歯性感染症が原因で深頸部膿瘍を形成した症例は多数報告されているが,側頭部に膿瘍を形成する症例はまれである.今回我々は下顎の歯周炎が原因で側頭窩および側頭下窩に膿瘍を形成した1症例を経験したので,その治療と臨床経過について報告する.症例は63歳女性で,左側頭部痛,開口時痛,開口障害を主訴に当科を紹介受診となった.造影CTにて左側頭部に膿瘍形成を認め,同日左側頭部に皮膚切開を加えて外科的ドレナージを行い,抗菌薬による加療を行った.膿瘍の治療として適切な切開排膿が重要であるが,今回側頭部の皮膚切開を工夫することで全ての膿瘍腔を開放できたことが早期治療につながったと考える.

Bifidobacterium globosum is expected to be applied to foods as a probiotic, and a method to rapidly distinguish it from the related Bifidobacterium pseudolongum is necessary for product development and quality control. In this study, we evaluated existing methods of bacterial species classification for B. globosum/pseudolongum with the published whole-genomes. The results showed that the Polymerase Chain Reaction (PCR) - Restriction Fragment Length Polymorphism (RFLP) method by the restriction enzyme HaeIII for hsp60 genes and the sequence analysis of hsp60 and rpoB gene sequences could not accurately distinguish B. globosum/pseudolongum, and both species could be accurately distinguished by the sequence analysis of clpC gene sequences. The clpC gene sequence similarity was at least 97.9% in the same species and no more than 95.4% in different species. Therefore, we developed a novel method of RFLP by restriction enzyme BspT104I on PCR products of clpC genes to identify B. globosum/pseudolongum. This method was considered to be faster and more accurate than previously proposed methods for identification of B. globosum/pseudolongum. Thus, the method presented in this study enables efficient species classification and it is very useful in utilization of B. globosum/pseudolongum for foods.

The vagus nerve is one of the 12 cranial nerves, which is considered as the 10th nerve. The vagus nerve extended from the medulla oblongata to the second segment of the transverse colon. This nerve innervates the majority of the muscle of the pharynx, soft palate and larynx. In cases of vagus nerve paralysis, palatal arch movement is restricted. The arches cannot move up and back in order to close off the nasopharynx. This causes hoarseness, nasal speech, aspiration and regurgitation of liquid through the nose. In this article, we report 10 cases of the vagus nerve paralysis. The patient’ mean age is 59.4 (range 34-83). There were 5 male and 5 female patients, among which there were 6 cases of right and 4 cases of left side paralysis. In six cases the chief complaint was vocal disorders. The causes included 5 cases of Varicella-Zoster virus infection (all confirmed) and 4 cases of central paralysis. In 9 cases of the 10 patients, nasopharyngeal incompetence, elevation or constriction of the paralyzed side and saliva pooling in the pyriform sinus were observed. In 4 cases of the 10 cases, the vagus nerve paralysis improved after treatment within 6 months. In cases of vagus nerve paralysis immediate and accurate diagnosis and treatment are crucial. It is important to observe the palatal arches as they contract and the soft palatal as it swings up and back in order to close off the nasopharynx.

本共同研究のテーマは、人々(特に子どもたち)が日常的に親しむ文化において科学がどのようなものとして描かれ、人々がそれをどう受容して、どのように「科学的なもの」を理解しているかである。子どもたちが接するコンテンツに描かれているのは、科学的な根拠を欠く「非科学」というよりは、科学的には実現されていない、いわば「未科学」である。これらのコンテンツには科学を批判的に見る視点が確かに含まれているが、最終的には科学技術のリスク認知よりもそれへの期待が上回るという分析結果となった。

2014年 1 月から2018年 6 月までの 4 年半に関西医科大学総合医療センターにおいて経験した10例の早期梅毒患者についてまとめるとともに,梅毒の最近の動向について考察した。当該地区においても梅毒患者は増えており,男性 6 例,女性 4 例とやや男性優位であった。風俗店に通う青壮年男性が多かったことは,全国的傾向に合致していた。男性同性愛者および HIV 重複感染例は皆無であった。 硬性下疳 3 例,丘疹性梅毒 2 例,梅毒性乾癬 1 例,膿疱性梅毒および扁平コンジローム 1 例,ばら疹 3 例と極めて多彩な臨床像を呈していた。ほぼ全例に対してアモキシシリン内服治療を選択し,ほぼ全例で STS 抗体価の速やかな低下とともに臨床的治癒が得られた。なお,今回は晩期梅毒を除いた集計とした。梅毒の多彩な臨床症状と梅毒血清反応の複雑な動きを十分理解することは皮膚科医の大きな責務であり,病院内において今後ますます主導的役割を果たすことが求められる。 (皮膚の科学,18 : 139-143, 2019)

CaCl2 is a low-toxic, inexpensive reagent that dissolves in large amounts in water to form highly concentrated aqueous solutions, or hydrate melts electrolyte. In this work, it was investigated that the concentrated CaCl2 aqueous solutions (aq) as novel lead (Pb) electrodeposition baths. While PbCl2 is poorly soluble in water at room temperature, PbCl2 dissolved up to 0.452 mol kg−1 ([Pb(II)] = 93.7 g kg−1) by the formation of PbCl42− complexes in the concentrated CaCl2 aq due to its high Cl− activity. Electrochemical measurements confirmed that the apparent exchange current density and the limiting current density of Pb electrodeposition decreased with increasing CaCl2 concentration. Since the diffusion of Pb(II) species was limited due to the high viscosity and ionic strength of the solution, agitation was effective in improving ion transport and electrodeposition rate. The presence of chloride ions in Pb electrolysis is usually detrimental due to the low solubility of PbCl2, however, the highly concentrated CaCl2 solution can be a potential candidate electrolyte for PbCl2-based electrolytic processes.

