(1)量子群の圏化の2圏構造をpivotalなものに変形できることを示した。(2)有限生成自由群の圏が対称モノイダル圏として可換Hopfモノイドにより自由に生成されるという事実の初等的・直接的な証明を与えた。(3)ハンドルボディ内の底タングルの圏BでKontsevich不変量を用いて定義される関手を構成し、圏Bの線形化のVassiliev-Goussarovフィルトレーションに付随する次数的線形圏の構造を決定した。(4)曲面の写像類群のJohnsonフィルトレーションとJohnson準同型の概念を、任意の群Gの任意のフィルター付き群Kへの作用に対して一般化した。

The pathogenesis of renal potassium wasting and hypokalemia in classic renal tubular acidosis (type 1 RTA) remains uncertain. The prevailing theory is that K+-Na+ exchange is stimulated due to an inability of the distal tubule to establish a normal steep lumen-peritubular H+ gradient. We encountered a 42-year-old woman with type 1 RTA associated with Sjogren's syndrome, in whom renal potassium wasting and hypokalemia persisted despite sustained correction of systemic acidosis with alkali therapy and increased intake of potassium. In addition, plasma renin activity was markedly increased and the serum aldosterone level was upper-normal despite the hypokalemia. Increased intake of sodium resulted in suppression on the serum aldosterone and correction of renal potassium wasting and hypokalemia. This case shows that secondary hyperaldosteronism, possibly due to an impairment of sodium conservation in the distal tubule, may contribute to the loss of potassium from the distal tubule even after the correction of acidosis.(Internal Medicine 31 : 1047-1051, 1992)

本稿の目的は,ソーシャルアクションの実践方法としてのプログラム評価の有用性および方法について考察することである.東京都渋谷区で行われた「渋谷スタディクーポン事業」へのプログラム評価の実践事例を取り上げ,ソーシャルアクションにおける展開方法としての,プログラム評価の有用性および方法について考察を行った.その結果,プログラム評価を通じた「プログラム理論の明示化」,「ニーズの明確化」,「プログラム効果の可視化」,「評価結果の提示・共有化」が,本事業の政策化というソーシャルアクションの実現に寄与したと考えられた.以上から,プログラム評価はソーシャルアクションの実践方法として,有用な実践方法である可能性が高いと考えられた.

To the public, highways and roads are the most familiar public utility. Therefore, every people have their own opinions and complain against highway and road administration. The public and the groups, for instance trucking industries and automobile manufacturers, have strong interests in highway and road policy, and infl uence its decision-making processes. During the last decade, Japan has experienced several failures in the highway and road policy-making. This paper picks up three policy topics such as the privatization of the Japan Highway Public Corporation, the transfer of automobile related taxes from the earmarked expenditure for highways and roads to the general account, and the change of the metropolitan expressway toll structure from a fl at fare system to a kilometer-based fare one. These three cases indicate that the decision-making process of the highway and road policy cannot result in optimal situations. For more appropriate highway and road policy, this paper proposes introducing constitutional rules into decision-making processes for the highway and road policy in Japan.

