<p>83 歳の女性,関節リウマチの診断で 20 年前からステロイドを含む免疫抑制剤が投与されていた。頭部に脱毛斑が出現し,近医皮膚科を受診した。頭部の皮疹はステロイド外用で悪化し,右手背にも落屑を伴う紅斑が出現した。頭髪の毛根部,頭部と手背の皮疹部鱗屑の直接鏡検で真菌成分がみられ,ケルスス禿瘡と手白癬と診断した。真菌培養にて <i>Microsporum gypseum</i> が原因菌であると同定した。テルビナフィン125 mg/日の内服を開始したが,瘙痒と疼痛が悪化し,頭部に厚い黄色の痂皮,いわゆる菌甲が生じた。テルビナフィンを 250 mg/日に増量投与したところ,症状は次第に改善し,内服開始から 5 カ月後に治癒した。関節リウマチの治療により免疫抑制状態にあり,通常量のテルビナフィンに治療抵抗性を示し,さらに菌甲を形成したと考えられた。</p>

<p>69歳,女性。当科初診の 2 カ月前から急速に増大する右こめかみ部の紅色結節を自覚した。近医皮膚科を受診し,切除目的で当科に紹介された。当科初診時,結節は 14 × 13 mm で,弾性やや硬の表面が平滑な淡紅色のドーム状を呈していた。ダーモスコピー所見で結節は淡紅色を呈し,黄白色内容物が透見された。また,不整な白色線条や不規則に分岐する血管拡張がみられた。生検による病理組織像で毛母腫と診断したが,腫瘍成分に重層扁平上皮を伴っていた。一般に毛母腫は若年者に好発し,正常皮膚に覆われ硬く触れる皮内あるいは皮下結節が多いが,表面に突出する腫瘤など多彩な臨床像を呈することもある。 本稿では高齢者に生じた毛母腫の症例を集計し,また非典型的症例ではダーモスコピー所見が毛母腫の診断に有用である可能性が示唆されたので報告した。</p>

In this study, the effect of shot peening on rolling contact fatigue life of bearing steel was investigated under contaminated lubrication. Shot peening was carried out on bearing steel SUJ2 specimens. Peened specimens were polished to remove the dimples generated by shot peening. The distribution of Vickers hardness and residual stress of the specimens were measured before and after the shot peening. The resistance to indentations was compared by examining the profile of the Vickers indentations. Then, rolling contact fatigue tests were carried out under contaminated lubrication. The material hardness was improved up to about 100 HV on the surface to a depth of 300 μm, and compressive residual stress was generated up to 1546 MPa at a depth of 25 μm due to the shot peening. In addition to the width and the depth of the Vickers indentation, the ridge height which causes the stress concentration around the indentation, was reduced by 17% by the shot peening. The rolling contact fatigue life associated with a 50% reliability (L50) was improved 62 % by the shot peening. However, the improvement of rolling contact fatigue life associated with a 90% reliability (L10) was only 26 % because of the increase in dispersion of the fatigue lives due to surface cracks created by the shot peening. Based on the present result, the increase of surface hardness and generating the large compressive residual stress near the surface by shot peening was found to be effective in increasing the rolling contact fatigue life in contaminated lubrication.

The circa dian rhythm of Ryodoraku has previously been reported, but the details are not yet well known. As a result Ryodoraku of three volunteers were examined several times a day w ith resistance and capacitance (R&C) using a multi-frequency body fat analyzer and/or oxida tion reduction potential (ORP), p H and volume of u rine.<BR>Accompanied with several small waves, Ryodo raku currents gradually increased from morning to afternoon and decreased from afternoon to night.<BR>Fluctuations in Ryodoraku were roughly s imilar to changes in urinary volume per hour. As urinary volume is regulated by anti-diuretic hormone, secretion of w hich is c ontrolled by the ce ntral nervous system, Ryodoraku might also be managed by the centrar nervous system.<BR>In most cases the changes in R yodoraku displayed the inverse associations to the large fluctuations of urinary ORP and to the alterations of R & C. It was thought that urinary ORP was affected by f ood intake or bathing etc. and R & C w ere influenced by cellular activities. Therefore Ryo doraku was considered to vary according to the living activities. From these findings, the circadian rhythm o f R yodoraku seems to be composed of a large wave controlled by the central nervous system and several small waves influenced by living activit ies.

Allergo-immunologic investigations and examinations of the autonomic nerve system were performed in a case of bronchial asthma with myasthenia gravis. The results were as follows:1) Asthma attacks and blephaloptosis were observed to tend to appear alternately.2) The same trend was shown by administration of mecholyl or other anti-cholinergic drugs and when such drugs were given asthma attacks increased, although blephaloptosis improved.3) Asthma attacks in this patient resembling those of many patients with bronchial asthma were observed mainly in the early morning.4) Allergy examinations showed normal serum IgE level, negative skin reaction tests and no peripheral eosinophilia.These results show that asthma attacks and symptoms of myasthenia gravis seemed to appear alternately in interaction of acetylcholine, showing a see-saw phenomenon and suggest that the autonomic nervous system participates in an induction mechanism or process in asthma.

