現在までに臨床導入された心筋保護法には多くの組成・方式があるがいずれも有効性や安全性に遜色なく,その選択は理論的特徴や基礎臨床研究よりもむしろ施設や術者の好みに委ねられてきた.このため心筋保護はすでに確立した術中手段としてその基礎理論の重要性や臨床的な検証が軽視される傾向にある.しかし現代の心臓外科においては手術適応拡大による重症例や拡大手術症例の増加,MICSの導入や修練外科医の教育などを背景に,予期しない長時間心停止あるいは心筋保護灌流不均衡など想定外の事態において,心筋保護法が生死をわける事態をまねくことも稀でない.また最近ではさまざまな心筋保護に関する前向きランダム化比較試験randomized controlled trial(RCT)が報告されるようになり不十分ながら客観的検証に基づくエビデンスがようやく構築されつつある.このため心臓血管外科医にとって学術的意義のみならず医療安全の観点からも心筋保護理論の習熟と最新情報の周知はきわめて重要であることが再認識されるにいたった.現在開心術のための臨床的心筋保護法とは,心筋保護液組成crystalloid/blood cardioplegia,心筋保護液温度cold/warm/tepid, 投与方式continuous/intermittent(multidose)/single dose,投与経路antegrade/retrograde deliveryなどさまざまな要素によって構成されるハートチーム全体で連携すべき総合的補助手段である.本稿では代表的な心筋保護法についてその適正な選択のための基礎的理論と臨床成績を概説するとともに,Del Nido cardioplegia,Microplegiaなど新たな心筋保護戦略,比較臨床研究結果などの最新の臨床知見を紹介する.

ゼラチンと寒天のゾルゲル変化を応用して、単層稠密にPDLC構造を作成するアイデアを考案し、実験検証した。プロセス中のゼラチンと寒天の温度をコントロールすることで、ゾル状態のゼラチンエマルジョンの上に、ゲル状態の寒天層が積層される状態を作り出した。その結果、固いゲル状態の寒天層が液晶ドロップを押し込む効果が発現し、多角柱に変形した液晶滴が単層稠密に配置され、かつ表面がフラットなPDLC構造を実現した。

光や温度など周囲環境に遍在するエネルギーを利用し,駆動環境の空間的限定が少ない状態で推進力を得るシステムを提案する.さらに直線的な推進に加え,光の強度の強い方向に向かう走行性など微生物に見られる機能を自律型ソフトマイクロボットで再構成することにより,自律型ロボットの制御に関する実験的なモデルの構築を目指す.

I previously (Takatsuki, 2009) proposed the following question: Although bears are often regarded as umbrella species, is this actually true? So far, I have not been able to find scientific evidence in support of this, and I feel that conservation activities should be based on scientific approaches. Indeed, this opinion is in accordance with that of S. Boutin (2005) in his review of the ecological effects of carnivores in boreal forests of Nordic countries, in which he discussed several effects of bears on ungulates, hares, rodents, and vegetation through cascade effects. Boutin emphasized that a "fine-filter" conservation approach that focuses on particularly charismatic carnivores often overlooks ecological processes and that carnivore-oriented conservation requires large refuges. However, actual refuges are often too small for such large carnivore species, particularly in Europe. Such approaches that focus only on carnivores as umbrella species risk the loss of endangered species or organisms requiring particular ecological processes. For biodiversity conservation, a "coarse-filter" approach that focuses on ecological processes such as wild fires, logging, and succession is more important and effective. Given that the social conditions of Japan in terms of biological conservation are often more similar to those of Europe than of North America, a "coarse-filter" approach may be more appropriate for bear conservation in Japan.

【目 的】 豪雪地域の居宅高齢者の日常生活における身体活動量の季節変動を明らかにすることを目的とした. 【対象と方法】 2008年からの1年間, 月毎に7日, 豪雪地域に居住する健常高齢者5名に歩数計を装着してもらい, 歩数と活動時間, 対象者が書いた生活日誌の内容を分析した. 研究目的, 方法, 匿名性, 研究協力への自由意思について説明し, 同意を得た. 【結 果】 対象者の内2名は春季から秋季にかけて歩数と活動時間が多く, 冬季は少なかった. 残りの3名にこれらの季節変動はみられなかった. 【結 語】 季節変動がみられた者の日誌には, 歩数目標の設定と自己評価, 目標を達成するための工夫が表現されており, 身体活動量の季節変動には積極的な自己管理態度や行動特性が影響しているものと考えられた. 冬季には活動量の維持が困難であることから, 豪雪地域においては冬季の不利な気象条件による影響が低減されるような活動場所と機会の整備が求められる.