【はじめに、目的】スタティックストレッチング(SS)は関節可動域(ROM)などの柔軟性改善効果が期待でき,臨床現場やスポーツ現場で多用されている.しかし近年では,特に45~90秒以上のSSにより筋力やパワーなどの運動パフォーマンスが低下することから,運動前のウォームアップにおいてSSを行うことは推奨されていない.しかし,実際のスポーツ現場では20秒以下のSSが用いられる場合が多いと報告されており,20秒以下のSSは運動パフォーマンス低下を生じさせない可能性がある.しかし20秒以下のSSの即時効果を検討した報告は非常に少なく,さらにその持続効果を検討した報告はない.そこで本研究では,柔軟性改善効果の指標としてROMと弾性率,運動パフォーマンスの指標として求心性収縮筋力(CON)と遠心性収縮筋力(ECC)を用いて20秒間のSSの即時効果と持続効果を明らかにすることを目的とした.【方法】対象は健常成人男性20名の利き足(ボールを蹴る)側の足関節底屈筋群とした.足関節底屈筋群に対する20秒間のSS介入前後に足関節背屈ROM(DF ROM),弾性率,CON,ECCの順に各々測定した.先行研究に従ってSS介入後の測定は,①介入直後に測定する条件,②介入5分後に測定する条件,③介入10分後に測定する条件の計3条件を設けた.対象者は,無作為に振り分けられた条件順にて3条件全ての実験を行った.DF ROM,CON,ECCの測定は多用途筋機能評価訓練装置(BIODEX system4.0)を用いて行った.なお,CONとECCの測定は足関節底屈20°から背屈10°の範囲で,角速度30°/secに設定して行った.弾性率の測定は超音波診断装置(Aplio500:東芝メディカルシステムズ株式会社)のせん断波エラストグラフィー機能を用い,腓腹筋内側頭(MG)に対して行った.統計学的検定は,SS介入前後および条件(直後条件,5分条件,10分条件)間の比較は繰り返しのある二元配置分散分析(時期×条件)を用いて検討した.さらに,事後検定として,SS介入前後における条件間の比較はBonfferoni法,各条件におけるSS介入前後の比較は対応のあるt検定を用いて検討した.【結果】DF ROMに有意な交互作用(p=0.004, F=6.517)と,時期に主効果を認めたが,MGの弾性率,CON,ECCには有意な交互作用及び主効果を認めなかった.事後検定の結果,DF ROMは全条件において介入前と比較して介入後に有意に増加した.またDF ROMは介入直後と比較して介入10分後に有意に低値を示した.【考察】本研究の結果より,20秒間のSSはDF ROMを即時的に増加させるために有効であり,その効果は10分後まで持続するが,5~10分の間で減弱することが明らかとなった.また,20秒間のSSは弾性率,CON及びECCに影響を及ぼさないことが明らかとなった.【結論】20秒間の短時間のSS介入は等速性筋力を低下させずにROMを増加させるが,弾性率を変化させることが出来ないことが明らかとなった.【倫理的配慮,説明と同意】本研究は本学の倫理審査委員会も承認を受けて実施された.また,本研究はヘルシンキ宣言に則っており,実験開始前に対象者に本研究内容を口頭と書面にて十分に説明し,同意を得た上で行われた.

要旨:【目的】ステントグラフト内挿術後には動脈瘤の急激な血栓化により凝固線溶系が亢進することが知られている.トラネキサム酸を投与することで術後の線溶系亢進およびtype 2 endoleak が抑制されるかについて検討を行った.【方法】腹部大動脈瘤51 例,胸部大動脈瘤38 例の計89 例を対象とし後方視的に検討を行った.周術期に凝固線溶系を測定,またtype 2 endoleak の評価は術後1 週間目および6 カ月目の造影CT にて行った.【結果】PT(INR),APTT,AT,血小板数においてはトラネキサム酸の投与に影響がなかった.Fibrinogen はトラネキサム酸投与群で有意に高値を示し消費の抑制が示唆された.FDP は非投与群で術後5 日目より再上昇を示し線溶系の亢進を認めたが,トラネキサム酸投与群ではその再上昇が有意に抑制されていた(43.8±36.6 vs 18.3±17.1 µg/ml: p=0.004).Type 2 endoleak は術後1 週間目において非投与群が22.0%であったのに対し投与群が6.3%と有意に抑制され(p=0.031),その傾向は6 カ月目でも持続した.【結論】トラネキサム酸を投与することでステントグラフト内挿術後の線溶系亢進およびtype 2 endoleak が抑制されることが示唆された.

