The middle section of the Itoigawa-Shizuoka Tectonic Line (Middle ISTL) is an active fault system that extends NW-SE for 50km from Matsumoto to Kobuchizawa, in central Japan. The Middle ISTL is characterized by high average slip-rate reaching 8 to 10mm/yr during the Late Pleistocene and Holocene. This is one of the highest slip-rate reported from active faults on land in Japan. Empirical relation between slip-rate and recurrence time indicates that the Middle ISTL may rupture more than once a thousand year. The previously known recurrence time estimates of 3500 to 5000 years were significantly longer than the expected recurrence time. The last faulting event on the Middle ISTL most likely occurred in 841 A. D. according to historic record and paleoseismological works. Since the elapsed time is about 1152 years, the estimation of recurrence time is critical to evaluate the potential of next earthquake. In order to know the history of recent faulting events, we excavated the Gofukuji fault, the northernmost segment of the Middle ISTL at Namiyanagi, south of Matsumoto. Investigation of 6 trenches, 3 test pits and topography around the trenches brought following results. The ages of three most recent faulting events are 445-1386 A. D., 150-334 A. D., and 839-189 B. C. The recurrence time is between 111 and 1236 years. Assuming the correlation of the last event with the 841 A. D. event, the recurrence time is estimated to be 338 to 1172 years. Average recurrence interval in this case ranges 515 to 840 years. The elapsed time of 607 to 1152 years is probably longer than the recurrence time. The average slip rate of the fault since c. a. 6000 B. C. is 9.4±4.5mm/yr left-lateral. The left-lateral coseismic slip during the last event is estimated as 7.5±1.5m.

Meta-analyses have revealed that the relative risk of hip fractures in patients with type 1 and type 2 diabetes mellitus is higher than that in non-diabetic subjects. The risk of fracture in patients with diabetes mellitus increases along with a decrease in bone mineral density (BMD) similarly to those in non-diabetic patients. However, the observed risk of fracture is higher than expected one by BMD in both type 1 and type 2 diabetic patients, indicating that precise estimation of bone fragility by BMD values in patients with diabetes is difficult. Bone strength consists of BMD and bone quality, for this reason, poor bone quality is a most suitable and explicable cause for elevated fracture risk in this population. This bone fragility observed in patients with diabetes mellitus is caused by unique pathogenesis in diabetes, suggesting that osteoporosis in diabetic patients may be one of the diabetic complications and that specific diagnostic criteria for this osteoporosis is required. Bone quality indicators closely related to bone fragility are required to be identified to establish a diagnostic method for osteoporosis in patients with diabetes mellitus.

ケアの求心力は人の国際移動を大きく促進した. 1つは香港, シンガポール, 台湾に存在する70万人をはじめとする家事労働者にみられ, 家事の補填のために雇用される途上国の女性である. 労働市場における非競合性, 雇用主にとっての「利便性」もあり経済成長や高齢化を背景として家事市場は拡大した. 外国からの豊富な労働供給を背景に市場は安定したが, 家事労働をめぐる階層化, 性役割分業の固定化を伴った. 次第に高齢化を背景とするケア需要が増大した. 各種補助金や税控除, 老親扶養法など「家族化政策」と連動し家事市場はさらに拡大したが, ケアの社会化は困難となった. 家族主義の問題点は家族形成を前提とするが, 日本, 韓国, 台湾など先進国における高齢者, 障害者などによるケア確保が一義的な国際結婚が増大した. 少子化や家族危機の言説と絡まり, 国際結婚は社会統合・多文化政策のきっかけとなった. 良き家族の一員としての統合は, 伝統回帰型かジェンダー平等型かという点で課題を抱えるが日本を除き政権にかかわらず推進されている. しかし, これらの移動は家事労働者の労働者性の担保, 婚姻過程の経済取引化などは結婚移民の脆弱性の原因となっている. 送り出しに伴う家族構成員の移動と欠如, 受け入れ側の家事労働者の家族接合, いずれにせよ従来とは異なるケアの供給体制であり, 近代家族自体が相対化されるものである.

