Tuberothalamic artery領域の右視床梗塞を来した結核性髄膜炎の32歳男性例を報告した.発熱と頭痛で発症し, 経過中に失見当識, 了解不良や傾眠状態が出現した.CTではtuberothalamic artery領域の右視床に造影効果のない梗塞巣と考えられる低吸収域を認め, これが失見当識などの原因と考えられた.脳底槽は等吸収域を呈し著明な造影効果を呈した.MRIでは脳底部の血管はflow void phenomenonのため低信号域を呈したが, 脳底槽はT1強調画像で等信号域, T2強調画像では髄液より低い高信号域を呈し, 著明な造影効果を呈し, これがgranulomatous basal arachnoiditisの部位を示していると考えられた.結核性髄膜炎で急に意識障害や精神症状を来した場合には脳梗塞の合併を考慮する必要がある.

The purpose of this study is to elucidate the effects of skeletal muscle volume improvement through a body mass-based resistance training intervention with or without the intake of Glavonoid. Thirty elderly men were randomly assigned to a Glavonoid(n=15)and to a Placebo (n=15)groups. Both groups underwent a series of resistance training programs, in which 3 exercises were conducted for the trunk and lower limb muscles(20 repetitions/exercise)for 12 weeks. During the training period, the subjects were performed in the exercises, once a week in the gym supervised by fitness instructors and 6 days a week at their own homes. Before and after the training intervention, body composition, muscle thickness and the score of performance test were determined. After the training for 12 weeks, skinfold thickness at the anterior thigh significantly decreased, and muscle thicknesses at the abdomen and anterior thigh increased in the Glavonoid group. In both groups, the sit-up test score significantly improved after the intervention. The relative change in the score of sit-to-stand test was negatively correlated with the pre-intervention score, indicating that the training effect depends on the baseline fitness level. It was concluded that Glavonoid has favorable roles for muscle size and against adiposity in the elderly men when it is combined with a body-mass based resistance training, and that the training intensity should be adjusted for each individual.

Large oyster shell beds are contained in incised-valley fill deposits of the Cycle 1 of the Miwa Formation, Shimosa Group, Lake Kasumigaura area, Ibaraki Prefecture . This paper describes taphonomic processes of large oyster shell beds well exposed in three localities . Six shell bed types can be recognized on the basis of their lithology, sedimentary facies, mode of fossil occurrence and associated fossils : 1) autochthonous aggregated, 2) autochthonous scattered, 3) parautochthonous aggregated, 4) cross-bedded aggregated, 5) crossbedded scattered and 6) shell-fragment aggregated types. The large oyster shell bed higher than 5 m and wider than 30 m at east Hama (Loc. 1) is characterized by type 1 with two layers of type 3 five to 20 cm thick that seem to have formed by sudden storm events. The second shell bed at west Hama (Loc. 2) shows two mounds 3 m high and or wider than 7-10 m composed of type 1 and covered by massive muddy sediments. This means that the oyster banks were buried under estuary mud. The last example at Sakihama (Loc.3) is the largest oyster shell bed in this area with height of 5m and width of 70m. It can be divided into three unit bounded by two erosional surfaces. The unit I is composed mainly of type 1 representing in situ oyster reefs, and subordinately of 3 formed by reworking through tidal current. The unit II composed of types 4 and 5 forms a shallow channelfilled structure that seems to have been formed as a tidal channel. The overlying unit III above a flat erosional surface mostly consists of type 6. Reworked oyster shell fragments might have concentrated intermittently and condensed through muddy sediment bypassing within tidal flat to embayment.

臭素系ダイオキシン類は,POPs 条約の規制対象物質ではないが,対象であるダイオキシン類と同様の特性を示すため,リスク管理の必要性が国際的に共有されている。国内では,環境省による臭素化ダイオキシン類の排出実態調査が実施され,臭素系難燃剤の decaBDE を使用する施設や含有製品を取り扱う施設で,ダイオキシン類の排出基準や作業環境基準を超過する値で検出されることが明らかにされた。臭素化ダイオキシン類の排出は,2017 年に decaBDE が POPs 条約上の廃絶対象物質となったことに伴い,動脈側で減少することが予測されるが,静脈側で含有製品の再資源化や廃棄を通じた排出が当面継続する見込みであり,引き続きその実態把握が必要である。DecaBDE の代替物使用の臭素系ダイオキシン類の排出への影響についても,今後の排出実態調査や関連研究で明らかにされることが期待される。また,臭素化ダイオキシン類の適切なリスク管理には,WHO と UNEP の専門家会合が指摘しているとおり,魚類・哺乳類毒性試験に基づく TEF を補完していく必要がある。

