Pulmonary hypertension (PH) induced by pulmonary tumor thrombotic microangiopathy (PTTM) can be fatal because its rapid progression confounds diagnosis, and it is difficult to control with therapy. Here we describe a woman with symptomatic PTTM-PH accompanying gastric cancer that was suspected from perfusion scintigraphy. PTTM-PH was diagnosed by gastroesophageal endoscopy and lung biopsy after partial control of PH using the platelet-derived growth factor (PDGF) receptor (PDGFR) tyrosine kinase inhibitor, imatinib. Treatment with sildenafil and ambrisentan further decreased PH, and she underwent total gastrectomy followed by adjuvant TS-1 chemotherapy. PH did not recur before her death from metastasis. Postmortem histopathology showed recanalized pulmonary arteries where the embolized cancer masses disappeared. PDGF-A, -B, and PDGFR-α, β expression was detected in cancer cells and proliferating pulmonary vascular endothelial cells. Thus, PTTM-PH was successfully controlled using a combination of imatinib, drugs to treat pulmonary arterial hypertension, and cancer management.

Overseas, the surveillance on companion animals have been reported by "NASPHV (National Association of State Public Health Veterinarians)", "CCWHC (Canadian Cooperative Wildlife Health Centre)", "ECDC-EFSA (European Center for Disease Prevention and Control - European Food Safety Authority)", "CSVID (Chilean Society of Veterinary Infectious Diseases)". According to pet boom in Japan, the disease surveillance of pets utilized IT (information technology) becomes available by spreading of "insurance-for-pets" and Electric Health Record in animal hospitals. We report challenges and results of new pet-surveillance by using of clinical data in animal hospitals. Clinical data of companion animals were analyzed by using cloud computing and electrical patient records without personal identifiers. Analyzed history, profile and clinical information of cases were rapidly feedback to veterinary surgeons in automated fashion from the system. The surveillance is designed with intention to answer some questions of the transmission routes of zoonotic diseases. Cloud computing combined with electrical patient records provided environment for easy daily operation with automated real-time feedback of data collation and limited descriptive analysis as additional advantage. Public and animal health epidemiologist may be able to provide further time trend, statistical, and geo-spatial analysis using with whole or subsets of this readily available database. Together the surveillance will improve understanding of pet animal health in the community and transmission of zoonosis in between human and animal.

This study aimed to investigate whether valproic acid (VPA) pretreatment enhances the therapeutic effectiveness of mesenchymal stem cells derived from bone marrow (BMSCs) transplanted into rats with an acute spinal cord injury (SCI). BMSCs were pretreated with VPA before transplantation and then intravenously injected 1 week after SCI. Before transplantation, levels of CXC chemokine receptor 4 (CXCR4) expression in BMSCs were tested using quantitative real-time PCR and Western blotting. Stromal derived factor-1 (SDF-1), the unique ligand of CXCR4, was quantified using RT-PCR and immunofluorescence. The locomotor function of rats with an SCI was evaluated using the Basso, Beattie, and Bresnahan (BBB) locomotor rating scale. Fluorescence microscopy and hematoxylin-eosin (HE) staining were also performed to evaluate pathophysiological changes after transplantation. On day 7 after SCI, the level of SDF-1 expression peaked. CXCR4 expression increased significantly in BMSCs pretreated with VPA. After intravenous transplantation, BrdU-labeled BMSCs were noted at the spinal injury site, and this was especially true for BMSCs pretreated with VPA. More significant functional improvement was observed in rats receiving BMSCs pretreated with VPA than in other groups of rats. AMD3100 partially inhibited improvement. This study demonstrates that pretreatment with VPA before transplantation enhances the therapeutic benefits of BMSCs in terms of greater cell migration and better neurological outcomes after traumatic SCI. The mechanism of this enhancement may be related to the SDF-1/CXCR4 axis. Therefore, pretreatment of BMSCs with VPA warrants further study in relation to the treatment of traumatic SCI.

