【目的】くも膜下出血患者の離床時期が歩行獲得に及ぼす影響について後向きに調査し,アウトカムに影響する要因についても合わせて検討した.【方法】対象は2011 年2 月から2013 年7 月までにSAH(subarachnoid hemorrhage)と診断され外科的治療を行い,理学療法を実施した63 例とした.発症後14 日以内に離床を開始したEarly 群(40 例)と,発症後15 日以降に離床したLate 群(23例)に分類し比較した.また,抽出項目の中からアウトカムに影響する要因を検討した.【結果】Early群の歩行獲得率は有意に高い結果であった(p<0.002).アウトカムに影響する要因は,重症度(p<0.001),年齢(p<0.04),発症から離床開始までの日数(p<0.05)があげられた.【結論】早期離床は,歩行獲得までの日数を短縮させ,早期にADL を再獲得させることを示唆した.アウトカムに影響する要因は,重症度,年齢,離床までの日数であった.

2013(平成25)年2月26日,「道徳教育の充実に関する懇談会」が文部科学省に設置された。新しい学習指導要領には,その懇談会で議論された内容が反映されている。本稿は,道徳を教科化することを前提に開催された懇談会での議論の過程を整理し,さらに,教科化に対する批判的見解についても取り上げ検討する。それら両者を対照させることによって,道徳教育が有している問題や,今後の課題について考察するものである。

本稿では、日本のホスピタリティ産業における外国人材が、当該産業に従事し続けていくプロセスにおける異文化適応の様相を明らかにすることを目的としている。そのため本研究では、外国人材への半構造化インタビューを実施し、そこから得られたデータを修正版グラウンデッド・セオリー・アプローチを用いて分析した。その結果、日本語独特の言語行動の困難さ、日本人顧客への対応や、職場内における言語的な摩擦や心理面を示す人間関係構築の課題が捉えられた。これらの課題に対応するため、人間関係構築の工夫、当該産業での支えと,沖縄という地域からの支えを軸にして,役割やその場に応じて日本文化と自文化を組み合わせる異文化適応の様相が重要であることが明らかになった。本稿において、異文化における外国人材に関する示唆や、ホスピタリティ産業における日本語教育及び、当該業界への実務面での示唆が提示できると思われる。

As of the end of November, 2021, the rate of completion for second-dose COVID-19 vaccine administration was almost 80% in Japan. We evaluated waning COVID-19 vaccine effectiveness in Japan, controlling for mutated strains, the Olympic Games, and countermeasures. The effective reproduction number R(t) was regressed on current vaccine coverage and data of a certain number of days prior, as well as shares of mutated strains, and an Olympic Games dummy variable along with data of temperature, humidity, mobility, and countermeasures. The study period was February, 2020 through November 4, as of November 25, 2021. Estimation results indicate that vaccine coverage of more than 90 days prior raises R(t) significantly. Especially, vaccine coverage with 90 or 120 days prior cancelled vaccine effectiveness completely. Results indicate significant waning of vaccine effectiveness from 90 days after the second dose.

Three different types of analytical methods for D,L-amino acid were developed. First, a method for simultaneous analysis of D,L-amino acids in food was constructed by combining pre-column derivatization with 4-fluoro-7-nitro-2,1,3-benzoxadiazole (NBD-F), UHPLC separation, and UV and circular dichroism (CD) detection. Using this method, D,L-amino acids in food samples was determined at a very short time of 5.5 minutes. Second, a pre-column derivatization LC/MS method (direct method) for D-alanine, D-serine, and D-aspartic acid and L-isomers in biological samples was developed. In this method, D,L-amino acids were derivatized with an achiral reagent, p-N,N,N-trimethylammonioanilyl N′-hydroxysuccinimidyl carbamate iodide (TAHS), separated with a chiral column, and detected using mass spectrometry (MS). Third, a pre-column derivatization LC/MS method (indirect method) for highly sensitive and selective analysis of 20 D,L-amino acids using a chiral derivatization reagent, (R)-4-nitrophenyl-N-[2-(diethylamino)-6,6-dimethyl-[1,1-biphenyl]-2-yl] carbamate hydrochloride ((R)-BiAC), was developed. The unique structure of the (R)-BiAC reagent, which has a biaryl axially chiral structure, allowed for higher resolution and a high sensitive analysis of the 20 D,L-amino acids within 12 minutes. This method has been applied to various D-amino acid research studies, and it can be applied to a wide range of analyses from food to biological samples. Lastly, an LC/MS method was established that simultaneously analyzes tryptophan and 15 of its metabolites, which is involved in neurotransmission and immunity in vivo.

