薬剤師の集中治療室定数配置薬の在庫管理業務による経済効果は十分に明らかにされていない。そこで,われわれは定数の過不足・破損が報告された薬剤数および損失金額について薬剤師が集中治療室に常駐する前(2016年9月1日〜2018年8月31日)と後(2018年9月1日〜2020年8月31日)を比較して,定数配置薬の管理状況が改善するのかを調査した。 定数の過不足・破損が報告された薬剤数は薬剤師常駐後に329個から229個まで30.4%減少し,損失金額は192,910円から110,090円にまで42.9%減少した。ヒューマンエラーによる破損よりも管理状況に由来する定数の過不足を理由に報告された薬剤数が著しく減少した。薬剤師が集中治療室において定数配置薬の在庫管理業務を開始した後は,定数配置薬の管理状況が改善し,薬剤の定数不足や破損による経済的損失が減少した。

The discoveries of thienamycin and related compounds, potent and broad spectrum β-lactam antibiotics, have led to interest in the synthesis of compounds with a 1-carbapenem and 1-carbapenam ring system. Advances in the syntheses and synthetic approaches of 1-carbapenems and 1-carbapenams, which have been published from June 1980 to August 1982, are reviewed according to the synthetic methodology of these novel ring systems.

症例は69歳,男性。併存症は慢性腎不全(維持透析),洞不全症候群(DDD型ペースメーカー留置)。コハク酸シベンゾリン(以下CZ)を約1カ月間内服後に倦怠感と不穏が出現した。来院時,ペーシング不全を伴う高度徐脈と低血糖,高カリウム血症を認めた。高カリウム血症を是正後もペーシング不全と低血糖が遷延したため,CZ中毒と判断して第3,4病日に血液濾過透析(以下HDF)を施行し,以降,ペーシング閾値は低下して低血糖も改善した。初診時の血中CZ濃度は1,294ng/mLと高かったが,各HDF前後で血中CZ濃度は低下した。CZは血液透析による除去効率が低い薬剤とされるが,本症例と同様にHDFが薬物の除去や中毒症状の改善に寄与した報告が散見される。本症例のようにペーシング不全により高度の循環障害を伴うCZ中毒では各臓器での薬物クリアランスの低下も予想されるため,HDFが奏効する可能性があると考えられる。

Ophidiomycosis is an emerging infectious disease caused by the fungus Ophidiomyces ophiodiicola, which has been affecting wild and captive snakes in North America, Europe, and Australia. We report 12 cases of suspected ophidiomycosis in captive colubrid snakes in Japan. Pathological and microbiological examinations were performed, and the results confirmed the diagnosis of ophidiomycosis in two snakes, which indicated that the remaining sympatrically raised snakes also had ophidiomycosis since they exhibited similar lesions. This is the first report of ophidiomycosis in Asia caused by O. ophiodiicola. To prevent the expansion of ophidiomycosis in the natural environment in Japan, there is a need to evaluate the ophidiomycosis carrier status of imported snakes, the pathogenicity of the infection in native snakes, and the prevalence and distribution of O. ophiodiicola in wild and captive snakes. Measures also must be taken to prevent endemicity globally.

This study investigated whether there are differences in bystander behavior when bullying is witnessed. We evaluated the number and types of bystander behaviors by means of the OBVQ, and then examined the psychological characteristics of the different bystander behaviors. 269 Japanese elementary school students and 503 junior high school students completed questionnaires assessing empathic concern, peer conformity, moral disengagement, authenticity and satisfaction in classes, global self-worth, and how they would respond if they observed a peer being bullied. The results of a multinomial logistic regression analysis and ANOVA demonstrated that there would be some types of bystander behavior with common characteristics such as empathic concern and developmental differences. Moreover, the results also showed that two types of behaviors, of which one indicated the most negative psychological state, might be caused by peer conformity. Other behaviors might be caused by moral disengagement and contain some subordinate modes owing to regulatory effects of peer conformity, such as pretending to be unaware of bullying. The practical implications of bystander behavior are discussed.

窒素雰囲気下で熱処理されたスギ辺材およびアカマツ辺材の耐朽性, 耐蟻性および吸湿性を検討した。熱処理の温度は140~240℃で, 熱処理の時間は24時間とした。スギ辺材の熱処理木材から木口断面20mm×20mm, 繊維方向10mm の試験片を切り出して, オオウズラタケおよびカワラタケによる12週間の強制腐朽試験を実施したところ, 処理温度が高いほど腐朽による質量減少率は低下し, 220℃および240℃の処理で質量減少率は1%以下となった。また, ファンガスセラー試験においても, 12カ月経過時点で, 無処理試験体の腐朽度が5となっても, 220℃および240℃処理試験体の腐朽度は0のままであった。一方, 室内耐蟻性試験では, 240℃処理試験体のイエシロアリ食害による質量減少率は11%であり, 耐蟻性は十分ではなかった。JIS K 1571: 2004に準拠した野外試験の結果, 奈良県森林技術センター明日香実験林におけるスギ辺材熱処理材の腐朽とヤマトシロアリによる被害も, また鹿児島県日置市吹上浜におけるアカマツ辺材熱処理材のイエシロアリによる被害も, 処理温度が高いほど軽減されることが明らかになった。高い生物劣化抵抗性を示す熱処理木材は, 吸湿性も大きく低下しており, 両者に密接な関係があることが示唆された。

