左被殻出血によって軽度の表層失読を呈したと考えられた失名辞失語例を報告する。本例の障害機序を検討するため, 漢字刺激を用いた実在語と非語の音読課題, 同音擬似語を含む語彙性判断課題, 読解課題, 呼称課題を実施した。その結果, 漢字の非同音非語の音読成績は正常範囲内であり, 漢字から音韻へ変換する機能は保存されていると思われた。一方, 実在語の音読課題では低頻度かつ非典型読みの実在語の音読において基準値以下の成績を示した。誤反応はすべて LARC (legitimate alternative reading of components) エラーであり, 表層失読の特徴を認めた。また, 同音擬似語を用いた語彙性判断課題の成績が低いことから, 文字単語の同定過程に障害があると考えられた。二重経路モデルを用いた分析では, 意味システムの障害では本例の障害機序を説明できず, 文字列レキシコンや音韻列レキシコンの障害によって軽度の表層失読を呈したのではないかと考えられた。

本領域は、数学と物質・材料科学の連携により、「次世代物質探索のための離散幾何解析学」を創成することを目指すものである。原子・分子のようなミクロ構造やナノ粒子などのメゾ構造(これらを階層的ネットワークと理解)と、物質・材料のマクロな性質つまり物性・機能の関係を幾何学的に記述し解析することで、物質のミクロ・メゾ構造とマクロな物性・機能の関係を解明し(順問題)、求められる物性・機能を持つミクロ・メゾ構造の予見(逆問題)、更に構造を生成する動的構造形成の制御(最適化・制御)を行うことを目指し数理モデルの構築、シミュレーションと理論による検証を踏まえた最適構造の提案とそれに基づく物質合成を計画している。総括班は、このような異分野融合研究を効率よく行うための議論の場の提供、チュートリアル的な勉強会の企画・運営、領域全体に資する国際研究集会の運営、成果発信のためのニュースレターの発行などを行った。また、研究班でカバーできない研究課題を分析し、新手法の提案・提起に柔軟に対応するため公募計画を策定し、領域会議において計画研究、および公募研究の間の情報交換と議論を行った。異なる分野、班の間をつなぐためのインターフェースとなる若手研究者を配置した。彼らは領域内の項目間の融合を促進するための勉強会やワークショップの企画、ニュースレターの作成を行うとともに、彼ら自身も連携研究を行い、論文および国際研究集会等において成果を発表した。公募研究をより有効に連結させ相乗効果を上げるために、チームを超えた連携研究を奨励するための連携研究支援を行い論文として発表することができた。領域代表者は特に本領域研究に関していくつかの重要な国際会議でプレナリー講演やサーベイ講演を行い、領域の確立に向けて情報発信を積極的に行った。

3種のポリスルホン膜、PS-1.6UW、APS-150、BS-1.6を血液透析(HD)および血液透析濾過(HDF)に使用し、膜断面における蛋白付着状態を共焦点レーザー走査蛍光顕微鏡で観察した。観察した蛋白は、アルブミン(Alb)、IgG、β2-microglobulin (β2-MG)、C3a、顆粒球Elastase、interleukin-1β、interleukin-6、tumor necrosis factor-αの8種類である。Alb、β2-MGは各膜ともHD、HDFで同様の強い蛍光が観察されたが、サイトカインの付着は各膜とも少なかった。緻密層の厚さや細孔構造の違い、表面polyvinyl pyrrolidone配合比率、電位の違いなどによると思われる、若干の差異を認めたものの、今回比較観察した3種類のポリスルホン膜では、これらの蛋白付着パターンに大きな違いを認めなかった。

This study aimed to evaluate the effect of pulmonary arteriovenous computed tomography (CT) value and CT value difference on the pulmonary arteriovenous automatic extraction ability of a medical workstation. We classified patients who previously underwent contrast-enhanced CT as those with a pulmonary arteriovenous CT value difference of <50 Hounsfield unit (HU) and ≥100 HU. The groups were further divided into four subgroups each, with a total of eight groups, based on low pulmonary arteriovenous CT values of 200 or more and <250 HU, 250 or more and <300 HU, 300 or more and <350 HU, and 350 HU or more. A radiographer conducted a visual evaluation, and we judged whether it could extract pulmonary arteries A1–A10 and pulmonary veins V1–V10 without errors. When the CT value difference was <50 HU, the low pulmonary arteriovenous CT value of 200 or more and <250 HU significantly decreased the extraction ability compared with the ≥350 HU group (p<0.05), but when the CT value difference was ≥100 HU, no difference in the CT value was found. The pulmonary arteriovenous CT value and CT value difference affect the pulmonary arteriovenous automatic extraction ability of the medical workstation, but revision by the creator is necessary because misrecognition is included even if a CT value and CT value difference is high.

濁音,半濁音,拗音を表す仮名文字の書字障害を認めた失語症例に対して,キーワードを用いた書字訓練を行った.本症例は漢字の形態想起が困難であったため,仮名単語をキーワードとして用いた.キーワードの語彙特性として,親密度,表記妥当性,および心像性が高い単語を選択した.訓練方法としては,言語聴覚士が1モーラを音声呈示し,症例が呈示された音声をもとにキーワードの音韻列,仮名文字列を想起したうえで,1モーラに対応する仮名文字を書字した.訓練は,ABABデザインによる単一事例実験計画に基づき有効性が認められた.最大13日間という短い訓練期間で濁音,半濁音,拗音表記のすべての仮名文字が書字可能となり,訓練経過後1年半においても仮名書字が実用的であったため訓練効果は維持されていると思われた.

