Discussions about fictional names provide us an interesting perspective when we try to make clear important notions in the philosophy of language, such as sense and reference, because, so far, few theories have been successful in treating fictional names.<BR>In this paper, I first point out that fictional names raise problems for two influential views in the philosophy of language, namely, Mill's view that proper names do not have meaning but only refer to their object, and Frege's view that proper names do have both reference and meaning(or sense). And then we shall examine three options to overcome the difficulties, namely, the predicate view, Walton-Evans' make-believe theory, and Thomasson's theory of dependent abstracta. Each view has its own merit and demerit when sentences/statements containing fictional names are divided into two types: internal and external. Internal statements express what is the case in a story(say, 'Sherlock Holmes is a famous detective)and external statements express what is the case from the real-world point of view(say, 'Sherlock Holmes is created by Conan Doyle'). I would like to propose that the make-believe theory would be the best option for internal statements and Thomasson's view for external statements. But what about statements that are both internal and external at the same time, such as 'Sherlock Holmes is a detective created by Conan Doyle'? This problem is discussed in§4 and one option is proposed there.

It is the representative example that the castle walked it with the history of the modern cultural properties protection as a historic spot in the modern times, and it is the example of the park of historic interest in modern times, but there are extremely few places where it may be said that the maintenance was over.This study considered it about the historical park and the change of the use patterns that the change of a preservation and the idea of practical use, the characteristic performed in a castle in the modern times before the full-scale business of "preservation and maintenance about the historic spot" of 1965 enforcing it after the abandoned castle of modern times castle appointed to a history of nation trace.As for the reconstruction maintenance, as for the element of the point such as the building, the space of the circle that used the reconstruction as a garden of the parks moves to the reconstruction of an open space and the building, but suitable correspondence is late by the reason of the use as the park about the tree in the castle. In addition, it is clear to be the obstacle which is covered the scene as the historic spots such as Ishigaki or the building and the tree which became higher, and fail even if it planted a tree by a plan.

本研究の目的は、小学生児童(5、6年生)を対象とした抑うつ低減のための認知行動的プログラムの効果を検討することであった。217名(10、11歳)の児童がスクールベイスドの介入群と統制群に割り当てられた。プログラムの内容は、多様性のあるコーピング、リラクゼーションを習得することを目的とした。対象児は、プログラムの実施前後に自記式質問紙を用いて抑うつとコーピングについて評価された。その結果、抑うつ症状について時期と群に有意差がみられ、もともと抑うつの高い児童において、プログラム前に比べ、プログラム後に抑うつの有意な低減がみられた。コーピング得点は、プログラム前に比べて、プログラム後のほうが有意に高かった。このことから、多様性のあるコーピングとリラクゼーションに焦点を当てたプログラムが、児童の抑うつの低減に対して効果的な技法であることを示された。最後に、本研究は児童の抑うつに対するスクールベイスドのプログラムとして、有効性が示唆された。

温水洗浄便座は,排便時における清潔維持,便秘や痔の予防・改善に役立つ器具として広く使用されている.我々は,温水洗浄便座局部洗浄水中の細菌,排便後の局部洗浄水の飛沫によって引き起こされる臀部・股間に付着する糞便細菌について調査し,温水洗浄便座の使用に潜む問題について検討した.本研究結果により,温水洗浄便座局部洗浄水中の細菌生息が確認され,さらに一般家庭の局部洗浄水中生菌数は公共施設のものに比して約3倍多いことが明らかとなった.これは,補水が少頻度の一般家庭においては残留遊離塩素が揮発したためと考えられた.一方,排便後の局部洗浄より,臀部・股間における糞便細菌の付着が確認され,洗浄後に臀部において糞便細菌の飛沫付着が確認されたことから,医療従事者は,局部洗浄水利用者との接触感染に注意が求められる.

鼻に発生した痛風結節の1例を経験したので報告する。症例は53歳男性。鼻根部に徐々に増大する3cm大の腫瘤を認め,局所麻酔下に摘出術を施行した。摘出標本中に針状結晶とその周囲の異物炎が認められ,痛風結節と同様の病理組織所見であった。痛風結節は一般に急性関節炎の起きやすい足,膝,手指,肘関節に好発し,頭頸部領域では耳介にしばしば認められる。その他にも顎関節,胸鎖関節,脊椎,眼瞼,鼻,舌,喉頭,声帯などにまれに見出され,時には他の良・悪性腫瘍との鑑別診断に苦慮することもある。長期罹患中の高尿酸血症患者では,頭頸部腫瘤の鑑別診断として痛風結節を念頭に入れておく必要があるものと考えられた。

メニエール病に対し副作用の少ない治療法としてハーブティー療法 (HTT) を考案した.メニエール病患者 15 名に対し,日本めまい平衡医学学会の治療効果基準に基づき,HTT 開始 6 か月前から,めまい回数記録,純音聴力検査,能力低下および耳鳴の自覚的評価に関するアンケートを毎月行い,HTT 開始以降の 12 か月間と比較した.めまい回数は効果判定基準に基づきめまい係数を算出した.HTT 施行例のうち 10 症例が再発せず良好な経過をみせた.めまい係数では 75%が軽度改善以上を示した.聴力は全例で悪化を認めなかった.能力低下アンケートでは改善傾向を認め,耳鳴アンケートでも悪化を認めなかった.再発 5 例もより少ない投与回数で症状のコントロールが可能であった.HTT は効果が緩やかであるため急性発作期の治療には向かないが,緩解期の再発予防には十分な効果を発揮し,急性期でも他の通常医療薬との併用で効果を高めることが期待された.

【背景・目的】食物アレルギー児が安全な学校生活を送るためには学校生活管理指導表(以下,管理指導表)を用いた管理は必須である.しかし非専門医にとって必要な情報を過不足なく記載することはかならずしも容易ではない.そこで管理指導表の作成を支援するアプリ(以下,アプリ)を開発し,その診断性能を検証することとした.【方法】必要な問診項目が表示され,保護者がそれに回答することにより管理指導表の記載例が出力されるアプリを開発した.鶏卵,乳,小麦,ピーナッツのアレルギーのいずれかが原因食物の患者の問診票データをアプリに入力して出力された結果と当院アレルギー科医が記載した管理指導表を比較して,その一致度を検証した.【結果】94名の患者で解析を行い,一致率は食物アレルギー病型で100%,原因食物で90.4%であった.このアプリの運用により,非専門医でも容易かつ適切な管理指導表を作成することが期待できる.
