction, basilar artery occlusion (BAO) thrombectomy, as demonstrated by several randomized clinical trials. Intravenous thrombolysis (IVT) bridging to mechanical thrombectomy has also been studied via several randomized clinical trials, with the overall results indicating that IVT should not be skipped in patients who are candidates for both IVT and EVT. Simplification of neuroimaging protocols in the extended window to permit non-contrast CT, CTA collaterals have also expanded access to mechanical thrombectomy, particularly in regions across the world where access to advanced imaging may not be available. Ongoing study of areas to develop include rescue stenting in patients with failed thrombectomy, medium vessel occlusion thrombectomy, and carotid tandem occlusions. In this narrative review, we summarize recent trials and key data in the treatment of patients with large ischemic core infarct, simplification of neuroimaging protocols for the treatment of patients presenting in the late window, bridging thrombolysis, and BAO EVT evidence. We also summarize areas of ongoing study including medium and distal vessel occlusion.

A 19-year-old womanpresented with acute onset of cough and dyspnea. She started smoking two weeks before the appearance of symptoms. On admission, arterial blood gas analysis on room air breathing revealed PaO2 55 Torr. Chest roentgenogram and high resolution computed tomogramsshowedlocalized fine nodular shadows at the right lower lung field. Bronchoalveolar lavage fluid revealed a high eosinophil count. Eosinophil infiltration was also observed in transbronchial lung biopsy specimens. The final diagnosis was acute eosinophilic pneumonia (AEP). Although few reports have demonstrated diffuse fine nodular shadows in AEP, localized fine nodular shadows on chest roentgenogram and CT may some-times be the sign of AEP especially in the early phase of the clinical course.(Internal Medicine 42: 88-91, 2003)

Two patients, 48- and 50-year-old sisters, presented with a characteristic facial appearance with slowly progressive deafness and cerebellar ataxia starting in their 30s. Genetic testing identified compound heterozygous pathogenic variants in the ERCC6 gene: c.1583G>A (p.G528E) and c.1873T>G (p.Y625D). A diagnosis of Cockayne syndrome (CS) B type III was made. CS is usually diagnosed in childhood with well-defined facial characteristics and photosensitivity. This case report describes rare cases of adulthood CS with a primary presentation of slowly progressing deafness and cerebellar ataxia. CS should be considered in adults with characteristic facial and skin findings, deafness, and cerebellar ataxia.

Background: Various types of arrhythmia are observed in patients with cardiac amyloidosis, but the prevalence of arrhythmia has not been fully investigated. This study investigated the prevalence and treatment of arrhythmias in patients with cardiac amyloidosis before the introduction of new agents for amyloidosis, such as tafamidis.Methods and Results: Of 53 patients who were histologically diagnosed with cardiac amyloidosis at 10 centers in western Japan between 2009 and 2021, 43 who were diagnosed on the basis of immunohistochemical staining were evaluated in this study. Of these 43 patients, 13 had immunoglobulin light-chain (AL) amyloidosis and 30 had transthyretin (ATTR) amyloidosis; further, 27 had atrial tachyarrhythmia, 13 had ventricular tachyarrhythmia, and 17 had bradyarrhythmia. Atrial fibrillation (AF) was the most common arrhythmia in patients with cardiac amyloidosis (n=24; 55.8%), especially among those with ATTR amyloidosis (70.0% of ATTR vs. 23.1% of AL). Eleven (25.6%) patients were treated with a cardiac implantable device. All 3 patients with pacemakers were alive at the last follow-up (median 76.7 months; interquartile range [IQR] 4.8–146.4 months). Of the 8 patients who underwent AF ablation, there was no recurrence in 6 (75%) after a median of 39.3 months (IQR 19.8–59.3 months).Conclusions: The prevalence of various arrhythmias was high in patients with cardiac amyloidosis. AF occurred most frequently in patients with cardiac amyloidosis, especially among patients with ATTR.

