A 42-year-old man was referred to our hospital because of left nasal obstruction. Anterior rhinoscopy disclosed a mass in the left nostril attached to the nasal septum. CT scanning showed a well-demarcated soft tissue density mass in the anterior part of the left nasal cavity. Biopsy revealed that the mass comprised spindle cells without atypia. Under local anesthesia the tumor was removed by the transnasal approach. The histopathological diagnosis was neurilemmoma.

Schwannoma occurs everywhere neural tissue exists, but it is relatively rare in oral regions. It orally originates mostly in the tongue and very little in the lips. Recently we experienced a case of schwannoma arising in the left lower lip. The patient was a 73-year-old man and visited our hospital with the chief complaint of a lower lip. The tumor was excised under local anesthesia, and it was found to be an elliptical mass with a capsule measuring 35 × 20 × 20 mm. From both clinical and light microscopical findings it was diagnosed as schwannoma (Antoni A and B type) and was compared with other reports of schwannomas which had been already reported in Japan. From electron microscopical and immunohistochemical findings the origin of the tumor was suggested to be schwann cells in the present study.

神経鞘腫は良性腫瘍であるが,稀に悪性化するとの理由で,神経とともに完全摘出される例が報告されている。しかし,われわれが検索しえた限りでは,神経鞘腫が悪性化した例についての報告例は現在まで29例であった。その中で頸部に発生した例は10例であり極めて稀であった。この結果,神経鞘腫の治療は,臨床経過を考慮したうえで,神経機能を温存し腫瘍を完全摘出する術式(被膜間摘出術)が妥当であると考えられた。

国内で人気の高い動画投稿閲覧サービスに YouTube とニコニコ動画が有る.これらのサービスでは膨大な動画が蓄積されており,利用者が好みの動画を探すのは容易ではない.サービス運営側は検索語と並び替えによる動画検索を提示しているものの,細やかな動画検索は実現できていない.そこで,多数の視聴者が動画に付与するタグを用いた動画分類を考える.本研究ではニコニコ動画を対象とし,動画分類や類似動画提示のために,タグの出現頻度および共起出現を用いたタグの階層化を行った.階層化には ISR (Inter section ratio) 手法を用いた.

チェコ、ドイツ、オランダなどヨーロッパ各地の図書館で文献調査を行い、『世界図絵』の様々な異版本を閲覧した。『世界図絵』の挿絵が初めてカラーで印刷された1883年版の特徴を明らかにしたことも重要な成果であるが、とりわけ「第3章 天空」の挿絵が1658年に発行された初版では、回転するものであったことを確認できたことは、きわめて大きな成果である。その結果『世界図絵』が世界で最初の挿絵入り教科書であるということにとどまらず、世界で最初の手でさわって動かせることのできる教材であることを明確に指摘することができた。

本研究では,慢性化している都市高速道路渋滞の緩和対策として,交通状況に応じてJCT合流部の車線数を動的に変更する動的可変チャンネリゼーションを提案し,交通シミュレーションを利用したその効果の評価手法の提案,首都高速道路ネットワークを対象としたケーススタディ,ドライビングシミュレータを利用した実験によるその安全性の評価,動的可変チャンネリゼーションの実用化に向けた課題の整理を行った.

既存の螺旋スロープに点字ブロックを敷設し、視覚障害者(全盲者)とアイマスクをした健常者を被験者として、螺旋スロープの空間把握や探索行動の調査をした。結果、点字ブロックは、周回数の把握において役立つが、方向の把握においては効果が低いことがわかった。点字ブロックの敷設は、1/4周毎とし1カ所あたりに2枚が最適と結論付けられた。ただし、点字ブロック以外の手がかりも同時に複数必要であると考えられた。また、全盲者は、周回数から方向を割り出そうとする傾向はなく、音や歩いた感覚等によって方向の把握をする傾向があった。

■相談メモ 以前、相談させていただいた患者との問題が長引いています。診断後に急性増悪した患者で、「初診時に見逃した」とクレームを言っていた患者ですが、2カ月に1度くらい謝罪と慰謝料を求めてやって来ます。私自身は責任がないと思っていますが、穏便に済ますためにも、謝ってしまった方がいいのでしょうか。

EBP プログラムである IMR(Illness Management and Recovery:疾病自己管理とリカバリー)と FPE(Family Psycho Education:家族心理教育)を並行して実施することで,家族間の負担感の軽減や自己効力感など相補的・相乗的な効果を実証的に示すことを目的とし,当事者とその家族を対象に介入を行った。 リカバリーゴールの達成に向けて前向きな行動変容が見られることで QOL の改善傾向や再発予防として示唆された。また,家族はエネルギーの使い方が減少し,体験の共感や苦労の分かち合いから心理的サポートの実感が得られていた。

Aim: The aim of this study, the YOung TAiwanese Cohort (YOTA) Study, was to investigate the relationship between a childhood overweight/obese status and young adult preclinical atherosclerosis, including assessments of the carotid intima-media thickness (CIMT) and prehypertension or hypertension. Methods: From among children who participated in the 1992-2000 mass urine screening program in Taiwan, we recruited 303 subjects with an elevated blood pressure (EBP) and 486 subjects with a normal BP in childhood during the period of 2006-2008. These 789 young adults received health check-ups for cardiovascular health, including examinations of blood and urine parameters, anthropometrics, BP and the CIMT, a subclinical cardiovascular risk index. Data analyses were used to evaluate the associated risks in both childhood and young adulthood. Results: The school students with a childhood overweight/obese status had a higher risk of prehypertension or hypertension, with a relative risk of 3.20 (1.40-7.33) for being overweight and 6.51 (3.36-12.63) for being obese in young adulthood at an average age of 21. A childhood overweight/obese status also predicted a higher risk of having a thicker CIMT, with a relative risk of 2.82 (1.26-6.28) and 4.17 (2.21-7.85) for being overweight and obese in adulthood, respectively, after a mean follow-up of 8.5 years. The body mass index exhibited a consistent trend from childhood to adulthood, with an adjusted R square of 0.551. The participants who were not overweight/obese in childhood also demonstrated a higher risk of prehypertension or hypertension if they became overweight or obese in adulthood. Conclusions: This study highlights the importance of preventing and treating an overweight or obese status in childhood for the primary prevention of cardiovascular disease in adulthood.