Mi sono interessato particolarmente alla parte del canto XV del "Paradiso" che costituisce il preambolo al famoso incontro con Cacciaguida, in cui il trisavolo del poeta rivolge a Dio parole incomprensibili ai mortali. Si tratta di un linguaggio mistico che il poeta non ha modo di intendere. Gli angeli e i beati non hanno bisogno di nessun mezzo di comunicazione, perche la loro intelligenza non viene coperta "cum grossitie atque opacitate mortalis corporis humanus". Essi mirano "nello speglio in che, prima che pensi, il pensier pandi". Non era necessario quindi per un beato come Cacciaguida parlare a Dio che "tutto discerne". Ci deve esere un altro motivo per cui egli parla. A mio parere nelle parole che Cacciguida rivolge a Dio vi e un elemento mistico e liturgico. Nel "De vulgari eloquentia" Dante spiega l'origine della lingua umana, basandosi sulla narrazione del "Ggenesi", ma aggiunge delle considerazioni personali che non si trovano nel testo originale. Il poeta pensa che Adamo abbia cominciato a farsi sentire prima che a sentire, perche nell'uomo il farsi sentire e piu essenziale che il sentire. Adamo, unico uomo esistente, avrebbe rivolto la sua prima parola a Dio per farsi intendere. Ma Dio discerne tutti i segreti della mente del primo uomo, e non c'era necessita che Adamo parlasse. Dio tuttavia volle che anch' egli parlasse, affinche fosse gloirficata la Sua opera. Io penso che Cacciaguida rivolga le sue parole a Dio per una ragione analoga : glorificare l'opera del Signore, ringraziandolo per l'eccezionale generosita con cui ha concesso al suo discendente l'alta missione di "scriba Dei". Di fatto il linguaggio mistico di Cacciaguida si configura come uno sfogo religioso simile a una preghiera. Una situazione non molto diversa si puo osservare nel dialogo tra Dante e Cacciaguida. Dal momento che quest' ultimo prevede tutto quanto il poeta concepisce nella mente senza che venga espressa con il linguaggio, non sarebbe stato necessario che Dante si esprimesse. Cio nonostante il trisavolo gli chiede di parlare con voce "sicura, balda e lieta". Anche qui si tratta di un atto prettamente rituale. Che significato ha dunque in Dante il ricorso a tale azione rituale? Senza dubbio il poeta voleva conferire autorita alla missione decretata dall' Eterna Volonta senza tuttavia esplicitarne il vero significato. In conclusione il linguaggio mistico nel episodio considerato e un artificio retorico utilizzato abilmente dal poeta per rendere ancora piu solenne l'incontro memorabile con il suo trisavolo.

Maternal care is indispensable for survival of neonates in mammals. Lactating females consume a large amount of energy for nurturing their pups by lactation. Management of energy expenditure through lactation is important for survival of themselves and pups. We previously reported that the orexigenic neuropeptide neuropeptide Y in the dorsal raphe nucleus (DRN) regulated maternal care, depending on food intakes of lactating females. In the present study, we investigated the neuronal mechanism for regulating maternal care by melanocortin 3 and 4 receptors (MC 3/4R), which was regulated by the anorexigenic neuropeptide alpha-melanocyte-stimulating hormone (α-MSH) and the orexigenic neuropeptide agouti-related peptide (AgRP) in an opposing manner. Neuronal processes immunoreactive to adrenocorticotropic hormone, which was a precursor of α-MSH, or AgRP, were distributed in the DRN. Furthermore, the pre-synaptic marker synaptophysin was co-localized with ACTH or AgRP in the DRN. We next investigated how the MC3/4R antagonist SHU 9119 affected maternal care. Injection of 100 pmol SHU 9119 into the DRN prevented maternal care in fed dams. Additionally, we examined whether the agonist melanotan II could affect maternal care following fasting for 8 h. Fasting for 8 h abolished maternal care in lactating females. But injection of 100 pmol melanotan II into the DRN partially recovered maternal care. These results indicate that MC3/4R signaling in the DRN regulates maternal care depending on feed intake in lactating mice.
