訪問看護部に所属してから、特定看護師として介入した、褥瘡を有する在宅療養者A氏の事例を報告する。A氏は40年前より下半身不随で、蜂窩織炎により入院中に、右臀部に褥瘡を形成したが、本人と妻の希望により、褥瘡が治癒しないまま退院することとなった。退院までには、本人と妻を含めた多職種間での連携会議を開催し、創処置の統一やリハビリ導入を行った。退院後は医師の包括的指示の下、デブリードマンや創傷被覆材の選択、薬剤の調整・変更を行った。創部の画像を見てもらいながらA氏と妻の訴えを傾聴し、肯定的アプローチを実践することでA氏の治療への参加が促され、退院後37日目に褥瘡は治癒した。特定看護師が、療養者と家族の治療参加を促し多職種と連携・協働しながら、局所療法、除圧、栄養管理を行うことで、在宅における褥瘡管理が可能となることが示唆された。

This study investigates the factors that contribute to broadening social understanding of e-sports events. e-sports is a method for viewing computer games as sports, and has attracted strong interest from around the world. The brand image of e-sports in Japan is derived from home video game consoles and is often pointed out by customers as being aimed at children. This is one of the factors hindering the spread of the sport. Therefore, we conducted a survey to find out what kind of e-sports tournaments would help revitalize local communities. As a result of factor analysis, it was found that the participants were divided into two groups: the spectator group and the experience value group. As for the game titles to be handled, the participants preferred titles that they had played on home video game consoles rather than IPs used in the world championships.

シイタケ菌糸体抽出物は癌化学療法との併用時に,患者のQOLを維持・改善することが報告されている.本研究では,様々な治療背景および様々なステージのがん患者のQOLに及ぼすシイタケ菌糸体抽出物の影響を検討した.16施設で73症例を対象にシイタケ菌糸体抽出物を4週間(1,200 mg/day)連日経口摂取させ,摂取前後のQOLをEORTC-QLQ-C30調査票でスコア化した.シイタケ菌糸体抽出物摂取後のQOLスコアは被験者全体では,摂取前スコアに比べ,心理的,疲労スコアで有意に改善した.特にステージ3,4の被験者では,総体的,身体的スコアの改善も観察された.シイタケ菌糸体抽出物の経口摂取の併用は,進行性がん患者のQOLを改善することが示唆された.

We herein report the first case of foveolar-type gastric adenocarcinoma that developed after the initiation of vonoprazan (VPZ). A 51-year-old man had heartburn at the first visit and reflux esophagitis endoscopically, so he started taking VPZ. An approximately 5-mm-sized reddish polyp with a raspberry-like morphology was detected at the anterior wall of the upper body of the stomach 156 weeks after starting maintenance therapy with VPZ 10 mg/day. It was diagnosed as foveolar-type gastric adenocarcinoma based on a biopsy. Another approximately 4-mm-sized foveolar-type gastric adenocarcinoma was also detected at the posterior wall of the middle body of the stomach.

A lot of medieval castle ruins were seen in and around Noubi Plain (south of Gifu Prefecture and west of Aichi Prefecture) and Mikawa, in 1960s. In the 16th century, the warlords who lived in these castles became vassals of Oda Nobunaga and Toyotomi Hideyoshi. Forty years have elapsed since then. The land was developed and most of the castle ruins disappeared. I photographed these remains in my boyhood. The remains do not exist now,but I have many photographs and see them even now. First, I would like to introduce those pictures of castle remains,and aerial photos next. I also introduce cadastral maps, drawing map made in Edo period, and the books on cultural assets. I want to imagine the old state of those castles with those pictures and books and my heart goes out to the history, preservation and destruction of the ruins.

The magnitude of mechanical stress at the shoulder and elbow appears to be directly correlated with the degree of maximum shoulder external rotation (MER) during throwing. Therefore, it is very important to prevent excessive MER to minimize the risk of throwing injuries. The purpose of this study was to investigate the relationships between MER during throwing and the kinematic parameters of throwing mechanics, shoulder muscle strength, and shoulder range of motion in high school baseball players. The subjects were 40 male high school baseball players with no elbow or shoulder joint problems. Three-dimensional analysis was performed to calculate the MER angle. Then, the shoulder and elbow angles at initial foot contact (IFC) were computed. ROM and muscle strength of shoulder joint were also measured in each subject. Multiple linear regression analysis was used to relate the MER angle to these factors. Significant correlations were observed between the MER angle and the external rotation (ER) angle (r=−0.51, p<0.001) at IFC, and the ER range of motion (r=0.84, p<0.01). The MER angle significantly correlated with shoulder internal rotation (IR) at IFC. This finding suggests that stress on the shoulder and elbow could be increased by the degree of shoulder IR angle at the moment of IFC. Further, excessive ER range of motion may also be a risk factor.

狂犬病は予防できるが,治癒させられない代表的ウイルス性人獣共通感染症である。日本では1957年以降ヒトの狂犬病も動物の狂犬病も発生の報告はないが,狂犬病が常在する国々はいまなお多数ある。したがって,狂犬病が日本に侵入する可能性は常に考えておく必要がある。近年日本から海外に旅行ないし出張する人々は年間1600万人を超えた。こうした状況では狂犬病常在地への旅行者がイヌやネコなどに咬まれて狂犬病ウイルスに感染し,帰国後に狂犬病を発病する輸入狂犬病が発生する可能性は高い。日本では狂犬病ワクチンを常備する医療機関が少なく,狂犬病免疫グロブリンは製造も輸入もされていないため,海外での動物咬傷被害者に狂犬病曝露後発病予防を迅速に実施できる状況にない。日本での輸入狂犬病発生を防ぐためには,医療機関における狂犬病への備えが必須である。

<p>緩和ケア・終末期医療の医療ソーシャルワーカー(MSW)の役割遂行の構造に関連する要因と因果関係性を明らかにするために,がん診療拠点病院などのMSW 1,134人を対象に質問紙調査を実施した(回収率37.1%).本研究では,コンピテンシー,コーピング,死に対する態度,職場・職業への適応感は相互に影響し合いながらMSWの役割遂行を規定するという多重指標モデルを作成した.共分散構造分析の結果,有意な推定値が得られ,適合度はGFI=.920,AGFI=.898,RMSEA=.060でありモデルは妥当であると評価した.緩和ケア・終末期医療のMSWの役割遂行の構造(情報共有,ソーシャルワーク実践,代弁,意思決定支援,精神的支援)が本調査により明らかとなり,関連要因を含む仮説は支持された.これら現状の向上・改善は,医療・ケアチームにおけるMSWの至適な役割遂行につながることが示唆された.</p>

要旨:攻撃的ユーモアを笑うということを優越感から生まれる快の笑いとは異なる観点から検討した。攻撃的ユーモアを含む4 コマ漫画と非攻撃的ユーモアを含む4 コマ漫画をユーモア刺激とし, 大学生および大学院生142 名を対象に質問紙調査を行った。それぞれのユーモア刺激について, 対象者をユーモア高評価群とユーモア低評価群に分類し, 与えられたユーモア刺激を笑ったり, おもしろいと感じたりする場合の笑いについて比較検討した。その結果, 攻撃的ユーモアを笑うためには, 感覚的不適合とともに論理的不適合も必要である可能性が示唆された。日常生活におけるコミュニケーションの手段として笑いを重視していることがユーモア評価に影響していることも示された。