The Japan Meteorological Agency has deployed strain-meters in Shizuoka, Aichi, and Nagano Prefectures, aiming to detect a slow slip phenomenon that is expected to occur just before the anticipated great earthquake of M8 class at the plate boundary. In this paper we propose a stacking method in which data at different stations are added according to the following procedure. First, the observed waveform at each station is normalized by the noise level. Then, the normalized wave form is stacked one by one in the order of the magnitude of the signal to noise ratio, by reversing the polarity considering an assumed source location, if necessary, so as the signal to be all positive. This stacking procedure is stopped when the synthesized signal to noise ratio of the stacked waveform becomes the maximum. Members in the optimum data set that gives the largest signal to noise ratio become different if the location of the assumed source is different. It is shown that the signal to noise ratio of the stacked waveform obtained by this method is enhanced 2.3 times of that of the most favorable individual data. A slow slip event as small as Mw 5.0 would be detected if it occurs within the source area of the anticipated Tokai earthquake. We think the stacking method is applicable not only to the strain-meter data, but also to the GNSS and tilt-meter data and such an extension of the method may be effective to detect rising of magma beneath volcanoes.

False recognition can occur at high levels after participants study lists of associated words and are tested with semantically related lures. The present study attempted to determine the differences of young and elderly adults in retention interval on false recognition and remember-know judgments of critical lure. Young participants' mean age was 21.6 years (range 21-23 years). Elderly participants' mean age was 69.7 years (range 65-74 years). Participants engaged in estimating the frequency of usage of visually presented words, and they were tested both immediately and one week later after the study phase. We found that elderly adults were relatively more susceptible than young adults to this false recognition effect. Moreover, a reliable increase of false recognition for critical lures was observed across retention interval in young adults but not in elderly adults.

The effects of inhalation anesthesia (2% isoflurane, sevoflurane, or enflurane) and intraperitoneal anesthesia with pentobarbital (65 mg/kg) were compared in rats using an electrocardiogram (ECG) and determination of blood oxygen saturation (SPO2) levels. Following inhalation anesthesia, heart rate (HR) and SPO2 were acceptable while pentobarbital anesthesia decreased HR and SPO2 significantly. This indicates that inhalation anesthesia is more preferable than pentobarbital anesthesia when evaluating cardiovascular factors. Additionally, pentobarbital significantly increased HR variability (HRV), suggesting a regulatory effect of pentobarbital on the autonomic nervous system, and resulted in a decreased response of the baro-reflex system. Propranolol or atropine had limited effects on ECG recording following pentobarbital anesthesia. Taken together, these data suggest that inhalation anesthesia is suitable for conducting hemodynamic analyses in the rat.

This paper compares satellite microwave radiometer and ground-based radar observations with the Japan Meteorological Agency nonhydrostatic model (JMA-NHM) simulations, using a bulk microphysical parameterization, for a typical rainfall system associated with the Baiu front around the Okinawa Islands, Japan, on 8 June 2004.  The JMA-NHM correctly replicated the shape, location and intensity of the precipitation associated with the observed rainfall system. Radar reflectivities and microwave brightness temperatures (TBs) were simulated using output from the JMA-NHM simulations. They were then compared with concurrent corresponding observations by the National Institute of Information and Communications Technology (NICT), CRL Okinawa Bistatic Polarimetric Radar (COBRA), and Advanced Microwave Scanning Radiometer for EOS (AMSR-E). Fairly good agreement was obtained between the simulated and observed reflectivities under the melting layer and TBs at low frequency (18.7 GHz), indicating that the JMA-NHM adequately simulated the amount of liquid hydrometeors. However, the intensity of scattering in the simulations was stronger than that in the COBRA observations above the melting layer and the AMSR-E observations at high frequencies (36.5 and 89.0 GHz). This was due to the fact that the JMA-NHM overestimated the amount and size of snow particles as a result of large depositional growth.  The excessive snow contents were reduced by adjusting some of the microphysical processes in the JMANHM: the snowfall speeds were increased and a riming threshold for snow to graupel conversion was changed. These adjustments helped to reduce the amount and size of snow, resulting in further agreement with the COBRA observations. These adjustments also further improved the simulated TBs at high frequencies, especially at 36.5 GHz. However, differences still exist between the simulated and the observed TBs at high frequencies, suggesting that additional adjustment to and improvement of the snow microphysical processes are needed for the application of the model to microwave remote sensing of precipitation.

