The Japanese population is rapidly aging because of its long life expectancy and low birth rate; additionally, the number of patients with heart failure (HF) is increasing to the extent that HF is now considered a pandemic. According to a recent HF registry study, Japanese patients with HF have both medical and care-related problems. Although hospitalization is used to provide medical services, and institutionalization is used to provide care for frail older adults, it can be difficult to distinguish between them. In this context, multidisciplinary management of HF has become increasingly important in preventing hospital readmissions and maintaining a patient’s quality of life. Academia has promoted an increase in the number of certified HF nurses and educators. Researchers have issued numerous guidelines or statements on topics such as cardiac rehabilitation, nutrition, and palliative care, in addition to the diagnosis and treatment of acute and chronic HF. Moreover, the Japanese government has created incentives through various medical and long-term care systems adjustments to increase collaboration between these two fields. This review summarizes current epidemiological registries that focus not only on medical but also care-related problems and the 10 years of multidisciplinary management experience in Japanese medical and long-term care systems.

A streptomycete was isolated from an Easter Island soil sample and found to inhibit Candida albicans, Microsporum gypseum and Trichophyton granulosum. The antibioticproducing microorganism was characterized and identified as Streptomyces hygroscopicus. The antifungal principle was extracted with organic solvent from the mycelium, isolated in crystalline form and named rapamycin. Rapamycin is mainly active against Candida albicans; minimum inhibitory concentration against ten strains ranged from 0.02 to 0.2 μg/ml. Its apparent activity against Microsporum gypseum and Trichophyton granulosum is lower because of its instability in culture media on prolonged incubation required by these fungi. No activity was observed against gram-positive and gram-negative bacteria. Acute toxicity in mice is low.

パクリタキセル(以下,PTX)による末梢神経障害にラフチジンとトコフェロールニコチン酸エステル(以下,TN)の併用が著効した例を報告する.【症例】72歳男性,左上葉肺腺癌,肝転移と診断されたがPTX投与後に四肢のビリビリした痺れによる歩行困難を来たしたため化学療法は中止となり,転院となった.痺れはTN300mg/日では無効であったが,ラフチジン20mg/日の併用により急速に改善した.痺れはTNの減量で増悪したが,再度300mg/日への増量で改善し屋外歩行も可能となった.【考察】PTXによる末梢神経障害に対しTNとラフチジンの併用が有効であった.末梢神経の再生速度が緩徐であるのに対して薬剤量の変更で痺れが速やかに変化する点から痺れの改善はTNの微小循環改善作用とラフチジンのカプサイシン感受性知覚神経を介した血流増加および脱感作による効果と考えられた.

The composition of urine of Rhacophorus schlegelii var. arborea and Rana nigromaculata taken from the bladder was qualitatively analysed, in order to compare with that of the fluid in the peri-vitelline space of the early empryos of Rhacophorus schlegelii var. arborea. RICHARD, BORDLEY, WALLKER and HEUDRIX ('33) verified the existence of uric acid, phosphate, creatinine and chlorine, but not the other organic and inorganic constituents in the urine of Rana pipiens. The present study of analysing the urine of Rana nigromaculata reveals the presence of sodium-ion, chlorine-ion, potassium-ion, phosphate-ion, sulphate-ion, calcium-ion and ammonium-ion, and besides them, nitrate-ion and nitrite-ion in the case of Rhacophorus-urine as the inorganic constituents. As the organic substances, the urine of the two species contains uric acid and urea, but not any trace of creatinine so far as the WEYL reaction is consulted. The urine of Rana nigromaculata in summer months contains uric acid and urea in higher concentration than in winter months. On the basis of the present analysis the chief end-product of the protein metabolism of Amphibia seems to be urea, since the uric acid is contained in much low concentration in the urine of Amphibia.

うつ病は主要な精神疾患であり,抗うつ薬を中心とする薬物療法が主流である.しかし現在では,身体運動の抗うつ効果を用いた運動療法も研究が進んでいる.過去の研究では,うつ病患者や比較対照群の統制が不十分であり,結果が一貫しないものが多かった.質の高い論文に絞ったメタ解析では,運動は非介入群に比べて有意な抗うつ効果があり,薬物療法の補助療法としても効果があることが示されている.しかし,運動療法が対象・方法ともに治療法として確立されたわけではなく,有効性については慎重に見極めていく必要がある.実際においては,対象患者の精神状態だけでなく合併症など身体状態にカスタマイズした運動を勧めていく必要があるだろう.

Under difficult COVID-19 situations, restricted movements and increasing stress demand casual counseling. Counseling in a virtual space would lower the psychological barrier to consultation by concealing the client and the counselor’s figure. This study examines whether the willingness to consult would change by the seriousness of consultation and the different appearances of the counselor’s avatar. As a result, in the serious consultation situation, the willingness to consult with the avatar in a white coat was high. In the informal consultation situation, the willingness to consult with the avatar wearing a hood was high. Furthermore, with eye-tracking, we found that people’s attentive areas were different by the situation and by the appearance of the avatar. These results suggest the potential for the avatar counseling system in the Metaverse and remote virtual reality (VR) counseling.

This paper presents the application of a noncoated carbide drill having a sharp cutting edge and multistage point angle (drill A) for drilling carbon fiber reinforced thermoplastics (CFRTP). The cutting characteristics were evaluated by comparing the drill A with a diamond-coated carbide drill having a constant point angle (drill B). The influence of the contact state between the cutting edge and carbon fiber on the cutting force was evaluated using a CFRTP with a unidirectional fiber. The drill A exhibited a maximum thrust force at the drill rotation angle where the fiber orientation and cutting edge travel direction matched. It was found that the thrust force of the drill A was lower than that of the drill B at any drill rotation angle. Moreover, the thrust force of the drill A was always lower than that of the drill B during the drilling of the CFRTP with a plain-woven fabric. Additionally, the uncut fiber of the drilled hole obtained by the drill A was more satisfactory than that obtained by the drill B. The temperature of the area near the cutting point during the drilling was lower in the drill A than in the drill B. In the drill A, the thrust force tended to increase with the number of drilled holes. However, no considerable reduction in the drilled hole quality was observed at 200 holes.

本研究では選択性緘黙の中学2年女子に学校場面における発話行動形成のため、包括的支援アプローチを適用し、その効果を検討した。彼女の選択性緘黙は発現前条件が学校場面で誘発されるストレス反応、維持条件が彼女の代替発言をする生徒および筆談をする教員の存在と考えられた。そこで、彼女の学校場面でのストレス反応低減と発話行動形成を目的に、不安階層表の段階を唾液アミラーゼ評価で確認し、その段階のストレス反応の程度に合わせて刺激フェィディング法、系統的脱感作法、現実的脱感作法、主張反応法を併用適用した。また、彼女の選択性緘黙維持条件除去の目的で学校介入をした。専門支援機関でのセッション4回、学校訪問指導4回の10カ月の支援の結果、彼女の選択性緘黙は解消し学校場面における活発な発話行動が形成され、予後も良好であった。包括的支援アプローチの選択性緘黙への有効性が示され、今後の課題として技法選択基準の明確化をあげた。
