Emotional crying has been described as the shedding of tears consequent to an emotional event, a universal and uniquely human behavior. The purpose of the present study was to develop the Japanese version of the Beliefs About Crying Scale (BACS), which assesses beliefs about whether crying is beneficial for gaining positive feelings in both individual and interpersonal contexts. In Study 1, we examined factor structure and re-test reliability. The results indicated that the factor structure of the Japanese version and original version of the BACS are similar and possess acceptable re-test reliability. In Study 2, we tested validity. The results revealed that the Japanese version of the BACS had theoretically reasonable correlations with assumed variables. These results suggest that the Japanese version of the BACS has similar properties as the original version and has acceptable validity and reliability.

本稿の目的は,批判的に考える態度(批判的思考態度)の伸長を図った統計指導の設計ならびに実践を行い,生徒がどの程度,批判的思考態度の重要性を認識したかを明らかにすることである.教材開発にあたり,「現実の文脈の使用」「『読解の文脈』での活動の重視」「データの提示の方法」という点に留意した.そして,複数のデータの読み取りや,個人の考えを小グループで検討し,他者の考えを聞いた上で改めて個人の考えを再検討するという活動を導入した教材を開発した. 指導実践を通して,データの提示の仕方を工夫することで複数のデータを参照することの重要性が理解され,また,結論を何度も他者と意見交換することで,意見の変容が促され,他者との意見交流の重要性が理解された.そして,本実践を通して多くの生徒が批判的思考態度の重要性を認識したこと,また,批判的思考態度が表出する場面として,「新たなデータの必要性を問う場面」「複数のデータを参照して答えた場面」「最終的な意見を述べる場面」があることが明らかとなった.

38歳,男性。30分間のランニングをした後に市販の栄養ドリンク剤を1本服用したところ,30分後に全身に蕁麻疹が出現した。その後意識消失し,救急搬送された。後日,原因精査を希望し当科を受診された。栄養ドリンク剤as isのプリックテストにて陽性を示したため,成分別プリックテストを施行した。合成タウリン(1%)のみ陽性を示し,ほかの成分はすべて陰性であったため,栄養ドリンク剤中の合成タウリンによるアナフィラキシーと診断した。栄養ドリンク剤による即時型アレルギーの報告は少なく,合成タウリンでの報告例は韓国からの1例のみであり,調べ得た限りではわが国で初の報告例である。タウリンは哺乳類の成長に重要な栄養素で,牡蠣やイカなどの食品に天然タウリンとして含まれるほかに,多数の栄養機能食品に配合されている。自験例はイカなど天然タウリンを多く含む食品の摂取制限は必要とせず,合成タウリンを含有する栄養機能食品の禁止のみで,再発はない。(日本皮膚免疫アレルギー学会雑誌,3(2):313-317,2020)

In the experiments performed in the advanced course of the National Institute of Technology, Numazu College, data science education was conducted remotely by using asynchronous and synchronous interactive methods. The Tellus platform with satellite and terrestrial data was used to analyze data in a cloud environment. We adopted the development of data engineering, data analysis, and value creation skills as basic educational guidelines. From the answers to the questionnaire, we conclude that 1) the asynchronous interactive method made us aware of many benefits, 2) the synchronous interactive methods gave us some issues to be solved, and 3) the three guidelines were successfully achieved.

We report a case of postoperative fracture of the right femoral tuberosity with a resultant pain in the lateral part of the right hip experienced at the start of walking. During steady-state gait, right pelvic rotation with medial rotation of the right hip occurred in the right stance phase of the patient. However, at gait initiation, medial rotation of the right hip was limited. We focused on and treated the anterior fibers of the right gluteus medius muscles, which are responsible for internal hip rotation. Post treatment, the patient reported an improvement in the pain felt at gait initiation, an improvement in the medial rotation of the right hip during the right stance phase, and an improvement in walking stability.