Background: A recent randomized trial demonstrated that catheter ablation for atrial fibrillation (AF) in patients with heart failure with reduced ejection fraction (EF) is associated with a reduction in death or heart failure. However, the effect of catheter ablation for AF in patients with heart failure with mid-range or preserved EF is unclear.Methods and Results: We screened 899 AF patients (72.4% male, mean age 68.4 years) with heart failure and left ventricular EF ≥40% from 2 Japanese multicenter AF registries: the Atrial Fibrillation registry to Follow the long-teRm Outcomes and use of aNTIcoagulants aftER Ablation (AF Frontier Ablation Registry) as the ablation group (525 patients who underwent ablation) and the Hokuriku-Plus AF Registry as the medical therapy group (374 patients who did not undergo ablation). Propensity score matching was performed in these 2 registries to yield 106 matched patient pairs. The primary endpoint was a composite of cardiovascular death and hospitalization for heart failure. At 24.6 months, the ablation group had a significantly lower incidence of the primary endpoint (hazard ratio 0.32; 95% confidence interval 0.13–0.70; P=0.004) than the medical therapy group.Conclusions: Compared with medical therapy, catheter ablation for AF in patients with heart failure and mid-range or preserved EF was associated with a significantly lower incidence of cardiovascular death or hospitalization for heart failure.

Die beiden Filme, "Modern Times" und "Metropolis" , teilen dasselbe Thema, das in der Kritik an der kapitalistischen modernen Zivilisation besteht. Im kritischen Licht stellen sie die mechanisierte Lage der Menschen in der hochindustrialisierten Gesellschaft dar, die Entfremdung und Manipulierung des Individuums verursacht. Welche Weltanschauung und welches Werturteil die beiden Filme in ihren Zivilisationskritiken zeigen, ist jedoch anders, ja sogar gegensä tzlich. Einerseits wurzelt die Zivilisationskritik in "Modern Times" im Glauben an den Individualismus, andererseits beruht diejenige in"Metropolis" auf dem Autoritarismus. Chaplin sucht in "Modern Times" ständig als Individuum seine Freiheit zu erringen, während die Menschen in "Metropolis" nur erwarten, aus ihrer unterdrückten Lagegerettet zu werden, indem sie sich "einen guten Herrscher" wünschen, der sie zu ihrer Befreiung führen sollte. Diese gegensätzlichen Einstellungen zum Leben, die trotz derselben Zeitlage parallel bestehen,werden nicht auf verschiedene persönliche Weltanschauungen der beiden Filmautoren, C. Chaplin und F. Lang, zurückgeführt, sondern spiegeln die gegensä tzlichen Weltanschauungen der Gesellschaften wider,in denen die beiden Regisseure lebten.

Japan's economy made rapid and steady progress in the post-war half century and children grew by 2 cm per decade. Economic development in South Korea was some two decades behind Japan, due to the Korean War (1950-53). Teens in Korea were 2-3 cm shorter in height than their Japanese peers in the 1960-70s, caught-up with the latter in the early 1990s and then outgrew Japanese teens by 3 cm in the mid-2000s. They ceased, however, to grow any taller afterwards, whereas the national economy remained prosperous and per capita supply of animal-sourced foods, including milk increased appreciably.School boys in Korea were 1.5 cm greater than their Japanese peers in growth velocity from 1st graders in primary school to high school seniors in the early 2000s but began to fall persistently in velocity to be 2 cm below Japanese in the end of the 2010s.Analyzing Household Expenditure Surveys, 1990 to 2019, the authors were stunned to discover that Korean children started to turn away from vegetables in household consumption in the mid-1990s and ate as little as 10% of vegetables as the control group (people in their 50s) in the mid-2010s. Children in Japan started to steer away from fruit and vegetables in the end of the 1970s, when supply of per capita meat and milk was expanding. It is suspected that vegetables and fruit may be among essential nutrients for child height development.

The salak fruit (snake fruit) contains one to three seeds covered with an aril. The size of the fruit primarily depends on the number of seeds. Fruits with more seeds grow larger and attain higher commercial value in fresh markets owing to their superior appearance. In eastern Thailand, during the hot period in the early rainy season, the fruit set of salak decreases, which is empirically believed to be caused by high night temperatures. In this study, we pollinated and incubated salak spadices at various temperatures (18–36°C) to determine the optimal post-pollination temperature range needed to produce valuable fruits. Chronological pollen-tube elongation in the pistil and development of the early embryo and endosperm were then observed anatomically, followed by fruit-set estimation. At ≤ 21°C, pollen-tube elongation was limited. At 24–27°C, although elongation was slow, pollen tubes attained embryo sacs in > 60% of florets within 36 h after pollination. Pollen tubes elongated fastest at 30–33°C and attained embryo sacs within 12 h after pollination. At 36°C, the difference in the elongation speed tended to be higher among the pistils. The percentages of ovules with developing embryos were the highest at 27°C at 48 h after pollination; zygote and early embryos were observed in 10.8% and 55.9% of the ovules, respectively, and the primary endosperm nucleus, dispersing endosperm nucleoplasm, and free endosperm nuclei were observed in 8.6%, 38.7%, and 19.4%, respectively. The second highest percentage was observed at 24°C. The percentages tended to decrease at 30°C and decreased significantly at ≥ 33°C. At ≥ 30°C, shriveled embryo sacs were observed. The estimated fruit-set percentage based on embryo development as the consequent fruit set was the highest (≈80%) at 27°C, while the second highest percentage (≈75%) was at 24°C. At ≥ 30°C, the estimated percentage decreased to less than half that at 27°C. Fruits containing three seeds were expected to grow in the range of 17.6–28.0% at ≤ 27°C. One- or two-seeded fruits were expected to grow at ≥ 30°C. Limited fruit set was expected at 36°C. Our results indicate that salak prefers relatively cool temperatures of approximately 25°C for the fertilization and set of valuable fruits.

