Peliosis hepatis(肝紫斑病)は,類洞の拡張と肝内に多発する血液の貯留腔を認めるまれな疾患で,WHOの肝腫瘍の組織学的分類では腫瘍類似病変に分類されている.本邦では腫瘍との鑑別に苦慮した症例の報告が散見されるが,肝全域にわたってPeliosis hepatisが発生,進展し,致命的な転帰となった症例は,過去にわずか1例が報告されているのみである.今回我々は,特徴ある組織学的所見を呈し,経過観察中に突然病態が悪化し急速に致命的な経過をたどった,肝全域にわたる特発性Peliosis hepatisの極めてまれな1例を経験したので報告する.

Objective: To identify the impact that role-lettering has on promoting self-understanding and understanding of others in nursing students, and to discover the educational implications of acquiring interpersonal and communication skills. Method: The research was designed as an exploratory validation of inductive and descriptive factors of role-lettering, and for validation of clinical hypotheses. Subjects were 215 2-year course nursing students in Kyushu Prefecture. They were mailed a self-administered survey and a qualitative analysis of their responses was conducted. Results: 102 surveys were returned, with 93 valid responses (43.3%). The valid surveys contained the following responses: "dissatisfaction and anger," "complaints and predicaments," "tension and mixed feelings," "expectations for others," and "confirmation of others' feelings." The returned surveys contained the following responses: "dissatisfaction and criticism," "valid complaints and justifications," " difficulty in understanding and response," "different perspectives," and "expectations and hopes for self." Based on the feeling of good or ill will towards others, limitations for self-understanding and understanding for others were revealed. Of the seven clinical hypotheses, five were validated. Discussion: Clarifying one's emotions through role-lettering and sharing the standpoint of others aids in self-dialog, promotes empathic understanding, displays an interest in and concern for others, and deepens self-understanding and understanding of others. Five of the seven clinical hypotheses were validated, and educational implications that promote self-understanding and the understanding of others obtained.

The purpose of this study is to examine the educational significance of the communication the pre-school children maintain with elder citizens. The study was conducted at the nursing care centers for the elderly that also provide day care survice for pre-school children. Chosen were the centers that maintained constant contacts between the elderlies and the pre-school children, and the situations involving communication between the two parties were observed, while the staff at the centers were interviewd. The data gathered were qualitatively analyzed. The results describe four aspects of the pre-school children's experiences: verification of one's own value, acceptance by elders, recognition of one's own self through communication with adults, and cultural education. The elderlies played a role similar to that of their grandparents, suggesting that the children felt at ease with them. In addition, children seemed to have mini-experience of the real world by communicating with the elderlies. Furthermore, the results of our interview suggest that the environment in which the children's activities are taking place in the presence of elderlies with disability may very well provide them with a basic understanding of normalization on a daily basis. in other words, exposure to such an environment may help the pre-school children to be easily accepting people with disability while preparing them to be adaptable to society.

光−電子相関顕微鏡法(Correlative light and electron microscopy: CLEM法)とは,同一試料を光学顕微鏡と電子顕微鏡を用いて観察し,両顕微鏡により得られた像の相関を得る解析法である.これまでに様々なCLEM法が開発され報告されているが,主に動物の培養細胞等が用いられており,植物の組織や細胞に適した方法は報告例は乏しい.植物組織・細胞においても,GFP等の蛍光で標識した生体分子の局在を高分解能で正確に捉えるためにはCLEM法の開発が重要である.我々は植物材料において,GFP蛍光を放つ細胞小器官の超微形態を高分解能走査電子顕微鏡で可視化する「GFP-走査電子相関顕微鏡法」の開発を進めている.細胞小器官をGFPで標識したシロイヌナズナ形質転換体の根端や子葉などの組織・器官を固定・脱水後,樹脂包埋する.そして,ミクロトームにより準超薄切し,導電性スライドガラスに載せ,そのGFP蛍光を共焦点レーザー顕微鏡により検出する.その後,その切片を電子染色し,高感度な反射電子検出器をもつ電界放出形走査電子顕微鏡により,蛍光を撮影した同一箇所の微細構造を撮影する.最後に,蛍光像と電顕像を重ね合わせることで,蛍光を放つオルガネラを特定し,その超微細構造を明らかにする.本技術は,蛍光タンパク質が普及した植物科学分野の超微細構造解析研究に大きく貢献できると期待される.