目的:看護職員のワーク・エンゲイジメントと看護師長の自己効力感の関連について,看護職員による所属部署の看護師長に対するリーダーシップの認識の媒介効果を明らかにすることを目的とする.方法:16医療機関の看護師長,看護職員を対象に看護職員のワーク・エンゲイジメントと看護師長の自己効力感,リーダーシップについて質問紙調査を行った.看護職員269名を対象にマルチレベル媒介分析を行った.結果:看護職員による看護師長のリーダーシップの認識は看護師長の自己効力感と看護職員のワーク・エンゲイジメントを完全に媒介していることが明らかとなった.看護師長の自己効力感と看護職員のワーク・エンゲイジメントにおけるリーダーシップの認識の媒介効果は個人レベルでの効果であることが示唆された.結論:看護師長の自己効力感は看護職員によるリーダーシップの認識を介して,看護職員のワーク・エンゲイジメントに影響を及ぼしていることが明らかとなった.

Coronavirus disease 2019 (COVID-19) has been spreading globally since 2019;however, comprehensive rehabilitation of elderly patients with COVID-19 pneumonia remains a challenge. A 76-year-old American woman with COVID-19 pneumonia was admitted to our hospital. Because her disease was complicated by acute respiratory distress syndrome (ARDS), she was treated with intensive care, including invasive ventilation and extracorporeal membrane oxygenation (ECMO). During and after intensive care, she exhibited physical symptoms such as weakness, pain, shortness of breath, and difficulty in movement and exercise. Furthermore, during approximately 3.5 months of hospitalization, she received swallowing and speech therapies along with physical therapy. These rehabilitation therapies enabled her to get home in the United States. Her rehabilitation schedule had to be carefully planned according to her symptoms and infectiousness of COVID-19. This paper highlights few important points regarding the difficulty in rehabilitation including that of physical function, mental health, and cognitive function of patients with COVID-19. Furthermore, this report provides a problem-solving approach for long-term rehabilitation in elderly patients with COVID-19 pneumonia.

For investigation of the relationship between plant growth and environmental factors in a plant factory, an experimental plant factory has been built and some experiments have been carried out. Leaf lettuce was grown in a hydroponic system under artificial light condition. Arial environment such as air temperature, humidity and CO2 concentration were controlled. Root environment such as EC and pH were monitored and controlled by a microcomputer.In the first experiment, EC level was maintained at a constant set-point and the difference between the set-point and the actual value was summed up when the latter value was corrected. Transpiration was also calculated from the water depression in the tank. Then it was summed up through the growing period.Following results were obtained:(1) The EC decrement showed high correlation with the transpiration, regardless of light intensity and (2) the transpiration rate per EC decrement decreased as the EC of nutrient solution became higher.In the second experiment, the effects of light intensity, lighting cycle and EC of nutrient solution on tipburn injury were examined.Following results were obtained: (3) The tipburn occurrence became later under the conditions that the EC was low and the duration of a lighting cycle was short and (4) the tipburn occurrence became later under the conditions that the light intensity was low and the duration of a lighting cycle was short.Effect of the duration of a lighting cycle on the tipburn occurrence showed larger as the light intensity increased. From the results of Tibbitts et al (1985), it is, therefore, considered that the pressure of laticifer or the Ca concentration in the wall of laticifer increases with the increase of the duration of lighting cycle and this could cause the tipburn.

本論文では,銀座や竹下通りといった外国人に対する著名地区ではなく,裏路地に極めて多くのインバウンド旅行者が回遊する裏原宿に着目し,その誘引性を考察した。結果は,裏原宿への最も多い来街者は中国からの旅行者であり,裏原宿独特のストリートファッションブランドに誘引されて訪れていることが分かった。また,これらのブランドは,近年まで日本のファッションの主軸として語られてきた百貨店やSCを中心に販売するブランド群とは異なるものであり,当該エリアならではのカルチャーを有するユニークなブランド,そしてオリジナリティーや希少性ある商品が評価され,推奨の対象となっていることが,Kotler5Aモデルを修正したカスタマージャーニーにより示された。
