[Purpose] Electromyography biofeedback therapy is applied to various diseases during physical therapy for motor learning. Our aim was to develop a low-cost electromyography biofeedback device kit that students could build by themselves in class and to evaluate whether this kit was an adequate educational tool for physical therapy students. [Subjects and Methods] This study included 34 volunteers from a 3 year physical therapy vocational school and a 4 year university course. All students were tasked with: 1) completing a pre-questionnaire; 2) attending basic lectures on electromyography biofeedback therapy and how to build the device; 3) building the device; and 4) completing a post-questionnaire. [Results] Mean time required to build the device was 50.8 ± 34.1 minutes for the vocational course students and 27.4 ± 8.8 minutes for university students. For both groups, there was a significant increase in the number of students stating “have an interest in,” “be involved with,” and “have knowledge of electromyography biofeedback” when pre- and post-questionnaires were compared. In the post questionnaire, the frequency of the “knowledge increased” response was higher. [Conclusion] Physical therapy students can easily build our new device in a classroom, and this kit is a useful educational tool to motivate learning in electromyography biofeedback.

[Purpose] The aim of this study was to compare immediate effects between new medical stretching (NMS) and conventional medical stretching (CMS) techniques on Hamstrings flexibility. [Subjects and Methods] Thirteen healthy adult males, with finger floor distance (FFD) less than zero centimeter, without known musculoskeletal and neurological impairment in spine or lower extremities, were included. The subjects were randomly allocated to two groups. The subjects were instructed to perform NMS and CMS (hold for 30 seconds once, twice for each side of lower extremity) for both sides (total two minutes, only one session for one day). The interval between the two techniques was one week. FFD was measured with digital standing trunk flexion meter at the pre-intervention and post-intervention of both techniques. [Results] The mean values of FFD improved at the post-interventions of both techniques. The tests of within subject effects indicated that the main effect of treatment was not significant but the main effect of time was significant and the interaction of treatment and time was also significant. [Conclusion] The results of this study indicated that both medical stretching techniques were effective on Hamstrings flexibility immediately after the intervention and NMS technique was more effective on improving flexibility.

To investigate the influence of briquette size and density on the combustion behavior in both flaming and char combustion stage, the combustion experiments with the highly densified columnar briquette made from cypress sawdust were carried out. Three sizes of the briquette, φ30 mm×60 mm, φ35 mm×70 mm and φ40 mm×80 mm, of which diameter/length ratio is kept constant at 1/2 and three types of the briquette density, 900, 1100 and 1300 kg/m3 were employed. The water content of the briquette was kept around 10 wt%. The mass loss rate and the duration in both combustion stages were measured. In the flaming combustion stage, it is observed that the average mass loss rate per unit specific surface area is proportional to the briquette surface area, while it doesn't depend on the briquette density. It is expected by considering Fourier number that the flaming combustion duration is proportional to the product of the inverse square of the specific surface area and the briquette density and it is shown that the experimental results correspond to the expectation. It is found that the relationship between normalized mass loss rate and normalized time is nearly the same regardless of the briquette size and density. In the char combustion stage, normalized mass loss can be approximately expressed with the shrinking core model by assuming the effective oxygen diffusion coefficient in the ash layer for each briquette density used in this study. And it is found that the effective oxygen diffusion coefficient in the ash layer obtained by the shrinking core model decreases with increasing the briquette density.

To investigate the permeability of deep dentin following immediate sealing with different etch-and-rinse and self-etch adhesives (Single Bond 2, Adper Prompt L-Pop, Clearfil Protect Bond, Clearfil S3 Bond, G-Bond) and a dentin desensitizer (Gluma). Fluid-transport model was used to measure fluid conductance during and after application of adhesives. Polyvinylsiloxane impressions of bonded dentin were taken to monitor fluid transudation from the surface of the adhesives. The area and number of dentinal fluid droplets and/or blisters were calculated using image analysis. None of the adhesives were able to block fluid conductance completely. The fluid conductance values of the adhesives displayed the following statistical ranking (p<0.05): G-Bond≤Clearfil Protect Bond<Smear-layer-covered dentin<S3 Bond≤Single Bond 2≤Adper Prompt L-Pop<Gluma Desensitizer<Acid-etched dentin. Highly significant correlation was observed between the permeability of the tested adhesives and the area fraction of fluid droplets/blisters on the adhesive surfaces (r=0.99, p<0.01).

