Introduction: This study aimed to determine the trends in the use of medicines that can affect the recognition function of patients aged ≧70 years. Since 2015 when the “Safe Drug Treatment Guidelines for the Elderly” was revised, these medicines are dispensed at a community pharmacy located in a satellite city. Methods: We compared the dispensing results from 2014 to 2015-2019 and investigated the ratio of prescriptions containing the medicines that can affect the recognition function using the clinical department and medicine type. Results: Prescriptions containing medicines that can affect recognition function have been on a downward trend since 2015. During this period, the number of medicines per prescription did not decrease, and the medicines that can affect recognition function were replaced with other medicines. On the other hand, no decrease in the number of medicines that can affect recognition function was observed in internal medicine. In medicine type, the ratio of benzodiazepines drugs decreased; however, it increased again in 2019. Conclusion: There was a decreasing trend in the use of medicines that can affect recognition function of the elderly in departments other than internal medicine after 2015.

糖尿病領域では,患者の自己管理能力を引き出す診療・服薬指導が望まれており,医療者のコミュニケーションスキル向上,多職種連携によるチーム医療が重要視されている.COVID-19パンデミックを機に,遠隔診療・服薬指導の実臨床への普及が喫緊の課題とされたが,糖尿病領域に必要なスキルの研修機会は乏しい.そこで今回,遠隔診療・服薬指導スキル向上のために,多職種対象オンライン研修会を開催した.

温湿度,降雨,日射の環境作用がコンクリートの乾燥収縮に及ぼす影響を把握するため,同時に作製した角柱試験体を全国 4 地点で屋外暴露し,水分量と収縮量の経時変化を測定した.その結果,各地の一年間の収縮量はほぼ同等であったが,季節ごとの収縮の進行が異なることが明らかとなった.温湿度の変動・日射・降雨の影響を考慮できるコンクリートの水分移動および収縮に関する数値解析法を用いた検討により,湿度の変動と降雨が収縮量に及ぼす影響が大きいことを明らかにした.湿度の変動と降雨の影響を考慮する係数を平均湿度と降雨時間割合から算出し,平均湿度に乗じた見かけの相対湿度を乾燥収縮予測式に用いることで,屋外におけるコンクリートの収縮を簡易的に予測できることを示した.

In this paper, a broadband tunable power divider (TPD), whose power division ratio (PDR) can be theoretically tuned from -∞ to +∞, is proposed. The new design is based on a Wilkinson power divider, a branch line coupler, and a new combination of phase reconfigurable synthesized transmission lines (PRSTLs) that comprises varactor diodes as the phase shifters (PSs). The operating principles and design equations will be provided with thorough discussion. For proof-of-concept demonstration, a prototype was designed at 2.4GHz. The performance in the measurement is summarized as follows: the operating bandwidth = 25% (2.1GHz-2.7GHz), very wide PDR tuning range of ±29dB, maximum insertion loss = 1.6dB, and switchable output phase of 0° or 180°.

5月に行われたコンピュータ将棋選手権において,拙作の Bonanza が接戦のリーグ戦をすり抜け,幸運に助けられながらも優勝することができた.Bonanza の思考アルゴリズムは,チェスで広く用いられている全幅探索の手法に基づく.将棋においても,全幅探索が有効な手法の一つになり得ることが示された.本原稿では,このプログラムの仕組みを,探索アルゴリズムの概要と,特に将棋ドメインにおける futility pruning の応用に的を絞り,解説する.Futility pruning を行うことによるプログラムの棋力上昇が,次の一手問題の正答率に基づいて示された.

日本の紀要,特に大学紀要の現状について,その定義の困難さ,歴史,掲載論文の特性,質についてのこれまでの議論に着目してレビューした。紀要が学会誌や専門誌に取って代わられなることなく継続して刊行されている理由を,特に人文社会科学における研究や研究者の意識などから検討した。また紀要の電子化について,機関リポジトリの発展および大学図書館間のILLの状況を踏まえて現状を述べた。最後に,学術情報メディアとしての紀要の質の確保と,紙媒体と電子版が共存するハイブリッド環境下において書庫狭隘化に直面している大学図書館における合理的な紀要管理の方向性について議論した。

Since slurry transportation systems are very useful to transport bulk materials, the systems are used in transporting ores and tailings in mining industries and in dredging harbors. This paper describes the characteristics of the slurry transportation systems, and the plan and application of the systems, written as follows, in mineral development in Japan. 1) The plan of Ishikari coal slurry tranport pipeline. 2) The hydraulic transportation system in Sunagawa coal mine. 3) The backfill slurry transportation system in cut-and-fill stoping. 4) The Oodate-Noshiro tailling slurry transport pipeline. 5) The pilot plant system of slurry transportation for sediment sand in Sakuma dam reservoir. 6) The manganese nodule mining system using airlift pump by National Research Institute for Pollution and Resources. 7) The control of gas kick and drilling mud flow in drilling well for methane hydrate production.

A case in which vestibular dysfunction as well as hearing loss was caused by acoustic trauma was reported. The patient was a 22 year old woman that developed hearing loss and dysequilibrium at a rock concert after 20 minutes exposure. She was a college student and in order to prepare the term examination, she did not sleep or take any food for 24 hours prior to the onset of these symptoms. This extreme fatigue condition seemed to be closely related to the appearance of the vestibular symptoms in addition to the hearing loss. The generation mechanism of the acoustic trauma was discussed in the light of the available literature and the importance of good physical condition was stressed.

肺炎はガイドライン2017において,市中肺炎,院内肺炎,医療・介護関連肺炎がいったん統合される形で診療されることになった.但し,A-DROPシステムがSOFA/qSOFAシステムと併用されて重症度の判定に使用され,定型・非定型肺炎の鑑別法も引き続き使用される.抗菌薬はペニシリン系薬を軸とする基本的考え方は今後も同様であり,インフルエンザ診療でのワクチンなど,予防的対応にも重点を置くことが改めて確認された.

Esophageal anisakiasis is a rare disease, accounting for about 0.2% of all cases of anisakiasis. A 26-year-old woman visited our hospital because of severe epigastric pain and heart burn several hours after eating pickled mackerel, which progressively worsened, and subsequently presented with fever. Contrast CT showed marked hypertrophy of the gastric wall and a significant edematous change in the lower esophagus, in addition to inflammation of the mediastinum. Upper gastrointestinal endoscopy revealed an Anisakis worm in the gastric corpus and lower esophagus. After removing it by endoscopy, her condition immediately improved. The present patient was complicated with mediastinitis. It was suggested that anisakiasis can become severe in an esophagus with a thin wall, as can be the case with the small intestine, where the worm may burrow in some cases. As in this case, we can achieve the marked amelioration of anisakiasis through the removal of worms. Thus we should perform endoscopy in the early stage. In addition, since some patients may ingest several worms of Anisakis, we should carefully check whether or not worms burrow in sites other than the stomach, where worms have already been identified.