笑いに関する研究は世界中でおこなわれています。本欄では、英語で発表された笑い学の最近の研究成果を紹介しています。笑いに関する研究は、医学、心理学、社会学、哲学、文学、言語学、動物行動学など、多様な学問領域の専門雑誌に掲載されています。幅広い分野で展開されている世界の研究動向について共有することで、国内での笑い学の研究がさらに発展することにつながればと考えています。 本号では計6本の研究論文についての紹介記事を掲載することになりました。記事の執筆には、6名の研究者にご協力いただきました。どうもありがとうございました。

【緒言】ブプレノルフィン経皮吸収製剤(BTDP)の自己中断により,急性オピオイド退薬症状を呈した在宅医療患者を報告する.【症例】84歳,在宅訪問診療利用中の男性.腰部脊柱管狭窄症の悪化により,4カ月前からBTDPで鎮痛されていた.症状改善傾向と考えた家人が,患者本人に無断でNSAIDs経皮吸収製剤に貼り替えたところ,約50時間後から5分ごとの頻尿や失禁,水様性下痢,発汗,血圧低下,足裏の不快感,不眠などの多彩な症状が表出された.Clinical Opiate Withdrawal Score(COWS)では12点の軽度退薬症状に該当した.発症後24時間で激しい症状はほぼ自覚されなくなり,48時間後には完全に消退した.【結論】BTDPの急速な中止による退薬症状の報告例は少ない.医学薬学的な側面のほか,在宅医療におけるオピオイド製剤使用上の社会的問題点も明らかとなった.

粉製品に含まれたダニの経口摂取によるアナフィラキシー2症例を経験したので報告する.症例1は50歳女性で,開封後に常温保存したお好み焼き粉を,自宅にて調理摂取した約30分後に全身浮腫性紅斑や動悸,嘔気が出現した.患者が摂取したお好み焼き粉は,賞味期限より2年が経過していた.患者血清ダニ抗原特異IgE値はヤケヒョウヒダニ23.4 IU/ml,コナヒョウヒダニ33.7 IU/mlで,コムギ関連特異IgE値はコムギ<0.35 IU/ml,グルテン<0.35 IU/mlと陰性であった.持参した同一銘柄の未開封お好み焼き粉と冷凍保存された薄力粉でのスクラッチテストは陰性であったが,ダニ抗原(鳥居薬品)のプリックテストはscore 3+の陽性を示した.症例2は28歳女性で,開封後に数カ月間常温保存したお好み焼き粉(以下事故粉)を自宅において調理中に喘息症状が出現し,摂取中に呼吸困難感や嘔吐,腹痛が出現し,アナフィラキシーショックとなった.患者血清ダニ抗原特異IgE値はヤケヒョウヒダニ51.6 IU/ml,コナヒョウヒダニ55.6 IU/mlで,コムギ関連特異IgEはコムギ<0.35 IU/ml,グルテン<0.35 IU/mlと陰性であった.プリックテストは事故粉でscore 3+の陽性,ダニ抗原(鳥居薬品)でscore 4+の陽性であった.また冷凍保存された薄力粉は陰性であった.事故粉にはコナヒョウヒダニが50匹/g検出され,ELISA法で測定したところ,ダニ主要抗原であるDer f 1が64.1 μg/gと多量に検出された.事故粉と患者血清(症例2)を用いたImmunoblot法ではダニ抗原と思われる25 kDa部位にバンドが検出された.さらに血清とダニ粗抽出液を用いたInhibition immunoblot法ではダニ粗抽出物と共通分子量付近に認められた複数のバンドは事故粉より消失した.この結果から事故粉に認められたバンドは粉中のダニ抗原に反応したバンドと考えられた.上記の検討を踏まえ,お好み焼き粉におけるダニ繁殖の実態を調査するため,我々は,お好み焼き用ミックス粉3銘柄と薄力粉におけるダニ数及びダニ抗原の増殖性の違いについて検討した.各粉にコナヒョウヒダニを添加培養し,3週間後と6週間後に評価した.培養6週間後にミックス粉のダニ数とダニ抗原は,薄力粉に比較して3銘柄ともに増加傾向を認め,1銘柄は有意に増加していた.粉類でダニの増殖を防ぐには冷蔵保存が良いとされる.市販のミックス粉の注意書きには冷蔵保存と明記する必要があり,また我々皮膚科医は小麦アレルギーと誤診しないよう注意が必要である.

太陽光発電所では,カラス属の落石によるものと思われる太陽光パネルの破損が確認されており,パネル破損の発生メカニズムを理解するために,米倉山太陽光発電所(山梨県甲府市)でカラス属の飛来頻度とパネル破損の関係を調査した.ビデオカメラによるカラス属の飛来観測を2つの調査区画(区画A,B)で,2期(第1期:2017年10月24日-2018年1月18日, 第2期:2018年1月19日-3月9日)に亘ってのべ3,792時間行った.調査の結果,飛来頻度が最も多かった第2期・区画B(41個体/時)でのみパネル破損が確認され,パネル破損やパネル上の石の残留の発生確率はカラス属の飛来頻度と正の相関関係が認められた.本研究から,カラス属の飛来頻度が増加すると太陽光パネルの破損するリスクが上昇することが示唆された.