IoT(Internet of Things)とCPS(Cyber Physical Systems)は共に,物理的な世界とサイバー空間とを融合させることで,世の中に新しい価値を生み出す。本記事では,まず,IoTとCPSに関して,国内外の企業や政府等における具体的な事例を紹介する。次に,第5期科学技術基本計画において,その実践が提唱された「超スマート社会」(Society 5.0)と,その実現に向けたプラットフォームの概要について述べる。さらに,その基盤となる技術の体系化を行い,CRDS(研究開発戦略センター)が作成した研究開発の俯瞰報告書を踏まえて,今後の研究開発課題について述べる。

Ehlers-Danlos syndrome, hypermobility type (EDS-HT) is unexpectedly common and is associated with a high rate of gastrointestinal manifestations. We herein report the first documented case of mobile cecum associated with EDS-HT. A 21-year-old woman with repeated right lower abdominal pain was initially diagnosed with EDS-HT. Abdominal examinations performed in the supine position, such as CT and ultrasonography, showed no gross abnormalities. In contrast, oral barium gastrointestinal transit X-ray images obtained with changes in the patient's body position revealed position-dependent cecal volvulus with mobile cecum. She was finally discharged with a dramatic resolution of her symptoms after laparoscopic cecopexy for mobile cecum.

Background:Up to 25% of patients with ST elevation myocardial infarction (STEMI) have ST segment re-elevation after initial regression post-reperfusion and there are few data regarding its prognostic significance.Methods and Results:A standard 12-lead electrocardiogram (ECG) was recorded in 662 patients with anterior STEMI referred for primary percutaneous coronary intervention (PPCI). ECGs were recorded 60–90 min after PPCI and at discharge. ST segment re-elevation was defined as a ≥0.1-mV increase in STMax between the post-PPCI and discharge ECGs. Infarct size (assessed as creatine kinase [CK] peak), echocardiography at baseline and follow-up, and all-cause death and heart failure events at 1 year were assessed. In all, 128 patients (19%) had ST segment re-elevation. There was no difference between patients with and without re-elevation in infarct size (CK peak [mean±SD] 4,231±2,656 vs. 3,993±2,819 IU/L; P=0.402), left ventricular (LV) ejection fraction (50.7±11.6% vs. 52.2±10.8%; P=0.186), LV adverse remodeling (20.1±38.9% vs. 18.3±30.9%; P=0.631), or all-cause mortality and heart failure events (22 [19.8%] vs. 106 [19.2%]; P=0.887) at 1 year.Conclusions:Among anterior STEMI patients treated by PPCI, ST segment re-elevation was present in 19% and was not associated with increased infarct size or major adverse events at 1 year.

【目的】 味噌汁は加熱によって匂いが劣化するが、焼き味噌は風味が向上するため、焼きおにぎりなどさまざまな調理で利用されている。そこで本研究では、味噌は高温加熱により低温とは異なる新しい特有香気成分が生成し、風味の向上に寄与すると考え、その組成の変化を明らかにすることを目的とした。さらに、味噌を焼くことによる官能評価への影響も検討した。【方法】 味噌を150℃・200℃で加熱し、三点比較法によって官能評価を行い、官能的に識別できるか検討した。次に、焼き味噌のヘッドスペース(Headspace)に存在する香気成分を固相マイクロ抽出(Solid Phase Micro Extraction)法で抽出し、GC、GC-OおよびGC-MS分析によりその組成を明らかにした。【結果】 官能評価では、150℃と200℃で加熱した味噌を1%の危険率で有意に判別できた。また、正解者のうち78%が150℃で焼いた味噌が好ましいと評価した。GC、GC-MS分析の結果、200℃で10分加熱した味噌では未加熱味噌と比較して増加したピークは22種、減少したピークは10種であった。GC-O分析では同じ試料で51種のにおいが感知された。このうち、未加熱味噌と比較すると増加または新しく生成した香気は34種あり、減少した香気は23種であった。今後は特に新しく生成または増加した香気成分の特定を試みる予定である。

