<p>フォワーダの積載性能が長尺材の集材生産性に与える影響を明らかにするため、大型(最大積載量6,000kg)と中型(同4,800kg)のフォワーダを対象として、材長ごとの生産性を調査した。長尺材として6mと8m材を集材し、比較のために4m材も集材した。集材作業は、13tクラスのグラップルによるフォワーダへの積込み、実走行、グラップルによる荷おろし、空走行の各要素作業に区分した。先山と土場の各グラップル操作も含め、1人作業として生産性を比較した。その結果、大・中型機とも4m材を生産したときに最も生産性が高く、6m、8mと材が長くなるにつれ低下した。しかし、大型機では8m材の生産性が4m材の約7割だったのに対し、中型機では約6割と積載性能によって低下率が異なった。各作業における材長の影響は積込み作業で大きく、長尺材になるほど時間がかかる傾向がみられたが、走行や荷おろし作業への影響はそれほど大きくなかった。長尺材の生産性は大型機の方が高く、また、長尺材になったときの生産性低下率も大型機の方が小さいため、長尺材の集材には大型機の方が適しているといえる。</p>

症例は75歳女性.嚥下困難,食欲不振と唾液が常に流れてくる症状で近医を受診.上部消化管内視鏡検査で,逆流性食道炎と慢性胃炎の診断でプロトンポンプ阻害薬(PPI)を処方されていた.しかし,症状の改善を認めず当科を受診.難治性かつ上部消化管内視鏡検査にて食道粘膜に非連続性に地図状のびらんが散在し,易粘膜剥離も認めたため食道天疱瘡を疑った.抗デスモグレイン(Dsg)3抗体価が23 Indexと陽性であり,蛍光抗体直接法にて食道の表皮細胞間にIgG,IgA,C3の沈着を認めた.皮膚,口腔粘膜病変はないものの,食道天疱瘡と診断しプレドニゾロン(PSL)30mg/日を開始したところ,2日後より自覚症状が著明に改善し,食事も摂取できるようになった.その後はPSLを漸減し,現在はPSL 5.5mg/日投与中で再発を認めていない.本症例は,皮膚,口腔粘膜病変がなく内視鏡検査で食道天疱瘡の診断となった1例であり,また併存した胃病変もPSLにて改善したことから,食道天疱瘡と何らかの関係がある可能性も示唆される.

<p>『法華経』の「常不軽菩薩品」に登場する常不軽という名の出家菩薩は,増上慢の四衆にひたすら成仏の授記をし続け,そのことで彼らから誹謗,迫害を受けた.増上慢の四衆は常不軽を誹謗・迫害した罪で死後無間地獄に堕ちたが,自ら罪を畢え已わった後に常不軽と再会し,彼から『法華経』を教示され(=成仏の授記を受け),無上菩提へ向かう者となったとされる.ところが羅什訳の『妙法華』のみ,死時に臨んで常不軽が「其の罪,畢え已わって」(其罪畢已)としている.しかし,なぜ彼に罪があるのか,あるとすれば何の罪なのかが解明されないまま,今日に至っていた.</p><p>従来看過されていた事実として,Suzuki Takayasuは「常不軽菩薩品」に二種類の誹謗者が表されていることを明らかにした.二種類とは "『法華経』に出会う前の,『法華経』という仏語なしに無効な授記をしていた常不軽"(常不軽 ①)を誹謗した者たち(誹謗者 ①)と,"『法華経』に出会った後の,『法華経』という仏語をもって有効な授記をしていた常不軽"(常不軽 ②)を誹謗した者たち(誹謗者 ②)であり,後者の誹謗者 ② のみが,『法華経』説示者を誹謗した罪で堕地獄する.ところが『妙法華』のみ,常不軽 ② が『法華経』を説示していたという記述を欠いており,常不軽 ①と常不軽 ② との差違が判別しがたくなっている.</p><p>『法華経』の主張(『法華経』抜きに如来滅後に一切皆成の授記はできない.だからこそ,如来滅後にはこの『法華経』を説いて如来の名代として授記をせよ.如来のハタラキを肩代わりせよ)から判断して,常不軽 ① と常不軽 ② の差違は『法華経』にとって本質的であり,原典レベルで「其罪畢已」に相当する記述があったとは考えられない.羅什の参照した「亀茲(クチャ)の文」が特殊であったため,常不軽 ① と常不軽 ② との差違が判別しがたく,誹謗者 ① と誹謗者 ② を分ける必要が,漢訳段階で生じたものと考えるのが妥当である.しかし,そのために「其罪畢已」という文章を編み出したのは,羅什が『妙法華』訳出以前に『金剛般若経』を知っていたためと考えられる.</p><p>この「其罪畢已」という一節が,日本の日蓮に絶大な影響を与えた.日蓮宗の開祖である日蓮は,以前に念仏者・真言者であったため,「其罪畢已」の「罪」を過去の謗法罪と理解した上で,自らを常不軽と重ね合わせ,〈法華経の行者〉としての自覚を確立し,深めていった.</p><p>もし『妙法華』に「其罪畢已」の一節がなかったとしたら,日蓮は〈法華経の行者〉としての自覚を確立できず,その結果,『法華経』に向き合う姿勢を変えた可能性が高い.あるいは『法華経』信仰を捨てていた可能性まで考えられる.『開目抄』に見られる,「なぜ自分には諸天善神の加護がないのか」「自分は〈法華経の行者〉ではないのか」の解答の源は,「其罪畢已」以外には見出せないからである.</p><p>もし『妙法華』に「其罪畢已」の一節がなかったとしたら,中世以降今日に至る日本仏教は,現在とは大きく違った姿をしていたであろう.まさに,「大乗経典が外的世界を創出」(下田正弘)した好例である.『妙法華』に存するたった一個のフレーズ「其罪畢已」が,今日の日本の宗教界のみならず,社会の一側面を創出したのである.</p>

