痙縮筋の特徴として,安静時の短潜時伸張反射は亢進する一方で,歩行能力に関与する中・長潜時伸張反射は減弱することがあげられる.本稿では,近年の研究を参照しつつ,痙縮筋に対する筋力強化について概説した.痙縮筋に対して抵抗運動を行う際には,連合反応を抑制する必要性はなく,また,拮抗筋の過緊張ではなく,主動筋の筋力低下に対するアプローチにより機能向上を図ることの重要性を示した.具体的な事例として,脳性麻痺児・脳卒中後片麻痺患者に対する抵抗運動により筋力強化に効果が示され,歩行能力が高まることを解説した.しかし,麻痺側の随意性がない場合には,直接的に上肢や下肢筋群を収縮させることは難しい.その場合は,遠隔の随意性の高い部位からの抵抗運動による間接的アプローチ法により歩行の改善が可能なことを紹介した.また,抵抗運動により,中・長潜時伸張反射の減弱が改善される生理学的なエビデンスについて十分ではないが,H 波と fMRI による研究を紹介した.

産婦人科志望の初期研修医が執刀した帝王切開の安全性と,その経験が初期研修医の心境に与える影響を検討した.2014-2019年に当院で産婦人科志望の初期研修医9名が執刀した帝王切開66例(初期研修医群)を対象とした.指導医8名が執刀した帝王切開147例(指導医群)を対照群とし臨床背景,手術情報,転帰を検討した.心的影響の評価に初期研修医へアンケート調査を行った.手術時間,執刀開始から児娩出までの所要時間の中央値は初期研修医群で83.5分と14分,指導医群は61分と7分で初期研修医群は有意に長かった.臍帯血動脈pHの中央値は初期研修医群で7.306,指導医群は7.33で初期研修医群は有意に低値であった.アンケートでは研修意欲向上や進路への影響など執刀経験による肯定的意見が多数みられた.初期研修医の帝王切開は指導医の管理下ならば安全性は許容範囲で研修意欲の向上やリクルートに寄与する可能性がある.

許しと感謝のエクササイズは、ネガティブな出来事や自身の弱さ等に「ゆるします」や「ありがとう」と唱えることで,それらをありのままに受け入れ、さらにポジティブな面を見出し、最終的には些細で当たり前な出来事(目が見えること、家族がいること等)への気づきを通してより前向きな捉え方を身に着けていくことを目指すものである。うつ病16名を含む23名の患者を無作為比較試験を行った結果、BDI-II、自己受容感、日本版寛容性尺度、反応スタイル尺度の問題への直面化および日本版-外傷後成長尺度は、介入により有意に改善することが明らかとなり、本プログラムはうつ病を主とした患者に対して有用であると考えられた。

災害の影響を緩和する上で救援物資輸送は重要な課題である.救援物資の迅速かつ的確な輸送のために,我が国ではプッシュ型とプル型の二種類の戦略を計画している.しかし過去の災害では,ラストワンマイル輸送や被災地からの情報伝達でボトルネックが発生し,これらの戦略は期待通り機能しなかった.この経験に基づき,被災地に直接輸送する体制やICTに依存しない情報伝達体制が提案されている.本研究では,提案されたプッシュ型とプル型の戦略を定量的に評価するために,直接輸送が可能な輸送ネットワークや継続的な通信が不可能な情報ネットワークにおける在庫輸送戦略を分析する.その結果,直接輸送が最適であることを証明し,情報伝達が断続的な場合にプル型がプッシュ型よりも性能が低下する可能性とそれを回避する方策を示した.

猫の口腔内悪性神経鞘腫に対してカルボプラチン混和リドカインゼリーの局所注射とオルソボルテージ照射の併用療法を実施した.局所注射に起因する有害症状は認められず,腫瘍は顕著に縮小した.治療開始時からPEGチューブ栄養管理を行い,全身状態の改善を行った.今回用いた併用療法はオルソボルテージ照射単独治療と比較して効果的である可能性があるが,症例の集積による検討が必要である.