【目的】在宅緩和ケアを受けた終末期がん患者の現状と診療実態を調査すること.【方法】在宅緩和ケアを専門的に提供する6診療所(在宅特化型診療所)から在宅診療を受け,2012年1~6月に死亡または診療中止したがん患者352名を対象として,診療録調査を実施した.【結果】対がん治療終了後に在宅診療を開始した290名の有効回答が得られた.男性165名(57%),年齢は平均72±13歳.訪問看護を238名(98%),訪問介護を95名(39%)が利用し,死亡または診療中止前1カ月間に,輸液療法を72名(30%),強オピオイド鎮痛薬投与を127名(52%)の患者が受けていた.転帰は自宅死亡242名(83%),在宅診療中止48名(17%)であり,中止の理由は,患者の身体的問題と並んで,家族の精神的・身体的な問題が多かった.【結論】在宅特化型診療所における在宅緩和ケアの現状と診療実態が明らかとなった.

To evaluate the excitement caused by baseball plays, we examined the relationships among the audience's cheering volume, changes in win probability, and events such as achieving a hit during a professional baseball game. The total number of events and mean cheering volume throughout the game were 70 and 86.99 (± 6.35) dB, respectively. There was a significant correlation between the cheering volume and win probability. On comparing the former among events, the volume was significantly higher when achieving hits compared with outs or walk/hits by pitch. It was also significantly higher when there were runners in a scoring position than when there were no such runners. Based on the results, the audience may become more excited when win probability markedly changes, there is a hit, or there are runners in a scoring position.

The purpose of this paper is to elucidate the details about dialectal words referring to “fishing tools”, “fishing methods”, “fish names” and “fish processing” used in Todohokke, which is located in the eastern region of the Oshima Peninsula. The words have been collected by our fieldwork, some books of the local fishing industry and a couple of dictionaries of the Hokkaido dialect. Each item concerned includes a dialectal word in katakana letters, the meaning with brief comments, the example sentences, the recorded places and other sources. It is revealed that the dialectal words of Todohokke have unique characteristics closely related to the kinds of fish, distinguished from those of the western region of the peninsula.

2015年4月からスタートした機能性表示食品の中で機能性成分としてルテインとゼアキサンチンがあるが、消費者の認知はまだそれほど高くない。表示例としては「ルテイン、ゼアキサンチンには眼の黄斑色素量を維持する働きがあり、コントラスト感度の改善やブルーライトなどの光刺激からの保護により、眼の調子を整えることが報告されています」というのがある。表示の科学的根拠は何か又安全性はどうかということについて紹介したい。

We attempt to simulate the activity of earthquakes of moderate to large sizes in and around the Japanese islands using a block and fault model, in which slip deficit rates were derived from triangulation and trilateration data covering one-hundred years. The original block and fault model has 104 faults. However, the original fault size is too large to simulate moderate earthquakes. Therefore, we divide each modeled fault into 5 × 5 elements to generate earthquakes as small as M5. In total there are 2600 elements. We assume stress accumulates according to the estimated slip deficit rates. Interaction between fault elements is represented by changes in Coulomb Failure Function (hereafter Δ CFF) induced by the movement of other faults that are determined by the geometrical relationship and the direction of slip deficit rates. When stress reaches the threshold level, accumulated CFF is released by a forward slip and redistributed to surrounding faults according to the CFF changes calculated above. If the redistribution of CFF induces the next rupture, the same process is repeated until there are no more rupturing elements. We assume rupture thresholds to be 2.5 MPa for interplate boundaries and 10 MPa for inland faults, respectively. We simulate seismicity for 10000 years with a time step of 1 year.In this simulation large events that rupture almost all elements of a fault rarely occur. This suggests that strain rates derived from geological data or historical earthquake catalog might be underestimated. Simulated seismicity does not satisfactorily fit the GutenbergRichter's law, because moderate events occur more frequently than small or large events. This suggests that we have to incorporate heterogeneity in the rupture threshold or the size of elements on a fault plane. The correlation between interplate earthquakes along the Nankai trough and inland events in southwest Japan is not clear, but there seems to be a complementary relationship in activities between both regions. Migration of large events along the Nankai trough is occasionally seen in this simulation, but its direction is different from time to time.