The capability trained by studying information design is considered as follows. A) Evaluating the relation between object and man, B) Arranging and analyzing information, C) Constructing the process, D) Cooperate in a group, appealing to society, E) Representing by applying technology. In order to enhance the above mentioned A) and B), we have conducted a message transmission game in the information design class. In this game, students were experienced the unusual method of communication, thus they could learn how to analyze the game results from various points of cognitive science view. As a result, we have concluded that the message transmission game was an effective method in the information design education.

Background:Distinction of paroxysmal atrial fibrillation (PAF) from non-PAF is important in clinical practice, but this is often difficult at the time of first documented AF. Given that fibrillation cycle length (FCL) is longer in PAF than in non-PAF, the aim of this study was to compare various clinical parameters including FCL to establish a scoring system to distinguish PAF and non-PAF.Methods and Results:The subjects consisted of 382 consecutive patients with AF on digital ECG at the present institute between 2008 and 2011. They were divided into PAF and non-PAF groups according to the following clinical course. Propensity score matching yielded 88 matched patient pairs with similar mean age and gender between the 2 groups. FCL was evaluated using customized fibrillation wave analyzer with fast Fourier transform analysis. On multivariate analysis, higher HR, longer FCL, and smaller LAD were independent predictors of PAF. For the scoring, cut-offs for each parameter were determined according to highest sensitivity and specificity on the ROC curves, and 1 point assigned for each parameter. Using this scoring system, 2 points detected PAF with 64% sensitivity and 84% specificity.Conclusions:We propose a scoring system including FCL to distinguish PAF from non-PAF. Further studies are needed to validate the results.

Charles G. D. Roberts, a leading poet of the "confederation group" in the late 19th century Canada, reworked the images of "Canada" created by Henry Wadsworth Longfellow's Evangeline (1847) to represent his Maritime homeland. Focusing on two of Roberts's early sonnets, "Blomidon" (1885) and "Tides" (1890), this article explores the images particularly chosen from the "American epic" to construct the representation of the Maritimes. The examination of Evangeline-driven images in his verses initially published in Century (New York) shows how Roberts wanted his readers to imagine his own "home". Readers will find that Roberts put emphasis on the sublime, rather harsh seascape which directs his readers' attention to the "Acadia destroyed" after the Acadian expulsion (1755), instead of the picturesque idyll emphasized in Longfellow's work. Such images of the Maritimes, however, may not be consistent with the tourism-oriented images of the area that Roberts as a "local authority" involved in its construction.

1670年にチャールズ2世によって与えられた特許に基づいて、毛皮貿易の独占のみならず行政権を含む種々の特権を有する植民地支配会社として設立されたハドソン湾会社が、1867年にカナダが自治領となって特許をはじめそのすべての特権を失った後、どのようにして今日まで生き残り繁栄してきたのかを概観したものである。本論文は、17世紀に設立された同様な植民地会社はすべてその姿を消してしまった中で、この会社だけが21世紀まで繁栄を続けることができた原因の幾つかを明らかにしている。

妊婦体操や分娩時の呼吸法については, 積極的な奨励により広く実施されている。本研究は, 妊娠の過程に伴う呼吸機能を測定し, 分娩時に用いられる呼吸法の指導に関する基礎的資料を得ることを目的とした。対象は正常経過の妊婦8人と非妊婦25人である。妊婦では, 妊娠初期, 中期, 後期の3回測定した。内容は, (1) 肺活量の測定, (2) 呼気ガス分析 (呼吸法実施時, 運動負荷実施時), (3) 心拍数 (運動負荷実施時), (4) 二酸化炭素分圧測定 (呼吸法実施時) である。呼吸法は腹式深呼吸, 胸式深呼吸, 短促呼吸および努責の代わりに息止めを行い, 各呼吸法の間に安静3分間をとって実施した。運動負荷は安静2分間を前後に4METS設定で10分間実施した。結果より腹式深呼吸, 胸式深呼吸では呼吸効率は良いが, 短促呼吸や息止めでは二酸化炭素のwashout効率が悪いため, 十分な深呼吸が必要である。運動負荷による換気量や心拍数は, 妊娠の経過とともに増加した。