Feline renal diseases are increasingly noted in veterinary practice. It is important to diagnose and identify the pathological basis of renal dysfunction accurately at an early stage, but there are only a few reports on this area in clinical veterinary medicine. We investigated the efficacy of measurement of urinary albumin (u-Alb) and urinary transferrin (u-Tf) for early diagnosis using 5-µl urine samples collected noninvasively by catheterization from normal (IRIS stage I) cats and cats with stage I chronic kidney disease (CKD). The u-Alb levels in normal and stage I CKD cats were 6.0 ± 4.5 and 11.2 ± 8.4 mg/dl, respectively, and the u-Tf levels were 0.09 ± 0.42 and 0.52 ± 0.79 mg/dl, respectively. Based on ROC curve analysis, the sensitivity and specificity of u-Alb and u-Tf were higher than those of the currently used biomarker, the plasma creatinine level. The sensitivity of u-Alb was higher than that of u-Tf, whereas the specificity of u-Tf was higher than that of u-Alb. The validity of the threshold albumin level (20 mg/dl) was confirmed by measurements using SDS-PAGE. Since leakage of u-Tf in urine precedes leakage of u-Alb, inclusion of u-Tf in biochemistry tests may be appropriate for IRIS staging as a diagnostic marker of early diagnosis of renal disorder in cats.

High temperature volcanic sublimates including so-called “molybdenum blue” sampled around the high-temperature fumaroles at Iwodake volcano, Satsuma-Iwojima, Kyushu, Southwestern Japan, were characterized by X-ray powder diffraction, X-ray micro-diffraction, Raman micro-scattering, electron microprobe and X-ray photoelectron spectroscopic analyses. The identified minerals are molybdenite, sphalerite, tugarinovite, molybdite, hematite, halite, sylvite, anglesite, wulfenite, quartz, tridymite, cristbalite and potassium alum. Occurrences of both tugarinovite and molybdite are observed in Kudriavy volcano, too, and the coexistence of their minerals is characteristic of high-temperature volcanic sublimates. The molybdenite and sphalerite from Iwodake are exceedingly rich in Re (up to 0.77 wt%) and In (up to 1.69 wt%), respectively, distinguished from those of a different origin by chemical composition. The molybdenite consists of both of 3R and 2H1 polytypes. Sublimates from Iwodake and Kudriavy volcanoes are similar in mineralogy. Although the difference in the host rock arises between the Iwodake (rhyolite) and the Kudriavy (basaltic andesite) volcanoes, their volcanic gases resemble each other in F and Cl contents. This resemblance might cause the similarity in sublimate mineralogy between these volcanoes.

現在,日本国内の橋梁維持管理の現場では,目視や各種センサを使ったモニタリング(Structural Health Monitoring:SHM)によって定期的に変状検知が実施されている.特に常時監視についてはSHMの有効性を示す研究成果が報告されている.一方で,SHM導入・維持に関する各種コストや技術的課題が指摘されており,より簡便な手法の開発が必要となっている. 本研究では,SHMに関する各種問題を解決する手段の一つとして,国内の多くの橋梁がスパン10m~20m程度の短スパン橋梁であることを鑑み,それらを対象に,公共交通機関である路線バスを利用した新たなモニタリング手法の提案および検証と実証実験を行った.

Two concepts: social orientedness and individual orientedness, relating to two-dimensionality of self-consiousness, were proposed in order to grasp personality traits and adjustmental level and developmental level. Three questionnaires: an orientedness scale, a SD self-concept scale, and a self-esteem scale were administered to adolescent and adult subjects. The results showed that orientedness scores rose with increasing age, but a difference between males and females on both changing phase and process was found. Each orientedness changed in content, such as social orientedness following the next process: from dependency and others-direction to coexistence; on the other hand, individual orientedness followed the next process: from egocentrism to autonomy. These orientednesses mean the content of horizontal axis and vertical axis of two-dimensional developmental schema. From the relation with self-esteem, and discrepancy scores between real self image and two ideal-images: ‘social ideal self’,‘individual ideal self’, by SD self-concept scale, it was clear that the individuation process on emotional aspect, and the socialization process on the cognitive one could be observed.

A 91 year-old male who was orally receiving dabigatran and digitalis for chronic arterial fibrillation was hospitalized for cerebral infarction (CI). The complications of anemia due to gastrointestinal bleeding and the development of renal failure were also found, which enhanced the effect of digitalis. This enhanced drug action induced bradycardia and hypotension. The hypotension was improved by implanting a temporary pacemaker, and the neurological abnormal findings also disappeared with the improved blood pressure. In many cases, dabigatran is efficient in preventing CI, but in our case it in fact triggered a CI hemodynamically due to bradycardia and hypotension with gastrointestinal bleeding.

