In most libraries in Japan, you can find a voluminous dictionary of loan-words, compiled by &Mei Arakawa. This dictionary is characterized by its rich records, including the first written record of each of the loan-words. Every record tells us about the cultural history concerning the loan-word.In this paper, the writer will examine the process by which S. Arakawa compiled the dictionary, as said to be the definitive work among dictionaries of loan-words, though he was a mere English teacher at a local junior high school under the old education system.And the author will also examine the scholars by whom he was greatly influenced. S.Arakawa was quite unknown when he published Japanized English (at his own expense), yet the famous novelist and scholar Shôyo Tsubouchi was the first person to order a copy.S. Arakawa decided to specialize in loan words after being inspired by a treatise on loan-words by Yoshisaburo Okakura. Okakura remained Arakawa's most important mentor throughout the remainder of his life.Arakawa decided to compile Japanized English after being inspired by Sanki Ichikawa's book, English Influence on Japanese (sic). After receiving a letter from S. Ichikawa, saying that the dictionary would be of no value if it had no examples, Arakawa devoted himself to searching for these examples. In the end, he spent more than 60 years of his life searching records of loan-words.This report is based on the paper read by the writer at the monthly meeting on November 3, 2001.

はじめに アレルギー疾患の小児は,病気を抱えながらも,普段の日常生活を病院ではなく,家庭,学校,幼稚園,保育所など病院の外で過ごす.そこには,診察室の短時間の診療では見えてこないさまざまな悩みや問題点がある.これからの小児医療スタッフ,とりわけ小児アレルギーに関わる者には,病院でアレルギー児が受診するのを待っているだけではなく,病院を飛び出して日常生活の中にいる小児を捉え,問題点を抽出し,そこへ介入していく姿勢が必要である.そのためには,医師の診察のみでは不十分である.看護師,薬剤師,管理栄養士,さらには保健師,学校・園・保育所関係者,行政関係者などを幅広く巻き込んだ多職種連携が必要である. 幸い,わが国には,小児アレルギー疾患の専門知識と患者指導のスキルを習得する専門資格である小児アレルギーエデュケーター(pediatric allergy educator:PAE)制度があり,本資格を獲得した医療スタッフが全国で活躍している.本稿では,PAEの活動を軸にして,多職種による小児のアレルギーケアの実態と課題を考察する.

本研究は,退院の意向を持ちながら,10年以上の長期入院に至っている2名の精神分裂病患者が持つ,現状や退院に対する意識の特徴について,半構成的面接を通して述べられた患者の体験に基づいて検討したものである。その結果,以下の点が明らかになった。1)患者は,退院が現実的に困難な理由として,社会資源利用についての情報が不足していることや,家族の状況として受け入れが難しいことを認識していた。2)患者は,保護的環境である病院の中で,病院に順応するという形で無理をしない生き方を獲得してきたにもかかわらず,社会に適応するための準備をするという意味では無理をしなくてはならないという矛盾を抱えていた。つまり,今の自分にとって社会復帰が実現不可能かもしれないという自己評価の低下が,リハビリテーションなどへの活動意欲の低下へとつながり,現実には長期入院に至っていた。3)患者は,自分自身が入院に至った理由を病気によるものではなく,家族や周囲の理解不足や,生育暦に影響された自分の対応のまずさにあったと考え,さらに長期入院という経過そのものが,自分の社会適応を阻んでいるとしていた。同時に,過去の体験を消化できず,未だに拘り続ける自分を認識しており,それが現実レベルの不安となり,対処方法が見つからずにいた。4)患者は,面接を通して,病院の中でどのような生活をし,どのような体験をしてきたかを言語化することによって,客観的に自己を捉え,現状を認識し,洞察することができたと考えられた。

