環境問題は、公害に始まり、資源・エネルギーの大量消費による地球全体の環境問題、更には、将来世代の権利に及ぶ問題へと空間軸・時間軸ともに拡張された。並行して持続可能な開発の課題も、環境と経済の両立から、環境が経済活性化の駆動力となるグリーン経済の実現、そして得られた富が公平に配分される社会の実現へと進化した。その集大成として国連総会で決定されたSDGsの意義としては、1)民主的なプロセスで作成された目標を基に各国が実施計画を策定したこと、2)多様な発展レベルの国々からなる現実世界にマッチした目標であること、3)目標の相互関係の理解が進んだこと、 4)地域コミュニティでのローカライゼーションが行われること、及び5)環境保全が開発の配慮事項ではなく、環境保全を推進することで持続可能な経済・社会の構築に繋がることが明確化されたことが挙げられる。

米国の医療は複雑化する医療制度や国民の健康問題,医療技術の高度化,医師不足,研修医の勤務時間の制限,診療報酬の削減,入院期間の短縮化,安全対策の強化のために,従来の医師中心の体制からNurse Practitioner(NP)やPhysician Assistant(PA)を含めた多職種チームによる体制へ移行しつつある。 そのなかでNPは主要なチームメンバーであり,裁量権をもち,従来の医師の業務を行なうことが多い。本稿では筆者がNPとして所属する大腸直腸外科における多職種チームの介入の例を紹介し,米国におけるNPの効果をケアの質,コスト,医療アクセスの面から考察する。 そして最後に,日本の特定看護師(仮称)の導入にあたり,医療施設における管理や教育の課題について触れる。

Autism spectrum disorder (ASD) is a heterogeneous neurodevelopmental disability that demonstrates impaired social interactions, social communication deficits, and restrictive/repetitive behaviors. The endocannabinoid (eCB) system in the brain is a major regulator of synaptic plasticity and neuromodulation. It is reported that the eCB system have been altered in children with ASD and some animal models of ASD. To determine the causal role of the eCB system in the ASD, we have investigated the relationship between declines of the eCB system and the ASD-like symptoms, using the cannabinoid CB1 receptor knockout (CB1KO) mice. We found that male CB1KO mice demonstrated reduced sociability (3-chambered social approach task), elevated repetitive grooming behaviors (hole-board test) and deficits in short-term memory (Y-maze test). Moreover, the CB1KO mice also showed emotional instabilities (elevated plus-maze test and hole-board test). The serum progranulin, which is lower levels in patients of ASD, was significantly decreased in CB1KO mice. These findings suggested that CB1KO mice showed behavioral phenotypes including social deficits, which have face validity as an animal model of ASD. Therefore, the CB1KO mice will be a valuable tool for the exploration of pathological mechanisms and development of novel therapeutics in the ASD.

大学や病院の多くの医師は, これまで慣例のように文献検索, 文献の入手および学会等のスライド作成などを, 製薬企業の医薬情報担当者 (MR)のサービスに依存してきた。このサービスが平成5年 (1993) 4月より, 製薬会社242社が加盟する「医療用医薬品製造業公正取引協議会 (公取協)」が定めるサービス提供の制限を超えるものとして, そのサービスの自粛が行われた。その結果, 多くの病院図書室ではオンライン検索の端末機やCD-ROMによるコンピュータ検索が新たに導入され, 文献入手の依頼が集中することとなり業務上大きな戸惑いがみられた。その状況を把握すべくアンケート調査を行い, 262機関より回答が得られたので報告する。

Takamura Kōtaro, one of the most distinguished poets in the modern literary history in Japan, has been discussed from many points of view. It is a common approach for exploring the reality of the poet who took the initiative in singing “Ego and Self in Modernity of peculiar atomosphere in peculiar country“ through his personal experiences abroad, especially his encounters in London and Paris.Though several fruitful contributions to an understanding of this great poet have appeared in the last two or three decades, it is often impossible to discover any commentaries on his life in the United States.The aim of this paper is twofold: to indicate how Takamura Kōtaro lived in New York and indicate a more comprehensive view for touching the heart of his poems. At the same time, I focus on the relationship between a Jap art student Kōtaro and an American sculptor Gutzon Borglum in this study and try to broaden the horizons of the disputes over this poet.

