Anaplastic carcinoma arises from differentiated carcinoma and generally shows a dire prognosis. Anaplastic transformation may occur not only in primary tumors but also in metastatic lymph nodes. We encountered 5 cases of papillary carcinoma showing anaplastic transformation in lymph nodes that were curatively resected. Patient ages ranged from 67 to 85 years. Two of these patients showed anaplastic transformation at the initial surgery and the remaining 3 showed anaplastic transformation after repeated recurrence to the lymph nodes. After resection of anaplastic lesions of the nodes, 2 patients underwent radiation therapy, whereas the remaining 3 did not receive any adjuvant therapy. One patient died of rapid growth of lung metastasis 5 months after the resection. One patient died of carcinoma 63 months after surgery. Two patients have survived to date, 6 and 85 months after resection, respectively. The remaining one patient died of heart failure 11 months after surgery. It is therefore suggested that long-term survival can be expected for patients with differentiated carcinoma showing anaplastic transformation in the lymph node if the lesions can be curatively resected.

ルネサンス期声楽ポリフォニーの演奏において,我々は通常,近・現代の指揮法を用いる。これはバロック,古典派,ロマン派の各時代の音楽を支配した拍節的リズムを基盤として19世紀に確立せられたものである。一方。ルネサンス期の教会音楽を中心とした声楽ポリフォニーは拍節を持たず,グレゴリオ聖歌における朗唱的自由リズムの基盤の上に成立,発展したものである。従って,近・現代の指揮法はルネサンス・ポリフォニーとはその拠って立っところの基盤の遠いにおいて相容れないものと考えられる。本論ではこの観点に立ち,ルネサンス期声楽ポリフォニーの演奏においてグレゴリオ聖歌の指揮法であるところのキロノミーの原理を導入することを提唱する。In performing the vocal polyphony music of Renaissance,We generally use modern technique of conducting. It was founded in the 19th century, based on a rhythmic mode of baroque, classical, and romantic music,which are called《Metrical music》. On the other hand,polyphony music in Renaissance does not have metre, and mesure and its mode of rhythm is based on《Le rythme libre ortoirea》in Gregorian chant. Consequently, the modern technique ofconducting is against the characteristics in polyphony music of Renaissance. In this essay, I aim to introduce《Chironomie》, a mothod of conducting Gregorian chant, into the interpretation of vocal polyphony music of Renaissance.

The purpose of this article was to review the representative research findings on the effects of walking in water on health-related responses from multiple points of view to elucidate the beneficial effects of walking in water on health for older people. Research findings on positive effects of walking exercises in water were identified and discussed in terms of biomechanics, cardiorespiratory responses, body composition, and brain circulation compared with walking exercise on land in young and older adults. Collectively, it was suggested that walking exercise in water would be a recommended form of exercise for older adults, especially with the use of walking poles (i.e. aqua-pole walking).

Under the pressures of the criteria for the EMU, Italian administrative and fiscal reforms have been directed by the Center-Left governments and technocrats during the 1990s. They have obtained legitimacy through the tripartite agreements ("social pacts") signed by the Government, the employers' federations and the labor unions.<br>Despite many provocative statements made by "Eurosceptic" ministers, Berlusconi Government has syntonized the domestic labour market reforms to the EU's targets of occupation rate, following the "White Paper on Labour Market" drafted by the labour law experts including prof. Biagi, the assassinated "pro-Europe" consultant of the minister of Labour. The focus of the arguments is the Article 18 of Labour Rights Act, which guarantees rehiring and compensations to workers dismissed without just cause. The new social pact, the "Pact for Italy", which partially loosens the rigidity of the Article 18, was signed by the Government and major social partners except the CGIL, Italy's largest left trade union. This means a partial collapse of the "concertation" of the 1990s.<br>The "Pact for Italy" also demands to augment domestic investment especially to the Mezzogiorno, the less developed southern half of Italy. Between the domestic pressure to public spending and the external requirements of the EU's Stability and Growth Pact, Italian Center-Right government must seek a difficult balance of public finance. The Center-Left opposition is missing the opportunities to indicate the alternative way for reforms by the internal split between the moderate center parties and the Left activists of the "girotondo" (human-chain demonstration) movement.

