サイズの生物学によれば, 時間は体重の 1/4 乗に比例し, 体重当たりのエネルギー消費量は体重の 1/4 乗に反比例する。よって, 動物の時間の速度はエネルギー消費量に比例するという関係が得られる。 この関係は現代人の社会生活の時間にもあてはまると考えた。そしてエネルギーによりつくり出された異常に速い時間が現代人のストレスの源になっていると心配した。現代人はエネルギーを使って時間をつくり出す。一つにはコンピュータなどの時間加速装置を用いて余暇を生み出している。もう一つは医療により死という不活発な時間を活発なものに変換している。こうして人工的に生み出された時間とどうつきあうかは, 高次脳機能障害の治療に関わる方々にとっても, 大きな課題であろう。

Factors that affect career development in adulthood and systems for supporting it wereinvestigated. First, the dual characteristics and processes of acquiring an adult identity wereexamined. Then, the psychological and social effects of identity crises in adulthood, as well asthe effects of leaving a job and divorce, which cause identity crises, were discussed. Theresults indicated that re-organization of identity, improvements in business systems, CareerDevelopment Programs(CDP) and Employee Assistance Programs(EAP) are necessary tosupport career development in adulthood. Moreover, it is suggested that integration of EAP, aform of psycho-educational support, and CDP, career educational support, are important.

When Satô Haruo pointed the similarity between Shunkinshô (1933) and Thomas Hardy’s Barbara of the House of Greve, Tanizaki Junichiro had no objections to it and he rationalized the situation in his own way. Looking at it now from our contemporary perspective, it would surely be considered imitation, however to appreciate the originality of a certain work of art we have to consider the conditions that gave birth to it.Tanizaki has read tremendously the fin de siecle psychologists like Krafft-Ebing and the literary works of Poe, Baudlaire, Stringberg or Gorky in an atmosphere when modernism or cosmopolitanism was at its climax.How shall we judge his work? Was it a mere imitation? Or is it an original work of art? The fact is that Tanizaki has produced his work after having read and digested the western civilization thoroughly.The artistic production is only the tip of an iceberg of the author's creative genious. Though the critics or the authors sometimes try to explain the literary works, they often neglect the subconscious which is constitutes the larger part of the iceberg.Investigating the subconscious, will enable us to grasp the multilayeredness of a work of art. We then, will be able to recognize its originality through a series of analytic interpretations, which, even the author himself might be unaware of.I examined Tanizaki’s Shônen (The Children, 1911) critically and tried to analyze the symbolism in it with a fresh insight. Tanizaki, since the early stages of his career has always been at the mercy of the censorship authorities and often resorted to symbolism in his works; but it has reached to its peak in Shônen. In this work he did not imitate the west, but he took it as a model to compare it with his own. He laid down the tools and values in two sets like “Japan” and “The West” and knitted a highly structured symbolism around four children in Shônen. It was also his earliest declaration of his aesthetic programme" and it guided him through the literary path during the whole course of his 60 years’ authorship.

1972年、Jennet & Plumは外的刺激に対する反応の欠如する状態の継続する脳障害の患者について、PVS(persistent vegetative state)と命名した。かれらはこの状態は継続するが、永続性(permanent)か、不可逆性(irreversibk)であるかは断言できないとして遷延性(persistent)という語を用いた。しかし今日、意識障害が12ヶ月続いた場合には不可逆的である(Multi-society task force on PVS, 1994,英国Bland事件判決etc.)とし、PVSに対し、permanentを用いる識者が散見される(Mason, 1996.Truog, 1997)。また「PVSにおいては大脳皮質の機能はpermanentに喪失している(institute of medical ethics 1991)」などの記述が見られ、それが植物状態からの栄養補給の停止の根拠とされることがある。しかし不可逆性はあくまで経験的なものであり、100%担保されたものではない。勿論、不可逆性が担保されないからといって、正確な知識に裏打ちされたliving willや、医療資源の問題からの医療中止の議論が成り立たないことはなかろう。しかしpermanentという語は、living willなどの作成時などに誤解を招き、不適当と考える。

