深部静脈血栓症の診断には客観的な画像診断が不可欠である.現在,超音波診断法とCT静脈造影(CT venography以下CTV)が主に使用されている.超音波診断法は低侵襲で深部静脈血栓症診断の第一選択である.方法はプローブでの圧迫法を基本としてカラードプラ,パルスドプラを併用する.超音波検査は術者の技量・習熟度が検査成績に影響し,下大静脈/腸骨/下腿領域で診断率が下がってしまう.CT肺動脈造影はすでに急性肺塞栓症の第一選択の画像検査である.CT肺動脈造影撮影後にCTVを行うことは早急な診断を要する病態では適している.CTVは下大静脈,腸骨静脈,大腿静脈,膝窩静脈の描出力は高いものの,静脈径の小さい下腿では分解能に限界がある.腎障害,造影剤アレルギー,被曝線量も問題となる.深部静脈血栓症の診断に関しては超音波検査が第一選択であるが,CTVは肺塞栓症合併例や超音波検査困難例などに施設の現状にあわせて超音波診断の相補的な役割を担い,重要性は増している.

This study aims to investigate the characteristics of effective interaction between a therapist and his/her child client with autism spectrum disorder during a session of occupational therapy. Four sessions were video-recorded and analyzed, including one conducted by a novice therapist and three by expert therapists. Three children with autism spectrum disorder who were able to take part in everyday conversations also participated in this study. In study 1, we coded the therapists’ utterances according to their function in the context through a session. In study 2, we segmented the video-recording of a session based on the goal of child behavior, recorded the therapists’ physical support, and examined not only the temporal relationship between the therapists’ utterances and physical support but also the children’s proactive behavior. The results indicated that the expert therapists appropriately provided attentive utterances and physical support according to the children’s actions. We discuss a new and useful framework for understanding therapist support and children with developmental disabilities.

症例は54歳, 女性で1968年特発性血小板減少紫斑病 (ITP) と診断され副腎皮質ホルモン投与されていたが, 1971年に慢性関節リウマチ (RA) を併発した. 1979年尿糖および高血糖が出現し, 糖尿病と診断され, その後ITPの増悪により副腎皮質ホルモン (プレドニン) を30mgに増量し, インスリン注射を開始した.この時点での尿中C-ペプチド (CPR) は36μ9/日であった. 1989年頃より血糖コントロールは徐々に悪化し1991年には尿中CPRが10μ9/日以下となり, グルカゴン試験も無反応を示した.また, 既往に流早産を数回経験し, 検査所見ではICA, RA因子, 抗血小板抗体, およびループアンチコアグラントが陽性を示した. 以上より時間的に推移しながらITP, RAおよび抗リン脂質抗体症候群にslowly progressive IDDMなどの自己免疫疾患を合併したもので, 免疫学的背景を考える上で興味ある症例と思われた.

The phenomenon of taste is best explained by selective adsorption of taste stimuli onto a taste receptor. The sulfur-containing compounds taste sweet, sour, salty, or flavorous, depending on their chmical structures. The structure-taste relationships of sulfur-containing compounds, such as sugars, amino acids, peptides, sulfamic acids, saccharin derivatives, oxathiazinone dioxides, thioureas, and 5'-nucleotides, are discussed. The recent concept of the taste receptor sites is also described.

I. A 26 years old woman, complainig of general malaise and hypermenorrhea was admitted to our hospital in Feb.,1978. She had been suspected of idiopathic thrombocytopenic purpura (ITP) by her home doctor.Laboratory examin ations (Tab.1, Fig.1) revealed thrombocytopenia (26,000/cmm) and abnormal immuno-serological findings, including LE phenomenon (+), LE test (+), DNA test (×640), ANA (×160), BFP (+), RAT ( + ), Coombs test D (+) and hypergammaglobu linemia (1.84-1.36g/di). Her bone marrow picture was typical of ITP, i. e. an increased count in megakaryocytes lacking platelet production.Clinically, the patient showed slig ht fever and sometimes complained of bilateral knee joint Pain.She was diagnosed as SLE, chiefly manifesting thrombocytopenia, and was treated with corticoid (prednisolone), resulting in an improved platelet count (253,000/cmm) in three weeks.II. This case drew our attention to the relationship between ITP and SLE, which led us to investigate the autoantibodies in ten cases of ITP (Tab.2, Figs.2,3) treated at our department during the last five years.1) The positive rates o f the immuno-serological tests in our ITP cases were as follows: Anti-platelet antibody 1/7, LE phenom.0/6, LE test 0/9, DNA test 3/6, ANA 2/9, Microsome test 4/9, Thyroid test 4/9, RAT 4/8, BFP 4/8 and Coombs test 2/4 (Figs.4,5). Comparing these data with those of the SLE cases reported in the literature (Fig.4), some differences suspected to exist between the immunological failure in ITP and that in SLE.2) In all the four ITP cases responding well to corticoid therapy, the BFP was positive: in the non-responding cases, it was negative.In each case, little correlation could be seen between the number of positive tests and response to therapy.The han dling. of ITP cases necessitates a follow up in which attention is paid to the possible concomitance or succession of other auto-immune diseases.An accumulation of case studies in this direction may reveal the pathogenic correlations between ITP and SLE or other auto-immune diseases.

<p>本稿はケーブルカーを核とする観光鉄道の現況を,主として経営面から概説したものである.路線の類型化,運行面における特徴,コスト構造,行楽地アクセスとしての地位,入込客数との相関を示し,廃止路線の要因分析を試みた.経営は厳しく廃止もみられるが,地域活性化に資し環境適合性の高い観光鉄道を再評価,存続維持を模索する時期にきている.</p>

Recently, mindreading ability is the topic of intensive investigations, fired by the “Theory of Mind” research by Premack & Woodruff. Few researchers, however, tried to define “mindreading” algorithmically. We mindread other people everyday, and think we know what mindreading really is—e.g. to reproduce in ourselves the “thought” existing in other's mind. Unfortunately, this definition cannot be applied for machine systems. Even for human beings or animals we cannot examine the thought in other's mind directly, and there is no method to verify the coincidence. We propose a definition of “mindreading” independent of the inner representation (thought) of mindreaders and mindreads. Next, Multi-player Prisoner's Dilemma Game (MPD) is proposed as a task where mindreading is expected to be effective for the survival of the players. Computer simulation shows “(by our definition) mindreading programs” are actually acquired through evolution in the MPD society. Lastly the validity of our definition of mindreading, the implication of our definition, and condition for the emergence of mindreading are discussed.

In many instances a food stuff has both sweet and sour tastes and proper combination of sweetness and sourness is one of the factors in making the taste good.An experimental study has been carried out to find out the relationship between sweetness and sourness in a food stuff.The results of the experiments are as follows : Sweetness became less by the addition of only a small amount of acetic acid, and the rate of the sweetness lessened was directly proportional to the amount of acetic acid added.Sourness became less by the addition of sucrose. The rate of the sourness lessened was not directly proportional to the sucrose added, but it seemed that the rate of sourness lessened was in proportion to pH of the acetic acid solution. Namely, acetic acid solutions stronger than 0.3% in concentration were not masked by adding even a large amount of sucrose.