In psychotherapy of “gender identity disorder” in Japan, psychiatrists assess the “sex of mind” of their clienteles. In this process, medicine first treats the invisible “sex of mind” as more essential than the visible sexed body, then as visible in the clientele's looking, behaviors, talks etc. The aim of this article is to examine how medicine treats the sex category according to this paradoxical logic, using the review of medical discourses and scripts based on interviews of “gender identity disorder” clientele. Through this process of description of the medical practice, it is showing how, by seeing the clientele's display as trans-situational clues of the “sex of mind”, this practice may be actually seeing very “sex of mind”.

日本のポップミュージックやファッション,キャラクター,漫画やアニメなどを含む『かわいい文化』が海外で注目されており,国が推進する『クール・ジャパン戦略』の主要なコンテンツにもなっている.しかし,現在これらの大半は海外が主体となって広がっており,日本人自らが認知・発信できていない.先行研究では,かわいい文化の定義や情報発信の必要性は言及されているが,文化産業として発展させていくには,かわいい文化の起点や普及の仕方を明確にしておく必要がある.よって本研究では,かわいい文化が浸透していく際の『情報』に着目し,コンテンツごとに普及過程を調査し,そのプロセスや伝達手段,情報間の因果関係を明らかにした.

Spinal and bulbar muscular atrophy (SBMA), also known as Kennedy disease, is an adult–onset neuromuscular disease caused by an expansion of a CAG triplet–repeat within the first exon of the androgen receptor gene. Recent studies have indicated that SBMA is not a pure neurodegenerative disease, but affecting various non–neuronal tissues, including skeletal muscle. Furthermore, impairment in the interaction between motor neurons and skeletal muscles may be the key mechanism underlying the pathophysiology of SBMA. In the presence of testosterone, a disease–causing abnormal AR protein accumulates in the brainstem, spinal anterior horn, skeletal muscle, and several peripheral organs ; this induces a variety of symptoms, such as bulbar and muscular atrophy, liver dysfunction, and arrhythmia, in patients with SBMA. Studies using animal models have resulted in the development of promising therapeutic strategies for SBMA, including 1) testosterone suppression, 2) mutant androgen receptor gene silencing, 3) activation of protein quality control, and 4) rescuing of mitochondrial function. Efficacy of physical therapy has also been tested in small–scale clinical trials. In this review, we describe the clinical features and pathogenesis of SBMA, and summarize the therapeutic drug targets developed on the basis of results from recent studies using the well–established mouse models for this neuromuscular disease. Strategies for the development of disease–modifying therapy supported by basic experiments as well as well–planned clinical trials may lead to a breakthrough in the therapy for SBMA.

背景: 扁桃周囲膿瘍は重症な合併症を生じる可能性から適切な対応を要する. 目的: 当地域の扁桃周囲膿瘍の原因菌と薬剤耐性を評価する. 化膿性扁桃炎治療後に扁桃周囲膿瘍へ進展した頻度と処方抗菌薬の関連を検討する. 対象と方法: 2010年4月から2016年3月の扁桃周囲膿瘍214例の前医抗菌薬を, また2010年4月から2014年3月の119例の原因菌と薬剤耐性を診療録から記述した. 次に2010年4月から2016年3月の化膿性扁桃炎397例の扁桃周囲膿瘍進展頻度について過去起点コホート研究を施行した. さらに化膿性扁桃炎, 扁桃周囲膿瘍既往有無での解析も施行した. 結果: 扁桃周囲膿瘍への前医処方はセファロスポリン系37% (31/84処方), キノロン系抗菌薬25% (21/84処方) であった. 膿汁培養ではレンサ球菌属67% (98/66株), 嫌気性菌13% (13/98株) であった. 全検出菌の45% (42/94株) がキノロン系抗菌薬に耐性を示した. 化膿性扁桃炎へのキノロン系抗菌薬処方群が他抗菌薬処方群と比較し, 治療後に扁桃周囲膿瘍へ進展した頻度が有意に高値 (リスク比11.8, p=0.032) であった. 既往がある症例の検討も同様の結果であった. 結論: 化膿性扁桃炎, 扁桃周囲膿瘍の既往がある化膿性扁桃炎へのキノロン系抗菌薬投与は慎重を要することが示唆された. 今後さらなる検討が望まれる.