3色覚であるヒトの色覚異常(2色覚)のうち,Protanopia(P),Deuteranopia(D)およびTritanopia(T)型の場合にはそれぞれ赤と青緑,赤紫と緑および青と緑の色が識別困難とされる.本研究では生理学的に2色覚とされるニホンジカ(Cervus nippon;以下,シカ)がこれらを識別可能か否かについてオペラント条件付けにより検証した.シカ2頭(推定3歳:オス・メス各1頭)を試験に用いた.1セッションを20試行とし,正刺激として提示した色パネルの選択率80%以上(χ2検定,P<0.01)が3セッション連続でみられた場合,シカは2つの色を識別可能と判定した.オスは18,5および3セッション目,メスは12,14および4セッション目でそれぞれの色の組み合わせを識別できた.以上より,供試したシカはP, DおよびT型のヒトで区別し難い色の組み合わせをすべて識別可能であり,行動学的手法によって導き出されたシカの色識別能力はヒトの2色覚と一致しないことが示された.

The case of a 20-year-old woman presenting with delayed, potentially fatal intracranial hypertension as a result of a head injury is reported. On admission, the patient had a Glasgow Coma Scale (GCS) score of 10. A brain CT scan showed an epidural hematoma in the left temporal region and a mild contusion of the bilateral frontal lobes. Two days after her injury, the patient's consciousness level suddenly deteriorated (GCS 4). Transcranial Doppler (TCD) examination disclosed a marked reduction (18cm/sec) in the velocity of the blood flow through the middle cerebral artery and a significant increase (2.11) in the pulsatility index. A second brain CT scan showed diffuse brain swelling but no changes in the size of the hematoma. The intracranial pressure (ICP) was 58mmHg. Although the cause of the acute brain swelling was unknown, moderate hypothermia therapy was initiated. The ICP was difficult to control during the hypothermia treatment but was maintained at above 20mmHg for most of the time. Hypothermia treatment was continued for 18 days before ICP control was regained. The patient was discharged with mild memory disturbances 4 months after admission. A literature, review has suggested that the TCD and ICP values of this patient could have been fatal. Thus, moderate hypothermia treatment may improve the outcome of potentially fatal intracranial hypertension.

東京大学総合研究博物館所蔵の縄文時代人について,その骨折の病態を調べた.今回はそのなかでも福島県三貫地貝塚出土の古人骨について,骨折の鑑別診断が容易であった3例について論じた.3例は,橈骨遠位端骨折(Colles骨折),距骨変形治癒骨折,鎖骨骨折であるが,いずれも転倒や高所からの転落など,当時の人々の生業に起因したものと推定された.即ち獣を求め狩猟をすることや,果実などの採集のために,彼らは現代人のわれわれより,身体を酷使し危険な状態に身を挺したと考えられた.今回の症例は,全て現代的な治療を受けることが出来なかった時代の骨折であり,骨折は変形治癒している.骨折は,身体の自由度がかなり制限される疾患であることから,骨折の治療中は,家族や集落の仲間などからのさまざまな援助があったと思われる.このようなところに,科学的ではないにせよ,看護や介護の起源が求められると思われる.今後症例を集め,より多角的な見地からの検討することにより,看護や介護の起源を探ることが可能であると推測された.

原子力発電プラントは30~40年が寿命とされているが,最近の用地難あるいは経済性などの理由により20~30年の寿命延長の可能性についての議論が盛んになっている。 本「特集」では,他分野のプラントの状況分析なども含め,保守,管理,設計,材料などの観点から,原子力発電プラントの長寿命化について論じる。特に,安全性についてはPSAとの関係についても考え方を述べる。

Recently, telemetered microearthquake observation networks have been developed, and data acquisition and processing procedures have been automated in many networks, including automatic hypocenter determinations by using computers. As a result the detection capability and the accuracy of hypocenter determination of microearthquakes have been tremendously improved, which made a great advance in detailed microearthquake studies. We present a brief review on recent studies of seismicity based on microearthquake observations, focusing on spatial and temporal distributions of microearthquakes and their related problems.

電子ジャーナルやプレプリントサーバーなど,学術情報流通に係る技術の発展により,学術論文の出版数は,毎年増加しており,学術情報の洪水の中で,研究者や臨床家は,見逃せない論文を探すことが難しくなっている。獨協医科大学図書館は,教職員の学術成果へのアクセスの向上と研究と臨床活動を支援するため,世界中の研究者による生物学・医学領域論文の評価とレビューが参照可能なサービスFaculty Opinions(旧F1000Prime)の機関購読を,2020年度より開始した。本稿では,本学におけるFaculty Opinions導入までの背景,導入後の利用動態について報告し,医学・薬学系の高等教育機関でのFaculty Opinionsを利用するメリットと活用術について論じる。

The spring bedded ring model has been widely used for structural analyses of tire deformations under quasi-static and dynamic loading. Analyses on spring constants in the radial, the lateral and the circumferential directions, denoted by Kr, Ks and Kt respectively, have been conducted previously, however, were confined to only static cases. In this paper, by solving the equilibrated configuration of radial tire sidewall under inflation perssure and centrifugal force, we obtain finite deformations due to various loadings at the sidwall end near the tread. Then, we determined non-linear spring constants of a rotating radial tire Kr, Ks and Kt, which are compared with the experimental results given by referred papers.
