症例は60歳女性.右乳癌の診断にて入院した.手術当日グリセリン浣腸施行中に強い疼痛を訴え,その後も強い肛門部痛と嘔吐が持続した.臀部は腫脹し,肛門内から少量の出血を認めた.直腸診で直腸粘膜の欠損を触知し,浣腸時の直腸穿孔およびグリセリン液の管腔外注入が考えられた.浣腸後から自尿は無く,約10時間後の導尿では少量の血尿が得られた.補液と強制利尿にも反応無く,翌日急性腎不全と診断し,血液透析を開始した.計3回の血液透析で,腎機能は利尿期を経て約2週間後に正常に回復した.臀部の発赤,腫脹も受傷10日目には自然に消失し,直腸周囲での膿瘍形成も無かった.本症例は高濃度のグリセリン液が血中に入ったことにより,赤血球の膜障害と溶血が起こり,急性腎不全を引き起こしたと考えられた.以前より高濃度のグリセリンが血中に入ると溶血を起こすことは広く知られている.グリセリンが溶血を起こす機序については,赤血球の膜障害による高度の溶血が原因として推測されている.溶血が起こると大量の遊離ヘモグロビンが発生し,尿細管上皮内に再吸収されヘムとグロビンに分解される.ヘムは細胞毒として作用するため腎不全が発生するとされている.腎不全発生を予防するためには,遊離ヘモグロビンの除去が重要とされる.遊離ヘモグロビンは大分子物質であるため,その除去には血漿交換が有効と考えられている.また,遊離ヘモグロビンと結合し肝臓に運び処理するハプトグロビン投与も有効とされている.グリセリン浣腸時に患者が疼痛や気分不快および強い疼痛等を訴えた場合には,浣腸による直腸粘膜の損傷や穿孔の可能性がある.さらに腸管外へのグリセリン液注入は溶血から急性腎不全を発症する場合もあり,注意深い観察と迅速な対応が必要である.

This paper aimed to clarify the relationship between family communication and the structure of dwelling space in the period of growth and development (the 2nd grade of elementary school - the 2nd grade of junior high school). Consequently it was shown that 1) College students period: If the path of flow always passes through the family living space, a high percentage of families enjoy each other's company once a week or more, only for women and the total of men and women, 2) Growth period: there was no significant correlation between the structure of dwelling space and actual family communication, 3) Growth period: a relationship in terms of location between the parents' room (not the children's room) and the family living space correlates to the psychological distance of children to their parents.

Although barley has traditionally been one of the most important food grains, the intake in Japan has been decreasing during the last forty years. Barley provides many essential nutrients (carbohydrates, dietary fiber, vitamins and minerals) and functional components (dietary fiber and polyphenol). Studies on the health benefits of barley have been rapidly increasing in terms of the cholesterol-lowering effect, serum glucose and insulin normalization, decreased body fat accumulation, and blood pressure reduction. It has been scientifically proven that the soluble dietary fiber in barley, β-glucan, might reduce the risk of cardiovascular disease (CVD). It is inferred that two principal mechanisms may contribute to the cholesterol-lowering effects of barley and β-glucan: 1) reduction of cholesterol and bile acid absorption from the small intestine, 2) inhibition of cholesterol synthesis in the liver. Barley and β-glucan have therefore been approved as a potential health benefit against the risk of CVD in the United States and Sweden. Recent clinical studies have suggested that the consumption of barley and its products might reduce many risk factors associated with the metabolic syndrome, namely diabetes, hypertension and dislipidemia. This review presents information that will hopefully increase the awareness of professionals and consumers for the health benefits of barley.

本稿では,映像整理や視聴に要する労力軽減のために,サッカーの試合における試合中断区間に注目したイベント推定の手法を提案する.サッカーの試合ではイベントが発生すると一旦試合が中断される場合が多く,その中断区間からは原因となったイベントを推定するために有効と考えられる特徴量が得られる.提案手法ではまず,ショットの分割・分類,プレイ・ブレイクの分類などの映像の整理を行う.その後,ブレイク区間からブレイク時間,スローモーション映像の回数などの特徴量を抽出する.そして,学習データから得られたパラメータに基づきイベント推定を行う.実際の試合映像に対し実験を行い,提案手法の有効性と今後の課題についての検討を行った.

This paper examines the formation of history image and the relationship with development and the real world by taking up an image of the person who appears on historical novels. More specifically, this paper discusses the Ghost Story of Yotsuya, Juutarou IWAMI and Sasuke SARUTOBI and considered the difference between the forgotten existence and the existence left for the memory even now. A correlation with story culture and the real world is important. Moreover fans of entertainment works also referred to the tourism by visiting related places. I pointed out the importance of the archive facilities where it is supported.

コシアブラのストロンチウム集積はマンガンとカルシウムにそれぞれ相関があることが判明した。ヤナギ類、コシアブラおよびタカノツメ木の樹皮中にはCaとSrを含んだ結晶状の物質が観察され、木本植物に吸収されたSrは、樹皮中で不溶性の結晶状物質を形成し、植物体内に長く留まる可能性があることが考えらえた。ヤナギ表皮部分から採取したRNAについて発現量解析を行った結果、Ca結合性タンパク質などの候補遺伝子群が得られた。菌根菌の分布を比較したところ、菌根菌が多いところはセシウム137 が多いことが示唆された。

2010年、桜美林大学大学院大学アドミニストレーション研究科は専攻設置から10年を経過している。この専攻は、既存専攻の科目群を基礎として、大学改革に必要な科目を新たな授業科目として追加し、日本の大学が今日直面している諸課題を解決することを目的に設置され、本学教員の他に高等教育界の重鎮といわれている方々が教員として勢ぞろいした日本で初めての専攻でもあった。 2008年度、研究科への独立に伴い授業科目の見直しが行われたが、アカデミックな科目と時代の変遷にも対応できる実務を行う為のドリル的な科目、そして未だ配置されていない経営の根幹に係わる資源(ヒト・モノ・カネ)に関する事項を体系的に修得させる科目や論理的かつ構造的に考えられる力を身に付ける科目を融合したカリキュラムが、今後とも必要になっていくのではないかと考えている。

〔目的〕回復期リハ病棟における認知尺度と行動観察尺度を併用して用いる有用性を検討すること.〔対象〕当院回復期リハ病棟に入院した60名.〔方法〕行動観察尺度のNMスケールを用いて,認知症の重症度を4群に分類した.次に,各群間における入退院時のMMSE,NMスケールの比較と関連性を求めた.〔結果〕認知症が重度群の場合,MMSEでは失語症や鬱傾向により評価不十分だが,NMスケールでは有意な差が認められた.また入院時の軽度群と中等度群のみ相関が低かった.〔結語〕一方の評価だけでは信頼性が乏しいこと,また認知尺度は失語症や鬱傾向の影響を受けることがあるため,日常生活の様子を観察して評価する行動観察尺度は有効であった.
