As ambiguous package descriptions occasionally cause medication errors,measures against this should be taken to help ensure that medicines are used properly.An example is Speel plaster® M,a salicylic acid adhesive plaster,which is used for the treatment of dermal diseases with keratosis.It is supposed to be attached to the diseased skin in a size the same as that of the affected area or smaller than it,in order to prevent the normal skin surrounding the affected area from being irritated and/or being detached.However,we found that some patients covered an area larger than the affected area with the plaster.This may be due to misunderstanding of a Japanese expression in the package description.The expression is “Kanbu-dai”which means the same size as the affected area but some patients take“dai”to mean large making them think that the size should be larger.In a questionnaire given to 180 pharmacy school students,65.6% answered that the proper use of the plaster was to apply it in a size larger than the affected area,since they had misunderstood the meaning of the expression“Kanbu-dai”.This misunderstanding seemed to be connected with the fact that they had not used the plaster before.We therefore devised a user-friendly package with an explanation to patients using illustrations to help ensure that the plaster is used properly.

画像をはじめとする大容量のデータを取り扱う際,クラウドストレージだけではアクセス性能要件を満たせない場合がある.そこで,大容量かつ高速アクセスが可能なストレージシステムの構築作業を低予算で一から行った.記憶装置は安価なHDDを採用しながらも,ZFSによるソフトウェアRAIDを用いることで十分な性能と信頼性を確保することができた.また,アクセス用ネットワークにはスパコンで用いられるInfiniBandを採用し,56 Gbpsのスループットを確保した.このような装置を個人で構築・運用した事例について紹介する.

加齢に伴う筋力,身体機能の低下と定義されるサルコペニアは各種疾患でその概念が普及し,治療アウトカムにおける影響が盛んに報告されている.われわれも高齢胃癌患者におけるサルコペニア症例が非サルコペニア症例にくらべて術前摂取エネルギー量,タンパク質量が有意に少なく,サルコペニアが胃癌術後の重篤な合併症発生の独立したリスク因子であることを報告している.つぎのステップとして,初診時にサルコペニアと診断された65 歳以上の高齢胃癌術前患者を対象にして,エクササイズとしてハンドグリップ,ウォーキング,レジスタンストレーニング,栄養介入として1 日28kcal/kg(IBW)のエネルギー量と1.2g/kg(IBW)のタンパク質の摂取およびβ-hydroxy-β-methylbutyrate(HMB)の補充を推奨する,「術前栄養+エクササイズプログラム」を作成・実践しているので,その概要について報告する.

In this study, we focused on the effect of the information of others' memories and of the delay of recognition on conformable eyewitness memories to others. A2 (delay: no delay vs. one week) ×2 (experimental group vs. control group) experimental design was employed (both factors were between-subjects variables), and experiments were carried out by four participants per session (a total of 85 participants). First, in a recognition task about a video clip, participants in the experimental group were shown "false responses by others." After that, participants undertook a similar recognition task and a Remember/Know judgment task about these items. As a result, the participants that took the task after one week showed a higher conformity response rate to the false responses others' than participants that took the task immediately after encoding. Moreover, the participants in the experimental group showed a higher Remember-judgment rate in wrong answers than participants in the control group. These results suggest that even though testimony taken from an eyewitness group may be concrete and clear, it is possible that it does not reflect his/her true experience exactly.