吐[カ]喇列島の宝島および中之島から採集された雌雄成虫標本に基づいて, 新種A. takaraensis n. sp.タカラキイロアブを記載した。本種はA. kakeromaensis Hayakawa, Takahasi and Suzukiカケロマキイロアブに酷似しているが, 腹背面の斑紋が全くないことと, 翅脈R_4の小枝がきわめて短いかまたはそれを欠いていることにより, 容易に区別される。日本産キイロアブ属11種の検索表を掲げた。

The patient is a 14 year old boy, who admitted to our Department because of severe hemorrhagic diathesis and oliguria following a bite by Rhabdophis tigrinus. He had a snake bite on the 5th finger of right hand on September, 28, 1980. After 9 hrs, he noticed the bleeding at the sites of snake bite and gingiva. On the next day, he developed nasal, gastrointestinal, petechial and subcutaneous bleeding and hematoma 2 days later, and macroscopic hematuria 3 days later.Hemostatic examinations revealed complete incoagulability of whole blood clotting time, partial thromboplastin time, prothrombin time, serial thrombin time, and decreased amount of fibrinogen and elevated level of FDP. However, platelet count increased up to 50×104/mm3 at about 30 hrs after the snake bite. About 34 hrs after the first examination, platelet count decreased to 2.3×104/mm3, and at this time a diagnosis of disseminated intravascular coagulation (DIC) syndrome was made. As antiserum against this snake venom was not available, exchange blood transfusion was performed. His clinical feature and the hemostatic function were recovered well on the following days, while chronic renal failure has been persisted for several months.From above results, the exchange blood transfusion is favorable therapy in order to exclude the snake venom from the blood circulation.

これまで, ハロゲンランプ (HA) を光源とする光照射器が光重合型コンポジットレジンの重合に用いられてきた. 近年, キセノンランプ (XE) や青色発光ダイオード (LED) を用いた光重合器が開発され, 臨床応用されている. これらの光照射器は照射時間の短縮や発熱量軽減という利点をもつが, コンポジットレジンの重合や接着に与える影響については不明な点が多い. この研究では各種光照射器の使用によるコンポジットレジンの硬化と接着強さに与える影響について調べた. 各種光照射器の光強度を測定し, 3種類のコンポジットレジンを用い試料を重合後, ビッカース硬さ, 新鮮抜去牛歯に対する引張接着強さを測定した. 光強度はHA, XE, LEDの順に348mW/cm2, 1, 993mW/cm2, 131mW/cm2であった. XEおよびLED使用群で, すべてのコンポジットレジンのビッカース硬さはHA使用群に比較して低かった. XEはその高い光強度によって, 5秒間のみの光照射でボンディング材の重合に有利であると思われた.

As accuracy of industrial product was reduced by thermal deformation of a machine tool, a tool and a workpiece during cutting, there are several countermeasures for machining field. In the old days, we had developed and evaluated for a lathe with insensibility function for thermal and temperature change. The lathe has the structure of zero-center on three directions, the structure of self-compulsory cooling and the structure of thermal synchronism. Particularly the structure of thermal synchronism was developed for wet cutting. However the control method for the structure of thermal synchronism was yet to establish. Therefore the control using thermal synchronism at wet cutting in machine tool was developed and evaluated. The control system of fluid quantity on the structure of thermal synchronism using inverse analysis of neural network was developed for insensibility function of thermal and temperature change. Then thermal deformations of the developed lathe were measured and evaluated in the several experiments. It is concluded from the results that; (1) Thermal deformation of the bench lathe was very small in spite of no-forced cooling, (2) The control system of fluid quantity on the thermal synchronism using inverse analysis of neural network was effective for its working stability during wet cutting.

This paper presents a method for detecting and cancelling motion artifact related to standing and walking in a functional near-infrared spectroscopy (fNIRS) signal. Our fNIRS device has 22 channels. The motionless fNIRS signal from each channel is represented by a fourth-order autoregressive model (AR model), and the related parameters are estimated based on the motionless fNIRS signal using Yule Walker equation. The motion artifacts included in the fNIRS signal are cancelled using the Kalman filter constructed from the AR model. However, the cancellation may be insufficient when the motion artifacts are strong. To determine in which fNIRS channels the motion artifacts are cancelled insufficiently, we apply an observation prediction error related to the Kalman filter and a discrete Fourier transform. The brain activity of the user is then recognized from those fNIRS channels in which the motion artifacts are cancelled sufficiently. To evaluate the proposed method, a mobile robot is controlled using an fNIRS devise as worn by 10 subjects while standing, walking, or sitting. The success rate of brain-activity recognition by the proposed method was 64.2%, whereas that without the proposed method was 54.0%.