咳,嗄声,喘鳴といった上下気道の症状は,われわれ耳鼻咽喉科医が小児の患者を診察する際にしばしば経験する症状である。そのほとんどが鼻咽喉頭領域の感染症に関連した症状であるが,年齢により疾患も大きく変化するため患児の年齢にも注意が必要である。さらに身長や体重が大きくなる発育や知能や運動能力など機能が成長する発達を考慮しながら診断・治療にあたる必要がある。小児喉頭において,声門上の組織が脆弱なため吸気時に引き込まれやすいといった特徴があるため,吸気性の喘鳴の原因として喉頭軟弱症(喉頭軟化症)がもっとも多い。喉頭軟弱症のほとんどは2歳までに軽快するが,チアノーゼを伴うような呼吸障害や発育障害など重度の合併症があれば,Supraglottoplasty(声門上形成術)など外科的治療が必要となってくるため,そのタイミングの見極めが重要となってくる。

東部ヒマラヤ(インド北東部,アルナーチャル・プラデーシュ州,ネパール東部,ブータン東部)における埋没腐植土層の各種分析から,土地開発史を検討した.その結果,①古い段階での初期的な土地開発時期(約2000年前代が中心)も分散的に行われた。②集中的な土地開発時期は、約千年前以降、特に約500年前以降に行われた。③民族移動のルートは、東ネパールでの先行研究とは違い、東西ルートでの民族移動にともなう土地開発過程が示唆された。

Dated charcoal and humus materials in soil, both of which are evidence of forest fire, vegetation changes indicate occurrence of past deforestation and land development (paddy field) in Eastern Himalaya. Around the Ziro, Arunachal Pradesh, North Eastern India, human impact such as population growth and cultural changes may have accelerated an environmental and agricultural changes after ca. 2 ka BP. Relatively intense deforestation and land formation process occurring since the ca. 1 ka BP was due to the human impact.

症例は32歳,3経妊未経産.稽留流産,人工妊娠中絶術による2回の子宮内容除去術後,異所性妊娠による片側卵管摘除術を経て,今回凍結胚移植により妊娠成立した.妊娠8週に性器出血にて来院し,超音波検査にて巨大絨毛膜下血腫と診断されたが,巨大絨毛膜下血腫は縮小せず,持続する性器出血により高度な貧血を呈したため入院となる.鉄剤投与により保存的に経過観察されていたが,妊娠20週にコントロール不可能な多量の出血が持続したため,妊娠継続は困難と判断,ゲメプロスト腟剤による人工妊娠中絶術を行うに至った.頸管拡張術中に再び多量出血し母体が一時ショック状態になった.児娩出後にも胎盤遺残を伴った性器出血が続いたため子宮内容除去術も行い,誘発分娩開始から胎盤娩出までの出血量は約3000ml,濃厚赤血球12単位,新鮮凍結血漿4単位を輸血した.近年,絨毛膜下血腫はIVF妊娠で血腫発症率が上昇するとの報告もあり,今回の巨大絨毛膜下血腫と大量出血の原因としてIVF妊娠が関与している可能性が考えられた.〔産婦の進歩67(4):388-393,2015(平成27年10月)〕

The mechanism of nodulation in copper electrorefining process was investigated by experimental method and simulation, in particular as for its growth in height. Due to the high current density at the tip, the nodule height increased as an exponential function of the time for electrolysis. Therefore, the growth behavior of nodules was strongly affected by the size of the nucleus and the existence of the threshold size to lead an electrical short circuit was suggested. Since some nodules obtained in the industrial process included mold releasing agent carried from anode-casting process, mold releasing agents are considered to be one of the main causes of the large nodule and need to be removed for the improvement in the current efficiency.

The present study investigated the relationships between SES (Socio-Economic Status), attachment, and help-seeking in childhood. A total of 162 elementary school students responded to a questionnaire survey that assessed SES, attachment anxiety, attachment avoidance, and help-seeking. The results of a path analysis showed that SES is directly associated with seeking a person to get help. Regarding indirect association, the descriptive statistics and the results of a correlational test indicated a nonlinear association. In sum, while SES is positively related to attachment in children from all but the highest socio-economic groups, among children from the highest socio-economic group, attachment anxiety and avoidance are high. Furthermore, attachment anxiety is negatively related to revealing the reason for wanting help. Those results are discussed from the parenting point of view.

組織には,その後の組織のありかたを方向付ける開祖と呼ばれる人物が存在することがある。雪印乳業(株)(現,雪印メグミルク(株))の乳酸菌研究の開祖は,北海道大学農学部応用菌学教室二代目の佐々木酉二教授であると言って良いだろう。佐々木名誉教授は,雪印乳業(株)の前身である北海道製酪組合連合会に勤務していた1935年に北海道月形集乳場の生クリームから「ブルガリア乳酸桿菌(L. bulgaricus)」を分離し,この菌株を使った乳酸菌飲料を開発した。この製品は北海道製酪組合連合会を通じて製造販売されたのである。佐々木名誉教授は,北海道大学で学術研究に従事した後も,様々な形で北海道製絡組合連合会や雪印乳業(株)の乳酸菌研究に多大な影響を与えた文字通りの開祖である。佐々木名誉教授が分離した菌株は,後述するように,乳酸菌の命名法の変遷に伴いL. helveticus Lj-1株という菌種菌株名で現在でも使用されている。北海道限定ではあるが,雪印メグミルク(株)で製造販売されている乳酸菌飲料「カツゲン(現在は,ソフトカツゲンという名称で販売)」は,このL. helveticus Lj-1株を使用した製品の一つである。