昆虫には越冬前に特定のステージで発育停止, すなわち休眠に入り, 耐寒性を増加させ, そのステージに特異的な水分含量低下, 粗脂肪含量の増大を示すものが多い。クロゴキブリの大型幼虫は, あらかじめ15℃短日(8時間照明)条件下で飼育すると, 5.5℃低温に90日以上耐えられる。そこで, 脱皮後間もない終令幼虫を15℃短日下で150日間飼育すると, その間脱皮は行われず, 体重の増大に伴い, 含水率の低下, 粗脂肪含有率の増大がみられた。しかしこの変化は15℃短日下に特異的なものではなく, 27℃長日(16時間照明)条件下でも同様の変化がみられた。27℃では短期間で脱皮が起ったがその際には含水率は上昇し, 粗脂肪含有率は低下した。一方, 15℃短日60日間飼育の後27℃長日に戻した場合, 極端な羽化のおくれは認められなかった。又15℃短日に放置すれば, 発育はおそいが400日で半数以上が羽化した。したがって, クロゴキブリ終令幼虫の15℃短日下での予備飼育による耐低温性の獲得は, 休眠を伴っていないと思われる。1令幼虫から15℃短日下で飼育を行うと, ゆっくりではあるが次々と脱皮発育し, 650日で約40%が8令(残りは6∿7令)に達した。この8令幼虫は27℃飼育の8令(27℃では終令)より小型で, 低温下では適温下より脱皮回数が多くなることが暗示された。又15℃短日下では, 20℃短日下でみられたような2令のみに特異的な令期間の延長はみられなかった。しかし, 20℃短日下で得られた2令幼虫が5.5℃に60日間耐えられなかったのに対し, 15℃短日下で得られた2令幼虫は60∿90%生存し, 耐低温性が強まっていることがうかがわれた。

自閉症スペクトラム (ASD) の子供の聴覚過敏について保護者に対するアンケート調査による解析を行った. 症例は小学校2年~高校3年までで現在防音保護具を使用している15例である. アンケートは症状の履歴, 現症, 対策について40項目にわたる項目に対して保護者に記載してもらった. 症状に気づいた年齢は2~5歳が大半を占めたが, 聞こえは普通という例がほとんどであった. 症状は年齢が進むにつれ少しずつ改善している例, 不変な例が半々で一部に悪化している例があった. 嫌がる音は個人差が大きくさまざまであったが, その音圧レベルとは必ずしも相関していなかった. 特に嫌がる音では子供や赤ちゃんの声や泣き声が多く, 大丈夫な音はテレビなど画像を伴う音が目についた. 防音保護具は7歳で使用を開始している例が多く, 使用により落ち着いた生活を送れるという例が多かった. 音に慣れる治療は “有効と思わない” という保護者が多かったが, その一方で自分で掃除機をかけるようになって掃除機の音に慣れたという例もあった. ASD の聴覚過敏には環境対策のほか, 防音保護具の使用が有効であることが確認された.

米国の公衆衛生大学院における産業医養成システムと日本の課題―ハーバード公衆衛生大学院専門職修士課程を通じて―:辻 洋志ほか.大阪医科大学衛生学・公衆衛生学教室―目的:世界経済の急速なグローバル化により労働者を取り巻く環境は大きく変化してきている.わが国の労働安全衛生制度は,法律に準拠する形で戦後一貫して充実強化されてきた.しかし,社会の変化に伴い企業や労働者が産業医に求める内容も多様化し,対応を迫られている. 対象と方法:米国は結果責任を伴う自主対応型の労働安全衛生制度を取っている.本稿では体系的なトレーニングを行い,変化する社会の需要に柔軟に応えることができる基礎を持つ産業医を養成する,米国の公衆衛生大学院のシステムを紹介する. 結果および考察:今後の日本の労働安全衛生の維持向上を模索するに当たり,多様化する社会の需要に応えるには体系的トレーニングによる個々の産業医や関連職の技能の向上が不可欠であり,米国における公衆衛生大学院による産業医養成システムは一つの参考になると思われる. L'enjeu de cet article est d'analyser le rapport du politique et du religieux mis en oeuvre par Nerval dans Les Illumines, et plus particulierement dans <<Cagliostro>>. Cette question permettra de mieux saisir le sens de l'ensemble du recueil ainsi que les reflexions historiques de Nerval face a la rupture operee par la Revolution franchise. Le sens du titre des Illumines et le choix des personnages apparaissent plus clairement a travers la comparaison du recueil avec les <<Prophetes rouges>>. Il s'agit, dans les deux cas, de repenser le lien possible entre le religieux, le politique et le litteraire, notamment dans les idees de reformes sociales presentant des speculations metaphysiques. Mais les personnages des Illumines sont des marginaux foncierement differents des <<Prophetes>> qui sont tous des fondateurs d'ecoles exigeant une adhesion collective. De meme, dans <<Cagliostro>>, Nerval reste mefiant vis-a-vis de la resurgence des ideologies theologico-politiques, revolutionnaires ou sectaires. Cette reflexion s'appuie sur un parallele, en apparence surprenant, entre deux ceremonies : celle de l'Etre supreme de Robespierre et les rites initiatiques de Madame Cagliostro. Nerval y critique la Revolution pour ses abus et ses deviances, sans pour autant la rejeter. Contre tout pouvoir politico-religieux, Nerval imagine alors une persistance des sentiments religieux, populaires ou savants. L'histoire millenaire qui ouvre le recit de <<Cagliostro>> temoigne de ce souci de depasser la rupture historique operee par la Revolution franchise, et de maintenir, par dela la <<mort des religions>> qui s'ensuit, la possibility d'un renouveau spirituel.

