術後感染予防抗菌薬の目的は手術部位感染(SSI)の予防であり,遠隔部位感染を含むSSI以外の術後感染性合併症は対象としていない.手術を行う部位に常在する細菌をターゲットとし,広域スペクトラムを有する抗菌薬を使用しない事が原則である.術後感染予防に用いられる抗菌薬は,清潔創手術に対してはcefazolin(CEZ)あるいはsulbactam/ampicillin(SBT/ABPC)が推奨される.準清潔手術では,cefotiam(CTM),第2世代のセファマイシン系薬であるcefmetazole(CMZ),オキサ型のflomoxef(FMOX),あるいはCEZとmetronidazole(MNZ)の併用が推奨される.投与開始のタイミングは手術開始前1時間以内が推奨される.投与量・再投与は体重や腎機能によって調節が必要である.投与期間は術後24時間以内投与が基本である.

Soil is defined as a mixture of weathering products of rocks (sand, silt, and clay) and humus (or soil organic matter) derived from plant or animal. World soils are divided into 12 groups and their distribution is not uniform. Soil degradation includes soil acidification in humid region, salinization in arid regions, and loss of soil organic matter in both regions. Minimum tillage or optimized fallow systems can minimize loss of soil organic matter and improve soil fertility. Understanding of complex soil system is required for food supply and human survival.

It is important to understand the functional anatomy for motion analysis in the field of sport biomechanics and/or physical therapy. This article describes the functional anatomy of the muscles and joint movements in the lower extremity. The contents of this article are as follows: flexor hallucis, digitorum longus and ankle dorsiflexion; gastrocnemius medial head and tibial rotation; and hip external-rotators and hip flexion. As mentioned above, consideration of the relationship between the functional anatomy and impairments is important for physical therapy evaluation in the lower extremity.

NHKロボットコンテスト大学部門は1991年から開催され、既に20年継続している。筆者が所属する豊橋技術科学大学は、このロボットコンテストに1992年から参加しており、これまでに6回優勝している。ロボットコンテストの歴史は、3つの時期から成っている。第1期は1992年から2000年であり、この間は人がロボットに乗り、それを操縦して競技が行われた。第2期は2001年か2008年であり、この時期は、数台の自動ロボットと1台の手動ロボットが競技に使われた。試合展開が複雑になるルールであったため、この時期には、多くの種類のロボットと多彩な戦略が開発された。第3期は2009年以降である。本論分は、NHKロボットコンテストのこれまでの発展と現代の工学教育の問題、そして、ロボットコンテストが工学教育に与えた影響を述べる。

The recovering process of the anemia following 400ml blood donation in healthy female volunteers was studied hematologically. The advanced erythropoietic ability was observed from the first day following the phlebotomy. On the 14th day, the rates of reticulocyte and the mean corpuscular hemoglobin concentration normalized and the serum erythropoietin level indicated a maximum in spite of the decreases of serum iron and ferritin.There were no obvious side effects following a 400ml phlebotomy for healthy females. It was suggested that the 2 weeks after the 400m1 autologous blood donation would be a suitable period for orthognathic surgery.

【目的】日本摂食嚥下リハビリテーション学会嚥下調整食分類2013(学会分類2013)によって,病院や高齢者施設における嚥下調整食の段階の共通化が進んでいる.しかしながら,食事形態の説明が文言のみで物性値が示されていないため,特にコード3 や4 で人によって解釈が異なることがある.嚥下調整食分類をさらに発展させるためには,不均質な食品の物性測定方法の確立が必要である.本研究では,コード4 に基づいて調理された嚥下調整食のかたさを測定し,その目安を示すことを試みた.【方法】京都市内の介護老人保健施設の27 日間の昼食で提供した軟菜食(学会分類2013 コード外)の主菜および副菜中の199 食材とやわらか食(コード4 相当)の主菜および副菜中の156 食材を試料とした.施設で提供した嚥下調整食は,言語聴覚士,管理栄養士および調理師が学会分類2013 のコードに適応しているか否かを検討して提供可能と判断したものであった.やわらか食の喫食者は主に咀嚼機能が低下した利用者であった.使用食材を嚥下調整食の中から取り出してクリープメータRE2-3305C(山電)の試料台に置き,直径5 mm のプランジャーを用いて測定速度1 mm/s,試料温度20℃で破断測定を実施した.歪率0~90% における最大値をかたさとした.1 食材につき5 サンプルを測定して平均値を算出した.【結果】やわらか食では156食材のうち150食材(96%)でかたさは200 kPa 未満であったのに対して,軟菜食は199食材中81食材(40%)のかたさが200 kPa 以上であった.本研究のかたさ測定方法により,やわらか食と軟菜食のかたさの違いを評価できた.比較のために測定したユニバーサルデザインフードの歯ぐきでつぶせる(コード4 相当)に区分される商品に含まれる食材のうち,にんじんやじゃがいものかたさは200 kPa 未満であったが,しいたけや肉類は200 kPa 以上となった.【結論】この物性測定方法はコード4 に相当する嚥下調整食の新たなかたさ評価方法となる可能性が示唆された.