症例は36歳女性.主訴は乾性咳嗽,労作時息切れ.X-8年頃より左下腿腓骨部に皮下腫瘤を自覚したが放置.X-4年8月に顔面神経麻痺,右眼瞳孔散大が出現し,PSL 40 mg内服により約1 ヶ月で治癒.その後,原因不明の頭痛,難聴,嗅覚異常が出現.X-2年11月頃より乾性咳嗽および軽度の労作時息切れが出現.X-1年4月の健康診断にて胸部異常陰影を指摘され全身精査の結果,全身性サルコイドーシス(肺,副鼻腔,神経,皮膚,肝臓)と診断された.挙児希望およびステロイド恐怖症のため,吸入ステロイドを約1年間使用したが,自覚症状の改善は得られず中止.当科紹介後にご本人の希望で漢方薬(人参養栄湯7.5 g/日,桂枝茯苓丸加薏苡仁9 g/日)を開始したところ,開始4 ヶ月後より咳嗽および労作時の息切れはほぼ消失,開始6 ヶ月後には,胸部画像所見,呼吸機能検査所見の改善を認めた.その後も本治療を継続し,現在まで3年間にわたり病状は安定しており,再燃は認めていない.

本研究の目的は、アメリカの作曲家・ピアニストであるフレデリック・ジェフスキーの≪「不屈の民」変奏曲≫の楽曲構成を理解し、テーマと36の変奏曲の分析をしながら、シンプルな歌の旋律を複雑な現代曲に変奏していく過程を明らかにすることである。また、各変奏曲の多面的な探究を通して、曲に込められたメッセージが楽曲にどのように表現されているかを考察する。原曲の「不屈の民」はチリの民主化運動の時にセルヒオ・オルテガによって作曲された革命歌である。ジェフスキーはこの曲のメロディをテーマとして、36のピアノ変奏曲としてまとめた。楽曲構成としては、異なる音楽的特徴を持つ変奏曲が第1~第5ステージまで発展し、第6ステージではそれまでのすべての要素を要約していることが確かめられた。また、各変奏曲の分析結果から、サイクル5以外の変奏曲は三つの共通した音楽的特徴、①短2度と完全5度のモチーフ、②半音階や全音階のバス進行、③五度圏の調の動き、を持つことが分かった。本研究の結果から、三つの音楽的特徴が様々な要素を結びつける役割を果たし、巧みな変奏技法で長大な曲を一つに統合する過程を検証することができた。ジェフスキーはこれらのテクニックを使って、様々な人々が集まり、対立や衝突を経て団結し、新たな権力に抵抗していく姿を音楽で見事に表現したと言える。日本の演奏家や聴衆にとって、本研究がジェフスキーや彼の作品を理解するための一助となることを期待する。 This study examines The People United Will Never Be Defeated! composed by Frederic Rzewski, a prominent American composer-pianist, through an investigation of overall structure of the work and an analysis of the theme and 36 variations. This examination reveals the complexity and diversity of the work and how this complexity is related to the power of uniting the diverse groups of people. The theme is based on a Chilean revolutionary song,“¡El pueblo unido, jamás será vencido!” (The People United Will Never Be Defeated!), composed by Sergio Ortega, and Rzewski composed 36 variations from this theme. It is clear that Rzewski achieves an incredibly elaborate formal plan for the entire work and the momentum from the first stage to the sixth stage represents how the Chilean people united to oppose the new power. An analysis of the theme and variations reveals that the whole music except for cycle 5 has three common musical features: the minor 2nd /perfect 5th motive (6-20 hexachord according to Allen Forte’s classifications), descending diatonic/chromatic bass line, and the circle-of-fifths sequence of the harmonic progression. These musical elements are used both in tonal and atonal variations so that the composer manages to maintain a certain unity through the lengthy work. This work is a wonderful gift from the twentieth century and a landmark in the history of variations. The present study has aimed to introduce Rzewski’s piano works to pianists and audiences in Japan, and give them the opportunity to perform/understand his pieces.

This paper aims to verify whether Japan's commitments in the Trans-Pacific Partnership Agreement are consistent with the Liberal Democratic Party's manifesto and the Diet resolutions. The former is defined as a request to exempt any agricultural product from tariff elimination, while the latter to exempt all five sensitive agricultural products from any commitment. The paper concludes from an analysis of Japan's tariff schedule that, whereas the manifesto is observed, the resolutions are not observed since the share of the five agricultural products exempted from any commitment is 26 and zero percent, respectively, based on a narrow and broad tariff classification.