保健所は2022年 4 月時点で全国に468か所設置されており,「地域保健法(1994年)」に基づき,健康危機管理の拠点となる役割をもち,災害時や感染症対応には主体的に関わることになっている.新型コロナウイス感染症対応が始まってから,自治体はこの 2 年半,第 1 波から第 7 波の現在に至るまで,流行状況およびウイルス変異及び重症度等に応じて,「感染症の予防及び感染症の患者に対する医療に関する法律」に基づき様々な感染症対応に模索を繰り返してきた.基本的には全国的に共通する感染症対応業務(相談,検査,発生届受理,入院調整,患者の移送,健康観察,積極的疫学調査,入院勧告や就業制限通知等)を行うが,都道府県単位で,感染症の発生状況や医療資源の違いもあり,具体的な業務内容や方法は全国一律ではなく,現実的には地域の実情により,それぞれの自治体で工夫されてきた.流行状況を振り返ると,第 1 波,第 2 波,第 3 波は全国的に行動制限を要請され,PCR検査の需要と医療体制の供給がミスマッチであった.新型コロナウイルスは変異以前の特徴として呼吸器機能を低下させる病原性を持ち,有効な薬剤やワクチンがまだ普及せず,診療可能な医療機関も不足していた.第 4 はα株で高齢者の施設内感染で医療提供が困難となり,第 5 波は東京オリンピックの後でδ株の変異ウイルスが主となり,首都圏での流行が目立った.第 6 波および第 7 波はο株が中心で感染性が高く,病原性は低いが感染者数の急増かつ膨大なため,保健所の能力を大きく上回る対応が求められた.全国的にどこの自治体でも保健所の負担軽減策について外部委託も含めて対応するようになった. 2 年半の間に厚生労働省からの通知も多く,全国保健所長会は要望や提言などの意見活動も行った.日本は自然災害の多い国であるが故に,健康危機管理として災害や感染症においては,保健所が平時から備えとしての仕組みづくりや危機発生時の対応,被害からの回復という過程において,主体となることが期待されている.新型コロナウイルス感染症対策で得た教訓を生かしパンデミックとなりうる感染症対策を地域単位で行っていくため,住民の命と健康を維持する「保健所」を,医療機関や福祉施設等と有機的に連携し,持続可能な社会の枠組みとして活かしていくことを提言する.

To develop a garbage recycling system including garbage disposal, lactic acid and biodegradable plastics production, the preservation and deodorization of garbage during storage and the explication of their mechanisms are very important. The changes of counts of viable cell in garbage collected from university's dining hall during the storage were observed. In spite of seasons, lactic acid bacteria became preferential bacteria after the garbage was stored for 24 hours. On the contrary, coliforms and clostridia that are indexes of putrefaction and pollution remarkably decreased. The coliforms were not detected after the 72- or 96- hour storage. No food poisoning bacteria, such as staphylococcus aureus and bacillus cereus, were detected after the 24 hours storage. From the results of experiment adding the staphylococcus aureus to the garbage, it was proved that the lactic acid bacteria also easily became preferential bacteria even if food poisoning bacteria gained predominance at the beginning of the storage. It is clear from this study that lactic acid fermentation during the storage process inhibits the growth of putrefactive bacteria and food poisoning bacteria, which, as a result, makes it possible to realize the preservation and deodorization of garbage.

Gas storage and separation are closely related to various aspects of human society, such as the alleviation of greenhouse effect, promoting clean energy, and controlling harmful gases. In particular, hydrogen has been enthusiastically researched as a clean energy source that emits no greenhouse gases. Therefore, there are requirements for safe methods to manufacture, store, and transport hydrogen. Conventionally, hydrogen storage materials are metal hydrides and metal amides that react violently with moisture, and their usage environments are limited by the extreme conditions required for hydrogen gas generation. Metal-organic frameworks (PCP/MOFs) are porous coordination materials that are composed of organic ligands and metal ions. Because of their ability to efficiently adsorb small molecules, PCP/MOFs have potential applications in e.g., gas adsorption, catalysis, as well as energy storage and conversion. Functionalization and modification of the organic ligands to construct new MOFs are fascinating and significant areas in crystal engineering owing to the potential applications of the resulting materials. We describe the synthesis and gas-adsorption properties of novel MOFs using the functionalized organic ligands.