大規模システムの管理者の負担を軽減するため,統合監視ソフトウェアによる監視環境の自動構築と収集した監視情報に対して統計解析手法を適用できるシステムの試作を行った.監視情報の収集では,情報の改ざんやなりすましを防止することも考慮し,電子証明書による暗号化を施した.また,大規模なシステムでの利用を想定し,登録するICT機器の数に依る性能評価を行った.

2006年のスウェーデン選挙は,3期12年ぶりの政権交代によって注目された。その際,中道右派4党の勝利を可能にしたのは,前例のない周到な選挙協力であった。本稿では,(1)それに先立つ社民党政権期に,連立政権ではなく文書化した政策合意に基づいて多数派を確保しようとする「契約主義的議会政治」が現れたこと,(2)それが右派の結集を促し「選挙連合の政治」を生んだこと,(3)2006年での右派の勝利が今度は左派3党を次期選挙に向けた事前協力に踏み切らせたこと,を見ていく。その上で,選挙を前に左右両陣営が政権構想を固めて有権者の支持を奪い合う現在の状況は,同国の伝統的な「ブロック政治」の枠組みを越え出るものであることを指摘し,政党自身の変化や有権者の投票行動の変化にも言及しながら,この10年余りの間に同国の政党政治と選挙をめぐって構造変動が起きていると結論づける。

甲状腺手術後の副甲状腺機能低下症における臨床的問題は低カルシウム血症とその関連事象である。副甲状腺ホルモン不足による副甲状腺機能低下症に対する治療は,活性型ビタミンD3製剤の投与が主体となる。血清カルシウム濃度は,ある程度の過換気状態でも低カルシウム血症による自覚症状が生じないレベル(補正カルシウム濃度8.0mg/dL程度)を維持する。尿路結石症や腎機能低下のリスクを抑えるために,随時尿のカルシウム(mg/dL)/クレアチニン(mg/dL)比を0.3以下にする。高カルシウム尿症を生じる場合や尿路結石の合併例には,サイアザイド系利尿薬の併用を検討する。副甲状腺機能低下症患者に副甲状腺ホルモン製剤を併用した場合に,活性型ビタミンD3製剤の必要量の減少,および尿中カルシウム排泄の改善が報告されている。副甲状腺ホルモン不足による副甲状腺機能低下症を副甲状腺ホルモン製剤で治療することは今後の検討課題である。

症例は65歳男性である.急速進行性の認知機能低下で発症した.頭部MRIのFLAIR/T2強調画像で大脳皮質下に広汎な高信号域を認め,髄液よりJohn Cunningham virus-DNAを検出した.最終的に脳組織所見から進行性多巣性白質脳症と確定診断した.合併症として高安動脈炎と慢性型/くすぶり型成人T細胞白血病が認められた.発症早期にメフロキンとミルタザピンによる治療を開始したが,症状・画像共に改善なく約半年後に死亡した.本例において治療効果が認められなかった理由としては,HTLV-I感染に加えて高安動脈炎によるB細胞系の異常が影響した可能性を考えた.

1/f noises, the power spectrum density (PSD) of signals inversely proportional to the frequency, have been observed in various types of physical and physiological systems; e.g. current and heart beat. 1/f noise is suggested to be generated by superimposing stochastic processes. Among stochastic processes, autoregressive process is suitable for analyzing physical and physiological systems in that the autoregressive process is time-discrete. A time-discrete model is suitable for analyzing those systems because those systems are observed in time-discrete manner as well. However, it is obscure whether 1/f noises are generated by superimposing those autoregressive processes. In this study, first order autoregressive (AR(1)) processes were superimposed with varied the process parameter and a driving noise. As a result, PSD of superimposed time sequences was inversely proportion to the frequency when the process parameter and a driving noise were a uniformly distribution and white noises, respectively. This result suggests that 1/f noises could be generated from the superimposing AR(1) processes when the process parameter is distributed uniformly.