Using a historical approach, this study examines the transformation of graves from a material perspective, specifically the marketing strategy of gravestone shops. First, I reveal the relationship between the construction of graves for people who flowed into urban areas during the period of high economic growth and the commercialization of graves through advertising with its purpose of maintaining extended reproduction of these shops. Second, I demonstrate that, with the changes in the economic structure since the 1990s, gravestone shops have contributed to the social expansion of funeral and grave systems that do not require inheritance.

症例1は35歳男性である.6ヵ月前より禿頭予防にフィナステリド1 mg/日を内服していた.頭痛と痙攣発作を発症し,右前頭葉に出血をともなう低吸収域をみとめ,CT venography(CTV)で脳静脈洞血栓症と診断した.症例2は41歳男性である.2年前より禿頭のためフィナステリド1 mg/日,ミノキシジル6 mg/日を内服していた.頭痛を発症し,左頭頂側頭葉に散在する脳梗塞をみとめた.フィナステリド服用中の血栓症発症は,医薬品医療機器総合機構(The Japan Pharmaceutical and Medical Devices Agency; PMDA)へは,当院例を併せて14症例報告があり,脳卒中4例,心筋梗塞6例,その他4例であった.フィナステリドによるエストロゲン上昇の報告があり,血栓症発症への関与が考えられた.

This paper describes the experimental and analytical study on the parallel orientation of pins by sinusoidal vibration. A pin with a point at one end and a small spherical head at the other on a horizontally vibrating table is oriented in the direction parallel to the vibration. As long as the vibration amplitude and its frequency are appropriate, reorientation phenomenon occurs. Analytical study shows that the orientation depends on the magnitude of the maximum vibration speed of the table, that is, amplitude times frequency. The sense of the orientation of a pin depends on the initial condition.

日本における女性の過少代表性はつとに指摘されてきたが,近年,国会・地方議員に加えて首長にも女性が進出している。そこで本稿では,女性首長の増加の要因を明らかにするため,戦後全女性首長の経歴調査と現役女性市長の事例分析を行う。女性首長の経歴調査からは,女性を周辺化してきた労働市場構造を反映し,二つのキャリアパス(公務員・資格職と,地方議員)を経た女性たちが首長となっていることが明らかになった。とくに市町村では地方議員出身の女性首長が多い。また現役女性市長の事例からは,当選の背景に旧来の地方政治行政への批判があったこと,当選後はケアサービスの供給拡大と財政健全化という今日の地方政府が共通に直面する課題への対応がみられることが分かった。以上から本稿は日本における政治経済レジームの再編,すなわちケアの社会化と地方分権とが女性首長の登場を促していると主張する。

アミノ酸誘導体など食品や食品由来成分のもつ微生物不活化(消毒)活性を利用したスキンケアによい新しいタイプの消毒薬の開発を念頭に食品由来成分の探索とその作用機構の解析、ならびに実際の応用に向けたウイルス伝播力の解析を行った。塩基性アミノ酸のひとつアルギニンの持つ殺菌作用(ことに緑膿菌への)や梅酢ポリフェノールなどのウイルス不活化作用を見出し、ことに呼吸器感染症起因ウイルスへの消毒作用を明らかにした。また、医療環境を汚染したウイルスの持つ伝播力を汚染後の時間との関係において定量的に解析した。