Novel peptides derived from chicken collagen hydrolysate (CCH) were confirmed to have a vasoprotective effect after ingestion and absorption to vessels; however, their endovascular mechanism of action required clarification. Therefore, using human umbilical vein endothelial cells (HUVECs), in which inflammation had been induced by tumor necrosis factor-α (TNF-α), we investigated the mechanism by which CCH-derived peptides inhibited the vascular inflammatory response. Expression of soluble intercellular adhesion molecule-1 (sICAM-1), soluble vascular cell adhesion molecule-1 (sVCAM-1), and interleukin-8 (IL-8) in HUVECs was increased significantly by the addition of TNF-α, whereas treatment with proline-hydroxyproline (Pro-Hyp) and hydroxyproline-glycine (Hyp-Gly), which are present in human peripheral blood after ingestion of CCH, inhibited these increases. These peptides also reduced the mRNA expression of inducible nitric oxide synthase (iNOS) up-regulated by TNF-α. These results suggest that the vasoprotective effect of CCH-derived peptides is due to inhibition of endothelial inflammation.

Inflammasomes are multiple protein complexes that serve as molecular platforms to activate caspase-1 and regulate maturation of a potent proinfl ammatory cytokine, interleukin (IL)-1β, as well as proinfl ammatory cell death, pyroptosis. Although several types of inflammasomes have been reported so far, recent investigations indicate that the NLRP3 inflammasome recognizes non-microbial danger signals and leads to sterile inflammatory responses in various disease conditions. Sterile inflammatory responses are also implicated in the development of myocardial infarction (MI). In particular, IL-1β is an early and prominent mediator of inflammatory responses in MI, suggesting the pathophysiologic role of NLRP3 inflammasomes in MI. This review highlights the current state of knowledge regarding the role of NLRP3 inflammasomes in MI.

本論は,松下電器において真野善一が目指したデザインマネージメントについて,主として真野の発言,記述とヒアリング調査より,以下の内容を明らかにした。1) 真野は,様々な場面を利用して,経営幹部,関連部門に対してデザイン啓蒙の発言をした。2) 施策としては,デザイン協議会, デザイン研究グループを発足させ,初のデザイン方針発表会を開催した。3) 真野の施策はデザイン力強化とそのためのデザイン組織の一元化を目指したが,会社の理解を得ることは難しく,デザイナーは各事業部に分散した。4) 真野は,造形こそ主たるデザインマネージメントでありデザイン評価に繋がるとして取り組んだが,会社はデザイン組織マネージメントを求めていた。

In cancer chemotherapy, it is very important to take into account the patient’s background. In recent years, a simple suspension method has attracted increased attention as a method that prevents changes in the stability and safety of various drugs. However, of 135 oral anticancer drugs, only 28 have been examined using this method, as of April 2013. In this study, we carefully investigated whether 53 oral anticancer drugs could be adapted to the simple suspension method, except for the 28 drugs that had already been previously reported. The results showed that most of these oral anticancer drugs could be adapted to the simple suspension method. Of seven drugs that were not adapted, six were generic drugs. In addition, it was clear that the evaluation of bicalutamide tablets was significantly different from our expected results. In conclusion, we were able to qualitatively assess all 53 oral anticancer drugs. This is equivalent to half of 107 untested drugs. These results provide useful information to cancer patients using oral anticancer drugs prepared using the simple suspension method.

It has been reported that perivenous cystic structures near the parasagittal dura are associated with the leakage of gadolinium-based contrast agents at 4 hours after intravenous administration. The origin of such cystic structures remains unknown. While reading many cases of MR cisternography, we noticed that some of the cystic structures appeared to connect to the perivenous subpial space. This new imaging finding might facilitate future research of the waste clearance system for the central nervous system.

チトクロムP-450(CYP)を中心とした前臨床試験スクリーニング系の発達により,第II相代謝酵素で触媒される候補化合物が増加傾向にあると言われている.ヒトにおける代表的な第II相代謝酵素は,グルクロン酸抱合酵素(UGT)であり,近年多くの研究成果が集積されてきている.しかし,CYPと比べ前臨床試験スクリーニングへの応用は進んでいない.UGTのヒトin vivo代謝反応の予測系の確立は,UGTの様々な特性が原因で進展していない.特異的阻害薬が無いこと,活性化が認められること,さらに抱合代謝物によるUGT阻害などが試験系を難しくしている.さらに,種差および肝外臓器における情報は極めて少ない.グルタチオン抱合や硫酸抱合代謝物は,排出型トランスポーターの影響を受けるが,体内動態に及ぼす影響の検討が必要である.今後,CYP等の第I相と第II相酵素反応を同時に考慮できる評価系の構築が期待されている.

下顎隆起は骨性の隆起で,原因としては,遺伝,咀嚼圧,パラファンクションなどが挙げられるが,明確なことは分かっていない.今回,下顎隆起と咬合力およびパラファンクションとの関係を検討するために,当科受診の患者に対し,デンタルプレスケールを用いて最大咬合力,咬合接触面積を測定するとともに,パラファンクションに関するアンケートを実施した結果,下顎隆起の有無と最大咬合力との間には統計学的有意差が認められた.また,下顎隆起の有無と咬合接触面積との間にも統計学的有意差が認められた.アンケートの結果より,パラファンクションの中でもくいしばりおよび硬性食品嗜好と下顎隆起との間に統計学的有意差が認められたものの,歯ぎしりや筋痛およびストレスについては関連性は認められなかった.