The process of formation of austenite has been studied in low-carbon low alloy steels. The specimens with acicular structures such as martensite, bainite or tempered martensite are heated at temperatures between Ac1 and Ac3. The structual changes in these specimens during heating are observed in detail.The main results are as follows.1) Austenitizing processes are divided into two stages. In the first stage globular austenites are formed accompanying the dissolution of cementite. Acicular austenites are as well formed at an early stage.2) In the second stage a globular austenite is formed by the coalescence of the acicular austenite grains and those formed in the both stages grow to form extended austenite grains.3)γ′ transformation prevails in a boron containing steel.Based on these results, it is proposed that the reverse transformation are mainly controlled by the rate of recrystallization of austenite.

近年注目されている説によれば、飛鳥の須彌山石は、夷狄が当時の日本の朝廷に対して行う服属儀礼のための装置であったらしいとされる。そして、その儀礼は帝釋天や四天王に関連する神聖なあるいは呪術的なものであったらしいとされる。さらには、『倶舍論』にもとづきつつ、須彌山石と東大寺大仏蓮弁の須彌山図とには共通点が見られるので、須彌山石は意匠的にも須彌山を象徴したものであろうとされる。しかしながら、『日本書紀』や『倶舍論』などの内容を確認してみると、以上のように解釈することは適切ではなさそうである。須彌山石は、文化力誇示のための装置と見るほうがよいであろう。また、その意匠についても、さらに検討が必要であろう。

1970年から1979年までの厚生省発表のによる全死亡数6,982,264名の年齢別死亡率によると, 胃癌では55~59歳, 胃潰瘍では75~79歳, 十二指腸潰瘍では35~39歳が最高である. また1970年から, 1979年までの日本病理剖検輯報の剖検数257,868名の年齢別死亡率によると, 胃癌では70~74歳, 胃潰瘍では65~69歳, 十二指腸潰瘍では25~29歳が最高である.次にわれわれが宮城県で実施している胃集団検診成績のうち1961年, 1966年, 1971年, 1976年, 1981年の受検者総数407, 695名の年齢別頻度によると, 胃癌では70歳, 胃潰瘍では65~69歳, 十二指腸潰瘍では29歳以下が最高である. すなわち, これらの成績は胃癌と胃潰瘍が典型的な老人病であることを示している.人間の生命維持に最も重要な機能の1つは消化吸収で, これに関与するのは胃液と膵液の分泌機能である.われわれは内視鏡的胃粘膜正常者245名について胃液分泌能を測定したところ, 胃液分泌量, 最高酸濃度, 最高ペプシン濃度はいづれも70歳を過ぎると減少する傾向を認めた. 次に膵疾患を除外された507名について膵液分泌能を測定したところ, 膵液分泌量, 重炭酸分泌量, アミラーゼ分泌量はいづれも加齢によっては減少しない. それで健康者の脂肪吸収試験を行ったところ, 脂肪1日摂取量が30g以下では加齢による脂肪吸収の滅退はみられないが, 脂肪1日摂取量が35~40gになると, 70歳以上では滅退する傾向を認めた. したがって, 健康な老人では食餌量が適量であれば加齢による消化吸収の障害はみられない.最近日本人の平均寿命が伸びて世界1の長寿国となったが, 文献によると, 日本人は従来から長寿だったようである. 古代中国の史官陳寿 (233~277) の記した「魏志倭人伝」によると, 3世紀の日本人の寿命は百歳であった. また江戸時代の医学者貝原益軒 (1630-1714) の著わした「養生訓」(1713), 香月牛山 (1656-1740) の著わした「万民必用長命養生訓」(1716) でも, 日本人の寿命は百歳と記されている.ドイツの医学者 Christoph Wilhelm Hufeland (1762-1836) はその著書のなかで天寿二百歳説を提唱したが, これを現代日本人にあてはめると天寿120歳となる. また, 約140億の脳細胞が全部死滅するのは190歳と算出されているから, その過半数が残存する120歳位を天寿としてよいと考えている.われわれは, 今後天寿120歳を目ざして研究をすすめるべきであると考える.

