We analyzed the outcome of 25 SAH patients who were over 80 years old. We performed early operations on SAH patients between the ages of 80 and 85, and intentionally delayed operations on patients over 85 years of age (7 cases with neck clipping and 3 cases with endovascular coil embolization). Four of 10 patients in the operative group (40%) had favorable outcomes. The risk factors that worsened the outcome were cerebral vasospasm and general complications. All in the non-operative group had a poor outcome. In elderly SAH patients, an acceptable treatment plan must be made from a social and economic viewpoint.

This paper investigates whether the form of the legislative institution - citizen assembly versus elected parliament - a¤ects the level and composition of local public expenditure. Our empirical analysis focuses on medium-sized and mostly German-speaking communes in Switzerland that switched from assembly to parliament between 1945 and 2010. Event study estimates suggest that parliament adoption increases total spending by about 6 percent and that this increase is driven mostly by general administration and education spending. To understand potential mechanisms at play, we run a survey among assembly participants and document a sizeable under-representation of 20- to 40-year-olds, as well as of women in assemblies compared to both voters in elections and to the electorate at large. Since these two demographics have relatively strong preferences for public spending on education in our setting, switching from citizen assembly to parliament likely increased their representation in the political process.

近年の低成長期において価格競争が激化し,単一のコスト変動要因にのみに着目して,限界利益のみを見る経営が困難になりつつある.また,市場環境の変化に対して製品のコストや損益がどのように変化するかを迅速に把握する必要がある.本研究ではプロセスモデリングをコスト計算に応用し,市場環境の変化に即応して,製品のコストや損益を構造的かつ詳細に把握できるコスト管理方法を提案する.  プロセスモデルでは,コスト変動要因毎に,トランザクションと呼ぶアクティビティの集合を定義する.そして材料費,設備費などのコストをアクティビティ毎に把握する.そしてプロセスモデルをコスト構造マトリックスに変換し,市場環境変化を与えたときのコストと損益の算出方法を明らかにする.これにより経営状況の変化に対応でき,数値的な根拠を意思決定の際に与えるコスト管理モデルを提供する.

ジストニアは自分の意思に反して、筋肉の収縮や硬直が持続する病気である。遺伝性ジストニアであるDYT12はナトリウムポンプα3サブユニット遺伝子(ATP1A3)の機能喪失変異が原因である。DYT12の病態解明を目的にAtp1a3遺伝子欠損マウスを解析した。当該マウスはジストニアを引き起こしやすく、小脳の抑制性神経の伝達効率亢進が観察された。この結果から、Atp1a3の機能喪失が抑制性神経の機能に影響し、ジストニアを引き起こすと考えられた。

大規模CGM(consumer generated media)コミュニティでは,ユーザがコンテンツを作成し続ける必要がある.特にオンラインコミュニティでは,面識のない膨大な数のユーザを管理し,コミュニティを維持・活性化することは容易ではなく,面識のある知り合いベースのコミュニティとは異なる運営が必要となる.本研究では,持続可能なCGMコミュニティの構築を目的とし,流入,突破,継続,転換の4つの概念ループからなるCGMコミュニティ育成モデルを提案する.また,プログラミングに関する問題解決支援サービスteratailに本モデルを適用した.その結果,本モデルがCGMコミュニティの育成管理に有効であることが示唆された.同時に,定性的な側面であるコミュニティで生成されるコンテンツの品質管理についても考察し,コンテンツの品質を上げるためには,コンテンツ投稿者にコンテンツに対するフィードバックを与える手法が有効であることを示した.しかし,コンテンツ品質の向上施策は短期的かつ定量的なコミュニティの育成とは反比例することがわかった.以上より,持続可能な大規模オンライコミュニティの運用には,コミュニティを支えるプラットフォームの機能を充実化させる施策と,コミュニティの運用ルールを充実化させる施策の2 つの観点が必要不可欠であることを明らかにした.

The Katsura villa is very famous in Japan as one of the most excellent Sukiya architecture. It was the villa of Katsurano-miya Household. Katsurano-miya had four villas. One of them was ofcourse the Katsura villa. Other three of them were the Takagamine villa (Kita-ku, Kyoto City) , the Misasagi villa (Nishigyo-ku, Kyoto City) , the Kaiden villa (Nagaokakyo City). These three villas are now not in existence. This is the study on the Takagamine villa (Takagamine-oyashiki). The Takagamine villa was situated in Takagamine-dotenjo-cho, Kita-ku and Kinugasa-kagamiishi-cho, Kita-ku. There were five graceful and simple Chaya (small pavilion) , Getsuro, Kan-un, Machibito, Shunju, Kairaku.

11歳,避妊済み雑種雌猫の四肢掌球が腫脹し,漸次悪化したため,発症5カ月後に受診した。組織学的に真皮内に著しいプラズマ細胞浸潤を認め,血清蛋白分画で高ガンマグロブリン血症がみられた。プラズマ細胞性肢端皮膚炎と診断し,塩酸ドキシサイクリン5 mg/kg BIDで治療した。治療7週間後に略治,休薬した。その後8ヶ月再発を認めていない。

EBP, Evidence-Based Practice, appeared in the area of social work at the beginning of 2000. EBP was formally established in medicine in the 1990s, but now exists as a powerful methodology with which integrate science and intervention through social work. This article has three purposes: 1) to review the history of EBP and the reason for the appearance of EBP in the social work area; second, to examine definitions of EBP and steps for practice; thirdly, to pay attention to the use of EBP in social work education and social practices. The use of EBP in future social work will also be examined.

目的:看護師の直観に基づく意思決定に関する研究の動向を整理し,今後の研究課題を明らかにする。方法:PubMed,MEDLINE,CINAHL,医学中央雑誌Web版を用いて“intuition(直観)”“intuitive”“decisionmaking(意思決定)”“nursing(看護)”“nurses(看護師)”をキーワードに2017年9月までに発表された論文を検索し22件をレビューの対象とした。結果:22件の文献を分析した結果,【直観に基づく意思決定と経験】【看護実践における直観に基づく意思決定】【直観に基づく意思決定を育むストラテジー】の3つのカテゴリーが抽出された。介入研究は見当たらず,探索的研究と記述的研究のみであった。結論:今後は直観に基づく意思決定の思考プロセスの探究のため,他の学問分野と協働しその構造を具体的で簡潔に提示する必要がある。

National Statistics Center has started the initiative in promoting statistical literacy with a framework including a series of 3 activities since 2018. These activities are developing and disseminating SSDSE (Standardized Statistical Data Set for Education), preparing educational materials using the SSDSE, and holding an annual data analysis competition which also uses the SSDSE. This framework has potential to promote the use of official statistics, to make chances discovering interests in data analysis, and to rise positive attitudes to learn.
