Background:There have been few studies on the clinical course of hypertrophic cardiomyopathy (HCM) in a community-based patient cohort in Japan.Methods and Results:In 2004, we established a cardiomyopathy registration network in Kochi Prefecture (the Kochi RYOMA study) that consisted of 9 hospitals, and finally, 293 patients with HCM were followed. The ages at registration and at diagnosis were 63±14 and 56±16 years, respectively, and 197 patients (67%) were male. HCM-related deaths occurred in 23 patients during a mean follow-up period of 6.1±3.2 years. The HCM-related 5-year survival rate was 94%. In addition, a total of 77 cardiovascular events that were clinically severe occurred in 70 patients, and the HCM-related 5-year event-free rate was 80%. Multivariate Cox proportional hazards model analysis showed that the presence of NYHA class III at registration was a significant predictor of HCM-related deaths and that the presence of atrial fibrillation, lower fractional shortening and presence of left ventricular outflow tract obstruction in addition to NYHA class III were significant predictors of cardiovascular events.Conclusions:In our unselected registry in an aged Japanese community, HCM mortality was favorable, but one-fifth of the patients commonly suffered from HCM-related adverse cardiovascular events during the 5-year follow-up period. Careful management of HCM patients is needed, particularly for those with the above-mentioned clinical determinants.

Although ionic and metabolic gradients across membranes in living cells are known to be vital to cell functioning, in plants both the stiffness of cell wall and complexity of the cell compartmentalization sometimes prevent the study of membrane transport. This difficulty has, however, largely been overcome with the aid of two techniques, one employing a vibrating electrode1) and the other a patch clamp (Hamill et al., Pflügers Arch., 391, 85 (1981)). The former technique has revealed the ionic currents which flow locally around various type of cells. In the introduction to the present article, the relation of local currents with several physiological phenomena, such as pollination and cell polarity, is reviewed. The patch-clamp technique has directly led to the development of research strategies, not only on the plasma membrane, but also on the vacuolar membrane and other intracellular organelles. In fact, various types of ion channels, such as voltage-dependent, Ca2+-regulated, and phytohormone-modulated channels, have been found in plant cells. In the latter half of this article, the properties of several ion channels in the plasma and vacuolar membranes of characean cells which were identified using the patch clamp are reviewed, and their physiological functions are summarized.

【目的】本研究の目的は、戦後、導入された学校給食の牛乳について、文化的・社会的な影響等を検証し、今後の学校給食における牛乳利用について検討することである。【方法】学校給食と牛乳導入の歴史については、乳業関係会社の社史、学校給食に関する史誌や統計資料、調査資料等を広く収集して調べた。学校給食と牛乳摂取に関する意識、実態については、学校給食と牛乳摂取に関するアンケート調査を、学校給食の経験者である長野市内のN短期大学学生と3小学校の保護者を対象に実施した。 【結果】危機的な食料不足と学童の深刻な栄養失調を背景に、旧日本軍の保管食料やララからの救援食料をもとに再開した学校給食では、脱脂粉乳のミルクが導入された。「学校給食法」にも、脱脂粉乳のミルクは、学校給食の基本的メニューとして奨励され、小児の栄養改善のため最も費用効果的な方法の1つと考えられ継続されてきた。昭和32年度より、国策として国産牛乳が学校給食で使用されるようになった。アンケート調査の回収率は、小学校85.4%、短期大学100%であった。米飯給食のとき、牛乳を「全く抵抗なく飲んでいる」という設問に小学生78.9%、短期大学生64.5%、「少し抵抗はあるが飲んでいる」に小学生15.8%、短期大学学生25.6%の回答が得られた。その他、牛乳飲用の時期等、今後の学校給食での牛乳利用に関する資料が得られた。

MM, MLおよびCRF患者の血漿中NEAAは健常者の値に比べ高値である。しかし, MMおよびML患者のEAA値は健常者と類似し, CRF患者のEAAは著しく低値であるために, 3疾患患者のE/N比は健常者の値と比べ有意に低値である。健常者のFischcr比は高値, ついでCRF患者, ML患者, MM患者の順であり, MMおよびML患者のFischer比は低値で健常者の値と有意差を示している。栄養状態の改善策として, CRF患者は必須アミノ酸摂取量を増加してE/N比を, MMおよびML患者は分枝鎖アミノ酸の摂取量を増加してFischer比の改善をはかる必要が示唆された。

肝硬変患者におけるグルコース利用能を評価する目的でグルコースを経口的あるいは経静脈的に投与した後のグルコース利用率と非蛋白呼吸商を間接カロリーメーターを用いて計測した. 経口的, 経静脈的投与のいずれにおいても対照に比し肝硬変患者ではグルコースの利用が遅延する傾向を認めた. 肝硬変患者を代償期群と非代償期群とに, あるいはChild分類別に分けた比較では肝硬変がより進展した群においてグルコース利用能は亢進していた. 経口的投与後のグルコース利用率はbody mass indexとの間に有意な負の相関を, クレアチニン身長係数比との間に有意な正の相関を認めたが, 血中アルブミン, コレステロール, プロトロンビン時間およびアンモニアとの間には相関を認めなかった. 肝硬変患者では進行例においても遅延しながらもグルコース利用能は亢進しており, このことは末梢組織でのグルコース利用の亢進を反映していると考えられる.

The mathematics used for Quality Enginearing is not the one based on statistical mathematics but the one called quadratic expansion.The degree of freedom is defined as the rank of adjoined matrix of each quadratic term of the expansion. The reason that the writer used the name of degree of freedom is also explaned in this article. Quadratic expansion is related with Hermitian expansion in a complex domain in mathematical form and is related with power spectral expansion from the viewpoint of communication theory.

アマサギは人に運ばれることなく急速に分布拡大した例とされるが,分散や渡りなど長距離移動には不明な点が多い.筆者らは,茨城県で捕獲されたアマサギ2羽の長距離移動を,太陽電池式の人工衛星用送信器を使って追跡した.2羽とも,捕獲した2006年の秋にフィリピン中部へ移動して越冬したが,その内1羽が翌春に中国揚子江河口周辺へ移動し,繁殖期のあいだそこに滞在した.そこは,前年繁殖地とした可能性の高い茨城県から1,900 km西に位置する.この結果は,東アジアに生息するアマサギにおいて長距離の繁殖分散を確認した初めての例である.

The tradition of diary literature, nikki bungaku, is a long and distinguished one within Japanese letters. Critical attention, however, has focused on classical diarists and not those of modern times. lbuse Masuji (bn. 1898) is one such present-day writer whose diaries, both his own and those kept by his fictional characters, are of interest not only for what they reveal about the author himself, but for what they may imply about the very nature of the genre.Ibuse’s own personal diaries frequently recall ―and in a sense, thus memorialize and revive― his unusually numerous friends and relatives who died premature deaths. lbuse’s diaries may be a means of expiating a kind of guilt he experiences as their survivor ; a guilt more fully explored in his fictional diaries, the most notable of which is Shigematsu's in Black Rain (Kuroi ame). Here, a survivor of the atomic holocaust in Hiroshima attempts to conceptualize, and thereby master, the trauma of the bomb by rewriting years later his brief diary of August, 1945.In conclusion, diaries, which by their periodic entries come to resemble a ritual, serve to bring a disorderly or even incomprehensible external world under the control of the writer's literary and recreative act.
