症例は,75歳,男性.C型肝硬変症,肝細胞癌(HCC)にて当院通院中.2008年3月より血便を生じ,下部消化管内視鏡検査にて出血性直腸静脈瘤と診断.2008年7月HCCの治療目的にて入院.HCC治療時の上腸間膜動脈・脾動脈造影門脈相では遠肝性側副血行路は認めず,下腸間膜動脈造影にて下腸間膜動脈は直腸壁を造影し,その後排血路の一部として直腸静脈瘤が造影され下腸間膜静脈より脾静脈へと排血されていた.その後,直腸静脈瘤に対して内視鏡的静脈瘤結紮術(EVL)・硬化療法(EIS)併用療法を施行.直腸静脈瘤の口側端で,腸管壁外へ流出すると考えられた部位へEVL施行後,透視下でEISを施行.その後,造影CTにて直腸静脈瘤の血流は消失した.本症例は,血管造影にて直腸静脈瘤の血行動態を把握後にEVL・EIS併用療法を施行する事によって安全に治療しえた1例であったので報告する.<br>

東京都多摩地区のペットショップおよび一般家庭で飼育されていたイグアナ, プレーリードッグ, カメレオンなどのペット動物における腸管出血性大腸菌O157, サルモネラ, エルシニア, 黄色ブドウ球菌, <I>Listeria monocytogenes</I>およびクラミジアの保有調査を1996および1997年の10, 11月に行った. その結果, 1996年に調査した計140匹の動物の糞便のうち, サルモネラが3検体 (2.1%), 黄色ブドウ球菌が2検体 (1.4%), クラミジアが8検体 (5.7%) から検出された. 1997年には, 計101匹の糞便のうちサルモネラが5検体 (5.0%), <I>L. monocytogenes</I>が1検体 (1.0%), クラミジアが4検体 (4.0%) から検出された.

本研究は、京都語に隣接する京都府口丹波方言の敬語展開の現状を明らかにすることを目ざしたものである。口丹波地方では、従来、尊敬語として機能するハル敬語と、親愛語として働くテヤ敬語とが併存し、使い分けられていた。しかし、2012年から2015年までの現地調査の結果、京都市に近い南東部、特に中心部の亀岡地区で、テヤ敬語が廃れつつあることがわかった。そして女性は京都市女性話者に近い敬語運用を行い、男性は京阪地方で盛んな軽卑語のヨルを取り入れ、テヤ敬語の役割の一部を担わせつつある。また、ほぼ全域で、存在動詞がハルに承接する場合、オラハルとイハルないしイヤハルとのあいだでゆれがみられることがわかった。

2頭のオグロプレーリードッグ(Cynomys ludovicianus)に, 肺転移を伴う肝細胞癌と非腫瘍部の慢性活動性肝炎の同時発生と, 肝細胞の過形成が認められた. 過形成性の肝細胞は細胞質内に多数の好酸性, Orcein染色陽性の封入体を持っていた. 電子顕微鏡で, 細胞質内封入体に一致して鎖かたびらのような編み目状の構造物を観察した. これらの封入体の電顕像は, 肝癌に関連するヘパドナウイルスの封入体の形態と異なっていた. また, ヘパドナウイルス様の粒子も認められなかった.

Reconstruction aids for private housing structures lost in earthquakes were calculated on the basis of six criteria and published statistics. The criteria were generated from a principle given by the authors that the housing recovery does not degrade the quality of living of the affected, and their acceptability was confirmed through a questionnaire survey done in affected areas of a recent damaging earthquake. It was suggested that the financial assistance is needed only by families with an annual income less than seven million yen when earthquake insurance is not included and four million yen when included. Diffusion of earthquake insurance and financial standing of the affected were the significant issues carefully included in the development of assistance models.

Diminished ovarian reserve (DOR) has a high morbidity rate worldwide and has become a primary cause of infertility. DOR is a daunting obstacle in in vitro fertilization (IVF) and leads to poor ovarian response, high cancellation rates, poor IVF outcomes, and low pregnancy rates. Abnormal autoimmune function may also contribute to DOR. Dehydroepiandrosterone (DHEA) is a C19 androgenic steroid. DHEA is secreted mainly by the adrenal gland, and its secretion declines with age. DHEA has a pro-inflammatory immune function that opposes cortisol. The cortisol to DHEA ratio increases with age, which may lead to decreased immune function. DHEA supplementation helps improve this situation. A number of clinical case control studies and several prospective randomized clinical trials have observed a positive effect of DHEA supplementation in women with DOR. However, the underlying mechanism by which DHEA improves ovarian reserve remains unclear. DHEA functions as an immune regulator in many different tissues in mammals and may also play an important role in regulating the immune response in the ovaries. The conversion of DHEA to downstream sex steroids may allow it to regulate the immune response there. DHEA can also enhance the Th1 immune response and regulate the balance of the Th1/Th2 response. DHEA treatment can increase selective T lymphocyte infiltration in mice, resulting in a decline in the CD4+ T lymphocyte population and an upregulation of the CD8+ T lymphocyte population in ovarian tissue, thus regulating the balance of CD4+/CD8+ T cells. This review mainly focuses on how DHEA supplementation affects regulation of the immune response in the ovaries.

It has been well known that a room with a bright color is perceived larger than that with a dark color. In this report, we examined whether the pattern of wallpaper itself could affect the perceived size of a room. We measured the apparent size of the room having stripes on the walls; a miniature room (Exp.1) or a real room (Exp.2). The orientation of the stripes was horizontal or vertical. We found that the room with horizontal stripes looked wider (Exp.2) or wider and deeper (Exp.1) than that with vertical stripes, but that the room with horizontal stripes looked shorter in the height than that with vertical stripes (Exp.1).