In the oral region, traumatic neuroma is rarely encountered in daily clinical practice. As this lesion is caused by some kinds of trauma, traumatic neuroma may develop after operations such as tooth extraction. Traumatic neuroma appears to be a tumors, however, pathologically it is not thought to be a neoplasm, but nodular hyperplasia in nerve fibers. A typical case of traumatic neuroma is presented. A 32-year-old woman with a nodule arising in the left mental foramen region was referred to us. The nodule was first noted 6 years ago and persisted without any change until treatment. Before the nodule had appeared, the patient underwent surgical removal of a mucocele from the left mental foramen at another hospital. After the operation, the same region underwent surgery twice because of recurrence. The nodule was slightly tender, covered with normal mucosa, and red-bean size at presentation. A traumatic neuroma was diagnosed clinically. The tumor was removed under local anesthesia and examined histopathologically. Numerous distinct neural bundles with densely fibrous connective tissue were observed histopathologically in the specimen. The histopathological diagnosis of traumatic neuroma was established. No signs of recurrence have occurred so far.

Genetic defects of hormone receptors are the most common form of end-organ hormone resistance. One example of such defects is TSH resistance, which is caused by biallelic inactivating mutations in the TSH receptor gene (TSHR). TSH, a master regulator of thyroid functions, affects virtually all cellular processes involving thyroid hormone production, including thyroidal iodine uptake, thyroglobulin iodination, reuptake of iodinated thyroglobulin and thyroid cell growth. Resistance to TSH results in defective thyroid hormone production from the neonatal period, namely congenital hypothyroidism. Classically, clinical phenotypes of TSH resistance due to inactivating TSHR mutations were thought to vary depending on the residual mutant receptor activity. Nonfunctional mutations in the two alleles produce severe thyroid hypoplasia with overt hypothyroidism (uncompensated TSH resistance), while hypomorphic mutations in at least one allele produce normal-sized thyroid gland with preserved hormone-producing capacity (compensated TSH resistance). More recently, a new subgroup of TSH resistance (nonclassic TSH resistance) that is characterized by paradoxically high thyroidal iodine uptake has been reported. In this article, the pathophysiology and clinical features of TSH resistance due to inactivating TSHR mutations are reviewed, with particular attention to the nonclassic form.

Research in microneurosurgical anatomy has contributed to great advances in neurosurgery in the last 40 years. Many Japanese neurosurgeons have traveled abroad to study microsurgical anatomy and played major roles in advancing and spreading the knowledge of anatomy, overcoming their disadvantage that the cadaver study has been strictly limited inside Japan. In Japan, they initiated an educational system for surgical anatomy that has contributed to the development and standardization of Japanese neurosurgery. For example, the Japanese Society for Microsurgical Anatomy started an annual educational meeting in the middle of 1980s and published its proceedings in Japanese every year for approximately 20 years. These are some of the achievements that bring worldwide credit to Japanese neurosurgeons. Not only should Japanese neurosurgeons improve their educational system but they should also contribute to the international education in this field, particularly in Asia.

広島県のデニム生地製造企業のカイハラは、米国リーバイ・ストラウス社との取引開始を契機として、デニム生地の製造や開発に関する能力を構築し、成長を遂げてきた。本稿では、カイハラの事業展開のプロセスを時系列で記述することで、海外顧客との取引を契機とする、事業成長の可能性について考察を加える。