“The First Inspiration to Set One’s Mind on Seeking Enlightenment” (Shohosshinjibenjōshōkaku) is a key term from the “Kegon Sūtra” concerning the theory of attaining Buddhahood in the Kegon sect. “Shohosshinjibenjōshōkaku” has been the subject of a variety of debates since ancient times.In this paper, I discuss the Kegon interpretations of Shingon priest-scholars (such as Raiyu, Gōhō, Shōken, and Shōdō), who were active during the Middle Ages, from the perspective of “Shohosshinjibenjōshōkaku.” As a result, I am able to clarify differences between the interpretations of Shingi Shingon scholar-priests and those of Kogi Shingon scholar-priests.Furthermore, the method by which each school understands the “ideal world” provides a background for the differences between the two interpretations.

Five acidic 3', 5'-diesters of 5-fluoro-2'-deoxyuridine (FUdR) including 3-carboxypropionate, 4-carboxybutyrate, 5-carboxypentanoate, 6-carboxyhexanoate and 7-carboxyheptanoate were synthesized, and their susceptibility to porcine liver, rat liver homogenate, rat intestinal homogenate and rat plasma esterase preparations was studied. The susceptibility of the derivatives to porcine liver esterase preparation increased as the number of methylene groups in the ester promoiety increased in both acidic (pH 5.0) and neutral (pH 7.0) solutions. The derivatives showed about 100 times higher reactivity to the esterase at pH 5.0 than at pH 7.0. With the rat tissue homogenates and plasma preparations, the longer the ester chain the higher the susceptibility, except for 3', 5'-bis (3-carboxypropionyl) FUdR (I) with rat liver homogenate. Though the reactivity of I was the lowest with porcine liver esterase preparation and not measurable with rat intestinal homogenate and plasma, I showed the highest reactivity with the rat liver homogenate.