A 10-year-old 4.7-kg spayed female mix breed dog presented with mucoid vaginal discharge despite treatment for cystitis for a month. A malignant neoplasm was suspected based on atypical cells found on vaginal mucus cytology. A cystoscopy biopsy was performed, and the histopathologic diagnosis was consistent with transitional cell carcinoma (TCC). A subtotal urethrectomy and urethral-vaginal anastomosis were performed. Treatment with piroxicam (0.3mg/kg eod for 1278 days) and carboplatin (150~250mg/m2/3~4 weeks, nine times) was initiated 2 weeks postoperatively. Other than recurrent cystitis that started 3 weeks postoperatively, the patient maintained a good quality of life with neither recurrence nor metastasis. The patient died of renal failure 3 years and 9 months (1395 days) after the initial visit. While TCC is generally considered to have poor prognosis, it was suggested from the management of this case that early diagnosis and radical surgery could achieve good long-term survival.

Feline leukemia virus (FeLV) is a retrovirus that causes immune suppression and immunodeficiency, leading to opportunistic infections and leukemia/lymphoma in cats. Today, a variety of domestic mammals are kept in houses, and it is important to evaluate the possibility of interspecies transmission of FeLV. In this study, we assessed the infectivity of FeLV-B in ferrets that belong to Mustelidae. By pseudotype virus infection assay, we revealed that a ferret cell line, Mpf cells, is resistant to FeLV-B infection. The mRNA expression level of the FeLV-B receptor, Pit-1, was approximately half that of cat FEA cells in ferret Mpf cells. There was no significant difference in receptor usage between ferret’s and cat’s Pit1. These data may indicate the presence of the post-transcriptional modification and/or the restriction factor(s) against the FeLV-B infection in ferrets.

The purpose of this paper is to examine the current status and challenges to the final disposal of radiocesium-contaminated soil and to explore a theoretical framework for building shared knowledge and collaboration among a wide range of stakeholders. It is difficult to make effective solutions about the issues only by accountability from stakeholders. It is necessary to build common goals for cooperation among stakeholders.

Canine lymphoma/leukemia cell lines with p16 protein expressions: high (17–71 and GL-1) and low (CLBL-1, CLC, Nody-1, and UL-1) were treated in vitro with cyclin-dependent kinase 4 and 6 (CDK4/6) inhibitors, palbociclib or abemaciclib. Cell proliferation decreased as a result, with higher IC50 levels observed in the high p16 (17–71 and GL-1) and one low p16 (UL-1) cell lines compared with the low p16 cells (CLBL-1, CLC, and Nody-1). As expected, palbociclib and abemaciclib treatment reduced pRb phosphorylation in a dose-dependent manner, especially in cells with low p16. These results suggest that CDK4/6 inhibitors have potential as new chemotherapeutic agents for canine lymphoma and high p16 protein expression may be used as a biomarker for resistance to CDK4/6 inhibitor therapy.

A 12-year-old French bulldog presented with a mass on the right lower jaw. Computed tomography revealed that the mass originated from the salivary gland and was not continuous with the right mandible. The mass was surgically removed and subsequently diagnosed as extraskeletal osteosarcoma of the salivary gland following histopathological and immunohistochemical examinations. Although the surgical margin was clear, postoperative adjuvant therapy with nonsteroidal anti-inflammatory drugs (NSAIDs) was initiated. Lung metastasis was observed 249 days after the initial examination; therefore, toceranib was initiated with NSAIDs. The dog died 496 days after the initial examination of this disease progression but had good long-term management with a combination of surgery, NSAIDs, and toceranib.

本研究は、台湾金門島に現存する伝統的集落の移住経緯と空間特性を明らかにすることを目的とする。これまで個別に捉えられてきた全143の伝統的集落の立地環境、歴史的建築物の種類と残存数、集落規模、建物棟数、方位軸、抱護の種類と空間範囲、居住者の主要姓氏、移住前後の居住地をまとめ、集落空間を類型化した。その結果、立地環境を平地から山裾、斜面地までの10類型、自然地形と植生による抱護の種類を9つに分類した。さらに集落を構成する建物棟数から空間規模を小・中・大に設定し、歴史的建築物の残存数と合わせて、各集落の現状を明らかにした。また、居住者の主要姓氏から単姓村76、複姓村67が確認された。文献資料とヒアリング調査に基づいて長期間にわたる移動前後の居住地とその変遷を追い、現地調査により居住地の環境を理解することで、集落の移住形態を5つの型(移住群:拠点型Ⅰ、Ⅱ、転移型、定住群:発展型、安定型)に分類した。これらの結果から、周囲を海に囲まれた島嶼の環境と歴史的な居住地の移動によって構築された集落の空間特性を明らかにした。