Lung cancer can cause fatal central airway obstruction. Rapid airway clearance is necessary in some cases, but ventilator management may be insufficient to maintain oxygenation levels. Venovenous extracorporeal membrane oxygenation (VV-ECMO) may be an effective rescue therapy for respiratory failure, but its efficacy in treating tumor-related airway obstruction is unknown. We herein report a case of central airway obstruction and severe acute respiratory failure due to small-cell lung cancer successfully treated with VV-ECMO, bronchoscopic airway intervention, and chemotherapy. VV-ECMO can be an effective option for the treatment of central airway obstruction with acute respiratory failure due to lung cancer.

近代の日本人のほとんどは、まぎれもないレイシストであった。それは、中国(人)観が証明している。江戸期には、満州族の風俗である「弁髪」をもって、清国人を「芥子坊主」「ぱっち坊主」と呼んでいたが、そこに侮蔑的な意味合いは含まれていなかった。それが、江戸中期以降に少しずつ変化し、アヘン戦争などで中国が列強の前に敗退すると、知識人の中に中国を反面教師とする中国観が成立してくる。そして、明治に入るとすぐに「チャンチャン坊主」という侮蔑的な呼称が使われはじめ、「豚尾漢」などの呼称とともに都市部・居留地から地方へと徐々に広まっていった。その過程は、同時に、清国人を「豚」の漫画や絵で視覚化し、蔑視の対象にしていく過程でもあった。しかし、日清戦争までは、こうした中国人観が地方の民衆の中にまで浸透していたとはいえず、侮れない強国とする認識も存在していたが、戦勝により、国民的レベルで侮蔑的な中国人観が定着していった。

To computationally investigate the recent experimental finding such that extracellular ATP release caused by exogeneous mechanical forces promote wound closure, we introduce a mathematical model, the Cellular Potts Model (CPM), which is a popular discretized model on a lattice, where the movement of a “cell” is determined by a Monte Carlo procedure. In the experiment, it was observed that there is mechanosensitive ATP release from the leading cells facing the wound gap and the subsequent extracellular Ca2+ influx. To model these phenomena, the Reaction-Diffusion equations for extracellular ATP and intracellular Ca2+ concentrations are adopted and combined with CPM, where we also add a polarity term because the cell migration is enhanced in the case of ATP release. From the numerical simulations using this hybrid model, we discuss effects of the collective cell migration due to the ATP release and the Ca2+ influx caused by the mechanical forces and the consequent promotion of wound closure.

Purpose: To investigate whether deep cervical lymph node (DCLN) ligation alters intracranial cerebrospinal fluid (CSF) tracer dynamics and outflow using a rat model with intrathecal dynamic contrast-enhanced (DCE) MRI.Methods: Six bilateral DCLN-ligated and six sham-operated rats were subjected to DCE MRI with Gd-BTDO3A, and dynamic T1-weighted images were acquired. ROIs were collected from the CSF at the C1 level (CSF_C1), CSF between the olfactory bulbs (CSF_OB), CSF at the pituitary recess (CSF_PitR), and CSF at the pineal recess (CSF_PinR), upper nasal turbinate (UNT), olfactory bulbs, cerebrum, and the jugular region. Time-intensity curves were evaluated, and the maximum slope, peak timing, peak signal ratio, and elimination half-life for the four CSF ROIs and UNT were calculated and compared.Results: Delayed tracer arrival in the rostral CSF space and the nasal cavity with tracer retention in the ventral CSF space were observed in the ligation group. The maximum slopes were smaller in the ligation group at UNT (sham: 0.075 ± 0.0061, ligation: 0.044 ± 0.0086/min, P = 0.011). A significant difference was not detected in peak timings. The peak signal ratio values were lower in the ligation group at UNT (sham: 2.12 ± 0.19, ligation: 1.72 ± 0.11, P = 0.011). The elimination half-life was delayed in the ligation group at CSF_C1 (sham: 30.5 ± 2.70, ligation: 44.4 ± 12.6 min, P = 0.043), CSF_OB (sham: 30.2 ± 2.67, ligation: 44.8 ± 7.47 min, P = 0.021), and CSF_PitR (sham: 30.2 ± 2.49, ligation: 41.3 ± 7.57 min, P = 0.021).Conclusion: The DCLN ligation in rats blocked CSF outflow into the nasal cavity and caused CSF retention.