This study examined how the rapid spread of telework triggered by the COVID-19 pandemic has affected organizational behavior, especially teamwork. The spread of COVID-19 in 2020 led to a significant increase in telework in Japanese companies. Since telework involves non-face-to-face communication, it may have a negative impact on teamwork. We compared the teamwork of 23 teams in an organization by conducting a pre- (January 2020) and post-survey (May 2020) during the first wave of COVID-19 in Japan. We found that the proportions of telework and team virtuality increased significantly from pre-survey to post-survey. However, the teamwork and team performance data remained almost constant. Rather, on examining the data from May 2020 (post-survey), we found that team virtuality, especially technology use, had a positive correlation with teamwork. Thus, the rapid spread of telework and team virtuality does not necessarily hamper teamwork; rather, the technology-proficient teams may have excellent team processes and high team performance.

Background: Patients with acute pulmonary embolism (APE) often have negative T waves (Neg T) in precordial leads at presentation, but this is also found in acute coronary syndrome (ACS) caused by left anterior descending coronary artery (LAD) disease. Methods and Results: Differences in Neg T on admission electrocardiograms were studied between 107 patients with APE and 248 patients with ACS caused by LAD disease. All patients had Neg T in leads V1–4 and were admitted within 7 days from symptom onset. The number of leads with Neg T (4.8±1.8 vs. 5.5±1.7, P<0.001) and maximum magnitude of Neg T (3.4±2.0 vs. 4.7±3.3mm, P<0.001) were lower in APE. The frequency of occurrence of Neg T in each of the 12 leads, and the precordial lead with the greatest Neg T (peak Neg T) differed between APE and ACS (all P<0.05, respectively). APE was strongly associated with the presence of Neg T in both leads III and V1 and peak Neg T in leads V1–2. The combination of these 2 findings identified APE with 98% sensitivity, 92% specificity, and 94% predictive accuracy, which represented the highest diagnostic accuracy. Conclusions: Among patients with APE and ACS who have precordial Neg T, the presence of Neg T in leads III and V1 and/or peak Neg T in leads V1–2 simply but accurately differentiates APE from ACS.  (Circ J 2014; 78: 483–489)

【目的】台湾の家庭では薬膳スープを作り、食されている。四物湯はその中でもよく作られているスープである。本研究は台湾の家庭の調理方法により、加熱時間による呈味と呈味成分の変化を調べるともに、日本人を対象に嗜好調査を行った。【方法】生薬は台湾の漢方専門店で購入し、鶏肉は市販手羽元を用いた。四物湯の調製は鶏肉480gと生薬(当帰・熟地・川芎・芍薬)47gと純水1150ml(台湾ではこの1/3量を使用するのが一般的)を加えて加熱した。加熱時間は30、45、60、90分とした。加熱には「大同電鍋」(間接釜式の電気炊飯器)を用いた。スープは加熱終了後1000mlに定容した。呈味成分の測定試料は一定量のスープを同量のn-ヘキサンで脱脂し、終濃度80%のエタノールで除タンパク後、減圧蒸留した。呈味成分はIMP、イノシン、ヒポキサンチン(HPLC)、還元糖(ソモギ・ネルソン法)、乳酸(酵素法)、タンパク質(Lowry法)を測定し、さらに、スープの官能検査(2点識別・嗜好法変法)を行った。【結果】IMPは45分のスープにもっとも多く含まれ、タンパク 質や還元糖は90分のスープで多く、乳酸は60、90分で多い傾向がみられた。呈味成分全体としてみると、60分スープの量がもっとも多かった。官能検査では45分のスープを基本として比較した。30分はうま味やこくがなく、60分はうま味やこくが弱い傾向であったが有意差はなく、90分はうま味やこくが少なく好ましくないと評価された。官能検査では、45分のスープが最もおいしいと評価された。台湾の家庭では加熱時間を経験的におよそ1時間としており、その加熱時間の妥当性が明らかになった。

さまざまな宗教現象において、「享楽」と呼ぶべき何かがあると考えうる時点は少なくない。宗教の享楽とは何か。この問いに答えるための予備考察として、ジャック・ラカンの晩年に見られる「享楽の類型学」と呼びうる部分を簡潔に整理し、享楽の定義から始めて「絶対的享楽」「二つのファルス的享楽」および「対象aの剰余享楽」という、いくつかの享楽の類型を提示する。そして、それらの概念が明らかに「宗教的」なものと関係があり、宗教現象分析のための概念として使用可能であることを指摘する。また、彼が最後に提出した「大他者の享楽=女性の享楽」が、他の享楽を「超過する」ものであるばかりか、神秘家の伝統に関わるものとして、精神分析自体の「歴史的限界」を露わにすることを呈示する。