救急外来を受診する高齢患者の約半数は入院に至らずに帰宅する患者である。これらの患者が再受診や予定外の入院をすることなく,住み慣れた場所で生活の質を維持しながら療養を継続するためには,初回受診時に帰宅後の生活を見据えた医療機関と地域・在宅での療養をつなぐ統合的な療養生活支援が必要である。 日本看護管理学会では,2022(令和4)年度の診療報酬改定に向けて,「救急外来における非入院帰宅患者に対する看護師による療養支援」への評価を要望する。これは上述の療養生活支援の役割を外来看護師が担うことへの診療報酬上の評価を求めるものである。救急医療の限られた資源を適切に活用することにもつながり,病院経営上,組織運営上の効果も大きいと思われる。 本稿では要望作成に当たった検討委員会の立場から,要望の目的と意義を述べる。

症例は47歳の男性で,前日からの腹痛の増悪,嘔吐の出現,意識レベル低下にて救急搬送となった.来院時,意識レベルIII-200(JCS),血圧測定不能,血液ガス検査で代謝性アシドーシス認め,ショック状態であった.腹部造影CTにて腹腔内に多量の遊離ガスと腹水を認め,肝臓と腎臓実質の造影効果は不良であった.消化管穿孔に起因するショック状態と診断し,緊急開腹手術を施行した.多量の汚染腹水,遊離ガス,食物残渣を認め,肝臓は虚血性変化を呈していた.胃底部から体部前壁に壊死を伴う破裂部を認めたため,胃全摘術を施行した.成人の特発性胃破裂はまれであり,過食などによる胃拡張状態に,嘔吐などに伴う胃内圧の急激な上昇や,あるいは胃壁の血流障害に伴う壊死が原因とされている.破裂孔が大きいため,胃内容物の腹腔内流出量が多く,腹部コンパートメント症候群を呈し重症化することがあり,一般的に予後不良とされている.

Drug resistant Pseudomonas aeruginosa is responsible for severe nosocomial infections, and combination therapy is needed. We studied antibiotic combination effects by “break-point checkerboard Plate (BC plate)” and disk diffusion susceptibility test against drug resistant P. aeruginosa. Thirty-three strains of multi-drug resistant P. aeruginosa (MDRP) and metallo β lactamase (MBL) produced P. aeruginosa collected from February 2010 to June 2012 were tested using BC plate ‘eiken’ (Eiken Chemical Co. Ltd, Tokyo, Japan) to evaluate the effect with combined antibiotics. The isolates have combined effect rather than single drug effect were tested using KB disk (Eiken Chemical Co. Ltd, Tokyo, Japan). As a result, there were higher zone of inhibition against some isolates for AMK/AZT, CL/AZT, CL/RFP and RFP/AZT, and the other isolates were not strong zone (≤0.5 mm). Therefore, disk diffusion susceptibility test for MDRP and MBL produced P. aeruginosa is able to determine antibiotic combination effects when it is recognized apparent zone.

〔目的〕急性期脳梗塞患者を対象とし,発症時の大腰筋面積が転帰に与える影響について検討した.〔対象と方法〕対象は脳梗塞発症後3日以内に入院した46例とし,退院時の転帰より自宅退院群(14例)と転院群(32例)に分類した.入院時の腹部computed tomography(CT)にて大腰筋からpsoas muscle mass index(PMI)を算出するとともに診療録より検討因子を後方視的に調査し,2群間にて比較検討した.また,多変量解析より転帰に影響する因子を抽出した.〔結果〕自宅退院の可否と関連を認めたのは,PMI,入院時National Institutes of Health Stroke Scale(NIHSS),入院前modified Rankin Scale(mRS),高次脳機能障害の有無であり,多変量解析ではPMI,入院時NIHSS,入院前mRSが選択された.〔結語〕脳梗塞発症時のPMIは,自宅退院の可否に関連しており,発症後早期からの転帰予測に有用な評価指標である可能性が示唆された.

Assuming that sophistication in clothing/makeup has two aspects, we conducted this research for the following two purposes. The first purpose was to develop a scale about two aspects of sophistication in clothing/makeup. A questionnaire was given to 107 female college and university students and their 107 mothers in order to gather data on both younger and older females'. The results of the factor analysis on the data indicated that sophistication in clothing/makeup has two factor structures, "external sophistication" and "internal sophistication". We verified the construct validity between the scale and the lifestyle and habit of clothing/makeup behaviors. For the second purpose, we studied the association between external/internal sophistication and gender personality by generation. As a result of a covariance structure analysis, we found that gender identity promotes external sophistication only in younger females while androgyny promotes internal sophistication in both generations. We also found that internal sophistication promotes external sophistication only in older females.

This paper discusses a motion control of horizontal bar gymnast robot with two links and one passive joint on the bar. First, we show that the amplitude of the swing of the first link increases with resonance and parametric excitation by swinging the second link periodically. Next, we propose the control method using entrainment to produce each excitation by swinging the second link 'in phase' with the motion of the first link. In our method, the control input is consisted of periodic solution of van der Pol's equation inputted the angular velocity of the first link. Finally, we present experimental results that swing up control and continuous giant swings can be realized in the real robot.

Timely input sometimes can control periodic movement of mechanical systems. In the swing which is the parametric excitation system, when the center of gravity is moved in double cycle, the swing's amplitude increases, and it decreases when this phase is moved π [rad] . But it is difficult to control a swing in the wide range, because the cycle of the swing depends on its amplitude. This paper shows that the van der Pol's equation inputted the swing's angular velocity can compose the frequency observer, and proposed the new control form of a swing that the center of gravity is moved by nonlinear feedback which consists of equation's output.