急性腸間膜血管閉塞症は,絞扼性イレウスと共に腸管の血行障害をきたす代表的な疾患である.最近我々は塞栓摘除術のみで救命せしめた上腸間膜動脈塞栓症の1例を経験したが発症より手術による血流再開に至るまで3時間40分と短時間であったため腸切除を免れた.また横浜市立大学第1外科のイレウス症例中,術前に動脈血ガス分析をおこなった33例を検討したところ,腸切除を免れた.すなわち腸管が壊死に陥らなかった症例ではbase excessはすべて-2.8mEq/l以上であった.このことより絞扼性イレウスの手術適応決定の指標としてbase excess測定が有用であるとの結論を得たが,本来イレウスにおける手術適応の決定は遅くとも腸管の可逆的な血行障害の時点でなければならず,この点よりbase excessのみでの適応決定は慎重でなければならないと考えられた. これらの経験をもとに,腸管の血行障害における有用な補助診断法としての的確な指標を検討するために犬を用いて上腸間膜動脈を結紮する実験をおこなった.その結果,早期診断の一助として, total CPK, base excessが有用であるとの結論を得た.

糖尿病患者の不眠の頻度は高く,睡眠の質の低下はHbA1c の上昇との関連が認められており,不眠の改善が重要である。今回,酸棗仁湯が2型糖尿病患者の不眠に有効であった2症例を経験した。症例1は58歳男性。罹病期間10年の糖尿病で,インスリンにてHbA1c は7%代であった。数ヵ月前より不眠となり,酸棗仁湯を投与し2週間で不眠は消失した。症例2は79歳男性。罹病期間15年の糖尿病で,経口薬にてHbA1c は7%代。1年前より不眠症となり,酸棗仁湯投与にて,不眠は徐々に改善した。酸棗仁湯は2型糖尿病患者の不眠に有用であると思われた。

注射針の細径化が求められているが,細径化は座屈を容易に招来する.鍼灸治療では刺手で針を側方から支え,押手で針を皮膚直上にて支えるとともに皮膚に張力を与えることで,座屈を防止している.本研究ではこれにヒントを得て,針を筒内に保持し,筒を皮膚に押し付け,針を少しずつ繰り出す機構を提案する.支えが無い皮膚から上の針の長さを実質ゼロにすることで座屈荷重を高め,座屈を防止する.

The activity of porcine liver esterase towards diesters of 5-fluoro-2'-deoxyuridine with saturated aliphatic acids including acetic, propionic, butyric, hexanoic, octanoic, decanoic and dodecanoic acids was investigated. The susceptibility of the 3', 5'-diesters increased as the acyl chain was lengthened up to octanoyl, but further increase in the acyl chain length resulted in a sharp decrease in the susceptibility. The susceptibility of 3'-and 5'-monoesters increased as the chain was lengthened to decanoyl and slightly decreased on going to dodecanoyl. These results suggest that the higher antitumor activity of longer alkyl chain diesters of 5-fluoro-2'-deoxyuridine is partly due to their slow rates of hydrolysis by non-specific esterase.

The bioactivation characteristics of 3', 5'-diesters of 5-fluoro-2'-deoxyuridine (FUdR) esterified with saturated aliphatic acids, including acetic, propionic, butyric, hexanoic, octanoic, decanoic, dodecanoic and trimethylacetic acids, were studied by using rat tissue homogenates and plasma. The susceptibility of the esters to hydrolysis by the biological media increased as the acyl promoiety was lengthened up to octanoyl. Further elongation resulted in decreasing susceptibility. Antitumor activity against L1210 of the esters with a longer chain or branched acyl group administered intraperitoneally and orally to tumor-bearing mice was also examined. Both the antitumor activity and the therapeutic index (ILSmax/ILS30) of the esters improved as the acyl promoiety was lengthened from octanoyl to dodecanoyl. These results suggested that the higher antitumor activities of longer alkyl chain diesters of FUdR are due to their slow rates of FUdR regeneration with esterases.

症例は33歳, 女性. ほぼ終日持続する心房頻拍 (atrial tachycardia ; AT) に対してカテーテルアブレーションを施行した. 12誘導心電図のP波の形状から心房頻拍は右心耳または三尖弁輪起源と推定された. EnSite Multi-Electrode Array (MEA) カテーテルが三尖弁輪を跨ぐように右室心尖部に向けて留置してAT中に三尖弁輪部のNCMを行い, 自由壁側10時方向に心房頻拍の起源を同定してカテーテルアブレーションに成功した. ATの機序としては心臓電気生理学的検査 (electrophysiological study ; EPS) 所見より異常自動能と考えられた. 三尖弁輪部は中隔側, 自由壁側ともconventional mappingに苦労することがあり, non-contact mapping (NCM) が有用と考えられた.