Crowd control method at stadiums in emergency evacuation situation such as a fire, bomb threat, and terror is discussed. For its basis, egress and way home crowd behavior in eight stadiums was observed through walking with the crowd and mixed in like a tracer. In some stadiums, the crowd was controlled by closing some exits, temporary fences, and stewards’ guidance to relieve congestion outside the stadium site. In contrast, some stadiums let pedestrians walk freely; however, no accumulation was generated. The latter strategy is desirable for emergency evacuation, and its architectural design consideration and guidance policy are discussed.

「半福祉・半就労」が注目されるのは,非正規職が4割を占めるに至っている雇用の不安定化やそれに起因する実質賃金の停滞等によって,仕事に就いたとしても賃金だけでは健康で文化的な生活が保障されない労働者,就業者が少なくなくなってきたところにある。このような労働者,就業者に対しては,最低賃金の引き上げ等,労働サイドにおける政策的対応と併せて,「半福祉・半就労」による最低生活を保障しながらの就労支援が必要となる。 本報告は,「半福祉・半就労」を進めるためには,生活困窮者自立支援制度と生活保護制度のそれぞれの間口を広げるとともに,両制度の一体的実施や,切れ目のない支援が求められていることを指摘するとともに,両制度による生活保障と自立支援のあり方を,社会保障審議会等での議論を踏まえ検討するものである。

本研究では,自己評価であるCan-do statements(Cds)を利用して,日本のT大学と海外協定校の日本語科目の対応関係を明らかにした。具体的には,T大学と,協定校5校の日本語学習者を対象に調査を実施し,各授業科目受講生のCds平均得点を用いて,T大学各レベルの授業科目と各協定校各学年の対応表を作成した。対応関係が明らかになったことにより,協定校においてより適切に単位互換を行うこと,来日前に留学生の配置クラスを予測することが可能となった。さらに,T大学と協定校における各項目の回答結果を比較することにより,技能の習熟度における相違点を検討した。今回使用した項目には,海外においては遭遇する可能性がない言語行動もあり,そのような項目は,おおむね自己評価が低くなることも明らかになった。

A total of 58 spider species is reported from material mainly collected on three recent expeditions to Laos. Out of these, five species are described as new: Psechridae — Psechrus khammouan sp. nov. (male, female), Psechrus luangprabang spec. nov. (male, female), Liocranidae — Sesieutes thakek spec. nov. (male), Zodariidae — Storenomorpha anne spec. nov. (male, female), Sparassidae — Pseudopoda houaphan spec. nov. (female). The males of Caerostris sumatrana Strand, 1915 (Laos; first record for Asia) and Eurychoera quadrimaculata Thorell, 1897 (Singapore) are described for the first time. Copulatory organs of the latter species are illustrated for both sexes to facilitate identification of Eurychoera banna Zhang, Zhu & Song, 2004. New records of species formerly known from Laos are listed: Heteropoda dagmarae Jäger & Vedel, 2005, H. maxima Jäger, 2001, H. tetrica Thorell, 1897, H. venatoria (Linnaeus, 1767), Rhitymna verruca (Wang, 1991), R. plana Jäger, 2003.

1型糖尿病は,注射によるインスリン療法が唯一の治療である.近年の1型糖尿病診療は,新しい薬剤や血糖測定のデバイスの進歩が著しい.それでも,インスリン療法に加えて日々の生活の中で最も重要な食生活と運動の管理を生涯継続しなければならない.血糖管理が不十分な場合,糖尿病合併症の発生頻度は非常に高く,生命予後を悪化させる.小児思春期に多く発症する1型糖尿病では,血糖管理が不十分になりやすく,患者指導に難渋することが少なくない.筆者は,そのような患者の対応で困っていたときに動機づけ面接(motivational interviewing:MI)に出会った.MIは,アルコール依存症患者の面談から始まった面接法であり,今ではあらゆる医療面接,教育,司法,矯正などの幅広い分野でその有用性が認知されている.MIでは患者の発言の中の,チェンジトーク(よい発言)に注目し,チェンジトークを引き出して,変化へ向かうチェンジトークへと育てていくことを目的とする.思いやりを含んだ精神をもってクライアントとダンスを踊るように変化の方向へ導いていくのがMIである.筆者は,MIを学ぶことで患者のよい変化を多く経験するようになった.現在ではMIのトレーナーとしてMIの普及活動を行っている.

要求工学は,ソフトウェアシステムの外部仕様である振る舞いや制約を定義するための技術の集合である.そして,そこで定義された要求は,われわれが実際に生存している現実世界における意味や価値を反映したものでなければならない.極めて多岐にわたる要求工学の技術を,本稿では,正確な要求を定義するための技術,変動する要求に対処するための技術,および,視点の多様性に対応するための技術という3つの観点から概観する.
