Team medical care needs to be improved at the outpatient clinic to manage highly complex cancer chemotherapy. In May 2008, Kurashiki Medical Center was the first medical institution in Japan to start a pharmaceutical outpatient clinic called a “support clinic.” At the support clinic, oncology pharmacy specialists interview patients and propose several suitable options of drug regimens to the physicians to ensure safe and effective treatment. We evaluated the functions expected of the support clinic and the appropriateness of health insurance application to the clinic's services.Physicians and nurses highly evaluated the work of the oncology pharmacy specialists at the support clinic. The function that they expected most of the support clinic was the design of supportive therapies followed by the design of treatment strategies. All 93 patients surveyed said that the support clinic was beneficial to them. Ninety-six percent of the patients considered that the support clinic services were worth paying.The support clinic has been providing oncology pharmacy specialists with great opportunities to fulfill their potential. Its services are needed by medical professionals and patients who want improved outpatient cancer treatment.

多系統萎縮症(multiple system atrophy; MSA)は進行性の神経変性疾患で,パーキンソニズム,小脳失調,自律神経不全,錐体路徴候を経過中に種々の程度で認める.孤発性が圧倒的に多いが,主として常染色体劣性を示す家系も報告されている.パーキンソニズムが優位な臨床病型はMSA-P(multiple system atrophy, parkinsonian variant),小脳失調が優位な臨床病型はMSA-C(multiple system atrophy, cerebellar variant)と呼ばれ,欧米ではMSA-Pが多く,日本ではMSA-Cが多い.平均発症年令は55~60歳,予後は6年から10年で,15年以上生存する症例もある.早期から高度に出現する自律神経不全は重要な予後不良因子の一つである.発症時には,運動症状もしくは自律神経不全のいずれか一方のみを有する症例が多く,いずれの症状も出現するまでの期間の中央値は自験例では2年である.現在広く用いられている診断基準は,運動症状と自律神経不全をともに認めることが必須であるため,運動症状もしくは自律神経不全のみを呈している段階では診断が出来ない.しかし,自律神経不全のみを呈する段階で突然死する症例もあることを念頭に置く必要がある.MSAに伴う自律神経不全の特徴の理解と病態に基づいた責任病巣の特定は,早期診断に有用な情報をもたらすと考えられる.従来は稀とされてきた認知症もMSAにおける重要な問題である.前頭葉機能低下はMSAでしばしば認め,MRIやCTにて進行とともに前頭側頭葉を中心とする大脳萎縮も明らかとなる.最近では,前頭側頭型認知症の病型を示す症例も報告されている.MSAの病態と症候の広がりを踏まえた,早期診断方法開発は,病態抑止治療展開の上でも極めて重要である.

ソテツ味噌を試醸し, ソテツの有毒成分であり発癌性の知られているcycasinと, アフラトキシン汚染の有無を検索した。また, 長期間動物に投与する試験を行なった。1) cycasinはソテツ種子自身のもつβ-グルコシダーゼで分解され, 通常の原料ソテツ粉末中には残存しなかった。cycasinが残存するようにして調製した原料では, 麹菌の成育が抑制され麹ができにくいが, cycasinは速やかに分解された。2) Sprague-Dawley系ラットに, ソテツ味噌を10~50%混入した飼料を62日間, または10%混入飼料を190日間給餌した。これらには1年間飼育ののち, いずれの臓器にも腫瘍の発生はまったくみられなかった。3) ここで用いたソテツ味噌の原料ならびに製品, 自家製ソテツ味噌3点にアフラトキシンは検出されなかった。

本研究の目的は,地域子育て支援拠点の利用者の心情の変容を分析し,子育てによる親の孤立を防ぐための支援のあり方を明らかにすることである。拠点によく訪れた5人の利用者にインタビューし,複線径路・等至性モデル(TEM)によって質的分析を行った。その結果,親の複雑な心情の変容が4つの過程に分類された。本研究では,利用者が他の人の言葉やかかわりに非常に敏感であることが明らかになった。利用者の緊張と不安は表面上見ることが難しいため,拠点での支援では,「把握」「アプローチ」「創意工夫」「橋渡し」の必要性を提案した。

Last year the author's research team investigated 3587 college students from 13 universities and stated that one of the major factors which differentiate the upper from the middle and lower percentile groups was the use of cognitive strategies. This year using the same data as the last year, the author investigated the relation between their use of cognicative strategy and their intrinsic values among Japanese college students. The result shows that the cognitive strategic competence of the best performers and the middle group students are developed enough but the lowest students have not developed their cognitive competence and that the number of the intrinsic values is different among the groups. The higher the English proficiency of the students becomes, the more the number of their intrinsic values becomes. After analyzing the data, some coping instructions are suggested and educational implications are discussed.

〔目的〕認知症高齢者を介護する家族の介護肯定感とそれに影響する要因を明らかにすることを目的とする.ストレス対処能力(SOC),主観的健康感,ソーシャルサポートなどの影響要因について検討した.〔方法〕認知症高齢者を在宅で介護している主介護者に無記名の自記式質問紙調査を実施した.分析対象者は有効回答のあった188人とした.調査には介護肯定感尺度, SOC短縮版尺度, SF-8を用い,ソーシャルサポートと介護者・被介護者属性等についてたずねた.〔結果〕介護肯定感は平均値が36.5±7.9点であった.介護肯定感を従属変数とした重回帰分析の結果,介護肯定感に影響を及ぼす要因は介護者が女性(β=0.162, p=0.006), SF-8のMCSが高い(β=0.218, p=0.001), SOCが高い(β=0.133, p=0.043),「適切な介護の仕方が分かる」(β=0.410, p=0.000)ことが影響していた.これらの4変数により全体の35.7%を説明した.〔結論〕在宅で認知症高齢者を介護する家族の介護肯定感を高めるには, SOCや精神的QOLを高める支援とソーシャルサポートが重要なことが示唆された.

従来日本産Pleurobranchaea属としてはウミフクロウにP. novaezealandiae Cheeseman 1878が, ツノウミフクロウにP. sp.の学名があてられていたが, それらの学名について検討した結果, 前者にはP. japonica Thiele, 1925, 後者にはP. brockii Bergh, 1897の学名をあてるのが最も適当であることがわかった。ウミフクロウに以前にあてられていた学名P. novaezealandiaeは, 現在ではP. maculataとシノニムとされている。P. japonicaは神戸産の標本をもとに記載されたが, 腹足, 背楯, 歯舌, 顎板などの形態のみの不十分な記載であったために, P. maculataとシノニムである可能性が疑われていた。本研究によりP. japonicaはP. novaezealandiae=P. maculataとは外部生殖器官の位置や形態, 腹足腹面の色, 幼生の殻の形態と浮遊期間に明瞭な相違があるため, 独立の別種であることがわかった。ツノウミフクロウについては, Baba (1949, 1969)によりPleurobranchaea sp.として記録されていたが, 本研究で腹足背面の突起, 頭膜前縁の小突起の列, 内部及び外部生殖器官の特徴により従来インドネシア海域とアフリカ東岸から知られていたP. brockiiであると結論された。

Analysis of NOx removal by a discharge plasma is made with an equivalent circuit. The used NOx reduction system consists of a neon transformer and a plasma reactor based on a dielectric barrier discharge. Impedance of the neon transformer has been determined experimentally, The equivalent circuit based on a series connection of two capacitors is used for the plasma reactor. The capacitances are obtained from experimental V-Q Lissajous diagram. Comparison with experimental data of consumed energy and the dependence of NO removal on gap length is successfully done.