2017年に成人人工内耳 (以下CI) 適応基準が改訂され, 平均聴力レベルが 70dB 以上で, 補聴器 (以下 HA) 装用下の最高語音明瞭度が50%以下の高度難聴例に対しても適応が拡大された. これにより, 今まで HA 装用効果が不十分であった進行性難聴を含む両側高度難聴例や左右差のある高重度難聴例に対しても CI によるシームレスな補聴が可能となった. 高度難聴に対する CI の有用性は世界で多く報告されている. HA と CI の適応境界については議論の余地があるものの, HA 装用下の語音聴取能は CI による語音聴取能改善の予測因子として重要であり, 世界でも CI 適応基準として重視されている. また, CI に求められる効果が高度になるほど, 純音聴力検査や静寂下の語音明瞭度での評価には限界が生じる. 今後日本語における機能的アウトカムを含めた評価方法の確立が求められる. 新基準の導入に伴い, 高齢者に対する CI 手術も増加している. 高齢者に対する人工内耳では, 聴取能改善に加え, 認知機能や QOL に効果を及ぼす可能性が示唆されている. 高齢化社会において大きなインパクトを与えることが予想される. あらゆる聴力像に対して複数の選択肢が登場し, 切れ目のない聴覚補償が可能になりつつある. “きこえと QOL を維持する” ために CI を積極的に活用する時代が見えてきている.

〔目的〕スマートフォンのアプリケーションを用いて,妥当性のある肩甲骨の運動方向および運動角度が得られるかを調査した.〔対象と方法〕健常男性10名に上肢前方挙上と結帯動作をさせた.三次元動作解析装置とアプリケーションを用いて,各課題時の肩甲骨の前傾および上方回旋の角度を計測した.〔結果〕全課題において,三次元動作解析装置とアプリケーションで算出された結果には,相関関係を認めた.また,ICC(1,2),(2,2)において,高い判定を認めた.〔結語〕アプリケーションでの測定方法は,上肢前方挙上や結帯動作時の肩甲骨のアライメントを評価できる有用な方法の一つであり,肩関節の評価に役立てられると考える.

急性期医療を要する重症患者では、侵襲による過剰な炎症で代謝異化状態が亢進するため、まずは救命を目的に炎症を制御して呼吸循環動態を安定させる。その後は、急性期、回復期から退院後のquality of life(以下、QOLと略)向上を見据えた救急集中治療を継続的に行わなければならない。これに対して、栄養療法は救急集中治療の連鎖の一貫であり、急性期だけでなくQOLを改善させるための治療として認識する必要がある。さらに、ガイドラインやエビデンスの高い研究を活用して、各施設の実情にあったマニュアルやプロトコールを作成し、栄養サポートチーム(nutrition support team;以下、NSTと略)が遵守しながら適切な栄養療法を現場に提供することが重要である。

Arnold Schoenberg's Begleitungsmusik zu einer Lichtspielszene Op. 34 (1930) indicates the close relation between Schoenberg's work and film music. Although Schoenberg did not contribute to any films in his career, he had planned to accompany the performance of his work Die gluckliche Hand (1913) with a silent film. He emphasized the importance of sound film in the 1920s in texts such as "Der sprechende Film," 1927 ("The Talkie"). His relationship with Guido Bagier, the head of the department in charge of sound films department of Universum Film AG (UFA), also indicates Schoenberg's interest in films during that time. This paper explores Begleitungsmusik both in terms of its musical structure and with reference to the praxis of musical accompaniment of silent films. First, the musical character of Begleitungsmusik is analyzed from the perspective of its structure and the process of its composition. Discussion of an unpublished memo indicating the programmatic content of this piece and a detailed analysis of Schoenberg's musical manuscripts show that his revisions emphasized a dramaturgical structure, partly corresponding to the unpublished memo. Second, the relation between the film and Schoenberg's activities, based on the examination of related preliminary material including the diary of his second wife Gertrud and the unpublished autobiography of Guido Bagier, will be surveyed. Finally, the contemporary situation and the aesthetics of musical accompaniment of silent film are discussed, focusing on the relation of the three subtitles Schoenberg used in his Begleitungsmusik, "threatening danger," "anxiety," and "catastrophe." Musical scenarios published in Film-Ton-Kunst, a magazine that specialized in the film music praxis in the 1920s, are systematically explored. Consequently, this paper recontextualizes Begleitungsmusik within Schoenberg's own development and the historical background to clarify that although Schoenberg and contemporary praxis of musical accompaniment attached great importance to dramaturgical construction, never-theless, their musical languages and attitudes toward the excerpting of musical pieces differed.