〈はじめに〉当院手術室では、各出版会社からの手術室看護手順書などを参考に各科の手術手順書を作成している。近年手術技法の著しい進歩により、同じ予定術式であっても執刀医により手術手順、使用する鋼製小物や医療材料に違いがあるため手術手順書への追加記載が必要となる。しかし、スタッフ各個人のメモへの追記に止まりがちで、共有の手術手順書の活用は充分とは言えない現状である。そこで術後、器械出し看護師と外回り看護師によるシートに沿ったショートカンファレンスのデータ、スタッフに対する意識調査より、手術手順書の効果を明らかにしようと考えた。〈方法〉シート記載:平成19年6月~9月、腰椎麻酔・全身麻酔の手術260件を対象に担当した看護師がシートに沿って話し合い記載したものを項目毎に結果を単純集計した。それらから、手術に入る看護師すべてのメンバーが手術手順書を活用した群を1群とし、以下メンバー3人のうち2人の活用を2群、メンバー2人のうち1人の活用を3群、メンバー3人のうち1人の活用を4群、メンバー全員が活用しないを5群とした。手順書活用群別に、『医師から助言・指導を受けた』『手術室外に鋼製小物などを取りに出た』の有無。さらに予定通りの手術・途中術式変更の違い、予定手術・緊急手術の条件の違いによる差を解析した。アンケートによる意識調査:平成19年5月と10月に器械出し看護師担当時・外回り看護師担当時に『手順書の活用』は出来ているか、『医師からの伝達の共有』は出来ているかをそれぞれ5段階のスケール選択方法により意識調査を行い比較した。さらに、10月に自由記載法による意識調査を行った。<結果>1)手順書活用群別に、『医師から助言・指導を受けた』は、1群32%、2群36%、3群35%、4群24%、5群18%であった。『手術室外に鋼製小物などを取りに出た』では、1群68%、2群52%、3群71%、4群60%、5群66%であり、それぞれに明らかな差はなかった。さらに、予定通りの手術と途中術式変更の違い。予定手術と緊急手術の条件の違いでも、『医師から助言・指導を受けた』『手術室外に鋼製小物などを取りに出た』の有無にそれぞれ明らかな差はなかった。 2)手順書活用に関する意識の5月と10月の比較では、「器械出し担当時の手順書活用」の意識調査は、やや評価を下げてしまったものの「外回り担当時の手順書活用」「医師からの伝達は共有できている」の意識調査は評価を上げた。10月の自由記載による意識調査では、情報の共有ができる。手術に対し振り返りができる。手順書活用、追記の意識が高まった。などの記載が複数あった。<BR>〈考察〉手術室経験年数や個人の手術看護に対してのスキルの違いから手術手順書の活用方法が違ってくると推測される。手術後話し合うことは、手術を振り返りができ、医師からの伝達の共有ができるに繋がったと考えられる。また共有の手術手順書は最新の情報でなければならない。と言う意識が高くなったと考えられる。今回の研究で使用したシートの項目は手術治療の介助に関することが主であったため、今後はチーム自らの看護を振り返るきっかけにしていきたい。<BR>〈まとめ〉手術手順書を活用した場合と活用しない場合とでは、『医師から助言・指導を受けた』『手術室外に鋼製小物などを取りに出た』の有無のいずれにも関連性はない。<BR>

The purpose of this study is to analyze morphophonological &ldquo;realization&rdquo; processes of morphemes in the subject dialect, and to represent the results by ordered rules.<BR>Studied here is the dialect of Kuchinotsucho, Minamitakakigun, Nagasaki Prefecture.<BR>Two phases are assumed in the description: morphological phase, and phonological phase. Three units are assumed as constituent elements of the morpheme forms: morphophoneme (keitaionso), morphophone (keitaion), and phoneme (onso).<BR>In the morphological phase, morphophones are selceted from morphophonemes depending on morphological conditions. Morphological conditions are those total conditions in which are found the morphemes constituting the environment and the morphemes having the morphophoneme as a constituent element of their forms, and includes the tactical relation between the two morphemes. The following phenomena are treated in this phase: alternation of phonemes, e. g. t-d, s-h, e-u, e-i o-u, etc. and lengthening of vowels, e. g. toru (bird) qoori (dative form).<BR>In the phonological phase, phonemes are selected from morphophones depending on phonological conditions (the selection is conditioned by the phonemic system of the subject dialect). Phonological conditions are those total conditions in which are found the morphophones constituting the environment for the morphophone which happens to appear, and the tactical (phonotactic) relation between the two morphophones. The following phenomena are treated in this phase: alternation of phonemes as conditioned by the phonemic system, e. g. t-c, d-z etc.; assimilation of phonemes in the final morae of some words, e. g.-bu+bV&rarr;-Q+bV, -bu+mV&rarr;-N+mV etc.; dropping of consonants or semi-vowels, C1C2&rarr;C1, je, ji&rarr;e, i; shortening of long vowels, CVVN&rarr;CVN, CVVQ&rarr;CVQ etc.<BR>Each realization process is represented by a rule. Each rule consits of two parts: conditions, and result. Conditions are comprised of the logical product of the environmental element, tactical relation, and other conditions. e. g.(T<SUB>1</SUB><<M) &cap;(T<SUB>1</SUB>=1) &cap;(M=T<SUB>1</SUB>E<SUB>2</SUB>) &cap;(x<SUB>1</SUB>prM) &cap;(x<SUB>1</SUB>M=II):(T1&rarr;d)<BR>In general, the application of the rules of the morphological phase has precedent over the rules of the phonological phase.
