症例は49歳男性.2005年に気管支喘息重積発作による心肺停止状態となった.覚醒後に動作時のミオクローヌスが出現し,Lance-Adams症候群(LAS)と診断された.発症11年後にペランパネルを開始しミオクローヌスは著明に減少し,3年以上持続してactivities of daily lifeが改善した.近年進行性ミオクローヌスてんかん症候群にペランパネルが有効な報告があり,本例のようにLASのミオクローヌスに対してもペランパネルによる治療の余地がある.

生コンプラントで発生するスラッジ水の多くは, 脱水ケーキ化された後, 多額の処理費を伴って管理型の産業廃棄物として埋め立て処分されるかプラント内外で適宜処理されているのが実状である。また, 管理型廃棄物の処分場を将来にわたって確保・増設していくことは, 極めて困難であり, スラッジの再利用や処理方法の検討が望まれている。本稿は, 生コンスラッジの発生と処理の現状を概観したのち, 生コンスラッジに関する既往の研究を取りまとめたものである。

The article examines the constitutional debate surrounding racial minorities in the United States, especially segregation and affirmative action, in light of social realities as evidenced in the San Francisco Bay area (especially Berkeley, Oakland, and Piedmont). It examines census data regarding socio-economic status, as well as standardized test scores, and finds a consistent pattern of racial segregation and racial disparity in the area. The article then looks at systematic factors that are not evidently related to race, but still has an impact on race. These include 1) the system of local government in the United States, which enable local residents to establish city boundaries and then to create zoning regulations within those boundaries, as well as 2) the financing mechanism of local governments that tends to result in disparities in school funding. These factors, combined with the U.S. Supreme Court’s reluctance to expand the reach of its integration decisions to de facto segregation, or to remedies beyond district borders, have made it difficult for integration to be implemented on the ground. In the end, the dominant tendency to distinguish official discrimination and segregation with disparity as a result of “individual choices”; and an aversion to recognizing persons on the basis of group attributes such as race or gender, delineates the limits of the power of law to improve the status of minority groups in the case of the United States. This could also be observed in the case of the debate over affirmative action, where the initial purpose of improving the status of minorities (as a group) in the educational setting have been replaced with the rhetoric of equal opportunity among individuals or benefits to the larger society (not the minority groups). Whether the law can go beyond the confines of narrowly delineated cause-and-effect and reach into the confluence of structural and institutional factors that result in disparity may be the key to a jurisprudence of minority rights.

This study examined how differences in the center of foot pressure in the sagittal plane affect the activity of muscles surrounding the hip joint in healthy young male subjects. With the subjects standing on one leg, integrated electromyography values of the muscles surrounding the hip joint, the point of center of pressure in the sagittal plane (Y-axis center displacement), and the trace length of the center of pressure in the frontal plane (X-axis trace length) were measured for three different loads with the center of foot pressure in the forefoot, midfoot and hindfoot regions. There was no significant difference in X-axis trace length between the three loading conditions. The activities of the rectus femoris, tensor fasciae latae and adductor magnus muscles were higher when the load was borne on the hindfoot than in the other two loading conditions. This indicates that these muscles have a flexor effect on the hip joint in the sagittal plane, and that they control the posterior movement of the center of gravity of the body. In addition, we consider these muscles control the center of gravity of the body in the coronal plane at the hip joint.

OECD,WHO,国連等の国際機関は,保健医療分野における政策立案に資する国際統計報告として様々な保健医療指標の迅速な提供を各国に求めている.2015年に日本がOECDに提出した保健医療指標の数は35加盟国中最低で,改善が望まれる. 本研究は日本の保健医療指標の提出項目数の増加を目的として,既存の公的統計やレセプト情報・特定健診等情報データベースを活用して未報告の保健医療指標の推計を行う.さらに,今回の推計から得られたわが国における手術および画像検査の実施状況について考察する.最後に,保健医療指標推計にレセプト情報・特定健診等情報データベースを利用する際の課題について述べる.

Recent advances in the field of the β-lactam ring syntheses are reviewed. All the methods are classified according to the notation of m±n_??_4 cyclization (m and n=0, 1, 2, 3, …), which is first proposed by the author. Each reaction is briefly commented.

薬剤性過敏症症候群(DIHS)は薬物アレルギーとウイルス感染症が複合的に関与し多臓器障害を来たす疾患である.DIHSの神経障害として辺縁系脳炎を呈する症例が報告されており,human herpes virus 6の再活性化が症状の発現に関与していると考えられている.DIHSに伴う中枢神経障害は時に重篤な経過を示す場合があり,原因薬剤の中止とステロイドの全身投与を早期に行う必要がある.