The high flux source for the Pulsed High Density Experiment (PHDX) has been constructed, and field-reversed configuration (FRC) plasmas are being produced. To diagnose FRC plasma temperatures three kinds of diagnostic systems have been set up on PHDX; λ = 632.8 nm He-Ne laser interferometer system at the midplane for total temperature Ttotal, 16 channel spectrometer for ion temperature Ti and soft x-ray (SXR) measurement system for electron temperature Te viewed along z-axis, respectively. The SXR measurement system consists of five sets of collimators and AXUV100 photodiodes with directly deposited filters which have different bandpass regions. As a result of preliminary temperature diagnostics, the following plasma parameters are estimated during a relatively quiescent phase of FRCs; Ttotal ∼ 50 eV, Ti ∼ 50 eV and Te ∼ 25 eV. This result does not agree with the equilibrium relation of Ttotal ≈ Ti + Te, and it must be caused by plasma condition and our assumptions in the spectral analysis.

We estimated the protolith age and peak metamorphic temperature of the Yokokawagawa metamorphic rocks (YMR) east of the Itoigawa–Shizuoka Tectonic Line using detrital zircon U–Pb dating and Raman carbonaceous material geothermometry, respectively. U–Pb dating of a psammitic rock yielded a youngest age of ∼100 Ma, which corresponds to the protolith age. Raman carbonaceous material geothermometry results for five pelitic rock samples give metamorphic temperatures of ∼350–380 °C. The protolith age is consistent with those of the Sanbagawa metamorphic rocks (SMR), strongly indicating that the YMR are an extension of the SMR. The increase in peak temperature toward intrusive rocks along the eastern margin of the YMR may indicate that the relatively young ages yielded by previous K–Ar dating of the YMR reflect thermal resetting due to contact metamorphism.

Background: Cochineal dye is used worldwide as a red coloring in foods, drinks, cosmetics, quasi-drugs, and drugs. The main component of the red color is carminic acid (CA). Carmine is an aluminum- or calcium-chelated product of CA. CA and carmine usually contain contaminating proteins, including a 38-kDa protein thought to be the primary allergen. Severe allergic reactions manifest as anaphylaxis. The aim of this study was to review all Japanese reported cases and propose useful diagnostic chart.Methods: All reported Japanese cases of cochineal dye-induced immediate allergy were reviewed, and newly registered cases were examined by skin prick test (SPT) with cochineal extract (CE) and measurement of CE and carmine-specific serum IgE test. Two-dimensional (2D) western blotting using patient serum was conducted to identify the antigen.Results: Twenty-two Japanese cases have been reported. SPT and the level of specific IgE test indicated that six cases should be newly registered as cochineal dye allergy. All cases were adult females, and all cases except three involved anaphylaxis; 13 cases involved past history of local symptoms associated with cosmetics use. Japanese strawberry juice and fish-meat sausage, and European processed foods (especially macarons made in France) and drinks were recent major sources of allergen. 2D western blotting showed that patient IgE reacted to the 38-kDa protein and other proteins. Serum from healthy controls also weakly reacted with these proteins.Conclusions: SPT with CE and determination of the level of CE and carmine-specific IgE test are useful methods for the diagnosis of cochineal dye allergy.

Several vaccines have been developed for coronavirus disease 2019 - caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) - in record time. A few cases of immune thrombocytopenic purpura (ITP) following SARS-CoV-2 vaccination have been reported. We herein report a 90-year-old man who received the Pfizer-BioNTech SARS-CoV-2 vaccine (BNT162b2) and developed severe thrombocytopenia with intracranial hemorrhaging and duodenal bleeding, consistent with vaccine-related ITP. He was successfully treated with intravenous immunoglobulin, prednisolone, and eltrombopag and discharged without cytopenia. Vaccine-related ITP should be suspected in patients presenting with abnormal bleeding or purpura after vaccination.