In basketball, there is one of the intentional rules violations, which is called “foul game”. “Foul game” is the action that a defense player toward the end of a close game will intentionally foul players in possession of the ball in order to stop the clock. Although the dispute has so far been made from a viewpoint whether to be an act morally permitted to “foul game” or not, it has not resulted in the conclusion. The purpose of this study was to examine the point on right or wrong of “foul game” in basketball between Fraleigh and Simon who are regarded as renowned scholars in sport ethics with a fresh eye.There are three points in the controversy between Fraleigh and Simon.1. Role of the penalty for intentional fouls2. Importance of restorative skills3. Agreement of participants for intentional foulsIn this study, the three points at issue were examined by considering the contents of “official basketball rule” published in Japan Basketball Association. Especially, we analyzed the transition of the provisions of “intentional foul” and “unsportsmanlike foul” which have specified “foul game”. The results here are as follows.1.The role of the penalty for intentional fouls has shifted from the sanction for prohibited acts to the price for options.2.The importance of restorative skills by the free throw accompanying “foul game” has become more significant skills for which participants are asked in a game.3.The agreement of participants for intentional foul in “foul game” has moved turned to be acceptable.As we can see above, it has been evaluated as an act by which a “foul game“ is allowed in “official basketball rule”.

An algorithm for retrieving the macroscopic, physical and optical properties of clouds from thermal infrared measurements is applied to the Himawari-8 multiband observations. A sensitivity study demonstrates that the addition of the single CO2 band of Himawari-8 is effective for the estimation of cloud top height. For validation, retrieved cloud properties are compared systematically with collocated active remote sensing counterparts with small time lags. While retrievals agree reasonably for single-layer clouds, multilayer cloud systems with optically thin upper clouds overlying lower clouds are the major source of error in the present algorithm. Validation of cloud products is critical for identifying the characteristics, advantages and limitation of each product and should be continued in the future. As an application example, data are analyzed for eight days in the vicinity of the New Guinea to study the diurnal cycle of the cloud system. The present cloud property analysis investigates cloud evolution through separation of different cloud types and reveals typical features of diurnal cycles related to the topography. Over land, middle clouds increase from 0900 to 1200 local solar time (LST), deep convective clouds develop rapidly during 1200–1700 LST with a subsequent increase in cirrus and cirrostratus cloud amounts. Over the ocean near coastlines, a broad peak of convective cloud fraction is seen in the early morning. The present study demonstrates the utility of frequent observations by Himawari-8 for life cycle study of cloud systems, owing to the ability to capture their continuous temporal variations.

To understand sleep mechanisms and develop treatments for sleep disorders, investigations using animal models are essential. The sleep architecture of rodents differs from that of diurnal mammals including humans and non-human primates. Sleep studies have been conducted in non-human primates; however, these sleep assessments were performed on animals placed in a restraint chair connected via the umbilical area to the recording apparatus. To avoid restraints, cables, and other stressful apparatuses and manipulations, telemetry systems have been developed. In the present study, sleep recordings in unrestrained cynomolgus monkeys (Macaca fascicularis) and common marmoset monkeys (Callithrix jacchus) were conducted to characterize normal sleep. For the analysis of sleep–wake rhythms in cynomolgus monkeys, telemetry electroencephalography (EEG), electromyography (EMG), and electrooculography (EOG) signals were used. For the analysis of sleep–wake rhythms in marmosets, telemetry EEG and EOG signals were used. Both monkey species showed monophasic sleep patterns during the dark phase. Although non-rapid eye movement (NREM) deep sleep showed higher levels at the beginning of the dark phase in cynomolgus monkeys, NREM deep sleep rarely occurred during the dark phase in marmosets. Our results indicate that the use of telemetry in non-human primate models is useful for sleep studies, and that the different NREM deep sleep activities between cynomolgus monkeys and common marmoset monkeys are useful to examine sleep functions.