To investigate the description of the expiration date and manufacturing number on the drug packages, “pillow” packages and “heat-seal” packages, the questionnairing was carried out to pharmaceutical companies. Fifty-six drugs consisting of three hundred thirty-seven items, with sales of over 10 billion yen in 1998, were investigated at the corresponding 26 companies. The recovery rate of the questionnaire from the company was 100%, and the answer for all questions was also perfect. Three hundred fourteen items (93%) have had description regarding the manufacturing number and/or the expiration date on the heat-sealed packages. The manufacturing number took precedence over the expiration date. Of 269 pillow packages, 69 items (26%) have had description of manufacturing number and/or expiration date on the pillow package. There were 41 products (12%) that the company did not guarantee an expiration date being described on the box container when the packages were taken out from the box container.As a result, the description of the manufacturing number and expiration date on the packages, “pillow” and “heat-seal”, is still not complete even though the companies guarantee the expiration date when the drugs were stored with these packages. Since the information of the expiration date is necessary for the appropriate management of tablet and capsule products, it is required to describe manufacturing number and expiration date on every drug package including “pillow” and “heat-seal” packages.

The stability of powdery dosage forms of drugs prepared by unsealing the packages was investigated. For 247 articles purchased by the Yamanashi Medical University Hospital, the variables related to the evaluation of stability after unsealing these packages and investigating the contents based of an “Interview Form”. A longterm preservation test was carried out on 153 items (94%), and the mean length of the test was about 36 months. The acceleration and the severity tests were carried out using 17 items (10.4%) and 114 items (69.9%), respectively. The mean conditions of the acceleration test was 40°C/6 months, which agreed with the guidelines proposed by the “Drug Manufacturing Guidelines”. On the other hand, for the severity test, certain conditions for the evaluation could not be arranged, and testing was carried out under various conditions. In order to improve the efficiency of dispensing and the quality of life for patients, a further increased standardization of the testing conditions is recommended.

Di-2-ethylhexyl Phthalate (DEHP) was reported to dissolve from a polyvinyl chloride (PVC) administration tube during the infusion of an injection fluid containing a solvilizing agent, soybean oil or lecithin, etc. This paper describes the behavior of DEHP dissolution from PVC tubes during enteral tube feeding. The DEHP concentration was determined using HPLC. After the passage the ENSURE LIQUID® (EL) solution through the PVC tube, DEHP was dissolved in the EL solution. The EL solution is one type of enteral nutrition. This dissolution pattern was similar to that of injection fluid containing a solvilizing agent, in which the DEHP concentration in the EL solution increased linearly over time at first and thereafter reached a plateau. 170 μg of the DEHP was dissolved after 200mL of the EL solution had passed through the PVC administration set (the drip conditions were 25°C, 100mL/hr, tube length 115cm and inner diameter 0.3cm). The amount of DEHP which dissolved into the solution depended on the temperature (25-40°C), and no clear difference was observed between the PVC tubes (administration tube, feeding tube). In addition, the amount of DEHP that dissolved into the solution varied depending on the type of enteral nutrition.

生体内の細胞表層上には、糖タンパク質や糖脂質などの複合糖質中の糖鎖が外側に向かってアンテナ状に存在している。生体内に侵入してきたウイルスや細菌は、これらの糖鎖をマーカーとし、感染することが知られている。近年、糖鎖のもつ生理活性を高効率で発現させる糖鎖クラスター効果が注目を集め、様々な支持体を用いた糖鎖クラスター化合物の構築が盛んに行われている。これまで我々は有機ケイ素化合物であるカルボシランデンドリマーを支持体として液晶など様々な機能性基の集積化および物性評価を行ってきた。カルボシランデンドリマーは世代の拡張や分岐数・鎖長の制御が容易であるなどの特徴がある。この為、カルボシランデンドリマーを用いて糖鎖を集積化した場合、標的とするウイルス・毒素の糖鎖結合部位に適した分子設計がナノオーダーで可能であり、新たな糖鎖製剤となることが期待できる。