[Purpose] The purpose of this study was to compare hip range of motion between a lumbar stability group and a lumbar instability group, and to evaluate the effectiveness of hip exercises for low-back pain patients with lumbar instability. [Subjects] Seventy-eight patients with chronic low-back pain were the subjects. [Methods] The patients were divided into two groups: a lumbar stability group (n=45) and a lumbar instability group (n=33). They were assessed using the Korean version of the Oswestry Disability Index (KODI) to determine the level of disability of the patients with low-back pain. A 100 mm visual analog scale (VAS) was used to assess low-back pain. [Results] The limitation of hip range of motion of the lumbar instability group was significantly greater than that of the lumbar stability group. Comparisons among four groups at three weeks and six weeks after the start of hip exercises revealed that the VAS score of each group had significantly decreased. Comparisons among four groups at three weeks and at six weeks after the start of hip exercises revealed that the KODI score of each group had significantly decreased. [Conclusion] These findings suggest that the performance of hip exercises by chronic low-back pain patients with lumbar instability is more effective than conventional therapy at reducing low-back pain and levels of disability.

Coronary artery disease (CAD) is the most common cause of death and physical disabilities in developed countries, even though efforts to identify and target causal factors such as hypertension and dyslipidemia have brought tremendous improvements in prevention and treatment. A rapid advance in technology has unraveled new genetic variants associated with CAD and also provided great opportunities to identify novel pathogenic mechanisms and to develop new drugs with higher specificity. Whole-genome sequencing and whole-exome sequencing has made it possible to find rare alleles that are responsible for CAD in small, affected families and case-control studies in a very efficient manner. At present, genome-wide association studies have identified more than 50 loci that explain approximately 10% of the heritability of CAD, most of which is unrelated to traditional risk factors. Mendelian randomization studies enable identification of causal factors among numerous biomarkers and to narrow down promising therapeutic targets. This review highlights new genetic approaches and demonstrates the extent to which the outcome contributes to the finding of new therapeutic targets. (Circ J 2015; 79: 8–14)

A strategy named “Japan is back” adopted in June 2013 specifies that pharmacies shall be regarded as community-based places where health-related information is provided, and the public shall be encouraged to use the services of pharmacies and pharmacists who can advise on health and appropriate use of over-the-counter (OTC) drugs, and promote self medication. In Japan there are approximately 55000 pharmacies and 260000 pharmacists, and community residents are recommended to use these resources. As advisors on healthcare in the community, pharmacists are required to make judgments regarding drug use in individuals performing self medication and using OTC drugs in consideration of their symptoms and level of understanding of their health conditions, and recommend that they consult a medical center if necessary. To meet these requirements pharmacists need to have the skills to monitor each individual's lifestyle, behavior, and environment as well as trends in society, and assess their health status. However, education that allows pharmacists to practice such skills remains insufficiently developed. We consider that to be able to detect diseases early among community residents and appropriately support them using pharmacotherapy, it is very important to train pharmacists to do the following at pharmacies: 1) determine individuals who should be treated early using symptomatologic skills; 2) promote public awareness of disease; and 3) perform biochemical examination (blood is collected by fingerprick promptly to obtain biochemical data) in cooperation with the Department of Clinical Pharmacotherapy, Faculty of Pharmaceutical Sciences, Hiroshima University.

This paper summarizes the debate between Blaut and Downs (and his collaborator, Liben) concerning the development of mapping abilities of young children.Jean Piaget's theory of cognitive development has been one of the most influential theories in the field of cognitive mapping research. Two essential elements of Piaget's theory regarding the developmental sequence of spatial abilities with age are nativism and constructivism. A forum in the Annals of the Association of American Geographers journal in 1997 on the mapping abilities of young children helped to distinguish the intrinsic duality of Piaget's developmental theory and its influence on the debate between Blaut and Downs/Liben.Both Blaut and Downs/Liben studied young children's ontogenetic development of mapping abilities, but they examined different aspects of Piaget's theory. On the basis of nativism, Blaut recognized that children can perform various mapping tasks regardless of age, and he insisted that children naturally possess mapping abilities, whereas Piaget implied that they could not. In contrast, on the basis of constructivism, Downs and his colleagues emphasized that children's mapping abilities are the effect of both direct and indirect learning experience, and they pointed out some deficiencies in Blaut's testing method.In spite of their contrasting standpoints, both researchers were influenced by Piaget's theories, but emphasized different aspects of the complex process of cognitive mapping development.
